2.Development of SCI-covered papers list database in Dalian Medical University

After the contents and characteristics of SCI-covered papers list database in Dalian Medical University were described, it was pointed out that databases in academic library should provide more contents and links for the users to browse information and to use related resources by making full use of network technology. Technology and manpower should be well prepared and librarians should be qualified with patience and painstaking if an excellent SCI-covered papers list database is to be developed.
4.The application analysis of tracheal resection combined with primary end-to-end anastomosis in patients with benign cervical tracheal stricture

Objective To introduce the outcomes of tracheal resection combined with primary end-to-end anastomosis for benign cervical tracheal stenosis,and to discuss the strategy for prevention of surgical complications.Methods The clinical data of 22 patients due to different causes benign cervical tracheal stenosis surgery were analyzed retrospectively.Results The length of cervical tracheal stenosis ranged was 2.2-4.2 cm.Grade Ⅱ stenosis was present in 6 patients.Grade Ⅲ stenosis was present in 11 patients and grade Ⅳ stenosis in 5 patients.Successful extubation was achieved in all 22 cases.After surgery,temporary hoarseness occurred to 1 case;unilateral pulmonary atelectasis with pleural effusion occurred to 1 case; subcutaneous emphysema with infection occurred to 1 case; mild dysphagia occurred to 3 cases;a slight deepening of the tone of voice in 10 patients with women occurred to 5 cases,granulation tissue growth near the suture occurred to 3 cases,and suture dehiscence did not occur in any patient.The follow-up period ranged from 6-45 months,no patient developed restenosis.Conclusions It presents a high success rate and good functional result of tracheal resection combined with primary end-to-end anastomosis.Therefore,it is an effective and reliable approach for the management of benign cervical tracheal stenosis.To avoid complications,the preoperative assessment,patients selection and postoperative management should be emphasized.
5.Carbapenemase-producing Acinetobacter calcoaceticus Infection after Liver Transplantation

OBJECTIVE To study the carbapenemase genotypes among Acinetobacter calcoaceticus isolates infected after liver transplantation. METHODS The 5.4 software was used to analyze the resistance of Acinetobacter species. Eight strains of imipenem-resistant A. calcoaceticus which were resistant to multiple drugs were collected. Analytical isoelectric focusing electrophoresis was used to measure the pI of the ?-lactamase. The homology of the isolates was determined by pulsed field gel electrophoresis(PFGE). blaTEM, blaSHV, blaIMP, blaVIM, blaOXA Genes for resistant isolates were amplified and sequenced. RESULTS Imipenem-resistant A. calcoaceticus infected after liver transplantation was resistant to multiple drugs. All of 8 strains were produced OXA-23 carbapenemases (pI of 6.7). The results of PFGE indicated there was a trend of resistant clone transmisstion in the transplantation ward. CONCLUSIONS There is a transmisstion of A. calcoaceticus producing OXA-23 carbapenemase in the transplantation ward of our hospital. It is important to monitor powerfully the bacterial resistance to control infection more effectively.
6.The Regular ,Variant and Parasitic Arterial Blood Supply in Liver Cancer and its Effect on Transhepatic Artery Chemoembolization (TACE) Treatment

Objective To investigate the regular,variant and parasitic arterial blood supply in liver cancer,so as to improve the effects of treatment on liver cancer by TACE.Methods 52 liver cancer focus confirmed by pathology and imaging were retrospectively analysed First,the routine DSA was performed,if following conditions including no tumor staining or no vessel area within tumor on arterial phase and filling defect within tumor on parenchyma phase were presented,then DSA via superior mesenteric artery,left gastric artery,inferior phrenic artery,right adrenal artery,internal thoracic artery and intercostal artery were perfomed again.Results 39 focus with regular arterial blood supply(75%),13 focus with variant (25%) and 11 focus with parasitic arterial blood supply (21%) are observed .Conclusion The liver cancer focuses are mainly fed by the regular arterial blood supply of liver artery ,but still 25% are by variant arterial blood supply.The chemoembolization for liver cancer through regular ,variant and parasitic arterial blood supply can improve the therapeutic effect.
7.Arterial embolization in the therapy of prostatic hyperplasia(a report of 12 cases)

