1.The correlation study on syndrome differentiation of rheumatoid arthritis and joint high frequency ultrasound performance.

OBJECTIVETo observe the differential effect of joint ultrasound on the syndrome differentiation of rheumatoid arthritis (RA) by observing the high frequency ultrasound performances among inactive stage and different syndromes in active stage.
METHODSTotally 83 RA patients in the active stage were assigned to the dampness heat syndrome group (DHS, 59 cases)and the cold dampness syndrome group (CDS, 24 cases) according to Chinese medicine (CM) syndrome typing. Besides, 20 RA patients in the remission stage were recruited as the control group (abbreviated as the remission group). By using high frequency ultrasound and power Doppler ultrasound technology, a comparative observation of synovitis, tenosynovitis, synovial blood flow, and bone erosion in the 2nd-5th metacarpophalangeal (MCP) joints, proximal interphalangeal (PIP) joints, wrist joints, knee joints, the second and the fifth metatarsophalangeal (MTP) joints (a total of 24 joints) was performed in all patients. Correlation analyses were performed between the ultrasound performance, laboratory indices, and the disease activity. Ultrasound data of each RA patient were analyzed by their total scores. Χ2 test was used for enumeration data. The measurement data was expressed as x ± s. One-way ANOVA was used for data of normal distribution, while non- parametric test was used for data of non-normal distribution. Correlation analysis of two variables was performed for clinical indicators and ultrasound indicators. Its significance was detected using Pearson correlation.
RESULTSCompared with the remission group, the severity degree of synovitis, tenosynovitis, synovial blood flow, and bone erosion significantly increased in the DHS group (P < 0.01). There was statistical difference in ESR, CRP, anti-CCP, DAS28 score, and the positive rate of RF (P < 0.05, P < 0.01). There was statistical difference in the severity degree of synovitis and synovial blood flow, and DAS28 score in the CDS group (P < 0.05). Compared with the CDS group, there was statistical difference in the four ultrasound indices (P < 0.05, P < 0.01), ESR, CRP, anti-CCP, DAS28 score, and the positive rate of RF in the DHS group (P < 0.05, P < 0.01). There was no statistical difference in G, IgG, IgA, or IgM among the three groups (P > 0.05). There existed positive correlation between ESR and the synovitis degree, synovial blood flow, and bone erosion in the DHS group (r = 0.444, 0.397, 0.486, P < 0.05).There existed positive correlation between ESR and the synovitis degree, bone erosion, and synovial blood flow in the DHS group (r = 0.378, 0.270, P < 0.05). There existed positive correlation between the DAS28 score and the synovitis degree and synovial blood flow in the DHS group (r = 0.304, 0.351, P < 0.05).
CONCLUSIONSThe inflammation degree was the most severe in RA patients of DHS. High frequency ultrasound could provide better evidence for Chinese medical syndrome differentiation of RA patients.

2.Clinical research progression in chemoradiation therapy for resectable gastric cancer.

In recent years, based on the phase III clinical study, postoperative chemoradiation, perioperative chemotherapy with ECF regimen and postoperative adjuvant chemotherapy with oral S-1 have become the standard adjuvant treatment of resectable gastric adenocarcinoma in the United States, Europe, and Japan, respectively. Since the Southwest Oncology Group in 2001 reported a large phase III randomized clinical trial INT0116, adjuvant chemoradiotherapy has become a standard treatment for gastric adenocarcinoma. With the rapid development of chemoradiotherapy technique, clinical researches for using operation combined with chemoradiotherapy to treat gastric adenocarcinoma emerged one after another, including adjuvant postoperative chemoradiation, preoperative chemoradiation, and chemoradiation combined with intraoperative radiotherapy and so on. This review will summarize the recent treatment protocol using chemoradiotherapy for resectable gastric adenocarcinoma, and comprehensively evaluate the clinical value and significance of chemoradiotherapy for resectable gastric adenocarcinoma.

3.Functional status of bone marrow mesenchymal stem cells from osteoarthritis patients and disease development

