1.Influential factors of coronary collateral circulation formation in patients with severe coronary artery stenosis

Objective To study the related factors and how they influence the formation of collateral circulation in patients with severe coronary arteries stenosis. Methods A total of 111 consecutive patients were catagorized into 3 groups according to their grading of collateral connections which were: CC0 (n=27), CC1 (n=45) and CC2 (n=39) respectively. Serum lipid levels were measured including total cholesterol, triglycerides, high density lipoprotein cholesterol and low density lipoprotein. All patients received UCG examination within one week after admission. Objective:To remove murine embryonic stem cells(mESC)from the differentiating cell culture and purify the differentiated cells by Magnetic Activated Cell Sorting(MACS).Methods:Neural differentiation of mESC was induced by a 5-stage method.The specific cell surface marker,SSEA-1,was used to identify ES cells in the differentiating cells.The optimal dilutions of mouse anti mouse SSEA-1 IgM primary antibody and FITC conjugated goat anti mouse secondary antibody were determined before the flow cytometry test.The incubation time and incubation temperature of primary antibody were all optimized to make the cytometry test accurate.After the optimization,stage 4 cells were dissociated into single cell suspension,incubated with antibody of SSEA-1 and microbeads conjugated goat anti mouse IgM,and then sorted through the magnetic field.The rate of SSEA-1 positive cells in pre-and post-separation groups was assessed by flow cytometry,and the viability of cells was evaluated by trypan blue staining counting under light microscopy.Results:The proportion of SSEA-1 positive cells in the separated cells can be reduced from(7.19?1.36)% to(1.34?0.80)%.The survival rate of sorted cells was more than 92%,similar to that of pre-separation cells.Conclusions:The MACS system we used can effectively remove mESC from the differentiated cells.The sorted cells will be well provided for the subsequent studies about transplantation therapy.
4.Impact of health education on compliance of children undergoing megacolon radical operation

Objective To observe the influence of health education on compliance of children nn-dergoing megacolon radical operation. Methods A questionnaire investigation was conducted on compli-ance of children undergoing megncolon radical operation before and after health education. ResultsCompliance of children patients and their family members increased after health education (P<0.05).Conclusions Health education could improve the compliance of children patients and their family mem-bers as well as the cognition about the disease knowledge so that the rehabilitation was facilitated.
5.Research on current situation and impact factors of the stigma among the community women with urinary incontinence in Jinan

Objective To descript the current situation and to analyze the impact factors of the stigma among the community women with urinary incontinence in Jinan.Methods This was a crosssectional survey by purposive sampling.506 women with urinary incontinence from 3 communities in Jinan were measured by the International Consultation on Incontinence Questionnaire-Short Form(ICIQ-SF) and the revised version of the Social Impact Scale(SIS)in order to get the information about the urinary incontinence type,severity degree and the stigma.Results The participants' total score of the SIS was (39.62±8.10) points and the score index was 55%.Score index of three subscale including social isolation,social exclusion and inner sense of shame were 58%,45%,70%; age and severity degree of urinary incontinence were independent factors and totally accounted for 14.0% of the variance in stigma.Conclusions It suggested that,to make patients drop the cognitive errors about being incontinence,then decrease the stigma and improve the quality of life,the Health Care Sector should positively broadcast the relative knowledge of urinary incontinence and encourages patients to seek help,screen,diagnose,and get treatment earlier,and complete the tertiary prevention.
6.Screening of infection of malaria parasite with, atypical eosinophil distributions in the WBC scattergram and alarm in Sysmex XE-2100 hematology analyzer

