AIM: To research the application analysis of trabeculectomy combined with phacoemulsification treating patients with primary angle-closed glaucoma ( PACG) and cataract.
METHODS: Seventy-three patients from September, 2011 to September, 2013, with primary angle- closed glaucoma and cataract, were selected in our research. Patients were all treated with trabeculectomy combined with ultrasonic emulsification, and the therapeutic effect was observed.
RESULTS: The effects of trabeculectomy combined with phacoemulsification treating primary angle-closed glaucoma and cataract in reducing intraocular pressure were better, and postoperative visual acuity results of the research objects were satisfactory. And it could deepen the central anterior chamber depth effectively. All the research objects had few complications.
CONCLUSION: Trabeculectomy combined with phacoemulsification treating primary angle - closed glaucoma and cataract can achieve a good result for reducing intraocular pressure, postoperative visual acuity recovery and correction, and the surgical success rate is high, with low complication rates. It is worthy of widely clinical application.

OBJECTIVE:
To establish the method to analyze γ-hydroxybutyric acid (GHB) and its precursors 1,4-butanediol (1,4-BD) and gamma-butyrolactone (GBL) in urine through LC-MS/MS and provide evidence for related cases.
METHODS:
GHB-d6 and MOR-d3 were used as the internal standard. The urine sample was separated by LC after protein precipitation with methanol. The electrospray ion source was for ionization. Each compound was detected through multiple-reaction monitoring (MRM) mode.
RESULTS:
The limits of detection of GHB and its precursors 1,4-BD and GBL were 0.1, 0.1 and 2 μg/mL. The accuracy was 87.6%-98.1%. The intra-day and inter-day precisions were less than 15% and matrix effects were higher than 80%.
CONCLUSION
The method is high sensitive, simple, rapid, specific and with high reliability. This study has provided technical support and basic data for forensic cases involving GHB.

Objective One typical case with stress cardiomyopathy was reported and the current knowledge of the syndrome was reviewed to improve relevant knowledge. Methods A 71-year-old female patient presented dyspnea and chest pain due to emotional stress.ECG,echocardiography, selective coronary artery angiography, left ventriculography,~(99)Tc~m-MIBI single photon emission computed tomography (SPECT),~(18)F-FDG SPECT and MRI were performed. Results Electrocardiogram at admission showed ST segment elevation and T wave inversion in leads V1—V4.Pathological Q wave occurred 1 week later,it disappeared 1 month later however and severe T wave inversion occurred.Normal or slightly elevated cardiac enzymes in the blood were found during the course.Left ventriculogram at admission showed left ventricular apical ballooning with LVEF of 30%.The ballooning volume was about 3/4 of left ventricular volume, without any corresponding coronary artery diseases found in coronary angiogram. The abnormal apical ballooning decreased significantly in the follow-up left ventriculogram performed one month later. The LVEF rose up to 63.6%.~(99)Tc~m-MIBI and ~(18)F-FDG SPECT showed mismatch of perfusion and metabolism in the corresponding region, indicating presence of viable myocardium.MRI showed left ventricular apical ballooning without perfusion defect and late enhancement, indicating viability of corresponding myocardium. Conclusions Emotional stress can cause transient left ventricular apical ballooning called"stress cardiomyopathy".Either ~(99)Tc~m-MIBI SPECT associated with ~(18)F-FDG SPECT or delayed enhancement MRI plays an important role in identification of myocardial viability, which can efficiently guide clinical treatment.

During the process of growth, harvesting, transportation, processing and storage, Chinese herbal medicines (CHMs) can be easily contaminated by fungi and their metabolites like mycotoxins, which not only express negative effects on the quality and safety of CHMs and their processed products, but also pose great threats to human health. Now, some chemical synthetic fungicides have been frequently used to control the growth of fungi and accumulation of mycotoxins in the preservation of CHMs. However, the concentration and type of chemical fungicides allowed for postharvest application are restricted due to the disadvantages of their high residual toxicity, long degradation period and pollution to the environment and so on. Therefore, it is critical to research and develop some highly effective, safe and non-toxic, natural, environment-friendly fungistatic agents from plants to prevent CHMs from being contaminated by fungi and mycotoxins. The paper reviews mycotoxins and their harmfulness, the effective compounds of fungistatic plants as well as the antifungal mechanism to provide scientific evidences for developing novel and effective fungistatic agents plants. Then, the application prospect of fungistatic agents from plants in the preservation of CHMs was discussed.

Objective To investigate the diagnosis and treatment of renal neoplasm with calcification .Methods Retrospectively summarized the clinical data of the 2 patients with calcific renal neoplasm admitted in our hospital from the May to July in 2014, then analyzed and discussed the clinical manifestations , diagnosis and treatment com-bined with the literatures .Results The two cases were both suspected of renal malignant tumor preoperatively .The case 1 was a 32-year-old male , laparoscopic partial resection of the left kidney was performed , and the postoperative pathology was clear cell carcinoma (Fuhrman levelⅠ).The case 2 was a 18-year-old male, partial resection of the right kidney was performed because of the tumor size , and the postoperative pathology was adult nephroblastoma . Conclusions The calcific renal neoplasm is a rare disease , the property determination depends on postoperative pa-thology, and as to the choice of surgical method , the patients'age, the tumor size and the tumor location should be taken into consideration , and intraoperative frozen should be performed when necessary .

Objective To investigate the effect of motor imagery therapy based on mirror neuron theory on the motor function and gait in stroke survivors. Methods Forty-nine stroke survivors were randomly divided into a treatment group (n =24) and a control group (n =25) using a random number table. All the patients in both groups were given similar conventional rehabilitation treatment. In addition, the patients in the treatment group were given motor imagery training based on mirror neuron theory once a day for 20 min each time,5 days a week, lasting 8 weeks. The Fugl-Meyer assessment (FMA),Functional Ambulation Categories (FACs),average step length, the percentage of time spent on the intact foot and the paralyzed foot, and 6 minute walking distance were used to evaluate the subjects' motor function and gait before and after 8 weeks of treatment. Results Before the intervention there was no significant difference between the two groups in terms of any of the measurements. At the end of the 8 weeks of treatment all measurements in both groups had significantly improved compared with before training. The averages of all the measures were significantly better in the treatment group than in the control group. Conclusions Motor imagery training based on mirror neuron theory can improve the motor function and gait of stroke survivors.

A total of 104 patients with polycystic ovarian changes on ultrasound were divided into polycystic ovary syndrome (PCOS) and polycystic ovaries (PCO) groups according to their clinical symptoms and endocrine examinations. And 40 women with normal ovulation served as the controls.Ovarian volume (OV),ovarian stroma area (SA),ovarian total area (TA) and SA/TA ratio were measured by transvaginal uhrasonography.Their endocrine hormone levels were determined.Patients with PCOS showed significantly higher OV,SA,TA and SA/TA than those of PCO and control groups.SA/TA was positively correlated with testosterone. The median serum levels of anti-Mullerian hormone (AMH) and inhibit B in the PCO group were intermediate between those in control and PCOS groups.AMH and inhibin B of PCOS were higher than the PCO and control groups. It indicated that the levels of AMH and inhibit B were associated with the degree of polycystic ovaries and they could be used to differentiate PCOS and PCO.SA/TA is associated with the elevated serum testosterone of PCO.And SA/TA is an important ultrasonic parameter for diagnosing PCOS.