BACKGROUND:Bone marrow mesenchymal stem cels are crucial for bone and cartilage development and regeneration at a celular level. Insufficient quantity and functional impairment of bone marrow mesenchymal stem cels is widely considered to be one of osteoarthritis causes. OBJECTIVE: To explore the relationship between the functional status of bone marrow mesenchymal stem cels and disease progression in osteoarthritis patients.METHODS: Thirty patients with osteoarthritis were enroled from July 2013 to October 2014, and divided into control, mild osteoarthritis, and severe osteoarthritis groups, with 10 cases in each group. 5 mL bone marrow from the femur or tibia was extracted from each patient to isolate and culture bone marrow mesenchymal stem cels. Proliferation ability of cels at passage 3 was detected using cel counting kit-8; toluidine blue staining was performed at 14 days after chondrogenic induction; real-time PCR was used to detect the mRNA expression of Aggrecan and Col2A1 in the control group after chondrogenic induction. RESULTS AND CONCLUSION:Afterin vitro culture, bone marrow mesenchymal stem cels grew adherently in polygonal and fusiform shape with multiple processes at uniform size. The cytoplasm contained larger particles and the nuclei were ovoid. Most of cels were in cel division phase. The proliferation ability was strongest in the control group and weakest in the severe osteoarthritis group. Cels from the three groups were al at plateau phase after 1 week culture. At 14 days after chondrogenic induction, the cels were polygonal and quasi-circular, and purple metachromatic granules distributed outside of the cytoplasm. The expression of Aggrecan and Col2A1 in the control group displayed an overexpression trend. These findings indicate that the functional status of bone marrow mesenchymal stem cels from osteoarthritis patients is negatively correlated with the severity of disease, which can influence the disease progression in osteoarthritis patients.
4.Effect of conditioned medium from coculture of astrocytes and β-amyloid-induced apoptotic PC12 cells on the differentiation of neural stem cells and the possible mechanism

Objective To observe the effect of the conditioned medium obtained from the coculture of astrocytes and β-amyloid (Aβ1-40)-induced apoptotic PC12 cells on the neuronal differentiation efficiency of neural stem cells (NSCs), and investigate the possible involvement of the neurotrophins proteins including brain-derived neurotrophic factor (BDNF), nerve growth factor (NGF)and neurotrophin-3 (NT-3) in the induced differentiation of the NSCs. Methods PCI2 cells wereinduced by Aβ1-40 for different time lengths (0, 4, 6, 12, and 24 h), and after detection of the apoptotic rates using flow cytometry, the cells were cocultured with astrocytes for 2 days. The astrocyte-conditioned medium (ACM), after analysis of the neurotrophins proteins using erLzyme-linked immunosorbent assay (ELISA), was mixed with DMEM/F12 medium at the proportion of 1:3 to induce the differentiation of the NSCs. The cell differentiation was identified by laser confocal microscopy, NSE immunofluorescent labeling, and neuronal counting. Results Flow cytometry showed that induction by Aβ1-40 for 6 h resulted in the highest apoptosis rate of PC12 cells (P<0.05). BDNF content in the ACM derived from the coculture of astrocytes and the PC12 cells induced for 6 h was significantly increased, and the NSCs induced in this ACM showed the highest neuronal differentiation rates, showing significant difference from those induced by other ACMs (P<0.05). Conclusion The ACM derived from the coculture of Aβ1-40-induced PC12 cells and astrocytes can increase the neuronal differentiation rates of NSCs in vitro, and BDNF in the ACM may play a role in this process.
5.Expressions and activities of Rho GTPases in hypoxia and its relationship with tumor angiogenesis.

OBJECTIVETo study the expressions and activities of Rho GTPases in hypoxia and its relationship with tumor angiogenesis.
METHODSThree tumor cell lines were used in this study: gastric cancer cell lines AGS, SGC7901 and hepatocellular carcinoma cell line HepG2. Expression level of Rac1 mRNA was detected by semi-quantitative RT-PCR. Activity of Rac1 was determined by pull-down assay and expression of HIF-1alpha, VEGF, p53 and PTEN protein was detected by Westernblot.
RESULTSThe expression level of Rac1 mRNA was significantly increased in hypoxia compared to normoxia. Pull-down assay showed that hypoxia-induced activity of Rac1 was elevated in a time-dependent manner and climaxed at 3 hours. The expressions of HIF-1alpha and VEGF protein were up-regulated, while those of PTEN and p53 protein were down-regulated.
CONCLUSIONThese results indicate that hypoxia enhances Rac1 expression which might be involved in tumor angiogenesis by reacting with hypoxia-responsive genes.

6.Preliminary evaluation of coronary fractional flow reserve derived from CT based on tracer-kinetics principle

