Objective To design and synthesize derivatives of wanpeinine A, the main steroidal alkaloid isolated from the plant Fritillaria shuchengensis, and further study on the structure-activity relationship of the steroidal alkaloid. Methods Acylation and alkylation were used to synthesize the derivatives and their structures were identified via NMR and MS.Results The acylation of wanpeinine A (1) produced 3β,6α-diacetylwanpeinine A (2), 3β,6α-dipropionylwanpeinine A (3), 3β,6α-dichloracetylwanpeinine A (4), 3β,6α-dibenzoylwanpeinine A (5), and 3β-methoxyacylwanpeinine A (6). The alkylation of wanpeinine A formed 3β,6α-dimethoxymethylwanpeinine A (7). Conclusion All compounds are new except for 3β,6α-diacetylwanpeinine A.

Objective To select the calibrator for the conventional measurement system of serum a-Amylase (Amy).Methods The Amy levels of forty frozen serum samples were detected by the IFCC reference method (reference system),the conventional system A which used the Bioassay routine reagent and Randox calibrator,and was calibrated by the Roche PNPG7 method,and the conventional system B which used the Bioassay routine reagent and Randox calibrator,and was calibrated by the Rondox liquid stable PNPG7 method,respectively,and the acceptability of the two conventional systems was evaluated.Results The regression equations of the measurement values between the IFCC reference method and the conventional systems A and B were Y =0.964X +0.376 and Y =1.095X + 3.131,respectively.Among them,X and Y represented the results of the IFCC reference method and the conventional system,respectively.Compared with the IFCC reference method,the results of the conventional system A was reliable.Condusion With the guidance of the IFCC reference method,the domestic biochemical reagents matched with the suitable calibrators may provide the acceptable results.

OBJECTIVE:To study the effects of Salvionolic acids dialyzate on anaphylatoxin,C-reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-?(TNF-?)in peripheral blood of patients with maintenance hemodialysis(MHD)and to discuss whether the Salvionolic acids dialyzate could improve the blood-membrane biocompatibility.METHODS:Thirty-six patients undergoing MHD for at least half a year were randomly divided into 3 groups:control group(conventional hemodialysis,n=12),low-dose group(dialysate with salvionolic acids 40 mg?L-1,n=12)and high-dose group(dialysate with Salvionolic acids 80 mg?L-1,n=12).Dialyzer of the polysulfone membranes was firstly used in all patients.Patients' blood sampling was scheduled before dialysis,30 minutes after dialysis and at the end of dialysis(5 minutes before withdrawal of dialyzer)for measurement of serum levels of complement(C3a,C5a),CRP,IL-6 and TNF-? using enzyme linked immunosorbent assay(ELISA).RESULTS:As compared with before dialysis,the levels of C3a and C5a in both control group and low-dose group were evidently increased at 30 minutes,while the difference in high-dose group was nonsignificant.No significant differences of IL-6 and TNF-? were found between pre-dialysis and post-dialysis in three groups.As compared with before dialysis,the concentration of CRP in control and low-dose groups obviously increased but decreased obviously in high-dose group after dialysis.CONCLUSION:The production of C3a,C5a and CRP in the process of hemodialysis could be inhibited by a certain dose of Salvionolic acids dialyzate,which can improve the biocompatibility of the dialysis-membrane to some degree,but can't improve the levels of IL-6 and TNF-? after single-time dialysis.

Objective To explore the clinical value of the liquid based cytology test in the sputum/pleural fluid cytology diagnosis.Methods Collect the liquid based cytology test over the past year the sputun/pleural fluid specimen test results with the traditional detection methods of detection results,to compare the differences in the positive rate and other indicators.Results Two by detection of sputum/pleural fluid specimens of 225/136 cases,76/82 cases positive results of the liquid based cytology test; traditional technology for the 41/42 cases; data,P ＜ 0.05,statistically significant differences.Conclusion The liquid based cytology test in the sputum/pleural fluid cytology diagnosis is more superior to traditional methods,under conditions permitting,should be widely applied.

