1.The therapy of children with secretory otitis media by myringotomy with grommet insertion and the relevant management

Objective: To improve the diagnosis and treatment level of secretory otitis media (SOM) of children. Method :Clinical information of 80 children with SOM were colletcted and analysed. All of the 80 cases (144 ears)were treated by myringotomy with Grommet insertion. Among them, 30 cases by adenotonsillectomy. 10 cases by palatoplasty. Secretions of 54 ears were carded out by bacterial culture. Result:80 cases were followed up for 6 to 12 mothes. 120 ears of 144 were cured (83%) ,with hearing improved. The tympanograms and hearing threshold of pure tone were found to be normal. The Gram's positive and negative germs of bactevial culture were 20.7% (6/29 samples ) and 24% (6/25 samples ) respectively. Conclusion: Children with secretory otitis media should be treated as early as posible to improve the patients' hearing and develope their speech.

Objective To study the relationship between cleft palate and secretory otitis media(SOM)and the change of hearing of cleft palate patients with SOM before and after tube insertion.Methods 203 cleft palate patients(406 ears)were investigated by pure tone audiometry,acoustic immitance audiometry and auditory brainstem response(ABR).21 cleft palate patients with SOM underwent palatoplasty and myringotomy with tube insertion at the same time.Results The incidence of middle ear disfunction was 68.5% in 203 cases with cleft palate,which in the cases under 6 years the incidence was 80%.After operation,21 cases were followed up one to three years,their average hearing improvement was 17dB by tube insertion( P

Objective To investigate the distribution and migration of bone mesenchymal stem cells (BMSCs) in the brain after intravenous injection into brain-ischemic rats and to explore the effects on the learning and memory in rats. Methods Bone mesenchymal stem cells from adult rats were cultured in vitro. Then the cells were labeled by Hoechst 33342 and infused into the adult rats on day 3 after the ischemia/reperfusion model was established by cerebral artery occlusion. Morris water maze test was used to explore the effects on learning and memory function of the rats at 3 weeks after transplantation. Fluorescence microscope was used to identify BMSCs in rat brains at 4 weeks after transplantation. 5.Expression of psoriasin in actinic keratosis, Bowen's disease and squamous cell carcinoma and its significance

ObjectiveTo investigate the expression of psoriasin in actinic keratosis(AK),Bowen's disease and squamous cell carcinoma(SCC) tissues.MethodsImmunohistochemistry was carried out to quantify the expression of psoriasin protein in tissue specimens from the lesions of 20 patients with AK,25 patients with Bowen' s disease,21 patients with well differentiated SCC,and16 patients with poorly differentiated SCC,as well as from the skin of 18 normal human controls.ResultsThe expression rate of psoriasin was 11.1% in the control specimens,significantly different from that in the AK(95.0%),Bowen's disease (88.0%),well differentiated SCC (95.2%),and poorly differentiated SCC (92.3%) specimens (all P <0.01 ).Psoriasin was expressed in the cytoplasm of keratinocytes and dyskeratotic keratinocytes in stratum corneum and upper 1 to 3 layers of stratum spinosums in AK tissue,in the cytoplasm of keratinocytes as well as the membrane and cytoplasm of vacuolated cells in all layers of the epidermis in Bowen's disease tissue.In well differentiated SCC,cornified pearl and dyskeratotic keratinocytes located in the stratum comeum and all layers of the stratum spinosums stained positive for psoriasin,and in poorly differentiated SCC,psoriasin was distributed in the stratum corneum and upper 1 to 5 layers of the stratum spinosums,but absent in poorly differentiated squamous cells.The expression intensity of psoriasin in tissues sequentially increased from AK,Bowen's disease to well differentiated SCC.Though the expression intensity of psoriasin in poorly differentiated SCC decreased,but was still higher than that in normal skin tissue(P < 0.05).ConclusionPsoriasin is abnormally expressed in skin disorders with dysdifferentiation of squamous cells.
6.Morphology and histology of eleven medicinal plant roots of Dendrobium Sw.

Objective To observe the histological structure of plant roots of Dendrobium Sw. for assisting identification of crude drug Herba Dendrobii. Methods Morphological and histological studies were carried out on 11 medicinal plant roots in Dendrobium Sw. by microstructural observation. The 11 species were divided into three groups according to their stem morphology: a) pair fleshy-stem group, including D. chrysanthum, D. crepidatum, D. primulinum, D. hercoglossum, and D. crystallium; b) thick-and rigid-stem group, including D. fimbriatum and D. aurantiacum var. denneanum; c) node-or inter- node- bulgy-stem group, including D. findlayanum, D. gratiosissimum, D. pendulum, and D. wardianum. The surface descriptions of velamen were conducted for D. fimbriatum and D. aurantiacum var. denneanum which are similar in characters of cross section. Results There were differences in shape and layers of velamen cell, number of xylem bundles, maximum diameter of vessels, organization of pith, as well as the types, quantity, and size of crystals. D. fimbriatum could be distingushed from its adulterant species D. aurantiacum var. denneanum by the different surface structure of velamen cells. Conclusion The histological characters of the roots can be used for assisting identification of Herba Dendrobii.
7.Risk factors for postoperative cognitive dysfunction after orthopedic surgery for adolescent idiopathic scoliosis

