Objective To investigate the neonatal death causes and the relationship between the causes and the neonatal age,fetus age and the year in order to reduce neonatal mortality.Methods Data of neonatal autopsy and pathological records from 107 neonates in our hospital from January 1998 to May 2008 were collected,analyzed and summarized.Results 40.2%of the cases died within 24 hours and 72.9%died within 7 days after birth.The main death cause for those who died within 7 days was asphyxia(48.6%),and the main death causes ior those who died after 7 days were deformity(34.5%)and hemorrhage(34.5%).The most common death cause for premature bahies Was hyaline membrane disease(29.7%),and the most common one for term infants was congenital heart disease(23.3%).In the past 5 years.asphyxia Was still the most common death causein our country,those who died of infection were on the rise and the deformed babies increased too;on the contrary.those who died of obstetric causes and hemorrhage decreased.Conclusion Neonatal mortality could be reduced by prenatal management,prevention of premature delivery,improvement of the cooperation between pediatrics and obstetrics,and better management of NICU.

Objective To investigate the expression of matrix metalloproteinase 7(MMP7)in paraquat(PQ)?induced pulmonary fibrosis in rats. Methods Forty?eight SD rats were randomly divided into the control group and the pulmonary fibrosis model group(PQ model group),each group of twenty?four rats. Rats in the PQ model group received single intraperitoneal injection of 4 mg/mL PQ dilute solution and the control group were in?traperitoneal injected with the same dose of saline. Eight rats of each group were sacrificed on day 7,day14 and day 28 respectively. The pathological changes of lung tissues were observed and the hydroxyproline(HYP)content in lung tissues was determined. The severity of pulmonary fibrosis was observed. The expressions of MMP7 in lungs were observed by immunohistochemistry. Results The observation of general state of the experimental animals showed that except one rat died at day 28 d,all other rats survived to the end point of observation. After intraperitoneal injection with PQ,the weight of rats in the PQ model group gradually declined,and then increased around day 14,yet still much lower than that in the control group at day 28(P<0.05). After intraperitoneal injection with PQ,the pulmonary index in the model group increased gradually and then decreased after reach?ing the peak on day 14. The content of HYP in rat lung tissues in the PQ model group was remarkably higher than in the control group at day 7,day 14,and day 28,with statistical significance(P<0.01). In the PQ model group,the content of HYP was significantly up?regulated with the extension of infected time and reached the peak value at day 28. The results of HE staining showed significant pulmonary alveolitis at day 7,hyperplasia of abundant collagen fibers in alveolar septum at day 14,and obvious pulmonary fibrosis and collapse of alveolar structure on day 28 in the lung tissues of the PQ model group. A weak expression of MMP7 was measured in the lung tissues in the control group and the expression of MMP7 was higher in the PQ model group than in the control group at day 7,day 14,and day 28,with statistical significance(P<0.05). Conclusion Paraquat poison?ing was mainly manifested in inflammatory reactions of lung tissues in the early stage together with increase of fibroblasts and mainly in fibrosis in the late stage. The expression of MMP7was increased along with the severity of pulmonary alveolitis or fibrosis and showed significant changes compared to the control group at day 28 after poisoning,indicating that MMP7may be the marker of paraquat?induced pulmonary fibrosis.

Abstact:Objective To explore the effect of different arrangement of hollow screws in treatment of femoral neck fracture.Methods The clinical data of 160 cases with femoral neck fracture from March 2010 to May 2014 in our hospital were collected.They were randomly di-vided into triangle group and inverted triangle group according to arrangement of hollow screws.The operation time,intraoperative blood loss, fluoroscopy frequency in operation,length of stay,time of fracture healing,postoperative complications,Harris score and functional recovery of two groups were recorded and compared.Results The operation time and intraoperative blood loss in inverted triangle group were lower than those in triangle group,the difference was significant(P <0.05).The shortening rate of inverted triangle group and triangle group were 10%and 26.3% respectively.The shortening rate of inverted triangle group was lower than those of triangle group,the difference was significant (P <0.05).For the fluoroscopy frequency in operation,average hospitalization time,fracture healing time,rate of nonunion and femoral head necrosis,Harris score and functional recovery,there was no statistical significance between two groups(P >0.05).Conclusion Inverted tri-angle hollow screws treating femoral neck fracture makes surgery operation easier,operative time shorter and blood loss smaller.

Objective To investigate the therapeutic and preventive method of foreignbodies in esophagus.Methods Clinical date of 78 cases of foreignbodies in esophagus were analyzed retrospectively.Results Among the 78 foreignbodies in esophagus,76 cases were removed from esophagus in different way.2 cases fulled into mastach.No severe complication occurred.The cure rate was 97％.Conclusions Foreignbodies in esophagus should be removed as soon as it is diagnosed.It is important that correctly treating foreignbodies in esophagus can reduce the incidence of severe complications.

Trans-cinnamaldehyde, the main component of volatile oil from cassia twig or Cinnamomum cassia, which is a traditional Chinese herbal medicine. Trans-cinnamaldehyde is a kind olefine aldehyde of organic compounds and has many pharmacological properties, such as anti-inflammatory, anti-tumor, anti-bacterial, antidiabetic, anti-obesity, and neuroprotection etc. The compound has preventive and therapeutic effects on the nervous system, cardiovascular, cancer, diabetes and other diseases. Trans-cinnamaldehyde, as a preventive care of nature medicine, has great clinical and market potential. This paper gives a review about the pharmacological effects and mechanism of trans-cinnamaldehyde researched in the latest five years. We hope to provide some basic information for further research on trans-cinnamaldehyde.

