1.Scar hidden in laparoscopic appendectomy with suspending fixation

Objective To evaluate the feasibility and efficacy of suspended method in scar hidden technology in laparoscopic appendec-tomy ( LA) . Methods 27 patients underwent scar hidden laparoscopic appendectomy with the aid of suspended method. Silver probe was pierced abdominal cavity,bended into a hook shape and fixed appendix,then underwent appendectomy. Results 27 patients finished surgery successfully,the operative time was 25~100 min ( mean, 45 min) ,the blood loss during operation was 10~50 mL ( mean,20 mL) ,the hos-pital stay was 3~8 d ( mean,4 d) . Conclusion Scar hidden technology in laparoscopic appendectomy with the aid of suspended method is safe,feasible and further improves the cosmic result.
2.Incidence of Atrial Fibrillation after Off-pump versus On-pump Coronary Artery Bypass Grafting: A Meta-analysis of Randomized Clinical Trials and Propensity Score Matching Trials

The association between atrial fibrillation (AF) after coronary artery bypass grafting (CABG) and the surgical techniques selected has been extensively reported.However,no consistent results were obtained.In the present study,a meta-analysis was conducted by searching the electronic databases PubMed,Embase,Web of Science,and Cochrane to identify the association of post-CABG AF with on-pump (conventional CABG,cCABG) or off-pump CABG (OPCABG).Outcomes from randomized clinical trials (RCTs) and propensity score matching (PSM) trials were pooled by using the fixed-effect or the random-effect modeling method,and verified by the quality-effect modeling method.There were 35 studies with 36 independent reports that met the inclusion criteria and were eventually included in our meta-analysis.The total odds ratio (OR) of the incidence of post-CABG AF between OPCABG and cCABG was 0.80 (95% CI 0.71-0.91).The 25 randomized clinical trials (RCTs) had an OR of 0.69 (95% CI 0.56-0.86),while the OR of the 11 PSM trials was 0.88 (95% CI 0.77-1.00).Twenty-six studies involving the patients at a mean age no more than 65 years showed an OR of 0.76 (95% CI 0.64-0.90),whereas 10 studies with patients greater than 65 years old showed an OR of 0.90 (95% CI 0.78-1.05).The results of this meta-analysis suggest that OPCAB surgery may reduce the incidence of post-CABG AF when compared to cCABG and that younger patients may benefit more from OPCAB and have a lower incidence ofpost-CABG AF.
3.Effects of electroacupuncture of different frequencies on free radicals in hippocampus of mice with vascular dementia

Objective:To observe the effects of electroacupuncture (EA) of three different frequencies (2 Hz,80 Hz and 2 Hz/80 Hz) on the free radicals in hippocampus of vascular dementia (VD) model mice.Methods:A total of 100 Kunming mice were randomly divided into a sham operation group,a model group,a 2 Hz EA group,an 80 Hz EA group and a 2 Hz/80 Hz EA group,with 20 mice in each group.The ischemia-reperfusion VD model was established by repeated blockade of bilateral common carotid arteries.Mice in EA groups began EA treatment on the 4th day after the operation.Baihui (GV 20),Dazhui (GV 14),Geshu (BL 17) and Zusanli (ST 36) were punctured and then connected to EA instrument,with different waves of 2 Hz,80 Hz or 2 Hz/80 Hz (10 min/time) applied accordingly,once a day.During the jumping stand experiment,the learning performance,memory performance and hippocampal calcitonin gene-related peptide (CGRP),nitric oxide synthase (NOS),malondialdehyde (MDA),changes in superoxide dismutase (SOD) and true choline esterase (TChE) were observed.In hippocampus,the CGRP level was determined by radioimmunoassay;the MDA level was determined by thiobarbituric acid colorimetric method;the activities of NOS and TChE were determined by spectrophotometry;the activity of SOD was determined by xanthine oxidase method.Results:Compared with the sham operation group,the performances of learning and memory decreased significantly in the model group (P<0.01);in hippocampus,the CGRP level decreased,the MDA level increased,the activities of NOS and TChE increased,and the activity of SOD decreased in the model group.Compared with the model group,the learning and memory performances of the EA groups were significantly improved (P<0.05 or P<0.01);in hippocampus,the CGRP level increased,the MDA level decreased,the NOS and TChE activities decreased,and the SOD activity increased (P<0.05 or P<0.01).Among EA groups,the 2 Hz/80 Hz EA group was superior to the 2 Hz EA group and the 80 Hz EA group (P<0.05 or P<0.01).Conclusion:EA can improve the cognitive impairment of mice with ischemia-reperfusion VD.The mechanism may be related to the improvement of cerebral blood circulation,regulation of the central neurotransmitters,fighting lipid peroxidation and promoting nerve cell repair.The therapeutic effects of EA with different frequencies were different,and the intervention effect by EA at 2 Hz/80Hz is the most significant.
4. Computer-assisted three dimensional CT analysis of mandibular morphology in female patients with prominent mandibular angle

