Objective To observe the clinical features and outcomes of vitrectomy for diabetic retinopathy (DR) with central retinal vein occlusion (CRVO) in type 2 diabetes mellitus (T2DM).Methods A total of 192 patients (241 eyes) with proliferative DR (PDR) who underwent vitrectomy were enrolled in this study.All the patients were diagnosed as vitreous hemorrhage (VH) because of suddenly decreased vision.There were 93 eyes with tractional retinal detachment (TRD) and six eyes with neovascularization of iris (NVI).The patients were divided into PDR with CRVO group (group A,41 eyes) and PDR group (group B,200 eyes) according to the results of fundus examination.All patients received vitrectomy with silicone oil and C3F8 gas tamponade.There were 138 eyes with silicone oil tamponade which including 30 eyes in group A and 108 eyes in group B.The difference of number in silicone oil-filled eyes in two groups was statistically significant (x2=5.110,P＜0.05).There were 38 eyes with C3F8 gas tamponade which including six eyes in group A and 32 eyes in group B.There was no difference in C3F8 gas-filled eyes numbers in two groups (x2 =0.048,P＞0.05).The follow-up ranged from one to 60 months,with the mean of (28.69± 17.28) months.The corrected vision,retinal reattachment,persisting macular edema (ME),neovascular glaucoma (NVG) and repeated VH after surgery were comparatively analyzed.Results Of 241 eyes,there were 41 eyes (17.0％) with CRVO.Before surgery,the differences of corrected vision (Z=-0.138),intraocular pressure (t=0.966),whether there was TRDor not (x2=0.412),whether underwent panretinal photocoagulation or not (x2 =1.416) were not statistically significant (P＞0.05),but the difference of whether NVI were present or not was statistically significant (x2=31.724,P＜0.05)between two groups.After surgery,the corrected vision improved in both two groups (Z=2.319,4.589;P＜0.05).There was no difference of corrected vision after surgery between two groups (Z=0.782,P＞0.05).Postoperative complications occurred in 94 eyes,including 26 eyes in group A and 68 eyes in group B.The differences of incidence of reoperation (x2 =0.498),retinal reattachment (x2 =0.818),persisting ME (x2 =2.722) between two groups after surgery were not statistically significant (P ＞ 0.05).The incidence of repeated VH (x2 =5.737) and NVG (x2 =6.604) in group A were higher than those in group B (P＜ 0.05).Conclusions CRVO is commonly found to coexist with DR in T2DM patients with VH.Combined with CRVO patients are more likely to suffer NVI.Vitrectomy can improve the visual function in PDR with CRVO patients.

AIM:To observed the variation regularity of corneal endothelial cells in patients with different diabetes duration after phacoemulsification, and investigate the effects of diabetes and its disease duration on corneal endothelial cells.
METHODS: Ninety-seven ( 135 eyes ) cataract patients with diabetes were selected randomly and divided into GroupⅠ( which diabetes duration ≥10a) and GroupII(which diabetes duration <10a) according to their disease duration. Additionally 62 (89 eyes) age-related cataract patients were randomly selected as the control group. The corneal endothelial cell density ( CD ) , proportion of hexagonal cell and coefficient of variation ( CV ) in the three group patients were measured respectively before phacoemulsification and after surgery. And the measurement results were statistically analyzed.
RESULTS:The corneal endothelial CD and proportion of hexagonal cell in the three group were decreased after surgery compared with preoperative. But the CV of corneal endothelial cells was increased on the 1 st wk and in 1st mo after surgery compared with the preoperative. The difference was statistically significant (P<0. 05). The corneal endothelial CD and proportion of hexagonal cell in the two diabetic groups were lower than the control group after surgery. However, the CV of corneal endothelial cells was higher than the control group. The difference was statistically significant ( P < 0.05 ) . There was no significant difference in the corneal endothelial CD, proportion of hexagonal cell and CV between the two diabetic groups before phacoemulsification (P>0. 05). The proportion of hexagonal cell in Group Ⅰ was lower than which in GroupIIafter surgery. While the CV was higher than which in Group II. The difference was statistically significant (P<0. 05).
CONCLUSION: Phacoemulsification has some damage on the corneal endothelial. Since the impact of diabetes on the morphology and function of corneal endothelial cell was related to the diabetic duration. So phacoemulsification has more obvious damage on the corneal endothelial in diabetic patients. And the diabetic duration was longer, the damage on the corneal endothelial in phacoemulsification was more easily.

