1.Investigation and analysis of aircrew ametropia and related factors

AIM: To investigate the refractive distribution and analysis risk factors for aircrew ametropia.
METHODS: The number of 49 cases with ametropia from 1031 aircrew during May 2013 to May 2014 were reviewed. Various types of refraction composition, age, type, position, time of flight with the subjective assessment of aircrew were analyzed and compared.
RESULTS:Of 49 cases, 43 cases (88%) were myopia, 6 cases ( 12%) were hypermetropia. , Detection rates were higher in age over 50 years aircrew and flight time more than 3000h. Detection rates were lower in self-conscious symptom heavy aircrew, fighter aircrew and good habit of using eyes.
CONCLUSION: The myopia incidence in aircrew with age >50 years and long flight time is higher, than that of fighter pilots and good habit of using eyes. We should pay attention to the increasing late-onset myopia of aviators and habit of using eyes, work intensity and time of using eyes about aircrew.
2.Alteration of NO,ET-1,MMP-9 and TIMP-1 plasma levels in coronary artery stenosis and ectasia

Objective To explore the clinical effect of Weifuchun tablet combined with chemotherapy on patients with advanced non-small cell lung cancer (NSCLC).Methods Sixty-eight patients with advanced NSCLC was randomly divided into control group (n =34) treated with cisplatin + gemcitabine and treatment group (n =34) treated with Weifuchun tablet (1.436g × 2/d) and cisplatin + gemcitabine.After two treatment cycles,the clinical effect in both groups were evaluated.Results The clinical efficacy in the treatment group was 52.94% (18/34),which in the control group was 41.18% (14/34),there was no statistically significant difference between the two groups(x2 =0.94,P > 0.05).The quality of life and the level of T lymphocytes were markedly improved,and the reduction of hemoglobin,leucocyte,and platelet,and nausea reaction were all significantly inhibited in treatment group compared with that in control group after two treatment cycles (x2 =4.12,4.66,5.96,4.12,5.90,all P < 0.05).Conclusion Weifuchun tablet combined with chemotherapy effectively ameliorates the clinical symptoms of the patient with advanced NSCLC,reduces the toxic and side effects caused by chemotherapy,and improves the quality of life,which is worthy in the clinic.
4.Comparative proteomics of rat brain in the BCNU-induced model of cortical dysplasia

Objective To screen the differential proteins in the brain (neocortex and hippocampus) between the rats with cortical dysplasia (CD) and control ones,and investigate the role of their alteration in the development of epilepsy in CD.Methods Cortical dysplasia was induced in rat pups via in utero delivery of BCNU.A two-dimensional electrophoresis (2-DE)-based approach was used to construct the expression profiles of proteins in both the neocortex and hippocampus at different age groups (postnatal day 7 and 60) and to detect proteome changes between CD rats and control ones.Following gel image analysis,protein spots that differed in abundance between CD and control rats were identified by using Matrx-assisted laser desorption/ionization (MALDI) mass spectrometry (MS) and MS/MS.Results A total of 57 kinds of protein were screened out (P < 0.05),in which 35 were found up-regulated and 22 were down-regulated compared with the control,35 from neonatal stage (postnatal day 7) and others from adult stage.Finally,12 among them were identified,including tubulin,alpha-lB,Beta-actin,tubulin beta-2A,GAP-43,UbCKmit,GAPDH and TMBr-3,etc.Conclusions Changed expression of specific proteins which were found in our study are involved in construction of brain 's cytoskeleton,synaptic function,mitochondrial function and so forth.Thus,they may be related to the pathogenic mechanisms of epileptogenicity of CD.

