1.Practice and effect evaluation of gestational diabetes mellitus specialist practice model

Objective To evaluate the outcomes after implementing gestational diabetes mellitus(GDM) specialist practice model. Methods GDM specialist practice model contained four modules(first visit,follow-up visit,hospital-ization,postpartum),eight aspects of practice contents which included specialist evaluation,diet & exercise survey, diet & exercise prescription,GDM health guidance,weight management during pregnancy. Pregnancy outcomes were compared before and after GDM specialist practice. Results GDM specialist practice model was able to signifi-cantly decrease insulin utilization rate,reduce number of hospitalized patients due to poor blood sugar control,short-en length of hospitalization. Conclusion GDM specialist practice model can effectively save medical costs,and GDM specialist nurses play an important role in management of women with GDM.

The factor of antiapoptosis in cancer is recently identified as survivin. Survivin is prominently overexpressed in embryonic and fetal tissues, and various human cancers. It is potentially involved in both inhibition of apoptosis and control of cell division. Recently, survivin is found to be correlated with the genesis, progress, and prognosis of urological cancer. Manipulation of the antiapoptotic pathway maintained by survivin may be beneficial for cancer therapy. Progressive research of survivin in urological cancer.

4.The diagnostic value of combined detection of tumor markers for malignant pleural effusions

Objective To investigate the significance of combined detection of tumor markers in serum and pleural fluid on differential diagnosis of benign and malignant pleural effusion.Methods Three hundred and seventy six cases of pleural effusion were selected.The levels of carcinoembryonic antigen (CEA),neuronspecific enolase(NSE),cancer antigen 125 ( CA125 ),squamous cell carcinoma antigen (SCC) in serum and pleural fluid were examined and they were analyzed combined with histological or cytological evidence using statistical methods.Results There were 298 cases in malignant group and 98 cases in benign group.The levels of the four tumor markers in malignant group were significantly higher than in benign group both in pleural fluid (CEA:[279.9 ± 170.0]μg/L v.s.[ 12.6 ± 6.2 ] μg/L,t =6.29,P < 0.01; NSE:[ 112.3 ± 86.8 ] μg/L v.s.[14.7 ±7.3] μg/L,t =5.13,P <0.01 ;SCC:[ 10.6 ± 5.4] μg/L v.s.[ 1.2 ±0.6 ] μg/L,t =2.34,P <0.01;CA125:[ 409.2 ± 206.7] U/ml v.s.[ 44.0 ± 20.5 ] U/ml,t =7.46,P < 0.01 ) and in serum ( CEA:[ 86.7 ±42.0] μg/L v.s.[6.2±3.1]μg/L,t=3.14,P<0.01;NSE:[31.6±18.2]μg/Lv.s.[11.2±5.0]μg/L,t=4.61,P<0.01;SCC:[3.5±2.2]μg/Lv.s.[1.8±0.g]μg/L,t=1.70,P<0.01;CA125:[134.0±72.6]U/ml v.s.[ 19.8 ± 9.6 ] U/m1,t =4.04,P < 0.01 ).Moreover,the levels of tumor markers in pleural fluid were higher than in serum.The sensitivity were 100% by combined detection of pleural fluid and serum tumor markers in parallel and the specificity were 100% in sequence.Conclusion The levels of CEA,NSE,CA125,SCC in pleural effusion were more sensjtive than which in serum.Combined detection of tumor markers in pleural fluid and serum could improve the sensitivity of diagnosis for benign and malignant pleural effusion.
5.Influence of intraoperative posture intervention on drainage in patients after gynecological laparoscopic surgery

Objective To investigate the influence of intraoperative posture intervention on drainage in patients after gynecological laparoscopic surgery. Methods A total of 1250 patients who were scheduled for gynecological laparoscopic surgery including laparoscopic-assisted salpingoplasty,oophorocystectomy,ectopic pregnancy,vaginal hysterectomy,myomectomy and appendectomy were randomly divided into two groups. The patients' posture was changed from Trendelenburg position to level lithotomic position during suction by the circulating nurses in the experimental group,while after surgery in the control group. The volume and time of drainage,and the length of stay after surgery were compared between the two groups. Results The volume and time of drainage,as well as the length of stay after surgery had significant differences between the two groups (P<0.05). Conclusions Intraoperative posture intervention is helpful for the treatment effect of drainage after gynecological laparoscopic surgery.

