1.CYTOLOGICAL CHARACTERISTICS OF ADULT T-CELL LEUKEMIA

Cytological characteristics of a case of adult T-cell leukemia were studied by detection of morphologic, cytochemical and immunological methods. The leukemic cells found in the peripheral blood belonged to mature T-lymphocytes and presented the changes of polymorphous nucleus which had diagnostic significance in association with clinical features and positive ATLA antibody. The mechanism of formation of cytological characteristics of adult T-cell leukemia is also discussed.
2.Effects of 1,25-Dihydroxyvitamin D_3 on Inhibition of Proliferation and Induction of Differentiation of HL-60 Cells

Inhibition of proliferation and induction of differentiation by 1, 25-dihydroxyvitamm D3 of human promyelocytic leukemic cell line (HL-60)are reported. At the concentration range of 109 to 106 mol/ L, 1, 25-dihydroxyvitamin D3 had effect in inhibiting cell proliferation with time-dependent and dose-dependent manner. Strong inhibition of colony cell growth (74.8?11.1%)was observed at 107 mol/L concentration. It was also found that HL-60 cells could be induced into monocytes by 1, 25-dihydroxyvitamin D3at the same concentrations,which were demonstrated by cytochemistry and specific antigen of cell surface and NBT reduction test. In addition, the relationship between proliferation and differentiation is discussed according to the result of flow cytometry.
4.Bidirectional transmission and standardization of LIS and instrument in hospital

Objective:The one-way communication between traditional LIS system and equipment has been unable to meet the needs of business development of medical laboratory in our hospital, we need to upgrade our equipment and to develop the bidirectional communication interface between LIS and equipment.Methods: Our hospital has invested a lot of money to upgrade previous One-way communication interface based on ASTM, the new instrument specifications conform to HL7 standard, and develop bidirectional transmission program based on HL7.Results: After the implementation of the bidirectional communication, testing equipment can automatically identify bar code to get the test request information, after the completion of the test instrument, the inspection unit can automatically send sample results to LIS.Conclusion: Bidirectional transmission improves the work efficiency, transmission based on HL7 is not only fast, but also conducive to the sharing of data between heterogeneous medical information systems, and it is a general tendency.
5.Clinical efficacy analysis of atorvastatin in combination with aspirin and clopedogrel in the treatment of acute progressive cerebral infarction

Objective To investigate the clinical advantages and safety profile of the combinational treatment of acute progressive cerebral infarction with aspirin,clopedogrel and atorvastatin.Methods 170 cases with acute progressive cerebral infarction were randomly divided into the treatment group and control group,85 patients in each group.Both two groups were initially treated with aspirin and atorvastatin.After excluding the possibility of hemorrhage, the treatment group additionally received clopedogrel,while the control group was treated with aspirin and atorvastatin. The difference in clinical efficacy was evaluated between before treatment and 7 days,14 days or 28 days after treatment by using the NIH Stroke Scale (NIHSS)and Barthel Index (BI).Results The NIHSS scores of the treatment group after 7d,14d and 28d were (12.52 ±3.25)points,(9.10 ±3.21)points and (6.87 ±2.85)points, which of the control group were (13.65 ±2.93)points,(10.73 ±3.41)points and (9.07 ±2.96)points respectively, the differences between the two groups were statistically significant(t =2.340,3.170,4.877,all P <0.05).The BI scores of the treatment group after 7d,14d and 28d were (35.26 ±11.18),(53.73 ±13.74)and (74.61 ±17.35), which of the control group were (31.98 ±9.12),(46.65 ±11.79 )and (63.87 ±15.73 )respectively,the differences between the two groups were statistically significant(t =2.131,3.752,4.456,all P <0.05).The overall effective rates of the treatment group after 7d,14d and 28d were 62.7%,79.5% and 94.0%,which of the control group were 51.2%,68.3% and 84.1% respectively.The differences of overall effective rates were statistically significant after 14d and 28d(χ2 =4.711,8.531,all P <0.05).Few reverse reactions were observed in both two groups.Conclusion Compared with the aspirin and atorvastatin therapy,combinational treatment of acute progressive cerebral infarction together with clopedogrel has a better efficacy,safety profile and significant promotion on neurological recovery.
6.The clinical progress of antisense technology in the treatment of cancer