Objective:
To measure and analyze the bone volume and thickness of mandibular angle area by computer-assisted CT.
Methods:
From January 2010 to January 2016, 94 female patients with mandibular angle hypertrophy and bilateral mandibular angle osteotomy were selected. Twenty female patients with non-mandibular surgery were selected as control group (patient examined after nasal augmentation surgery), aged 18 to 37 years, with an average of 27.5 years. 5.Effect analysis of transconjunctival lower blepharoplasty combined with stromal vascular fraction-gel transplantation in suborbital rejuvenation

Objective:To investigate the effect of transconjunctival lower blepharoplasty combined with stromal vascular fraction-gel (SVF-gel) transplantation in suborbital rejuvenation.Methods:The clinical data of patients with suborbital rejuvenation who were admitted to the Department of Plastic Surgery, Wuhan No.1 Hospital from April 2019 to April 2021 were retrospectively analyzed. Hirmand classification of tear trough deformity was performed based on preoperative photographs. The expanded orbital septum fat was removed through the conjunctival approach, and the SVF-gel was extracted from the fat which was harvested from abdomen or thigh. The tear trough or suborbital depression was corrected by SVF-gel injection. The patients were followed up and the complications related to the operation were recorded. According to tear trough rating scale (TTRS), the depth of tear trough depression, the degree of pigmentation, the sagging degree of zygomatic fat pad and the degree of lower eyelid skin fold were scored before and after operation. The higher the total score indicates the greater severity of the lacrimal groove deformity. The morphology of the orbital-buccal junction was rated (0 to 3) according to the Barton aesthetic rating criteria. The aesthetic effects of lower eyelid bag and tear trough correction were comprehensively analyzed based on the satisfaction questionnaire (very satisfied, satisfied, dissatisfied 3 grades). SPSS 22.0 software was used for statistical analysis, and the normally distributed measurement data were represented by Mean±SD. Paired sample t-test was used to compare preoperative and postoperative TTRS scores, and P<0.05 was considered statistically significant. Results:A total of 92 patients with bilateral lower eyelid bag accompanied by tear trough or middle facial depression were included. There were 15 males and 77 females, aged (38.8±9.6) years (21-55 years). Preoperative Hirmand tear trough classification was performed as type Ⅱ in 37 cases, as type Ⅲ in 55 cases. All patients had conjunctival edema and lower eyelid swelling of different degrees 3 days after surgery, among which 35 patients had skin ecchymosis of different degrees, which recovered spontaneously 1 to 2 weeks after surgery, and no complications such as hematoma, diplopia, fat liquefaction, fat nodule and fat embolism occurred. Postoperative follow-up was conducted for 6 to 12 months. Comparison of preoperative and postoperative TTRS scores: compared with preoperative results, the TTRS scores of Hirmand type Ⅱ patients [(2.37±0.78) points vs. (4.83±0.98) points, t=36.54, P<0.001] and type Ⅲ patients [(2.61±0.60) points vs. (5.38±0.93) points, t=55.20, P<0.001] were significantly decreased after surgery, and the differences were statistically significant. Comparison of Barton aesthetic rating before and after surgery: for Hirmand type Ⅱ patients, preoperative rating was grade 1 in 7 cases, grade 2 in 19 cases, grade 3 in 11 cases, and postoperative improvement was grade 0 in 36 cases and grade 1 in 1 case; for Hirmand type Ⅲ patients, preoperative ratings were grade 2 in 20 cases, grade 3 in 35 cases, and postoperative improvement was grade 0 in 50 cases, grade 1 in 5 cases. Postoperative patient satisfaction survey: 85 cases (92.4%) were very satisfied, 5 cases (5.4%) were satisfied, 2 cases (2.2%) were dissatisfied. For the 2 patients who were not satisfied, one patient had no obvious improvement due to tear trough deformity, and was filled with SVF-gel for the second time 3 months later, achieving satisfactory results. One patient still had baggy eye on the outer side of the lower eyelid after surgery, which was corrected again 3 months later and achieved satisfactory results. Conclusion:For patients with lower eyelid bag with tear trough or suborbital depression without obvious skin relaxation, lower eyelid bag correction through conjunctival approach combined with SVF-gel transplantation can achieve the effect of rejuvenation of the suborbital region.

