1.The roles of Niacin in the treatment of diabetic retinopathy and its mechanism

Objective To investigate the protective effect of Niacin on blood-retina barrier (BRB) in diabetic rats and related mechanism.Methods The male Wistar rats (60) were divided into control (CON) group,diabetes (DM) group and Niacin-treated (NA) group,20 rats in each group.Rats diabetes models were induced with streptozotocin injection.Niacin (40 mg/kg · d) was administrated orally everyday in Niacin-treated group until sacrificed after 3 months.Pathological outcomes,total cholesterol (TC) and highdensity lipoprotein (HDL) were evaluated at month 3.Optical microscopy was used to observe the retinal structure.The integrity of BRB and the vascular permeability was quantified by analyzing albumin leakage using Evans blue (EB) method.The relative expressions of Claudin-5,Occludin,zonula occluden (ZO)-1 and GPR109A mRNA in rat retinas were detected by reverse transcription PCR (RT-PCR) and relative expression of GPR109A,tumor necrosis factor (TNF)-α and interleukin (IL)-6 by Western blot.Results Compared to CON group,the TC content was increased and HDL content was decreased in DM group (t=4.034,5.831;P<0.05).Compared to DM group,the TC content was decreased and HDL content was increased in NA group (t=6.868,3.369;P<0.05).The retinal structure of CON group was normal.Pathological changes were found in the DM group,such as tumescent nuclei and disorganized structures.The retinal structure of NA group was similar to the control group.Evans blue dye that the microvascular leakage in DM group was increased compared with CON group (t=24.712,P<0.05),while in NA group was decreased compared with DM group (t =16.414,P< 0.05).The mRNA expression of Occludin,Claudin-5,ZO-1 in DM group were decreased compared with CON group (t=11.422,12.638,12.060;P< 0.05),while in NA group were increased compared with DM group (t=5.278,3.952,8.030;P<0.05).The mRNA expression of GPR109A in NA group were increased compared with DM group (t=5.053,P< 0.05).The protein expression of GPR109A,IL-6,TNF-αin DM group were increased compared with CON group (t=4.915,11.106,6.582;P<0.05).Compared to DM group,the protein expression of GPR109A was increased (t=5.806,P<0.05),while the protein expression of IL-6 and TNF-α were decreased (t=10.131,5.017;P<0.05).Conclusion Niacin has the protective effect for BRB by up-regulating GPR109A expression which may suppress inflammation.
2.Relationship between vertebral artery incisures and the diseased regions evaluated using transcranial Doppler ultrasound

Objective To explore the relationship between the incisure changes in the intracranial vertebral artery (VA) segments on transcranial Doppler (TCD) and their diseased regions. Methods Incisure changes in VA intraeranial segments on TCD in 24 cases were found. Digital subtract angiography (DSA), computed tomography angiography (CTA)or coler Doppler flow imaging (CDFI)of carotid arteries were performed to confirm their diseased regions. Results (1)The group with end-systolic and pre-diatolic wide incisures was found in 9 cases: DSA or CTA showed 5 cases with severe stenosis, 2 cases with occlusion and 2 cases with congenital tenuity of the affected vertebral artery proximal part(VA-Pr). Within the above 8 cases, ipsilateral subclavian artery (SubA) was normal, 1 case was complicated with moderate stenosis of ipsilateral SubA,and 1 case was complicated with occlusion of contralateral SubA. Haemodynamics test showed (1)negative; (2)The group with systolic incisures was in 12 cases, and 12 patients were divided into two groups based on the haemodynamics test : ① positive group was in 6 cases. DSA showed that VA-Pr was normal but SubA had impaired with different degrees; ② negative group was in 6 cases. DSA showed VA-Pr was with occlusion in 4 cases and 2 cases was with congenital tenuity; The above 2 cases were complicated with stenosis of ipsilateral SubA and in 4 cases ipsilateral SubA were normal; (3) The group with small vibrated sharp waves on Doppler was in 3 cases, and DSA showed VA-Pr occlusion in 3 cases. In the 3 cases,one case was complicated with occlusion of ipsilateral SubA. Blood stealing was present in cervical muscular branches but not present between vertebral arteries . Ipsilateral SubA were normal in 2 cases. Haemodynamic tests showed negative. Conclusions VA incisures were not only present in the early subclavian steal syndrome (SSS) but also were found in the patients with impaired SubA . However, blood steal pathway is imperfect, and would be found in affected VA-Pr or congenital tenuity. Haemodynamic tests can help identify the above situation. The TCD screening method can be used in the routine diagnosis for SubA and VA from proximal to intracranial segments involvement, but there are limitations to some extent. It needs to combine with DSA and CDFI for diagnosis.
3.The effects of different types of subtotal gastrectomy on the secretion of insulin and gastrin

Blood samples were collected from 10 cases with Billroth Ⅰ gastrectomy(BI),15 cases with Billroth Ⅱ gstrectomy(BⅡ),and 15 cases with pyloroantral flap preserving gastrectomy(PAFPG)were examined to evaluate the changes of plasma insulin and serum gastrin.The blood samples of 8 normal subjects were examined likewise to serve as control.The blood samples were collected in intervals before and after the subject had taken hypertonic glucose solution.It was found that plasma insulin was significantly higher in BⅠ and BⅡ group than in PAFPG group,but it showed no significant difference between the PAFPG group and the control.Serum gastrin was signficantly higher in PAFPG group,BⅠ group,and the control than in BⅡ group.These findings suggest that the changes of insulin and gastrin secretion vary with the type of subtotal gastrectomy.
4.Suprapubic transvesical prostatectomy without partition of the prostatic cavity