According to the base complementation, the principle antisense technology is the sequence specific binding of complementary antisense nucleic acids or their chemical modification to the nucleic acids in cells, resulting in the suppression or prevention of gene translation. For the treatment of cancer, a lot of antisense drugs have been designed, which target the genes involved in the pathogenesis of cancer. And some of them have been in clinical trials. The author reviewed the progress of the clinical trials of these antisense drugs.
7.Clinical analysis of the effects of dexamethasone on ovulation induction in polycystic ovary syndrome

Objective To explore the effects of dexamethasone on ovulation induction in polycystic ovary syndrome (PCOS).Methods This clinical study included 68 women with PCOS, which of them were divided into two groups: 34 in dexamethasone treating(dexamethasone combined with ovulation induction) and 34 in con-trol(ovulation induction treatment only).Subsequently, comparisons were made between pre and after drug treat-ment in two groups, the main indexes were the levels of testosterone(T), luteinizing hormone(LH), follicle stim-ulating hormone(FSH), growth rate of follicle, and body mass index(BMI).And we have also considered the o-vulation rate and pregnancy rate, and compared these indexes in dexamethasone treating group with control group.Results There were significant differences between pre and after dexamethasone treatment in the levels of T in treatment group(P < 0.01), and there were not significant differences between pre and after treatment in the lev-els of T in control group (P >0.05).LH had decreased significantly after treatment in both two groups (P <0.01), FSH had not significant differences between pre and after treatment in two groups(P >0.05).The growthrate of follicle of two groups had elevated after treatment (P < 0.01).And the ovulation rate in dexamethasone treating group was higher than that in control group(P < 0.05), but there were not significant differences between the two groups in the clinical pregnancy rates(P >0.05).There were not significant differences between pre and after treatment in the levels of BMI in both two groups (P > 0.05).Conclusions Dexamethasone can decrease the T in plasm levels.It is good for ovulation induction in PCOS.
8.?-Lactamases Produced by Escherichia coli in Diabetic Patients:Their Classification and Distribution

OBJECTIVE To study the classification and distribution of the ?-lactamases produced by Escherichia coli in diabetic patients.METHODS To detect the ?-lactamases produced by E.coli in diabetic patients with a multi-disk synergy test.RESULTS Forty-nine strains of 60 strains in our study produced the ?-lactamases with total positive rate of 81.67%,From them 5 strains were producing penicillinases,17 strains producing BSBLs(28.33%),and 27 strains were producing ESBLs(45.00%),no carbapenem-hydrolyzing ?-lactamases were detected.In 27 strains producing ESBLs,the sensitivities to the ceftazidime,aztreonam and cefotaxime were similar.CONCLUSIONS The types of ?-lactamases produced by E.coli in diabetic patients are mainly BSBLs and ESBLs,without carbapenem-hydrolyzing ?-lactamases;the sensitivities to the ceftazidime,aztreonam and cefotaxime are similar.
9.Oxidative stress and type 2 diabetes

Insulin resistance and islet ? cell dysfunction are well-identified pathogenesis of type 2 diabetes. The generation of a great quantity of mitochondrial reactive oxygen species (ROS) impairs the function of mitochondria and induces oxidative stress during metabolic process of hyperglycaemia and hyperlipemia. Oxidative stress blocks insulin action pathway and induces insulin resistance; inhibit expression of insulin gene; decrease insulin secretion and impair ? cell function. In this paper, the mechanism of the ROS generation, insulin resistance and ? cell dysfunction induced by oxidative stress are reviewed.
10.Postoperative functional restoration of the elbow for adults with supracondylar or intercondylar fractures of the humerus

Objective To study the factors which affect the functional restoration of the elbow for adults with supracondylar or intercondylar fractures of the humerus treated with open reduction and internal fixation. Methods 22 patients with supracondylar or intercondylar fractures of the humerus were treated with open reduction and internal fixation from January, 1999 to December, 2004. They were followed up for an average of 30 months and complete clinical data were kept. The function of the elbow was evaluated with the Mayo elbow performance index. The relationship between the results and the factors —initial time of postoperative rehabilitation, fracture classification, operative approach and interval from injury to operation —were analyzed with cumulative logistic regression model of SAS software V8.2 (SAS institute, Cary, North Carolina). Results Of the 22 cases, 7 were excellent, 9 good, 5 fair and 1 poor. The excellent and good rates were 72.7%. And the odds ratio estimates of the 4 factors were 0.786, 0.299(B/A)0.221(C/A), 1.038, 0.197 respectively. Conclusions The initial time of postoperative rehabilitation, the fracture classification and the interval from injury to operation affect the functional restoration whereas the operative approach does not. Early operation, anatomical reduction, stable fixation and early rehabilitation will help improve the functional restoration.