Objective To understand the current cognition status of stroke rehabilitation nursing knowledge in young nurses of the department of neurology.Methods Self-designed questionnaires were used for investigating 273 young nurses between 20 and 30 years old of the department of neurology of 4 tertiary hospitals in Zhengzhou.Results The current status of stroke rehabilitation nursing knowledge of the nurses surveyed was generally at a low level.There were no significant differences among nurses with different education degrees,professional titles and whether their relatives suffered from stroke.However,there were significant differences among nurses with different professional lives and whether the department carried out rehabilitation care.Conclusions Nursing educators and managers need to find a new way to improve the level of stroke rehabilitation nursing knowledge of the young nurses.

ObjectiveTo observe the difference in analgesia effects of ropivacaine with fentanyl used intraarticularly after the single knee arthroscopy procedure.Methods40 patients performed knee arthroscopy under combined spinal-epidural anaesthesia( CSEA),were randomly divided into 4 groups (n =10),at the end of operation 10mlof different drug at the group F,R,FR and N(fentanyl 10μg,0.5％ ropivacaine,fentanyl 10μg +0.5％ ropivacaine,normal saline)were injected intra-articularly.The antalgic effects of four groups based on standard of VAS were observed at the 2,4,8,12 and 24h after operation.ResultsThe 2h postoperative VAS scores were lower in four groups,the differences in four groups were not significant.The 4,8,12,24h postoperative VAS scores of F,R and FR group were much lower than that of N group ( all P ＜ 0.05 ).Moreover,VAS scores of FR group were much lower than that of F and R group( all P ＜ 0.05 ).No other adverse effects were observed.ConclusionIntra-articular administration of ropivacaine with fentanyl could provide superior postoperative analgesia without side effects.It was an excellent regimen for analgesia after knee arthroscopy.

Objective To investigate the effects of probiotics on the occurrence of ovalbumin (OVA) induced food allergy and the regulatory effects on immune function of rat models. Methods Thirty female Brown-Norway rats aged 3 weeks were randomly divided into blank control group,food allergy group and probiotics intervention group(n=10).The levels of serum OVA-IgE and intestinal sIgA were measured by ELISA method.Splenic lymphocytes were isolated and cultured in vitro,and the Treg lymphocyte subgroups in the spleen were analyzed by flow eytometry.The levels of IFN-γ,IL-4 and IL-10 in the supernatant of cultured splenic lymphocytes were measured by ELISA method. Results The serum OVA-IgE level in food allergy group was significantly higher than that in blank control group(P<0.05),while that in probiotics intervention group was significantly lower than that in food allergy group(P<0.05).Compared with food allergy group and blank control group,the level of intestinal sIgA in probiotics intervention group was significantly higher(P<0.05).The percentage of CD4~+CD25~+T lymphoeytes in food allergy group was significantly lower than that in blank contml group(P0.05).The levels of IL-4 and IL-10 in food allergy group were significantly higher than those in blank control group(P<0.05).Probiotics intervention group had significantly lower levels of IL-4,IL-10 and IFN-γ than food allergy group(P<0.05).The ratio of IFN-γ/IL-4 in food allergy group was significantly lower than that in blank control group(P<0.05),while that in probiotics intervention group was significantly higher than that in food allergy group(P<0.05). Conclusion Probioties intervention could prevent the occurrence of food allergy in animal models by modulating the Th1/Th2 eytokine balance.

Objective To observe the effects of different compatibility ratios of Astragali Radix and Angelicae Sinensis Radix on vascular intimal hyperplasia; To ascertain the effective compatibility of Astragali Radix and Angelicae Sinensis Radix for antagonizing vascular intimal hyperplasia. Methods SD rats were divided into different groups by baseline geometric design method: Astragali Radix and Angelicae Sinensis Radix different compatibility ratios groups, Astragali Radix group, Angelicae Sinensis Radixgroup, positive medicinegroup and sham-operation group. A model of intimal hyperplasia of thoracoabdominal aorta was established by balloon catheter injury in vascular endothelium of rats. Then the thoracoabdominal aorta was taken out after gavage of Astragali Radix and Angelicae Sinensis Radix with different compatibility ratios for 14 days. Bloodletting was used to take thoracoabdominal aorta. Masson staining and Morphometric methods were used to analyze the intimal hyperplasia. Results IA, IT, HRIA and HRIT increased 14 day after intimal injury. Compared with the model group, IA, IT, HRIA and HRIT in Angelicae Sinensis Radix group, Astragali Radix and Angelicae Sinensis Radix 1:2 group, Astragali Radix and Angelicae Sinensis Radix 1:5 group, Astragali Radix and Angelicae Sinensis Radix 1:1 group and Astragali Radix and Angelicae Sinensis Radix 5:1 group were lower, and the effects of Astragali Radix and Angelicae Sinensis Radix 1:1 ratio were strongest. The effects on intimal hyperplasia in Astragali Radix group and Astragali Radix and Angelicae Sinensis 2:1 group had no significant differences compared with model group. Conclusion Astragali Radix and Angelicae Sinensis can inhibit vascular intimal hyperplasia in a certain compatibility ratios, and the effects of Astragali Radix and Angelicae Sinensis Radix 1:1 on intimal hyperplasia are the best.

