We herein report a witnessed cardiopulmonary collapse of a patient with difficult mask ventilation and near-impossible laryngoscopy-cum-intubation in the critical care unit. The airway was successfully rescued with a laryngeal mask airway Supreme™, followed by an open, crash tracheostomy by the otolaryngologist.

Fuzi is a medicine used for rescuing from collapse by restoring yang as well as a famous toxic traditional Chinese medicine. In order to ensure the efficacy and safe medication, Fuzi has mostly been applied after being processed. There have been different Fuzi processing methods recorded by doctors of previous generations. Besides, there have also been differences in Fuzi processing methods recorded in modern pharmacopeia and ancient medical books. In this study, the authors traced back to medical books between the Han Dynasty and the period of Republic of China, and summarized Fuzi processing methods collected in ancient and modern literatures. According to the results, Fuzi processing methods and using methods have changed along with the evolution of dynasties, with differences in ancient and modern processing methods. Before the Tang Dynasty, Fuzi had been mostly processed and soaked. From Tang to Ming Dynasties, Fuzi had been mostly processed, soaked and stir-fried. During the Qing Dynasty, Fuzi had been mostly soaked and boiled. In the modem times, Fuzi is mostly processed by being boiled and soaked. Before the Tang Dynasty, a whole piece of Fuzi herbs or their fragments had been applied in medicines; Whereas their fragments are primarily used in the modern times. Because different processing methods have great impacts on the toxicity of Fuzi, it is suggested to study Fuzi processing methods.

The ABCD2
score is validated for evaluating short-term stroke risk after transient ischemic attack
(TIA); however, whether it is able to predict the long-term risk of vascular outcome remains uncertain.
Recently a new tissue-based definition of TIA has been proposed. The ABCD2
scores of 145 TIA
patients admitted to our hospital were retrospectively calculated and stratified into two categories:
≤ 3 points (low risk); 4-7 points (moderate-high risk). At a median follow-up of 81 months, new
vascular events were recorded. Follow-up data were available in 107 patients. Seventy one patients
had a moderate-high ABCD2
score. Sixty six patients experienced a cerebral ischemic event; 8 a
myocardial infarction; 7 died of cerebrovascular or cardiovascular cause. Moderate-high ABCD2
score
was significantly associated with the further cerebral ischemic events (hazard ratio [HR], 1.755; 95%
confidence interval [CI], 1.019 to 3.024) and with the combined endpoint (HR, 1.818; 95% CI, 1.079
to 3.063). Our study shows that the ABCD2
score may also be used to predict long-term vascular
outcome after tissue-based definition of TIA. Moderate-high ABCD2
score is associated with an
increased general vascular risk in the long-term follow-up after TIA.

Objective To investigate the effects of emulsified isoflurane preconditioning on myocardial NF-κB activity during ischemia-reperfusion(I/R)in rats. Methods Forty-eight healthy male SD rats weighing 230-280 g were randomly divided into 4 groups with 12 animals in each group: sham operation group(group S),I/R group, lipid emulsion + I/R group(group L)and emulsified isoflurane + I/R group(group EI).In group I/R,EI and L,myocardial I/R was produced by occlusion of left coronary anterior descending artery(LAD)for 30 min followed by 120 min reperfusion. In group L,30% lipid emulsion 4 ml·kg-1 ·h-1 was infused intravenously for 30 min before myocardial I/R.In group EI,emulsified isoflurane 4 ml· kg- 1 · h- 1 was infused intravenously for 30 min followed by 15 min washout before myocardial I/R.In group S and I/R,normal saline was given instead.Blood samples were taken from femoral artery at the end of 120 min reperfusion for determination of serum cTnI and IL-6 concentrations and CK-MB activity by ELISA.The rats were then killed and the myocardial tissues were taken for determination of NF-κB activity by Western blot and observation of the ultrastructure by electron microscopy. Results The NF-κB activity, serum cTnI and IL-6 concentrations and CK-MB activity were significantly higher in group I/R,EI and L than in group S(P < 0.05 or 0.01),while lower in group EI than in group I/R(P < 0.05).Microscopic examination showed that emulsified isoflurane significantly attenuated the histopathological changes in group EI.Conclusion Emulsified isoflurane pretreatment can attenuate myocardial I/R injury through decreasing the NF-κB activity and inhibiting inflammatory response in rats.

AIM: To investigate the anti-inflammatory effect of partial liquid ventilation in the piglets with lung lavage-induced acute lung injury. METHODS: 16 piglets were treated by lung lavage to a partial pressure of oxygen in arterial blood (PaO_2) below 100 mmHg for one hour and randomly divided into mechanical ventilation group (MV group) and partial liquid ventilation group (PLV group). Blood-gas analysis, hemodynamics, the contents of TNF-?, MDA, SOD, and MPO activity in lung tissue were measured. RESULTS: (1) Lung W/D, PPI and the count of WBC in the BALF in PLV group were significantly lower than those in MV group. The contents of MDA and MPO in lung tissue in PLV group were significantly lower than those in MV group. However, no evident difference in SOD activity in lung tissue between two groups was observed. (2) The contents of TNF-? in lung tissue in PLV group were significantly lower than those in MV group. CONCLUSION: Partial liquid ventilation with perfluorocarbon has anti-inflammatory effect on lung lavage-induced acute lung injury in the piglets.

AIM: To observe the effect of partial liquid ventilation with perfluorocarbon on gas exchange, hemodynamics and lung histological change in the piglets with surfactant depletion-induced acute lung injury. METHODS: 12 piglets were treated by lung lavage to a partial pressure of oxygen in arterial blood (PaO 2) below 100 mmHg for one hour and randomly divided into gas ventilation group (control group) and partial liquid ventilation (PLV) group, in PLV group, piglets received PFC (FC3280) intratracheally at doses of 15 mL/kg. The parameters of gas exchange and hemodynamics were measured before lung lavage, after lung lavage when the acute lung injury (ALI) was established, and 1 hour, 2 hours after ALI. Animals underwent euthanasia at the end of the study, lung histologic analysis followed. RESULTS: Surfactant depletion by lung lavage induced a stable acute lung injury.Gas exchange increased markedly in the animals that underwent PLV, less hemodynamic damage was observed in PLV group compared with the animals in GV group. Lung histologic analysis demonstrated a less lung damage, including atelectasis, neutrophil excudation, intra-alveolar hemorrhage and interstitial edema in PLV group compared with control group. CONCLUSION: In piglets with surfactant depletion-induced acute lung injury, partial liquid ventilation with perfluorocarbon can improve the gas exchange with less adverse hemodynamic effect and less lung injury compared with conventional gas ventilation.
