2.Clinical characteristics of Littre hernia in children

Objective To investigate the clinical features of pedistric Littre hernia.Methods Clinical data of 11 cases of Littre hermia admitted from January 2002 to December 2010 was studied retrospectively.Results The diagnosis of Littre hernia was all established by laparotomy.All of the 11 cases were boys,the median age was 1.2 years (22 days to 3 years and 7 months).The main clinical features were:painful,irregular and nonresetable mass in the groin area (11/11),vomiting in 6 cases (6/11 ),fever (>38.5 ℃ ) in 4 cases (4/11 );X-ray showed intestinal obstruction in 9 cases (9/11 ),Ultrasound found mixed mass in the groin area in 11 cases ( 11/11 ),pouch-like structure were found in 6 cases (6/11).Eight cases (8/11) presented with elevated WBC ( > 10000).Palpable lobulated structure were fell in 5 cases (5/11).All cases of Littre hernia were successfully operated on,Meckel diverticulum perforation was found in 2 cases ( 2/11 ),Meckel diverticulum adhered to the hernia sac in 8 eases (8/11).All patients were cured by surgery,and postoperative follow-up (10ms-8y) found no recurrence.Conclusions Pediatric Littre hernia is rare,preoperative diagnosis is difficult.Avoiding preoperative violent reduction and intraoperative injuring Meekel's diverticulum or the small bowel helps improve the cure rate of Littre hernia in children.
3.Clinical trial of progesterone combined with nifedipine in the treatment of upper ureteral calculi

Objective To evaluate the clinical efficacy and safety of progesterone combined with nifedipine in the treatment of upper ureteral calculi.Methods A total of 94 patients with upper ureteral calculi werc divided into control group and treatment group,47 cases in cach group.Patients in control group were given anisodamine hydrochloride injection 5-10 mg by intramuscular injection,qd.Patients in treatment group received progesterone injection 20 mg by intramuscular injection,bid and nifedipine tablets 10 mg by under tongue,tid.All patients were treated for 14 d.The clinical efficacy,C-reactive protein(CRP),interleukin-6 (IL-6) and adverse drug reactions were compared between the two groups.Results The clinical efficacy in treatment group was 95.74% (45/47 cases),had significant difference with that in control group,which was 82.98% (39/47 cases,P <0.05).The levels of CRP and IL-6 in treatment group were (2.17 ± 0.25) mg · L-1,(4.72 ± 0.51) ng · L-1,had significant difference with those in control group,which were(3.45 ±0.42) mg· L-1,(8.34±0.89) ng · L-1 (P <0.05).The main adverse drug reactions in treatment group were dry mouth,blurred vision and rashes,the incidence rate was 6.38% (3/47 cases).The main adverse drug reactions were dry mouth,rapid heart beat,difficulty urinating and rashes in control group,the incidence rate was 10.64% (5/47 cases,P > O.05).Conclusion Progesterone combined with nifedipine in the treatment of upper ureteral calculi was effective with high safety.
4.Clinicopathological characteristics and prognosis of multilocular cystic renal neoplasm of low malignant potential

Objective:To analyze the clinicopathological characteristics and prognosis of patients with multilocular cystic renal neoplasm of low malignant potential and compare the clinicopathological charac-teristics of patients with multilocular cystic renal neoplasm of low malignant potential who underwent different surgical methods.Methods:Clinicopathological data and prognosis of patients admitted to Peking University Third Hospital from January 2010 to September 2023 were collected.Patients who underwent radical nephrectomy or nephron-sparing surgery and were pathologically diagnosed with multilocular cystic renal neoplasm of low malignant potential were identified.Based on the surgical methods,the patients were divided into radical nephrectomy group and nephron-sparing surgery group.The clinicopathological characteristics of the two groups were compared.Results:A total of 35 patients were enrolled in this study.The median age at diagnosis was 53.0(39.0-62.0)years.Among the 35 patients,23 were males(65.7%)and 12 were females(34.3%).Nine patients underwent radical nephrectomy(25.7%),while 26 patients underwent nephron-sparing surgery(74.3%).The clinical T-stage of 35 patients did not exceed T2a stage.The median operation time was 145.0 min,and the median estimated intraoperative blood loss was 20.0 mL.The median postoperative hospitalization days was 6.0 d.The postoperative pathological results did not indicate renal sinus invasion,sarcomatous change,adrenal invasion or lymph node invasion.Based on the surgical methods,the patients were divided into a radical nephrectomy group and a nephron-sparing surgery group.There was no significant difference in clinicopathological charac-teristics between the two groups.Except for one patient who was lost to the follow-up,all the other patients were followed up for 8-111 months,with a median follow-up time of 70.5 months.Only one patient died from non-cancer-specific reasons,other patients had no tumor metastasis or recurrence.Conclusion:Patients with multilocular cystic renal neoplasm of low malignant potential have a good prog-nosis.There is no significant difference in clinicopathological characteristics of patients between nephron-sparing surgery group and radical nephrectomy group for multilocular cystic renal neoplasm of low malig-nant potential.
5.Clinical Value of Acute Myeloblastic Leukemia 1-ETO Fusion Transcripts in Children with Acute Myeloblastic Leukemia by Using Real-Time Quantitative Reverse Transcriptase Polymerase Chain Reaction

