1.Puncture Point Hemostatic Effect Observation of Different Materials with Modified Seldinger Technique in PICC Catheter.

OBJECTIVETo explore the improvement the dingle technology through the PICC catheter puncture point elbow hemostatic effect. Selection.
METHODSBetween January 2013 and December 2013, chest hospital affiliated to Shanghai jiaotong university under the guidance of ultrasound improved the Ding Gehang PICC catheter patients of 997 cases were randomly divided into three groups A, B, C, respectively, using gauze pad, calcium alginate wound dressings, calcium alginate wound dressings with hemostatic gauze pad three methods to puncture point, compare the three groups within 48 h after puncture biopsy in patients with some local bleeding, treatment times and catheter after 1 week of the maintenance costs of the catheter.
RESULTSCompared with A, B two groups, patients of group C tube after 48 hours the puncture point local oppression hemostasis effect is better than that of group A and B, the difference was statistically significant (all P < 0.05); Catheter maintenance: group C within 1 week after catheter tube after local lowest maintenance cost.
CONCLUSIONPICC for surgery after the puncture point of oppression hemostasis method choice, the effect of calcium alginate dressings hemostatic gauze pad is better than that of gauze pads and calcium alginate dressings, calcium alginate dressings and gauze pad is more effective and economic, in clinical use.

2.Side effects of interferon αin treatment of chronic hepatitis B and its clinical significance study

Objective To investigate the side effects of interferonαin treatment of chronic hepatitis B,and the analysis of its clinical significance. Methods 164 cases of patients with chronic hepatitis B were selected as research objects,who received medical treatment in Hospital of Jilin Agricultural University from December 2010 to December 2011.Clinical data of these patients was retrospectively analyzed.Adefovir dipivoxil was applied to 78 patients(group A),and peginterferon-α(PEG)was used in 86 patients(group B).Then efficacy and side effects of two interferon drug were observed.Results The HBsAg and HBV DNA negative rates of patients in group B was 4.65% and 59.30%,respectively.The negative rates of group A were 2.56% and 43.03%.HBV DNA negative rate of both groups had a significant difference by statistics(P<0.05 ),and improving effect of liver function of group B was also better than group A.After receiving IFN-αtreatment,patients of both groups all appeared side effects,including fever, leukopenia,neutropenia,depression and insomnia.The side effects incidence of both groups didn't show significant difference.Conclusion IFN-αis an effective antiviral drugs for chronic hepatitis B.long-acting interferon has a better efficacy,but this drug also lead to side effect easily.Therefore,it is essential to strengthen the relevant prevention and care in the clinical treatment.
3.Protective effect of puerarin on anoxia-reoxygenation induced cerebral cell injury in culture

Aim To study the protective effect of puerarin(PUE) on cultured cerebral cells injured by anoxia-reoxygenation. Methods The anoxia-reoxygenation injury model were developed, anoxia for 60 min and reoxygenation for 30 min. the effect of PUE on cerebral ultrastructure was observed.[Ca 2+ ]_i was estimated with Adherent Cell Analysis and Sorting 570(ACAS 570) Laser Cytometer and measured with fluorescent dye Fura-2-AM, the lipid fluidity of cellular membrane was determined by fluorescence polarization technique. Results PUE improved the ultra-structure of cerebral cells, dose-dependently decreased[Ca 2+ ]_i and increased the lipid fluidity of cellular membrane. PUE markedly reduced the chromaticity value of pseudo-colour graphic model of Ca 2+ . Conclusion Puerarin has protective effect on anoxia-reoxygenation induced cerebral cell injury. The effect may be mediated by decreasing[Ca 2+ ]_i and increasing the lipid fluidity of cellular membrane.
4.Risk early warning management in the prevention of peripherally inserted central catheter-related thrombosis in lung cancer patients

Objective To evaluate the effect of risk early warning management in preventing peripherally inserted central catheter (PICC)-related thrombosis in lung cancer patients,in order to improve the efficacy and safety of PICC application in lung cancer patients.Methods A total of 761 patients receiving PICC in Shanghai Chest Hospital between January 2012 and December 2013 were observed.Based on the assessment of PICC-related thrombosis risk using Autar scale,the patients were divided into 3 groups,namely low risk group (n =332),medium risk group (n =364),and high risk group (n =65).Patients in the 3 groups were given grading nursing intervention,physical intervention and basic drug intervention for low and medium risk groups,compared with physical intervention,basic drug intervention plus emergency intervention for high risk group.873 lung cancer patients with PICC in the period from January 2008 to December 2011 served as controls.Results 32 cases in the control group (3.66%) developed PICC-related thrombosis,compared with 12 cases in the observation group (1.58%) (P =0.009).The incidence of PICC-related thrombosis was 1.51% (5/ 332),1.37% (5/364),and 1.54 (1/65) in low,medium,and high risk groups,respectively.None of the patients developed pulmonary embolism.Conclusion Risk early warning management can help reduce the risk of PICC-related thrombosis in lung cancer patients in hypercoagulable state.

The mainstay of treatment of laryngopharyngeal reflux focuses on a concoction involving proton-pump inhibitor and modified beneficial behaviors, sometimes referring to medications like H2-R blocker etc. Therapeutic effect seems to be promising although may at times be challenged by conflict researches. Voice therapy and voice parameters emerge to provide a grand new perspective in treatment and evaluation of this disease.

9.Deep neck abscesses accompanied by multiple underlying diseases: 2 case reports.

Patients who complained of pain, tenderness mass in neck, fever, limitation of mouth opening, pharyngalgia,dysphagia,with lab showing increased Neu, remarkably decreased PLT, upsurging Glu, urine-Glu, ALT, T-Bil, AFP, were finally diagnosed to suffer from deep neck abscesses after CT scanning. What was worth mentioning was that diabetes, hepatitis C, thrombocytopenia, and fungus infection added to the complexity of the treatment.