In this study, a novel dual-target surface plasmon resonance (SPR) biosensor was developed for S protein receptor binding domain (SRBD) and angiotensin converting enzyme 2 (ACE2) which are two key proteins in the process of SARS-CoV-2 invading cells according to characteristics of synergistic effects of multiple components and comprehensive regulation of multiple targets of TCM. 6.Application of volumet -displacement technique following breast conserving surgery in early breast cancer

Objective To explore the application and assess the cosmetic effect of mammaplasty with volume-displacement technique(VDT)following breast conserving surgery.Methods Breast conservation therapy(BCT) was performed in 77 patients of early breast cancer(EBC).Of the patients,42 cases received oncoplastic VDT (onco-plastic group)and 35 cases had only direct skin closure without treatment of the postresection defect (regular group) following BCT.The cosmetic results of the two groups were evaluated after operation and radiotherapy,respectively. Results All patients were followed up for 1 -36 months with median follow -up of 22.3 months,and no recurrence or metastasis occurred in one of the patients.1 month after surgery,the excellent or good cosmetic outcome were achieved in 31 cases (73.81%)in oncoplastic group and 17 cases (48.57%)in regular group respectively,and there was statistical difference between the two groups (χ2 =5.442,P =0.021);1 month after radiotherapy,the cos-metic excellent or good result were obtained in 27 cases (64.29%)in oncoplastic group and 6 cases (17.14%)in regular group respectively,and the difference between the two groups reached statistical significance (χ2 =9.045,P =0.008).Conclusion VDT can improve the cosmetic effect of mammaplasty following BCT in EBC and does not affect curative effect,it is worthy of promoting.
7.Recognition of experimental animal model with kidney disease.

Animal models with kidney disease are generally divided into two types. One belongs to the models which imitate human kidney disease by the artificial operations, such as anti-glomerular basement membrane antibody nephritis, Heymann nephritis, anti-Thyl. 1 antibody nephritis, BSA nephritis and puromycin nephropathy. The other one pertains to the models which make themselves kidney disease, and appear the pathological characteristics naturally as like as human, such as HIGA mice with IgA nephropathy and NZB/WF1 and MRL/1pr mice with lupus nephritis. In addition,the transgenic animal models with kidney disease can also be established by the modern molecular biologic techniques including gene knockout and siRNA transfection. As for the studies related with kidney disease in pharmacodynamics and pharmacology of Chinese herbal medicine (CHM), it is important to understand deeply the features of each animal model with kidney disease, and select accurately the proper models according to the different experimental objectives, and then, build the special models provided with the combination of disease with syndrome in traditional Chinese medicine (TCM). Therefore,it is the developmental direction for the further study to establish animal models with kidney disease, which should possess the characteristics of syndrome in TCM.

8.Microarray-captured next-generation sequencing as a diagnostic platform for detecting small mutations in Duchenne muscular dystrophy

Objective To set up a new diagnostic platform based on microarray exon-capture and next-generation sequencing for detecting small mutations in dystrophin gene.The sensitivity and specificity of the method were assessed in clinical settings and the distribution of small mutations in Chinese Duchenne muscular dystrophy/Becker muscular dystrophy (DMD/BMD) patients were also analyzed.Methods Forty-one DMD/BMD patients diagnosed by the clinical criteria without large deletion or duplication (≥ 1exon) were recruited from Peking Union Medical College Hospital consecutively.Genomic DNA was extracted from blood samples.The libraries were prepared.Then exon and intron-exon flanking sequences of DMD gene were captured by custom microarray.Targeted next-generation sequencing and Sanger Sequencing were conducted.The patients who were not detected any disease-causing mutation were performed muscle biopsy.Results Thirty-eight subjects were detected small mutations in DMD gene.All single nucleotide variants (SNVs) and insertion & deletions (INDELs) were validated by Sanger sequencing.Twenty-one novel mutations were reported.The distribution of SNVs and INDELs was similar to other international DMD databases.Upon immunohistochemistry staining of dystrophin protein,1 of 3 mutation-undetected patients was diagnosed as DMD,2 of them were excluded.The specificity of the method was 100%,while the sensitivity was 97.4%.Conclusions Our microarray-captured next-generation sequencing assay could detect SNVs and INDELs with high sensitivity and specificity.Its advantages are economic,time-saving and stable.The platform is suitable for clinical gene diagnosis.
9.Indomethacin for prevention of post-ERCP pancreatitis and hyperamylasemia