Objective To analyze the molecular characterization of PrM/C and E genome of Japanese encephalitis virus(JEV) strain,CQ11-66,a newly strain isolated from patients with epidemic encephalitis B Chongqing Municipal.Methods The samples were collected from Children's Hospital of Chongqing Medical University,and inoculating BHK-21 cells were used to detect and isolate the Japanese encephalitis virus(JEV) strain,computer analysis of the phylogenetic,nucleic acid data and deduced amino acid sequence was accomplished using the Clustal X(1.8) and MEGA5 programs.Results Only one JEV strain was isolated from patient's cerebrospinal fluid specimen,named CQ11-66.Comparison of the PrM/C genome sequence of strain CQ11-66 with other 31 JEV isolates showed a 74.8％-97.4％ nucleotide sequence homology among them,which resulted in 85.6％-98.7％ amino acid sequence homology; Meanwhile,comparison of the E genome sequence of strain CQ11-66 with other 35 JEV isolates showed a 81.6％-99.6％ nucleotide sequence homology among them,which resulted in 94.8％-99.6％ amino acid sequence homology.There were high homology between CQ11-66 and JEV isolates from Fujian province on nucleotide sequence and amino acid sequence.Phylogenetic analysis of PrM/C and E genome showed that the CQ11-66 belonged to genotype Ⅲ.Conclusion Only one JEV strain was isolated from patient's cerebrospinal fluid specimen.There were some differences between CQ11-66 strain and other JEV isolates,and CQ11-66 strain belonged to genotype Ⅲ.

One hundred and sixteen patients with lower limb fracture were screened by Doppler ultrasound for deep vein thrombosis(DVT) of bilateral lower extremities within the first 72 h, d7 and d21 after fracture. Results showed that DVT was detected in 31 (26. 7% ) out of 116 cases within 72 h; at d7 and d21 DVT was detected in 3 and 1 patient respectively with a cumulated DVT rate of 30. 2% in 3 weeks.Serial Doppler ultrasonography is of value in screening for DVT of the lower extremities in patients with lower limb fracture at early stage.

Objective:To explore the effects of infant sleep patterns on maternal sleep quality.Methods:Three hundred and nineteen pairs of mothers and infants from two grade three class-a hospitals in Nantong City were enrolled. Infant sleep patterns were assessed by the Brief Infant Sleep Questionnaire (BISQ). Maternal sleep quality was evaluated using the Pittsburgh sleep quality index (PSQI). The effect of infant sleep patterns on maternal sleep quality was analyzed by one-way ANOVA followed with multivariate analysis.Results:The maternal sleep quality scored 7.08±2.60. Single factor analysis pointed out that the sleep quality of women whose infants slept in bed near parents, woke up more than 3 times a night, was nocturnal wakefulness more than 60 minutes or nocturnal sleep duration less than 8 hours was significantly worse. After controlling for confounding variables, the way of baby falling asleep, numbers of night wakings, nocturnal sleep duration, maternal age, infant′s gender and primary caregiver were independent factors of maternal sleep quality.Conclusion:Infant sleep pattern is an important factor affecting maternal sleep quality. Nurses should provide information about the sleep patterns of infants to mothers and their family members to promote their babies′ sleep habits so as to improve the maternal sleep quality.

?AIM: To observed the clinical effect of twice fluid-gas exchange in vitrectomy for retinal detachment at the equator.?METHODS: The retrospective analysis of the 74 cases (74 eyes) with retinal detachment at the equator from January 2014 to September 2015 were reviewed. All these patients were performed standard three channel 23G vitrectomy under a wide angle lens, in which, the 37 cases ( 37 eyes ) were randomly selected and performed single fluid-gas exchange, and the other 37 cases ( 37 eyes) were performed twice fluid-gas exchange. The intraoperative surgical complications and the postoperative success rate of retinal reposition in 1wk, 1, 3mo after surgery of the two group patients were observed.? RESULTS: The intraoperative surgical complication rates of the twice fluid-gas exchange group were less than that of the single fluid-gas exchange group. The postoperative success rate of retinal reposition in the twice fluid-gas exchange group significantly increased in 1wk and 1mo after surgery, and the difference was statistically significant (P<0. 05). But the postoperative success rate of retinal reposition in 3mo after surgery had no significant difference (P>0. 05).?CONCLUSION:The twice fluid-gas exchange operation was simple and it was beneficial for beginners to master. The incomplete drainage of the single fluid-gas exchange and the pore drainage in the posterior pole or in the peripheral part of the retina were avoided during the surgery. The intraoperative surgical complication rates were reduced and the postoperative success rate of retinal reposition were improved. This operation method had great application value in clinic.

To describe a modified simple iris suture for pupillary dilation technique during vitrectomy in cases with a miotic pupil.Four translimbal incisions were created with a sharp straight blade at 1:30,10:30,4:30,and 7:30 o'clock,respectively.The straight needle of 10-0 polypropylene suture and a Sinskey IOL hook was used to displace the pupillary margin toward the limbus.In 3 cases,four sutures caused a 6-mm to 9-mm square-shaped pupil,and the pupil was allowed to return to a smaller size at the end of the operation.It is simple and may reduce postoperative complications.