Objective To determine the risk factors for postoperative cognitive dysfunction (POCD) after orthopedic surgery for adolescent idiopathic scoliosis.Methods Sixty-five patients with adolescent idiopathic scoliosis,aged 11-18 yr,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,scheduled for elective orthopedic surgery with general anesthesia,were enrolled in this study.On admission to hospital (T0),before surgery (T1),at the end of surgery (T2) and on 2nd day after surgery (T3),blood samples were taken from the peripheral vein for determination of serum interleukin-10 (IL-10) and tumor necrosis factor-alpha (TNF-α) concentrations.Cognitive function was evaluated using a set of neuropsychological tests at 1 day before surgery and 7 days after surgery.Baseline patient characteristics such as age,gender,years of education and medical history,intraoperative fluid intake and output,surgery time,extubation time,postoperative visual analog scale score and related complications were recorded.Patients were divided into either POCD group or non-POCD group according to the diagnostic criteria of POCD.Logistic regression analysis was used to pick out the risk factors for POCD.Results Sixty-three patients completed this trial.The incidence of POCD was 24%.Compared with non-POCD group,the serum IL-10 concentration at T0 and serum TNF-α concentration at T3 were significantly increased,the IL-10/TNF-α ratio was decreased,and the length of postoperative hospital stay was prolonged in POCD group (P<0.05).The results of logistic regression analysis showed that increased concentrations of serum IL-10 at T0 and TNF-α at T3 were risk factors for POCD.Conclusion Increased serum concentrations of IL-10 on admission to hospital and TNF-α on 2nd after surgery are risk factors for POCD in the patients with adolescent idiopathic scoliosis.
9.Relation of adaptation to adverse childhood events and personality characteristics in college freshmen

Objective:To investigate the relationship between college freshman adaptation and adverse childhood events,and to explore the mediating role of personality characteristics.Methods:A sample of 523 college freshmen from a key university in Sichuan Province was assessed with the China College Student Adjustment Scale (CCSAS),Adverse Childhood Events Questionnaire (ACEQ) and Eysenck Personality Questionnaire-Revised,Short Scale for Chinese(EPQ-RSC).Results:The rate of adverse childhood events of college freshmen was 87% in current study.The total scores of adaptation were negatively correlated with the scores of adverse childhood events influence and neuroticism (r =-0.13,-0.51,Ps < 0.01),while positively correlated with introversion-extroversion and lie (r = 0.47,0.25,Ps < 0.01).The scores of adverse childhood events influence were negatively correlated with scores of introversion-extroversion (r =-0.08,P <0.05) and lie (r =-0.12,P <0.01),while positively correlated with scores of neuroticism (r = 0.17,P < 0.01).Introversion-extroversion and neuroticism mediated the relationship between adverse childhood events influence and college freshman adaptation,the mediating effects were 51.65% and 58.10%.Conclusion:College freshman adaptation may be closely associated with adverse childhood events influence and personality characteristics.Personality characteristics may play a mediating role between adverse childhood events influence and adaptation in college freshman.
10.Effects of Intensive Induction Chemotherapy on Consolidation Radiotherapy in Young Children with High-risk Hodgkin Lymphoma

OBJECTIVE:To investigate the safety of induction chemotherapy and the implementation method of consolidation radiotherapy in young children with high-risk Hodgkin lymphoma(HL),by evaluating the effects of intensive induction chemother-apy on consolidation radiotherapy. METHODS:Six pediatric patients with high-risk HL received low-dose involved field radiation therapy (IFRT) via volumetric modulated arc therapy (VMAT) following 6 cycles of intensive chemotherapy [Ara-C/VP16 (cyto-sine arabinoside+etoposide),ABVE-PC(adriamycin+bleomycin+leurocristine+etoposide+metacortandracin+cyclophosphamide)and CHOP(cyclophosphamide+leurocristine+adriamycin+ prednisone)in turn]. Therapeutic efficacy and toxic effects were evaluated af-ter treatment. RESULTS:Intensive induction chemotherapy and consolidation radiotherapy were acceptable by young children,and mild acute or subacute toxic reaction were observed. Intensive induction chemotherapy didn’t affect toxic effects of consolidation ra-diotherapy. In this regimen,the cumulative doses of bleomycin and adriamycin were 20 mg/m2 and 270 mg/m2,respectively. VMAT optimized plan to ensure that the dose that involved organs received was safe. In the patient with mediastional radiation therapy,the mean lung and heart doses were 525.6 cGy and 503.9 cGy,respectively. CONCLUSIONS:It is safe to give high-risk HL young children 6 cycles of intensive induction chemotherapy(Ara-C/VP16,ABVE-PC and CHOP in turn)before radiotherapy. It is feasi-ble to conduct IFRT with 18-20 Gy and an additional 20-25 Gy boost,1.5-1.8 Gy/fraction. VMAT deserves to be advised.