Physical and psychological stresses inhibit or harm reproductive endocrine especially female one at many levels of hypothalamo-pituitary-gonadal (HPG) axis, and result in subhealth conditions or clinic diseases in reproductive endocrine. The hypothalamic corticotropin-releasing factor (CRF) and adrenal glucocorticoids (GCS) overreleases and other neuroendocrine alterations induced by stress are implicated in the mechanisms responsible for the disturbance of reproductive endocrine. The declines in activities of synzyms for reproductive hormones cause sexual hormones reduces and disfunction in reproductive endocrine system.
7.Effect of Two Kinds of Combination of Selected Acupuncture Points on Postoperative Enteroparalysis

ObjectiveTo observe the effect of two kinds of combination of acupuncture points on postoperative enteroparalysis.Methods96 patients with postoperative enteroparalysis were divided into the group A (n=44) and group B (n=52). The patients of the group A were treated with acupuncture on Mu points and lower-he point, while those of the group B on Back-shu points and distant lower-he point. The therapeutic effect of two groups was compared.ResultsThe two therapeutic methods had good effect, but the Back-shu points and distant lower-he points was superior to the Mu points and distant lower-he points ( P<0.05).ConclusionAcupuncture with combination of special points has better effect on postoperative enteroparalysis.
8.Association between helicobacter pylori infection and dyslipidemia in different gender population

Objective To assess the association between helicobacter pylori (HP) infection and dyslipidemia in different gender population.Methods We conducted cross-sectional analysis using data of 1921 cases of demographic characteristics,anthropometry,life style,lipid profile,etc.,from the subjects who received health examination from January 2010 to June 2012 in Department of Geriatrics,Tongji Hospital.Diagnosis of HP infection was achieved by using 14C-Urea Breath Test (14C-UBT).The participants were divided into HP infection positive group and HP infection negative group by 14C-UBT.Results In female subjects,the levels of total cholesterol/high-density lipoprotein cholesterol (TC/HDL-c) were higher in HP positive group than HP negative group (P < 0.05),but there was no obvious difference between the levels of low density lipoprotein cholesterol (LDL-c) in HP positive group and HP negative group.However,in the male subjects,the levels of LDL-c and TC/HDL-c were significantly increased in HP positive group than HP negative group (P < 0.05).HP positive group had a greater risk for high TC/HDL-c both in female and male subjects.In female subjects,the risk for high TC/HDL-c in HP positive group was 1.90 times of that in HP negative group (95% CI,1.06 ~ 3.38).In male subjects,the risk for high TC/HDLc in HP positive group was 1.56 times of that in HP negative group (95% CI,1.21 ~ 2.00).But only in male subjects,the risk for high LDL-c in HP positive group was 2.33 times of that in HP negative group (95 % CI,1.34 ~ 4.06).Conclusions We observed that HP infection was probability associated with dyslipidemia.
9.Cleansing effect of light catalyst compound atomization in the hospital environment

Objective:To check the cleansing effect of light catalyst compound atomization in the hospital environment. Methods: Bacterium quantities were compared in the rooms treated with light catalyst compound atomization or routine cleansing. Results: In the room treated with light catalyst compound atomization,the rate of air bacterium reduction was 90.83% at 1 h,53.61% at 24 h,and 55.82% in average from 1 week to 12 weeks.Bacterium on object surfaces were kept lower for 8 weeks in the light catalyst compound sprayed room than in the routinely cleansed room.Conclusion:The light catalyst compound atomization can chronically cleanse the hospital environment without pollution.