7.Mucosa advancement flap anoplasty in treatment of chronic anal fissures: a prospective, multicenter, randomized controlled trial.

Background: Anal fissure is one of the most common anal-rectum diseases, and approximately 10 percent patients with chronic anal fissure ultimately receive surgery. Relieving postoperative pain and protecting functions of the sphincter are central issues for coloproctologists. Objective: To evaluate the efficacy and safety of anoplasty in the treatment of chronic anal fissures. Design, setting, participants and interventions: In this prospective, multicenter, randomized controlled trial, 120 adult patients with chronic anal fissure were referred from Department of Coloproctology of Yueyang Hospital of Integrated Traditional Chinese Medicine Affiliated to Shanghai University of Traditional Chinese Medicine, Shanghai Ninth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine and Shanghai Municipal Hospital of Traditional Chinese Medicine. The patients were enrolled from January 2009 to April 2010 and randomly divided into study (mucosa advancement flap anoplasty, abbreviated as anoplasty) group and control (fissurectomy) group. The two groups were assessed separately, and the main outcome measures were observed for 2 weeks, with a short-term follow-up for 6 weeks. Main outcome measures: Degree of pain, haemorrhage and anal canal pressure were observed and recorded preoperatively, and on the third day, the fourteenth day and the sixth week postoperatively. The wound healing time was also recorded. Surgical complications of the two groups were recorded and compared on the third day and the sixth week postoperatively. The curative effects associated with the surgery were analyzed on the fourteenth day and the sixth week after surgery and the therapeutic results were evaluated. Results: Three patients were dropped out due to the early discharge from hospital and losing connection (1 in study group and 2 in control group). Overall the surgery showed that the anoplasty group had better results than the fissurectomy group in the curative effect on the sixth week after operation (P<0.05). Time of wound healing in the anoplasty group was (17.22±4.41) d and was better than (21.24±7.44) d of the fissurectomy group (P<0.05). Concerning the relief of wound pain, the anoplasty group achieved better results than the fissurectomy group at the third day, the fourteenth day and the sixth week after operation (P<0.05). Anoplasty reduced bleeding and had better efficacy than the fissurectomy at the third day and the fourteenth day after operation (P<0.05), however, there was no statistical difference at the sixth week after operation (P>0.05). There were no significant differences in relieving the anal canal pressure (P>0.05) and the surgical complications (dysuria, edema of anal margin, fever, infection, anal incontinence and anal deformation) between the two groups (P>0.05). None of the patients suffered postoperative complications by the sixth week after operation. Furthermore, there was no recurrence in either of the two groups at six weeks after operation. Conclusion: The results indicate that anoplasty for chronic anal fissures has advantages such as better therapeutic effects, less postoperative pain, a shorter healing time and no incidence of anal incontinence.
8.Increased coronin-1C expression is related to hepatocellular carcinoma invasion and metastasis.