Objective To analyze the clinical value of minimal residual disease(MRD) of acute myeloblastic leukemia(AML) with AML 1-ETO by using the real-time quantitative reverse transcriptase polymerase chain reaction(RQ-RT-PCR).Methods From Jan.2001 to Jan.2007,the MRD of 32 AML1-ETO-positive AML patients were analyzed by using PQ-RT-PCR.The detection of the AML1-ETO was taken after the induced chemotherapy every 1.5-2.0 months during the consolidation therapy.The survival of different stages in children with AML was analyzed by SPSS 10.0 software and calculated by using Kaplan-Meier analysis.Results Thirty-two patients received the induced chemotherapy and 29 patients with complete remission morphologically,3 patients had no complete remission morphologically and then gave up.Patients with molecular remission were associated with a high probability of survival(P=0.001 8).Patients with high transcript levels at diagnosis had no difference in event free survival with patients with low transcript levels.The quality of molecular response after induction,6 months in the chemotherapy as well as consolidation period,has significant impact on the event free survival(P=0.023,0.000 1,0.004 9).Conclusion The current study demonstrate that quantitative evaluation of AML1-ETO transcript levels is important and may be helpful for therapeutic decisions in future.
6.Identification of a Bw14 subtype and exploration for its molecular basis.

OBJECTIVETo identify a rare subtype of the ABO blood group system and explore its molecular basis.
METHODSBased on a standard serological assay, ABO subtype and haplotype were analyzed through PCR amplification of the 7 exons and adjacent introns of the ABO gene and TA clone sequencing.
RESULTSForward typing showed a B type, while reverse typing demonstrated an extremely weak anti-B on routine gel analysis, which indicated a forward and reverse typing discrepancy. Absorption-elution testing confirmed that there was no A antigen on the surface of patient's red blood cells. Sequencing of the ABO gene showed a G>A exchange at position 523 in exon 7, which resulted in a Val to Met substitution at codon 175. Clone sequencing of the amplificons of the ABO gene showed an ABO* Bw14/O01 heterozygote genotype.
CONCLUSIONMolecular method is useful for the identification of ambiguous blood groups. A 523G>A substitution of the ABO gene resulting in a Bw14 subtype probably underlies the weak B phenotype noted in the patient.

7.Association analysis between attention-deficit hyperactivity disorder and Val158Met polymorphism of catechol-O-methyltransferase gene.

OBJECTIVETo investigate the association between attention-deficit hyperactivity disorder (ADHD) in Han Chinese children and Val158Met polymorphism of catechol-O-methyltransferase (COMT) gene caused by the missense mutation of G158A in exon 4.
METHODSBy using polymerase chain reaction-restriction fragment length polymorphisms the Val158Met polymorphism of COMT gene was tested in 117 children with the diagnosis of ADHD as defined by DSM-IV and in 105 healthy controls living in Shanghai.
RESULTSThe frequencies of A allele were 25.21% and 23.81% in the ADHD group and the health controls respectively, which showed no significant difference between the two groups (Chi2=0.5197, P>0.05). There was also no significant difference in the distribution of all genotypes of COMT gene between the ADHD patients and the controls (P>0.05).
CONCLUSIONIt was suggested that for the Han Chinese children with ADHD in this study, there was no association between ADHD and Val158Met polymorphism of COMT gene.

8.Roles of adenosine and cytokines in the prostate tissue of rats with acute bacterial prostatitis.