Objective To investigate the preventive effect of Indomethacin for post-ERCP pancreatitis and hyperamylasemia.Methods A total of 600 patients,who were undergoing ERCP,were randomly divided into 3 groups to receive anal Indomethacin (n=200),intravenous octreotide (n=200) or no special medication (n=200) before ERCP.The level of serum amylase before and 24h after ERCP were measured,and the rate of acute pancreatitis and hyperamylasemia after ERCP were assessed.Results Serum amylase levels before ERCP of all groups were normal.The mean serum amylase level of Indomethacin group (101.3±77.7 U/L) after ERCP was significantly lower than those of octreotide group ( 176.6±138.3 U/L,P =0.040 ]and control group (227.2±264.9 U/L,P=0.048),while there was no difference between octreotide group and control group ( P>0.05 ).The incidence of post-ERCP pancreatitis in Indomethacin group (2.5%) was significantly lower than that of control group (9.5%,P=0.003),while there was no difference between octreotide group (4.5%) and control group ( P=0.05 ).The incidence of hyperamylasemia after ERCP in Indomethacin group (5.5%) was significantly lower than that of control group ( 13.5%,P=0.006 ),while there was no difference between octreotide group (10.0%) and control group ( P>0.05 ).Conctusion Anal administration of Indomethacin before ERCP can effectively reduce the incidence of acute pancreatitis and hyperamylasemia after ERCP.
10.Knee muscle function recovery of patients with knee osteoarthritis:Treatment and evaluation

BACKGROUND:Muscle atrophy and weakness exist around the knee joint of patients with knee osteoarthritis, comprehensive treatment combined with training muscle power around knee joint can obviously relieve symptoms and restore muscle power around the knee joint.
OBJECTIVE:To test the relationship between muscle function around the knee joint and the knee joint function with isokinetic muscle strength test before and after comprehensive treatment of knee osteoarthritis, and to investigate the pathological mechanisms of knee osteoarthritis in order to provide basis for the clinical rehabilitation and treatment.
METHODS:We screened out 30 subjects with unilateral knee osteoarthritis (stage Ⅱ and Ⅲ). Hospital for Special Surgery was used to evaluate the knee function, and visual analogue scale score was used to assess the patient pain. The knee muscle function of knee osteoarthritis patients was measured before treatment;the patients with knee osteoarthritis at stage Ⅱ and Ⅲ received comprehensive treatment for at least 1 month (active quadriceps isometric muscle strengthening exercises, oral celecoxib capsules combined with intra-articular injection of sodium hyaluronate), and knee muscle function was measured after treatment. Hospital for Special Surgery and the visual analogue scale score were evaluated during fol ow-up period.
RESULTS AND CONCLUSION:Compared with the healthy side, the muscle power of the affected side of the patients with knee osteoarthritis was decreased when the flexion and extension muscle peak torque and work volume tested at 60 (°)/s and 180 (°)/s (P<0.05). After comprehensive treatment, the Hospital for Special Surgery scores of the knee osteoarthritis patients were increased significantly (P<0.05), while the visual analogue scale scores were decreased (P<0.05), and the muscle power of the affected side was increased significantly (P<0.05);there was no significant difference in the muscle power
between affected side and healthy side after treatment (P>0.05). There was no significant difference in the peak torque ratio of quadriceps and hamstring measured at 60 (°)/s in the knee osteoarthritis patients before and after treatment (P>0.05), and there was significant difference measured at 180 (°)/s (P<0.05). Knee osteoarthritis patients have decreased muscle strength around the knee joint, and the comprehensive treatment of muscle functional training combined with effective pain relief and nutrition joint drugs can significantly relieve symptoms, improve muscle strength and restore knee function.