OBJECTIVETo search for hepatocellular carcinoma (HCC) invasion related biomarkers using the cell membrane proteomics approaches, and to validate the markers using experimental and clinical specimens.
METHODSThe HCCLM9 and MHCC97L cells with a similar genetic background and remarkably different metastasis behaviors were used for comparative membrane proteome profiling using sodium dodecyl sulfate polyacrylamide gel electrophoresis and electrospray ionization mass spectrometry technologies. Candidate protein makers were further validated by western blot on cells, immunohistochemistry (IHC) on animal tumor tissues, and tissue micro-array on clinical specimens.
RESULTSThe membrane proteins of MHCC97L and HCCLM9 cells were compared by sodium dodecyl sulfate polyacrylamide gel electrophoresis analyses. 14 proteins were identified by ESI-MS/MS among the differential bands. Coronin-1C was overexpressed in HCCLM9 (7.31+/-0.73) versus MHCC97L (2.84+/-0.99) validated by western blot. Elevated coronin-1C expression was observed in liver cancer tissues of HCCLM9 nude mice. IHC study in 115 human HCC specimens demonstrated that patients with higher coronin-1C expression had more advanced stage.
CONCLUSIONThe study suggests that coronin-1C could be a potential molecule to predict HCC invasive behavior.

9.Multiple viral transmission chains of enterovirus 71 co-circulated in Jilin Province during 2009-2010

Objective To analyze the genetic characterization of enterovirus type71 ( EV71 )associated with hand foot and mouth disease (HFMD) epidemics in Jilin province,during 2009-2010.Methods Randomly selected 31 representative EV71 strains from the eases of 8 prefectures to amplify and sequences of VP1 genes of EV71 strains,and analyzed with Bioedit and Mega4.0 program.Results All representative 31 EV71 strains belong to C4a subgenotype,the homology of nucleotide in VP1 region among the 31 EV71 strains were 94.5%-100.0%,and were clustered into 5 transmission chains respectively.25 strains out of 31 strains were associated with a predominant transmission chain,and circulating in 8 prefectures,while other 6 strains clustered into 4 lineages.Conclusion Multiple transmission chains of EV71 C4a subgenotype were co-circulating in Jilin province during 2009-2010,and a predominant transmission chain was circulating in 8 prefectures,associated with HFMD outbreaks of Jilin province.
10.Detecting anti-megakaryocyte antibodies in serum of systemic lupus erythematosus patients by indirect immunofluorescence.

This study was purposed to investigate the mechanism of thrombocytopenia in patients with systemic lupus erythematosus (SLE) through detecting anti-megakaryocyte antibodies in SLE patients. The serum anti-megakaryocyte antibodies in 36 SLE cases with thrombocytopenia were detected by using indirect immunofluorescence, the detected results were compared with detected results of 30 SLE cases without thrombocytopenia and 30 healthy persons. The results showed that the positive incidences of anti-megakaryocyte antibody in serum of 36 SLE cases with thrombocytopenia, 30 SLE cases without thrombocytopenia and 30 healthy persons were 19.4% (7/36), 6.7% (2/30) and 3.3% (1/30) respectively. As compared with SLE patients without thrombocytopenia and healthy persons, SLE patients with thrombocytopenia had higher incidence of anti-megakaryocyte antibodies, moreover there was significant difference between SLE patients with thrombocytopenia and healthy persons (p < 0.05), while there was no significant difference between SLE patients with or without thrombocytopenia (p > 0.05). It is concluded that autoantibodies against megakaryocytes exist in SLE patients and may partially contribute to the incidence of thrombocytopenia in SLE patients. The detection of anti-megakaryocyte antibodies with a enough case number is needed to make a final conclusion on thrombocytopenia pathogenesis in SLE.