Objective To explore a quick and feasible method of screening malaria parasite by using a Sysmex XE-2100 hematology analyzer though alarm information on high eosinophil count and atypical eosinophil distributions in the WBC scattergram. Methods Sysmex XE-2100 hematology analyzer was used for complete blood cell analysis. Microscopic review was used when high eosinophil count and atypicaleosinophil distributions in the WBC scattergram were found. If the review showed normal eosinophil cells, wecontinued to focus on red cell for searching malaria parasite. Results Among 1 501 specimens showing higheosinophil counts and atypical eosinophil distributions, nine cases with normal eosinophil cells were indisaccordance with the hematology analyzer, six of them showed high eosinophil count in the Sysmex XE-2100 hematology analyzer, whose distribution was located close to neutrophil clusters in scattergram. The otherthree had an abnormal WBC scattergram. There was no gap between eosinophil clusters and neutrophilclusters, which brought no classified results. But all the nine specimens had been found the trophozoite,schizont and merozoite in blood smears. Conclusions There were great possibility of the existence of themalaria parasite in specimens when hematology analysis showed high eosinophil count and atypical eosinophildistributions in the WBC scattergram in a Sysmex XE-2100 hematology analyzer, although these alarm wasnot comfirmed by microscopic review. This method provides not only a simple and reliable way for quickscreening malaria parasite but also has a great value in preventing the undetected ratio on malaria parasite.
7.Influence of atrial fibrillation on intravenous thrombolysis of acute ischemic stroke patients within different time window

Objective To assess the influence of atrial fibrillation on post-thrombolytic hemorrhagic transformation and functional prognosis in acute ischemic stroke patients within different time window.Methods We retrospectively reviewed the clinical and imaging data of patients of acute ischemic stroke with intravenous thrombolysis admitted from June 2009 to October 2013.According to onset-to-needle time,we divided patients into 3 groups and then assessed the effect of the comorbidity with atrial fibrillation on the occurrence of hemorrhagic transformation and favorable outcome (defined as modified Rankin Scale score≤2 at 90 days) after thrombolysis within different time window.Results A total of 345 patients were included in this study,among whom 101 (29.3%) were treated by intravenous thrombolysis within 3.0 h (≤3.0 h),157(45.5%) >3.0 h and≤4.5 h,87(25.2%) over 4.5 h(>4.5 h).Atrial fibrillation was observed in 50.5% (51/101) patients in ≤3.0 h group,37.6% (59/157) in >3.0 h and≤4.5 h group and 40.2% (35/87) in > 4.5 h group (x2 =4.362,P =0.113).There were no statistically significant differences among these three groups about the rate of hemorrhagic transformation (hemorrhagic infarction:16.8% (17/101),22.3% (35/157),20.7% (18/87),and parenchymal hematoma:5.0% (5/101),10.2% (16/157),10.3% (9/87),x2 =4.278,P =0.370) and favorable outcome (51.5% (52/101),53.5% (84/ 157),47.1% (41/87),x2 =0.913,P =0.633).Multivariate analysis demonstrated that atrial fibrillation was associated with hemorrhagic infarction for patients in > 4.5 h group (OR =3.637,95% CI 1.101-12.013,P =0.034),and the presence of atrial fibrillation independently predicted parenchymal hematoma for patients in > 3.0 h and ≤4.5 h group (OR =3.757,95% CI 1.133-12.457,P =0.030).There was no significant association between atrial fibrillation and favorable outcome at 90 days.Conclusions The presence of atrial fibrillation is not associated with the prognosis in thrombolytic patients.However,it enhanced the risk of parenchymal hematoma if patients were treated within the time window > 3.0 h and ≤4.5h.
8.Debridement and suturing combined with double vacuum sealing drainage technology for treatment of pressure ulcer

Objective To explore the clinical efficacy of ebridement and suturing combined with double vacuum sealing drainage (VSD) technology for the treatment of pressure ulcer. Methods Totally 32 patients of pressure ulcer (from July 2011 to October 2015) re-ceived debridement and suturing after infection control. Then F14 silicone ventricular drainage tube was placed in the wound for drainage while VSD was placed outside the wound. Both tubes inside and outside wound were connected to the central vacuum (20~40 kPa). Three days after the drainage,F14 silicone ventricular drainage tube was removed,and VSD material outside wound was replaced to continuous vacu-um treatment for another 3 to 4 days. Results All of 32 cases were healed up without hematoma,dead space and flap margin necrosis,inclu-ding 4 cases of poor healing,which were healed after active dressing. Conclusion Small area of the pressure ulcer can be closed up by de-bridement and suturing combined with double VSD technology,which is a simple operation with little injury and high clinical application value.
9.Value of clinical parameters in predicting the initiation of renal replacement therapy in acute kidney injury patients with cardiorenal syndrome