Objective:To evaluate the feasibility and diagnostic efficacy of the coronary fractional flow reserve derived from CT (CT-FFR) measurement method based on tracer pharmacokinetic principle.Methods:A total of 130 patients (159 coronary artery vessels) who were admitted to Beijing Anzhen Hospital from January 2015 to March 2019 were included in this study retrospectively. All patients had completed coronary CT angiography (CCTA) and invasive coronary angiography with invasive FFR. Subjective assessment of stenosis degree was performed on CCTA images and non-invasive FFR measurement was performed by using a tracer-kinetics based on CT-FFR measurement method. The Bland-Altman method was used to evaluate the diagnostic consistency of the two methods. Compared with the invasive FFR results, the sensitivity, specificity, positive predictive value, negative predictive value, and area under the ROC curve (AUC) of tracer-kinetics based on CT-FFR results for the diagnosis of coronary ischemic lesions were evaluated. All cases were divided into two heart rate groups:>65 bpm and ≤65 bpm. The diagnostic efficacy of tracer-kinetics based on CT-FFR in different heart rate groups was evaluated. χ 2 test and DeLong test were used to compare diagnostic performance in different evaluation methods and heart rate groups. Logistic regression analysis was used to evaluate the impact of factors such as heart rate, image thickness, image enhancement, and noise on the accuracy of diagnosis. Results:Bland-Altman analysis showed that the average difference between the two methods was -0.01. (-0.11-0.10). Compared with invasive FFR results, the tracer-kinetics based on CT-FFR method had a diagnostic sensitivity of 92.4%, specificity of 82.1%, positive predictive value of 87.6%, negative predictive value of 88.7%, and the area under ROC curve (AUC) value was 0.94. Compared with the diagnostic efficacy of luminal stenosis evaluated based on CCTA images, the difference was significantly statistical ( P<0.05). The diagnostic performance of CT-FFR had no statistically significant difference between the two heart rate groups. Factors such as heart rate, image thickness, image enhancement, and noise had no significant effect on the diagnostic accuracy of the tracer-kinetics based on CT-FFR method. Conclusions:The tracer-kinetics based on CT-FFR method may quickly complete the non-invasive FFR measurement on CCTA images with image quality that meets the needs of clinical diagnosis. It has a good diagnostic performance in the diagnosis of coronary ischemic lesions even for those cases with a faster heart rate. The diagnostic accuracy of tracer-kinetics based on CT-FFR method is not significantly affected by factors such as heart rate, image thickness, image enhancement, and image noise.
7.The cognitive and neural bases of abstract concepts.

Words denoting abstract concepts constitute nearly half of human lexicon and serve as building blocks of the human culture. Since the advent of non-invasive neuroimaging techniques, great progress has been made in revealing the neurobiological foundation of concrete object and action concepts, yet it remains unclear how abstract concepts are stored and processed in the brain. Here we review recent development in this field, focusing on both theoretical perspectives and neuroimaging findings. We found that abstract concepts can be represented via linguistic and experiential information; the neural correlates of abstract concepts are partly in line with such a theoretical framework. Future studies are warranted to uncover the cognitive and neural mechanisms of language and experience in abstract word representation, which will help to deepen our understanding of general computational principles of the human conceptual system and to promote the development of the brain-like artificial intelligence.

8.Stromal cell derived factor-1 inducing targeted migration of neural stem cells

Objective To investigate the effect ofstromal cell derived factor-1 (SDF-1) on the regulation of neural stem cells (NSCs) migration.Methods NSCs were obtained from the cerebral cortex of embryonic rats and cultured in serum-free medium,and their stem cell properties were assessed by means of induced differentiation in vitro into neurons and astrocytes.After in vitro cell culture,the purity of NSCs and the co-expression rate of CXCR4/nestin were detected by flow cytometry.Blind-well chambers were employed to detect the chemotactic effects of SDF-1 by counting the cells which had crossed a 8 μm pore membrane when confronted with varying concentrations of SDF-1 (0,1,10,50,100,500 and 1000 ng/mL),and the distribution of cells migrated out of the same neurosphere was overviewed by μ-slides in the persistent concentration gradient of SDF-1.Results Neurospheres were formed by persistent proliferation of NSCs, which were capable of differentiating into neurons (β-tubulin+) and astrocytes (GFAP+) in media without mitogens,and flow cytometry analyses showed that most of the cultured cells expressed nestin and the co-expression rate of CXCR4/nestin was nearly 80%.SDF-1 showed great chemotaxis to NSCs,and the amount of cells having migrated through the membrane in 500 ng/ml SDF-1 group was higher than that in other groups (P<0.05).When the cells were confronted with a linear concentration gradient (from 500 to 0 ng/mL),which was generated by diffusion and stable for at least 48 h,the cells migrated out ofa neruosphere could distribute irregularly with more cells locating in the region of higher concentration of SDF-1 and longer migration distance away from the center of the neurosphere than the opposite.Conclusion SDF-1 binding to its specific receptor CXCR4 was capable of inducing NSCs migrating directionally to the source of SDF-1.
9.Analysis of clinicopathological features and risk factors for postoperative complications in the elderly gastric cancer patients.

