2. QRS duration in evaluating ventricular dyssynchrony in patients with chronic systolic heart failure

Objective: To observe the types, prevalence of cardiac dyssynchrony in patients of chronic systolic heart failure (SHF) , and to investigate the value of QRS duration >135 ms in selecting patients for cardiac resynchronization therapy (CRT). Methods: Forty-five patients with SHF were randomly divided into two groups according to QRS width: Group 1 (QRS width <120 ms, 21 patients) and Group 2 (QRS width ≥120 ms, 24 patients). Electrocardiography, conventional echocardiography, and tissue doppler were performed in all patients to evaluate the cardiac systolic dyssynchrony. Meanwhile, the diagnostic value of optimized QRS width to evaluate the dyssynchrony indices was tested while using 135 ms as new cutoff value. Results: Systolic function: compared with Group 2, Group 1 had significantly higher LVEDD and LVEDV(P=O. 03 and 0. 01, respectively), lower LVEF(P=O. 004), and had significantly higher NYHA class(P = 0. 002) and more frequent use of diuretics and digoxins( P = 0. 01, both). Dyssynchrony parameters. Group 2 had a significantly higher prevalence of interventricular dyssynchrony (70. 8% vs 15%, P<0. 000 1) and higher IVMD value(P<0. 000 1) than Group 1 ; the prevalences of intraventricular dyssynchrony were high in both groups(71. 4% vs 79. 2%, P=O. 55), except that the APET value was prolonged in Group 2. Other indices(Ts-SD, Ts-12, and SPWMD) showed no significant difference between the two groups. Correlation analysis, a strong correlation was found between QRS duration and interventricular dyssynchrony (r=0. 58,P<0. 000 1), while it was fairly poor between QRS duration with intraventricular dyssynchrony(r=0. 07,P= 0. 64). ROC analysis: QRS width >135 ms could finely predict the presence of interventricular dyssynchrony, with a sensitivity of 80% and a specificity of 87. 5% ; while the same cutoff value to predict intraventricular dyssynchrony only yielded a sensitivity of 44. 1 % and specificity of 73. 6%. Conclusion: Intraventricular dyssynchrony and(or) interventricular dyssynchrony has a high prevalence in patients with SHF. A QRS duration cutoff value higher 135 ms can well evaluate the cardiac mechanical dyssynchrony in clinical practice, which may be of value for optimizing selection of CRT candidates and reducing the nonresponders.

Recommendation: Emphasize and build mental health service institutions in communities, and improve their functionality, establish an effective early warning and intervention mechanism for community mental health services in communities, enhance health education, health promotion and mental health knowledge education, and carry out the tertiary prevention and tertiary functionality principles for mental health. These measures can prevent community residents from mental illness, improve their mental health, and promote the development of mental health services in Chinas communities.
7.Expression of MMP-9 and TIMP-1 Proteins in the Decidua and Villi of Early Pregnancy

Objective To study the expression of matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1(TIMP-1) proteins in the decidua and villi of the early pregnancy. Methods Immunohistochemical staining was applied to detect the expression of MMP-9 and TIMP-1 proteins in the decidua and villi of 45 the early pregnant women and in the mid-secretory phase endometrium of 30 normal women. Objective To investigate the effect of nerve growth factor(NGF)in the process of muscular neurotization of denerva- ted vascularized skeletal muscle.Method By using NGF(0.5 ?g/d) to transplanted skeletal muscle,muscular neurotization was exa- mined with histological,histochemical,electron microscope and immunohistochemical methods at various time.Result NGF was effective in promoting muscular neurotization and diminishing denervated muscle atrophy in grafted denervated skeletal muscle.Conclusion NGF can promote the muscular neurotization and diminish denervated muscle atrophy.
10.The dynamic observation on splenocyte cytokines in mice immunized with the transgenic alfalfa containing Eg95-EgA31 fusion gene of Echinococcus granulosus

Objective To investigate the dynamic changes of splenocyte cytokines in mice immunized with the transgenic alfalfa containing Eg95-EgA31 fusion gene of Echinococcus granulosus (Eg). Methods Eighty-eight Balb/c mice were divided into 2 groups randomly according to body weights,and immunized orally or intranasally with 100μl or 10μl extracted leaf protein from the transgenic alfalfa(20 g/L) respectively once per 3 days for 2 months. Four mice randomized from each group were killed to get splenocyte on week 0(control),2,4,6,8,10,12,14,16,18 and 20 after the last immunization. The splenocyte were cultured in medium for 48 hours with EgAg or concanavalin A (ConA) stimulation to induce the interleukin (IL)-12,interferon γ(IFN-γ) and IL-10,and cultured for 72 hours with EgAg or lipopolysaccharide (LPS) stimulus to induce the tumor necrosis factor α (TNF-α). Then the supernatant was collected to measure the level of IL-12,IFN-γ,TNF-α and IL-10 by ELISA. Results In the oral immunization group,the level of IL-12,IFN-γ,TNF-α and IL-10 increased significantly from week 4 to week 6,week 2 to week 8,week 2 to week 6 and week 4 to week 12,respectively,reaching the highest level(25.0±5.8)ng/L on week 4,(575.0±28.9)ng/L on week 2,(50.0±11.5)ng/L on week 2 and (42.5±2.9)ng/L on week 8,respectively,as compared with the values on week 0[(11.3±2.5),(125.0±28.9),(11.3±2.5),(12.5±2.9)ng/L,all P < 0.01]; in the intranasal immunization group,it was similar about the values of IL-12,IFN-γ,TNF-α and IL-10 could be seen from week 4 to week 6,week 2 to week 10,week 4 to week 10 and week 6 to week 16,respectively,reaching the highest level(25.0±5.8)ng/L on week 6,(725.0±28.9)ng/L on week 4,(27.5±2.9)ng/L on week 6 and (60.0±11.5)ng/L on week 6,respectively,as compared with the values on week 0[(11.3±2.5),(125.0±28.9),(11.3±2.5),(12.5±2.9)ng/L,all P < 0.01]. The cytokine levels in the groups with EgAg,ConA or LPS stimulus were significantly higher than those in the corresponding splenocyte suspension groups(P < 0.05 or < 0.01),and the cytokine levels in the groups with ConA or LPS stimulus were obviously higher than those in the corresponding groups with EgAg stimulation (P < 0.05 or < 0.01). Conclusion The mixed responses of Th1 and Th2 types can be induced in mice immunized with the transgenic alfalfa in the early period post immunization(2-10 weeks).