Objective To investigate the value of clinical parameters in predicting the initiation of renal replacement therapy(RRT) in acute kidney injury (AKI) patients with cardiorenal syndrome (CRS).Methods A total of 75 AKI patients hospitalized with CRS were enrolled.All patients received pharmacologic therapy on the beginning 3 days.The patients whose heart function improved were divided into control group (n=39),and the patients whose heart function worsened were divided into RRT group (n=36).Clinical and laboratory data on the first day and the fourth day were collected and analyzed.The factors on the first day were labeled asⅠ ,and those on the fourth day were labeled asⅡ. The ratio of some parameters calculated were labeled asⅡ/Ⅰ .Area under curve (AUC) of receiver operating characteristic curve (ROC) of these factors was used to evaluate the sensitivity and specificity in predicting the initiation of RRT.Results The patients in RRT group had significantly higher levels of BNP-Ⅱ,BNP Ⅱ / Ⅰ and creatinine Ⅱ / Ⅰ (P < 0.01),and lower levels of 24 hours urine volume-Ⅰ and 24 hours urine volume-Ⅱ (P < 0.01).From ROC curve analysis,the AUC of 24 hours urine volume-Ⅰ,24 hours urine volume-Ⅱ,creatinine Ⅱ / Ⅰ,BNP-Ⅱ levels and BNP Ⅱ/Ⅰ to predict RRT were 0.736,0.875,0.747,0.779 and 0.894 respectively.When the cutoff values of 24 hours urine volume-Ⅰ,24 hours urine volume-Ⅱ,BNP-Ⅱ levels,BNP Ⅱ / Ⅰ and creatinine Ⅱ / Ⅰ were 905 ml (sensitivity 75%,specificity 94.9%),1450 ml (sensitivity 75%,specificity 100%),3360 ng/L (sensitivity 72.2%,specificity 100%),1.37 (sensitivity 75%,specificity 100%) and 1.25 (sensitivity 72.2%,specificity 94.4%) respectively,the value of the parameters to predict RRT was high.Conclusions The 24 hours urine volume,BNP levels after treatment and the dynamic changes of BNP levels and creatinine levels can be used as predictors of the initiation of RRT in the AKI patients with CRS.
10.Clinical application of cocktail double enzyme labeled antibody(D2-40/CK19)staining for the diagnosis of lymphatic vessel invasion in papillary thyroid carcinoma

Objective To analyse the relationship between lymphatic vessels invasion and clinical pathological features of papillary thyroid carcinoma ( PTC ) .Methods The expressions of D2-40 and CK19 were examined in the 104 specimens of PTC using immunohistochemical staining with combined monoclonal antibodies and cocktail double enzyme labeled antibody( D2-40/CK19) stainings.The two methods were compared in the diagnosis of PTC metastasis, and the factors affecting lymphatic vessels formation were analyzed.Results The positive rate of lymphatic vessels invasion was 37.5%(39/104) by using immunohistochemical staining with combined monoclonal antibodies and 53.8%( 56/104 ) by cocktail double enzyme labeled antibody ( D2-40/CK19 ) staining ( P<0.05).The lymph node metastasis rate was 83.9%(47/56) in the group with lymphatic vessels invasion, significantly higher than that without invasion 22.9%(11/48, P<0.01).The age of patients, diameter of primary tumor were the influence factors of lymphatic vessels invasion in PTC patients(P<0.05 and P=0.063).Conclusion Cocktail double enzyme labeled antibody ( D2-40/CK19 ) staining is a better method to detect lymphatic vessels invasion in PTC than immunohistochemical staining with combined monoclonal antibodies.