OBJECTIVETo investigate the clinicopathological features and postoperative short-term complications in the elderly gastric cancer patients.
METHODSClinical data of 270 elderly patients with gastric cancer who underwent gastrectomy in Department of Surgery, Ruijin Hospital, Shanghai Jiaotong University School of Medicine between July 2012 and June 2014 were analyzed retrospectively. Among 270 patients, 220 were 70 to 79 years old( old group) and 50 were ≥80 years old(oldest group). The clinicopathological features were compared between the two groups. Perioperative factors were analyzed to determine if they are associated with postoperative complications. Multivariate logistic regression model was performed.
RESULTSBefore operation, most elderly patients (n=161, 59.6%) had 2 or more than 2 comorbidities, including hypertension (n=154, 57.0%), anaemia (n=126, 46.7%), diabetes (n=53, 19.6%), arhythmia (n=52, 19.3%), cardiovascular disease(n=33, 12.2%), and chronic pulmonary disease(n=28, 10.4%). Elderly gastric cancers were more likely to locate at the lower third of the stomach (n=116, 43.0%). The pathological type was mainly the poorly differentiated carcinoma (n=152, 56.3%), and stage III was more common in TNM staging(n=138, 51.1%). As compared to the old group, the oldest group had more preoperative comorbid diseases(P=0.048), more previous surgery(P=0.029), more preoperative transfusion (P=0.019), more combined cholecystectomy (P=0.007) and feeding jejunostomy (P=0.037), but less tumor invasion of nerves(P=0.045). No significant differences in other clinicopathological parameters were found between the two groups (all P>0.05). A total of 121 (44.8%) patients presented postoperative complications, including severe complication in 30 cases(11.1%) and death in 4 cases(1.5%). Forty-seven patients(17.4%) presented operation-associated complications, including infection in 28 cases(10.4%) and leakage in 21 cases(7.8%). One hundred and seven(39.6%) patients presented non-operation-associated complications, including pneumonia in 48 cases(17.8%), hypertension in 23 cases(8.5%), and arhythmia in 17 cases(6.3%). Postoperative morbidities of Clavien-Dindo class II complication and non-operation-associated complication were higher in the oldest group compared with old group(P<0.05), while other postoperative complications were compared between the two groups, only urinary tract infection was significantly different(P<0.05). Univariate analysis showed that postoperative complications were significantly associated with age(χ(2)=7.308, P=0.007), number of comorbid diseases (χ(2)=10.872, P=0.001), cardiovascular disease (χ(2)=9.412, P=0.002), hypertension (χ(2)=4.934, P=0.026) and preoperative transfusion (χ(2)=3.911, P=0.048). Multivariate analysis showed that only the number of comorbid diseases was an independent risk factor for postoperative complications(OR=2.810, 95% CI: 1.710 to 4.616, P=0.000).
CONCLUSIONNon-operation-associated postoperative complications are more likely to occur in the elderly patients due to more comorbid diseases. Perioperative intensive care should be carried out for the elderly gastric cancer patients with comorbid diseases in order to improve surgical safety and efficacy.

10.Feasibility of delta-shaped gastroduodenostomy in totally laparoscopic distal gastrectomy for gastric cancer.

OBJECTIVETo investigate the feasibility of delta-shaped (DS) gastroduodenostomy in totally laparoscopic distal gastrectomy (TLDG) for gastric cancer.
METHODSFrom July 2013 to November 2013, 22 gastric cancer patients underwent DS gastroduodenostomy using laparoscopic linear stapler. All the patients underwent TLDG with D2 lymphadenectomy. In addition, modified DS anastomosis (when closing the common entry hole, previous duodenal staple line was also removed) was used in selected patients. Clinical data of these 22 patients were retrospectively analyzed.
RESULTSAll the patients underwent TLDG with D2 lymphadenectomy and DS gastroduodenostomy. Among them, 12 patients underwent modified DS anastomosis. The total operative time was (194.6±38.4) min, and the DS anastomosis time was (19.1±14.1) min. The number of linear stapler cartridges used per patient was 5.8±0.8. The intraoperative blood loss was (49.5±24.0) ml. The number of lymph nodes harvested per patient was 32.8±12.4. All the patients achieved microscopic cancer-free resection margin. The time to the first postoperative flatus, first water intake, and semi-liquid diet was (2.9±0.7) d, (4.8±1.1) d, and (6.6±1.2) d, respectively. The duration of postoperative hospital stay was (10.1±2.3) d. The postoperative complication rate was 9.1% (2/22). No patients developed anastomosis-related complications including anastomotic leakage, stenosis, or bleeding.
CONCLUSIONSDelta-shaped gastroduodenostomy is simple, easy, safe, and feasible. It will be an ideal choice for reconstruction after totally laparoscopic distal gastrectomy, and has great value in clinical practice.