Objective To compare the effects of dezocine and fentanyl on analgesia and cognitive state for elderly patients after neurosurgery.Methods A total of 42 elderly patients with brain tumors and American Society of Anesthesiologists (ASA) Ⅱ-Ⅲ were randomly divided into two groups:Dezocine group and fentanyl group (n=21,each).The time of recovery and extubation,pain intensity descriptive scale (PIDS),Ramsay score and mini-mental state examination (MMSE) before operation,and 1 h,2 h and 3 h after extubation were recorded.Results The time of recovery [(15.9±2.8) min vs.(16.2 ± 4.3)] min and extubation [(22.4 ± 5.1) min vs.(23.8 3.7) min] between the two groups had no differences (both P＞0.05).And there were no statistical differences between the two groups in Ramsay and MMSE score at 1 h,2 h,3 h after extubation and on PIDS at 1 h after extubation (all P＞ 0.05).The PIDS were lower in Dezocine group than in fentanyl group at 2 h[(2.01±0.79)scores vs.(2.55±0.51)scores,(t =-2.24,P=0.04))] and 3 h after extubation [(2.30±0.66)scores vs.(2.75±0.64)scores,(t =-2.44,P=0.03)].Conclusions The using of dezocine and fentanyl before the end of operation could have a good sedative and analgesia effect on neurosurgery without influence on the duration of recovery and extubation,and dezocine may maintain a longer time of analgesia than fentanyl in elderly patients.

Objective To compare the effects of propofol versus midazolam on serum hydrocortisone in the aged traumatic brain injury. Methods 64 patients (aged 65-72 years) with traumatic brain injury during mechanical ventilation were divided into propofol group (n=32) and midazolam group(n=32).The hemodynamics data,intracranial pressure(ICP) and the calculated cerebral perfusion pressure(CPP) were recorded.The serum hydrocortisone of patients was detectedby enzyme linked immunosorbent assay (ELISA) at 24 h,72 h and 4 weeks in intensive care unit(ICU). Results There was no differences in partial pressure of carbon dioxide,heart rate,meanartery pressure,pulse biood oxygen saturation (SpO2 ),armpit temperature,endexpiratory gas(PEr CO2),intraeranial pressure (ICP) and cerebral pertusion pressure (CPP) between the two groups (P＞0.05).The serum hydrocortisone in propofol group at 24 h and 72 h [(269.7±43.2) nmol/L and (235.0±67.0) nmol/L] were lower than in midazolam group [(278.0±75.5) nmol/L and (243.2±42.4) nmol/L] (t=5.312 and 5.919,both P=0.000).The serum hydrocortisone in propofol and midazolam group at 4 weeks were (209.1±73.9) nmol/L and (210.8±66.6) nmol/L,respectively,no significant difference was found(t=0.075,P =0.938). Conclusions There is a significant difference in the effects of propofol and midazolam on the early level of hydrocortisone in the aged patients with traumatic brain injury.

Objective To evaluate the efficacy of itopride hydrochloride combined with alpha-lipoic acid in treatment of diabetic gastroparesis.Methods One hundred and ten patients with diabetic gastroparesis were randomly assigned to two groups.Sixty patients received α-lipoic acid plus itopride (α-lipoic acid 0.6 g intravenously for 14 d plus itopride 50 mg orally t.i.d), and 50 cases received oral itopride only.The clinical symptoms including postprandial abdominal fullness, belching, acid reflux, anorexia, nausea and vomiting were observed;the gastric emptying rate was tested, and plasma motilin and serum ganstrin levels were measured groups, before and after two weeks of treatment.Results The clinical symptom scores of combination therapy group (4.62 ± 3.46 vs.12.26-± 3.43) and control groups (5.43-± 3.65 vs.12.31-± 3.37) were improved after treatment.The overall effective rate of combination group was 91.6％ and that of itopride therapy group was 78.0％ (x2 =4.098, P ＜ 0.05).Gastric emptying rate of combination group [(76.9 ± 9.7) ％ vs.(37.3 ± 10.2) ％] was increased more markedly than that ofitopridegroup[(57.2-± 10.4)％ vs.(36.8-± 11.2)％] (t =10.221, P ＜0.05).Plasma motilin and serum ganstrin levels in combination group [(640.2 ± 64.4) ng/L vs.(554.0 ± 67.6) ng/L, (152.6 ± 20.3)ng/L vs.(96.8 ± 22.2)ng/L] were reduced more markedly than those in itopride group [(636.3 ±66.4)ng/L vs.(589.4 48.3)ng/L, (153.5-± 19.4) ng/L vs.(120.9 ± 19.7)ng/L] respectively (t =3.100, 5.959, all P ＜ 0.05).Conclusion For patients with diabetic gastroparesis, α-lipoic acid and itopride combination therapy can significantly improve symptoms and gastric emptying rate, decrease motlin and gastrin levels;and the efficacy of combination therapy is better than that of itopride monotherapy.

Traditional Chinese medicine (TCM) is an important part of modern medicine. Application of modern ana-lytical techniques to identify the authenticity of TCM and evaluate its quality is an important and critical content in the process of the modernization and internationalization of TCM. This paper described the development and charac-teristics of a 1H-NMR-based metabolomics technology, and reviewed its application in the species identification, quality evaluation of Daodi Y aocai (i.e., genuine medicinal materials), processing theory and the best harvest time of TCM. Besides, based on previous work, further discussion was given on technical methods and key question of the 1H-NMR metabolomics method. This paper provided a methodological reference for the species identification and quality evaluation of TCM and other herbal medicines.
