2.Gabapentin in treatment for pruritus and insomnia among patients with hemodialysis

This paper aimed to study clinical effectiveness of gabapentin in the treatment for pruritus and insomnia among patients with hemodialysis. Eighty patients with severe pruritus and insomnia during regular hemedialysis in Jiaozhou People's Hospital, Jiangsu were randomly divided into two groups with 40 patients each. Gahapentin 0. 1 g were given each patient every night for 4 weeks in one group and a modified-re]ease capsule of 8 mg chiorphenamine maleate was given each patient daffy for 4 weeks in the other group. Effectiveness of treatment was evaluated by improvement of itching symptoms and Pittsburgh Sleep Quality Index (PSQI) scores for insomnia in the patients. Itching improvement was observed in 85%(34/40) of the patients and PSQI scores obviously lowered in them four weeks after gabapentin treatment.However, itching improvement was observed only in 42% ( 17/40 ) of the patients and little change was observed in PSQI scores in them four weeks after chlorphenamine treatment It suggests that gabapentin can significantly improve quality of sleeping and itching symptoms in patients with hemodialysis and it is worthy popularizing in those with it.
4.Study of different modes of delivery and outcome of pregnant women

Objective To study the different selections of delivery mode of pregnant women, and compare their outcomes. Methods Data of 7 509 pregnant women hospitalized in the General Hospital of Tianjin Medical University between January 2013 and December 2014 were retrospectively analyzed. Of them, there were 6 229 cases of primipara and 1 280 cases of multipara. The numbers of vaginal delivery and cesarean delivery were statistical analyzed between primipara and multipara. The cesarean delivery indications of multipara was also studied. The vaginal delivery rates and cesarean delivery rates, and their outcomes were compared between primipara and scar uterus maternal pregnancy. Results The cesarean delivery rate was higher in multipara than that of primipara (69.06%vs. 57.54%,χ2=59.19, P<0.05). Among them, 631 cases were uterine scar pregnancy, 39 cases (6.18%) were vaginal delivery and 592 cases (93.82%) were cesarean delivery. The uterine scar was the main indicator for cesarean delivery in multipara. Compared the outcomes of cesarean delivery, the weight of neonatal body mass was relatively light, the hospital stay was shorter, the amount of 24 h postpartum bleeding and postpartum fever were less in vaginal delivery in primipara and pregnant women with uterine scar ( P<0.05). There were no significant differences in scores of newborn. There were no significant differences in outcomes of vaginal delivery between primipara and pregnant women with uterine scar. Compared outcomes of cesarean delivery between multipara with scar uterus and primipara, there were increased maternal blood loss and postpartum fever in multipara with scar uterus (P<0.05). Conclusion The cesarean delivery is still the mainly mode for multipara. The vaginal delivery is the best mode for pregnant women with scar uterus compared with that of primipara. The vaginal delivery is the safer way of delivery for pregnant women.

Aimed at these psychological problems, the medical ethics requirements in the process of diagnosis and treatment were proposed:respect for the patient, protect of the patient′s privacy, patient communication and regular follow-up and health education.
6.The protection effect of cysteine rich-protein 61 on renal tubular epithelial cells against apoptosis induced by hypoxia

Objective To observe the expression of cysteine rich-protein 61 (Cyr61) on renal tubular cells,to explore its effects against hypoxic induced kidney injury and the underlying mechanisms.Methods A stably Cyr61 expressed tubular cell line Cyr61-HK2 was established based on HK2 cells and recombinant Cyr61-lentivirus.BrdU incorporation assay was used for cell proliferation.The apoptosis of cells was analyzed by flow cytometry with Annexin V and propidiumiodide staining.Western bloting was used to detect the protein expression of BAD,Akt and ERK.Results (1) Cyr61-HK2 cells displayed more proliferation ability than HK2 cells.(2) Under hypoxia condition,the apoptosis of both HK2 and Cyr61-HK2 cells increased,but the apoptosis of Cyr61-HK2 cells was lesser than HK2 cells.(3) The expression of Cyr61 led to the phosphorylation of BAD,Akt and ERK on 0 h,0.5 h,1 h (all P < 0.05).Conclusion The expression of Cyr61 can promote cell proliferation and dampen cell apoptosis induced by hypoxia,which may be involved in the Akt/ERK signal pathway.
7.Expression of the transcription factor CCAAT/enhancer-binding protein alpha in psoriatic lesions and its significance

Objective To detect the expression of the transcription factor CCAAT/enhancer-binding protein alpha (C/EBP-α) in the epidermis of psoriasis vulgaris lesions,and to investigate its correlation with abnormal keratinocyte proliferation and disease severity.Methods Biopsy specimens were obtained from the lesions of 30 patients with psoriasis vulgaris and normal skin of 30 healthy human controls.A two-step immunohistochemical procedure was performed to detect the expressions of C/EBP-αt and Ki-67 in these specimens,and Western blot to quantify the expression of C/EBP-α.The proliferation index of keratinocytes was calculated according to the expression intensity of Ki-67.Statistical analysis was carried out by using the SPSS 17.0 software,and Pearson correlation analysis was conducted to assess the relationship of C/EBP-α expression level with proliferation index of keratinocytes and psoriasis area and severity index (PASI) score.Results C/EBP-α was predominantly expressed in the cytoplasm of keratinocytes,while Ki-67 in the nuclei of keratinocytes.Compared with the normal skin,the psoriatic lesions showed a significantly lower expression of Ki-67 (t =7.82,P < 0.05),but higher proliferation index of keratinocytes (t =4.54,P < 0.05).The expression level of C/EBP-α was negatively correlated with the proliferation index of keratinocytes and PASI score in the patients (both P < 0.05).Western blot also showed an obvious decrease in the expression of C/EBP-α in psoriatic lesions.Conclusions The expression of C/EBP-α is decreased in lesions of psoriasis vulgaris,which might be involved in the pathogenesis of psoriasis vulgaris.

10.Effect of dietary fat acids on incretin and islet function

Objective To explore the effect of dietary fat acids on incretin and islet function in healthy adults .Methods Before each test, healthy subjects received a 1-week pre-experiment eucaloric diet .Fifteen subjects consumed two meals containing different fat acids , including high saturated fat acid ( HSF) and high monounsaturated fat acid ( HMF) .On two separate occasions,they underwent a minimum of 1-week washout between meals .At 0,30,60,120,180 and 240 min following meal intake, the plasma concentrations of gastric inhibitory polypeptide (GIP), glucagon-like peptide-1(GLP-1) and serum concentrations of glucose, insulin, triglycerides ( TG) and free fatty acid ( FFA) were measured.Results Postprandial glucose did not increase significantly following HSF and HMF meals (P>0.05).Compared with HMF meal, significant increase in AUCins240min,AUCTG240min and AUCFFA240minwas observed following HSF meal (P<0.05).Compared with HMF meal,a significant decrease in AUCGIP30min, AUCGIP240min, AUCGLP-1 30min and AUCGLP-1 240min was found following HSF meal (P<0.05).No significant difference was observed inΔI30/ΔG30 following HSF and HMF meals (P>0.05). AUCI/AUCG was significantly lower following HMF meal as compared with HSF meal (P<0.05).Conclusion This study demonstrates that the function of GIP ,GLP-1 andβcell is affected by the dietary fat acids in healthy adults .The HMF meal may stimulate GIP and GLP-1 secretion to a greater extent than HSF meal .