Objective To evaluate the efficacy of arterial embolization in the therapy of benign prostatic hyperplasia(BHP).Methods After super-selective arterial contrast medium administration and comfirmation of prostate blood-supply artery,polyvinyl alcohol(PVA)and gelfoam were injected via the arterial catheter in 12 patients diagnosed as BHP.Pre-and post-operative I-PSS,quality of life(QOL),maximum urine flow rate(Qmax),and residual urine(RU)were compared.At the same time,the changes of prostate volume and blood supply were evaluated by endorectal ultrasound and CT.The change of urethral diameter was also evaluated by urethral contrast medium.Results A total of 21 prostate blood-supply arteries were embolized,including 5 internal iliac arteries,9 inferior vesical arteries,5 internal pudendal arteries,and 2 obturator arteries.Bilateral arterial embolization was performed in 9 patients and unilateral embolization was performed in 3 patients.The post-operative I-PSS(4.8),QOL(1.3),Qmax(18.9 ml/s),and RU(3 ml)were obviously improved compared with those of pre-operation(24.2,4.8,9.6 ml/s,and 138 ml).The prostate blood supply was obviously decreased and the maximum speed of blood stream was slackened from(19.3?9.8)cm/s to(10.0?8.9)cm/s.The prostate volume decreased from pre-operative 127.6 ml to post-operative 65.1 ml.The reduction rate was 51%.Conclusion The arterial embolization in the therapy of BHP was a new method.It had some advantages,such as less injury,higher efficacy,better safety and fewer complications.
8.Study on clinical characteristics and therapeutic outcomes of 173 patients with Takayasu arteritis

ObjectiveTo study the clinical features, arterial involvement, therapeutic strategies and outcomes of Takayasu arteritis (TA). MethodsThe clinical symptoms, arterial images, inflammatory parameters and follow-up information of 173 patients with TA were retrospectively studied. Comparisons between groups were performed by t-test. ResultsThere were 136 female and 37 male patients in this study. The mean age at onset was(26±11 ) years. Hypertension, pulse deficit or asymmetrical pulse, and fever were present in 46.6%, 41.1%, 28.7% of patients, respectively. The distribution of arterial involvement were 64.7%in aorta, 9.8% in pulmonary artery, 19.1% in innominate artery, 65.9% in common carotid arteries, 65.3%in the subclavian artery, 36.2% in the renal artery, 12.1% in the vertebral artery, and 5.8% in coeliac axis.Elevated erythrocyte sedimentation rate(ESR) was found in 61.0% patients. Active tuberculosis or history of tuberculosis was implicated in 45 patients(26.0%). Ten patients(5.8%) were hepatitis B virus carriers.Among 105 followed-up patients, 98 patients(94.2%) achieved persistent remission, 17 patients relapsed when corticosteroids were tapered. ConclusionCorticosteroids combined with or steroid alone, supplemented with endovascular intervention procedures or surgical bypass procedures when necessary, can effectively control the clinical symptoms and inflammatory parameters and improve the quality of life of patients.
9.Twenty-nine cases of primary Sjogren's syndrome complicated with malignancies

Objective To investigate the clinical features and risk factors of primary Sjogren's syndrome (pSS) patients with malignancies.Methods The clinical features of 29 pSS patients with malignancies from January 1985 to March 2008 were retrospectively analyzed.Results The ratio of pSS with malignancies to all hospitalized pSS patients from January 1985 to March 2008 was 2.2%.In these patients,8 were lymphoma,4 were breast cancers,2 were myeloma,2 were thymoma,2 were lung cancer,2 were gastric carcinoma,2 were hepatoma,2 were colon carcinoma and 2 were tongue cancer.Uterine cervix cancer,renal carcinoma,thyroid carcinoma and carcinoma of parotid gland each were fund in this series of patients.Conclusion The most common malignancy in pSS is lymphoma and the major of pathological type is B cell non-Hodgkin's lymphoma.
10.Short-segment transpedicular fixation combined with augmentation vertebroplasty in treatment of thoracolumbar burst fractures

ObjectiveTo evaluate the efficacy of short-segment transpedicular fixation combined with augmentation vertebroplasty in treatment of thoracolumbar burst fractures. MethodsFrom November 2006 to September 2009, 37 patients with thoracolumbar burst fracture were admitted and received transpedicular fixation combined with calcium sulfate cement augmentation vertebroplasty, and the clinical data including fracture types, complications and following-up results were collected for analysis. The multimethod evaluation strategies involved the anterior vertebral body height, the sagittal Cobb's angle, the restoration of nervous function, internal fixation failure, visual analogue scale (VAS) and Oswestry disability index (ODI) were retrospective analyzed. Results All patients were followed up for average 19 months (range, 14-37). There were no internal fixation failure, loss of reduction, neurological complications in all the patients. In 16 patients with partial neurologic deficits, 14 initially improved at the final follow-up, with no deterioration of neurologic functions. The mean time of calcium sulfate cement obvious absorption and union was 3 months and 5 months postoperatively, respectively. The anterior vertebral body height was 55.40%before surgery and 85.46% after surgery on average, ended up with 82.35%. The sagittai Cobb's angle was improved from 22.45° to 6.86°, ended up with 9.66° on average. The mean VAS and ODI at the final followup were respectively 1.2 and 20.4 on average. ConclusionShort-segment transpedicular fixation combined with augmentation vertebroplasty appears to be effective in achieving stable biomechanics with high security,which seems to be a feasible option in the management of thoracolumbar burst fractures.