Objective To compare the efficacy of MRI and DSA in assessment of carotid artery stenosis and atherosclerotic plaque.Methods Forty-six patients with carotid plaque detected by ultrasound were enrolled in this study,and 89 carotid arteries were evaluated by MRI and DSA.MRI examination was acquired with 3.0 T MR scanner and 8 channel phase-array surface coil.The MRI sequences consisted of pre-and post-contrast T1WI,T2WI,PDWI,TOF.Anterior-posterior and lateral views of carotid artery were performed on DSA.The degree of carotid artery stenosis was evaluated by the NASCET standard.Fibrous cap rupture,intraplaque hemorrhage,and calcification were also evaluated on MRI and DSA.Statistical comparison was performed with the Kappa value and paired Chi-square test.Results The degree of carotid artery stenosis was 50％ (16％-78％) on MRI and 47％ (7％-73％) on DSA.Two imaging modalities were in good consistency in evaluation of the degree of stenosis ( Kappa =0.882,P ＜ 0.01 ).There was statistical difference in detecting fibrous cap rupture by MRI and DSA (34 vessels vs 10 vessels,respectively,x2 =20.346,P ＜ 0.01 ).Furthermore,thirty-seven vessels with intraplaque hemorrhage and 71 vessels with calcification in the plaque were found on MRI but none on DSA.Conclusion MRI is a reliable tool in assessment of the degree of the carotid stenosis and it is superior to DSA in detecting fibrous cap rupture,intraplaque hemorrhage,and calcification.

Objective To investigate changes of the interhemispheric coordination integrity in patients with hemi-parkinsonism using resting-state functional magnetic resonance imaging(rs-fMRI) homotopy technique called voxel-mirrored homotopic connectivity(VMHC).Methods Sixteen Parkinson disease(PD) patients with right body side motor symptom onset(RPD),15 patients with left side onset(LPD),and 19 age-,gender-,and education-matched healthy controls(HC) were included in this study.rs-fMRI scanning and pre-processed the raw data were performed.Then by using analysis of variance(ANOVA) and two sample t tset,we performed VMHC analyses on rs-fMRI data of these participants(P＜0.05,corrected with AlphaSim,clusters≥16 voxels).Exploratory linear correlations analyses were performed between the VMHC of regions showing significant group differences and the clinical features of LPD or RPD patients.Results Compared with HCs,patients with LPD had significantly reduced VMHC in visual regions,sensorimotor regions,and the cerebellar cortex(voxels size of 17-77,t=-5.06--3.42,P＜0.05).Patients with RPD exhibited decreased VMHC in the prefrontal cortex and sensorimotor regions.Both LPD and RPD groups had increased VMHC in subcortical regions.When compared with the RPD group,the LPD group displayed decreased VMHC in the visual regions,sensorimotor regions(voxels=16 and 18; t=-3.68and-3.87,respectively,both P＜0.05) and increased VMHC in the supramarginal gyrus(voxels=4,t=4.72,P＜0.05);ROI-based correlation analyses indicated that the VMHC in the inferior occipital gyrus and the postcentral gyrus was negatively correlated with the BDI-Ⅱ scores in the LPD group(r=-0.58 and-0.59,respectively; both P＜0.05),and positively correlated with the MMSE scores(r=0.56 and 0.52,respectively;both P＜0.05).In the RPD group,a positive correlation was found for the VMHC in the supramarginal gyrus and the illness duration(r=0.56,P＜0.05) and for the VMHC in the postcentral gyrus and the(mini-mental state exam) MMSE(r=0.53,P＜0.05).Conclusions The differential pattern of deficits in the interhemispheric coordination integrity in hemi-parkinsonism reflected by VMHC may provide insights into the neurological pathophysiology underlying the asymmetry of symptom appearance in PD.