OBJECTIVETo investigate the possible roles of adenosine and the cytokines TNF-alpha and IL-10 in the pathogenesis of acute bacterial prostatitis (ABP) in rats.
METHODSForty-eight male Wistar rats were randomly divided into groups A (ABP), B (ABP + theophylline intervention), C (sham) and D (blank control). ABP models were established by injecting Escherichia coli 0157 into the prostate, and those in group B were treated by intraperitoneal injection of theophylline immediately after modeling. At 4 and 14 days, the prostate tissues of the rats were collected for detection of the expressions of TNF-alpha and IL-10 by immunohistochemistry and the concentration of adenosine by high-performance liquid chromatography.
RESULTSAt 4 and 14 days, the concentrations of adenosine were significantly higher in group A ([48.38 +/- 17.27] and [26.54 +/- 11.22] microg/g) than in C ([0.45 +/- 0.25] and [0.46 +/- 0.29] microg/g) and D ([0.41 +/- 0.23] and [0.43 +/- 0.27] microg/g) (P < 0.05), and so were the expressions of TNF-alpha in A (0.23 +/- 0.08 and 0.21 +/- 0.03) than in C (0.07 +/- 0.03 and 0.07 +/- 0.01) and D (0.07 +/- 0.06 and 0.07 +/- 0.06) (P < 0.05), and those of IL-10 in A (0.13 +/- 0.03 and 0.25 +/- 0.01) than in C (0.07 +/- 0.03 and 0.07 +/- 0.03) and D (0.07 +/- 0.01 and 0.07 +/- 0.02) (P < 0.05). Compared with group A, the rats in group B showed significant increases at 4 and 14 days in the severity of inflammation, concentration of adenosine ([86.64 +/- 32.87] and [51.17 +/- 22.96] microg/g, P < 0.05) and expression of TNF-alpha (0.37 +/- 0.08 and 0.32 +/- 0.06, P < 0.05), but exhibited no remarkable difference in the expression of IL-10 (0.12 +/- 0.06 and 0.15 +/- 0.06, P > 0.05).
CONCLUSIONAdenosine may affect the progression of inflammation by regulating the expressions of the cytokines TNF-alpha and IL-10 in ABP rats through the adenosine receptor signaling pathway.

9.Detection of influenza viruses/avian influenza viruses and identification of virulence using a microarray

Objective To establish the DNA microarray to detect influenza viruses and avian influenza viruses,and identify their vindence. Methods Hemagglutinin(HA) ,neuramidinase(NA) and nuclooprotein(NP) genes were chosen simultaneously as targets for designing a microarray used for detection of viruses and identification virulence. The nucleic acid were amplified by single primer amplication (SPA). And then its specificity,seusitivity and reproducibility were evaluated. Results The microarray was able to specially detect H1N1 ,H3N2,B influenza viruses and H5N1,H9N2 avian influenza viruses. Their limits were 8HAU,16HAU,32HAU,and 8HAU,8HAU respectively .The limit for virulence was 32HAU. When samples were analyzed by both RT-PCR and microarray in parallel,the results agreed in 83.9% (47/56).Conclusion The microarray can detect and distinguish five tested viruses,and especially identify virulence. It not only supplies an assistant tool for clinical diagnosis and control of infectious disease,but also is valuable for controlling and preventing outbreak of avian influerza epidemic.
10.Expression of CCAAT-enhancer-binding proteins in colon cancer and its significance

Objective To explore the expression and clinical significance of CCAAT-enhancer-binding proteins (C/EBPα) in colon cancer tissues.Methods The expression level of C/EBPoα in colon cancer tissues and corresponding non-tumor normal tissues of 41 colon cancer patients was detected by Western blot.The correlation between the expression of C/EBPoα and clinicopathological parameters of colon cancer and survival rate of prognosis were statistically analyzed.Results In 41 cases of normal tissue adjacent to carcinoma,low expression of C/EBPeα appeared only 2 cases,high expression 39 cases,low expression rate was only 4.88%;and in 41 cases of colon cancer,C/EBP low expression appeared 32 cases,high expression only 9 cases,low expression rate was 78.05%.Compared with normal adjacent tissues,the expression of C/EBP in colon cancer was down regulated (P<0.05).In 41 cases of colon cancer,independent of C/EBPα expression and colon cancer,cancer size,sex,age and histological classification of colon cancer clinical pathological parameters (P > 0.05),but tumor node metastasis (TNM) and colon cancer staging was closely related (P < 0.05).The median survival times in low expression and high expression groups were 31.5 and 54.2 months,with significant difference(P <0.05).Conclusion The low expression of C/EBPα in colon cancer has a close relationship with TMN staging and survival rate of prognosis,which has a great significance on early diagnosis and targeted therapy of colon cancer in the future.