Objective To evaluate suprapubic transvesical prostatectomy without partition of the prostatic cavity. Methods From Nov.1999 to Mar.2000,20 cases of BPH were treated by suprapubic transvesical prostatectomy without partition of the prostatic cavity and the therapeutic result was evaluated. 5.Advances in the Isolation Methods of Fungal Polyketide Synthase Genes

Fungal polyketide synthases are responsible for the biosynthesis of secondary metabolites such as pigments, mycotoxins, and they are very important in pharmacology , food science and agriculture. The recent advances in the methods for the isolation and manipulation of multiple classes of polyketide synthase genes from fungi were introduced. It is useful for discovery of novel fungal polyketide synthase gene clusters. These methods can also be useful for revealing the genetic potential of fungi to produce multiple types of bioactive polyketide.
7.Analysis of clinical use of shuxuening injection in treatment of cerebral infarction based on real world.

To know the characteristics of Shuxuening injection used on cerebral infarction patients in clinical practice, 6 053 cases of Hospital information system (HIS) data from 20 hospitals were analyzed. Using the basic description method and association rules to analysis the data. By analysis the data we found that the average age of cerebral infarction patients who used Shuxuening injection is 67.96, 83.94% of patients were aged 46-80. The injection is administered intravenously,with most patients receiving a dosage of 15-20 mL per dose for between 1 and 14 days. It is always combined with aspirin (48.508%), cinepazide maleate injection (22.073%), atorvastatin calcium tablets (18.873%) in clinical practice. When it comes to two drug combinations, it always combined with cinepazide maleate injection and aspirin (8.178%), nicergoline capsules and aspirin (7.63%). Therefore, based on existing data, we give the conclusion that for the treatment of cerebral infarction Shuxuening injection is mainly used for older patients, and is often combination with similar pharmacological effects chemical drugs, which is complied with the guidelines. However, the wrong dose is still exist, doctors should realize the hiding risk.

8.Multiple myeloma: past, present and future--editorial.

Multiple myeloma is a neoplasm of mature and immature plasma cells, it remains an incurable disease using conventional chemotherapy and increasing aggressive approaches. In recent years, due to the better understanding of myeloma biology, genetics and tumor formation, there are lots of new active drugs or combinational chemotherapy regimens having been developed, such as proteasome inhibitors, immunomodulatory agents etc, they are more effective than conventional chemotherapy. This article summarizes the recent advances with the new options for the treatment of multiple myeloma.

9.Adult stem cells and possible mechanisms of its differentiation--editorial.

Adult stem cells are the multi-potential cells, which exist in fetal and adult tissues. It can reproduce itself (undergo self-renewal) or give rise to more specialized (differentiated) cells. Under certain inducing conditions, adult stem cells can acquire the ability to differentiate into different tissue cells. Multipotent adult progenitor cells (MAPC), an alternative name of adult stem cell given by Catherine Verfaillie, existing in bone marrow, can differentiate into cells with characteristics of mesodermal, neuroectodermal, and endodermal lineages in vitro at the single-cell level. MAPC can also contribute to most cell types when injected into the blastocyst. Adult stem cell differentiation implies that different cell lineages are derived from a single initial cell; all differentiated cell types are functional in vitro and in vivo; and engraftment is robust and persistent in the physiological and pathological situations. The possible mechanisms may underlie the differentiation: various tissue-specific stem cells are present in different organs; adult stem cells would be reprogrammed when removed from their usual microenvironment and introduced into a different niche that imparts signals to activate a novel genetic program needed for the new cell fate. And true multi-potential stem cells persist in postnatal life. In the future, multi-potent adult stem cells might then be used for therapies of degenerative or genetic disorders of multiple different organs.

10.Citation analysis of published articles in the Chinese Journal of Dermatology from 2002 to 2007

Objective To learn the academic level and scientific impact of the Chinese Journal of Dermatology by using bihliometric methods. Methods Citation frequencies, first authors and their affiliation, citing journals of cited articles published in the Chinese Journal of Dermatology from 2002 to 2007 were statistically analyzed according to the database of Chinese Medical Citation Index (CMCI) by using bibliometric methods. Results From 2002 to 2007, a total of 2259 original papers were published in the Chinese Journal of Dermatology. Of them, 1224 were cited with a total citation frequency of 3963, citation rate of 54.1%, average citation frequency per paper of 3.2 and top citation frequency of 204. The number of first authors of cited papers amounted to 893, and 61 (6.63%) authors published 3 or more cited papers during this period. These authors were distributed in 30 provinces, municipalities, autonomous regions and 1 foreign country; Jiangsu, Beijing, Zhejiang, Guangdong, Shanghai and Shandong ranked in the top 6 regions with high total citation frequencies. There were 517 journals citing articles published in the Chinese Journal of Dermatology from 2002 to 2007;the top citing frequencies were observed in the Chinese Journal of Dermatology, China Journal of Leprosy and Skin Diseases, The Chinese Journal of Dermatovenereology, Journal of Clinical Dermatology and Southern China Journal of Dermato-Venereology, etc. Conclusions The articles in the Chinese Journal of Dermatology have a high academic quality and citation frequency. Chinese Journal of Dermatology is an important publication in the dermato- venereology field as well as a core periodical in medical field of China.
