OBJECTIVE:To evaluate the efficacy of acarbose treatment in impaired glucose tolerance (IGT)and provide evi-dence-based reference for the clinical treatment. METHODS:Medline,Cochrane library,PubMed,EMBase,VIP database,CJFD and Wanfang database were retrieved to collect the randomized controlled trials(RCT)of comparison treatment(test group)of acar-bose and placebo(control group)in IGT. After the information collection and quality evaluation,Rev Man 5.0 was conducted for Mata-analysis. RESULTS:26 RCT,involving 2 221 patients were include in total. Meta-analysis result showed,compared with place-bo,acarbose could reduce FPG[WMD=-0.76,95%CI(-0.99,-0.53),P<0.001],2 h PG[WMD=-1.58,95%CI(-1.82,-1.34), P<0.001],HbA1c[WMD=-0.46,95%CI(-0.82,-0.11),P=0.01],BMI[WMD=-1.10,95%CI(-1.77,-0.44),P=0.001], TC[WMD=-0.35,95%CI(-0.59,-0.12),P=0.003],TG[WMD=-0.30,95%CI(-0.44,-0.16),P<0.001] significantly;it had no significant difference on HDL-C[WMD=-0.05,95%CI(-0.11,0.01),P=0.11] and LDL-C[WMD=-0.13,95%CI(-0.31, 0.05),P=0.17]. CONCLUSIONS:Acarbose has better efficacy in the treatment of IGT. Due to the low quality of included studies, it remains to be further verified by RCT with large sample and high quality.

Objective To study the anti-depression mechanism of Dingzhixiaowan concerning on histology of hippocampal formation.Methods 40 female rats were divided into control,depression model group(model group),distilled water group(DW group) and Dingzhixiaowan group(Chinese herb group).Behavior points were measured by open field method.Serum concentrations of cortisol and estradiol(E2) were checked with ELISA.HE staining and electron microscopy were applied to observe the histology of neurons in hippocampal formation.Immunohistochemistry method was employed to detect Nestin expression.Results Compared with the control,behavior points,serum E2 level and Nestin expression decreased obviously and serum cortisol level increased markedly,CA3 neurons were seriously damaged in model and DW groups.While behavior points,serum E2 level did not change,Nestin expression increased obviously and serum cortisol level decreased markedly,CA3 neurons were basically normal in Chinese herb groups.Conclusion Dingzhixiaowan can protect hippocampal formation,which might be one of the anti-depression mechnisms.

Objective To investigate the human papillomavirus 16/18 (HPV16/18) infective status and its relationship with the expression of p16,CK17,Ki-67 in immature squamous metaplasia(IM),cervical intraepithelial neoplasia (CIN) and cervical carcinomas (CC).Methods In situ hybridization (ISH) and immunohistochemistry were applied to detect the expression of HPV16/18,p16,CK17,Ki-67 on tissues from 30 IM,60 CIN and 30 CC.Results The expression rates of HPV16/18 in LSIL,HSIL and CC were 30 ％ (9/30),60 ％ (18/30) and 77 ％ (23/30),higher than that in IM [6.7 ％ (2/30)].HPV16/18 was positively expressed with the progression of cervical squamous epithelial lesions.The expression rates of CK17 in IM was 90 ％ (27/30),higher than that in LSIL,HSIL and CC [17 ％ (5/30),10 ％ (3/30),6.7 ％ (2/30)].The expression rates of p16,Ki-67 were 83 ％ (25/30) and 87 ％ (26/30) in LSIL,90 ％ (27/30) and 93 ％ (29/30) in HSIL,97 ％ (29/30) and 97 ％ (29/30) in CC,higher than that in IM [13 ％ (4/30) and 10 ％ (3/30)].The expression of p16,Ki-67 were particularly seen in HPV16/18-positive HSIL and CC,and the correlation were observed.Conclusion HPV16/18 infection is highly associated with the degree of squamous intraepithelial neoplasia and carcinomas of cervix and upregulated p16 and Ki-67,which suggested that HPV16/18 maybe cause mutation of p16 gene.With the progression of cervical squamous epithelial lesions,positive expression rates of CK17 were decreased compared with increased of HPV16/18,p16,Ki-67.Combined detection of HPV16/18,p16,Ki-67 is helpful for diagnosis and classification of cervical squamous epithelial lesions.

Objective To establish differential protein expressing profiles of human gastric adenocarcinoma cell in primary or metastatic lymph node tissues by two-dimensional differential gel electrophorosis (2-D DIGE) so as to investigate the metastatic molecular mechanism of the gastric cancer.Methods After obtaining 6 samples of human primary gastric cancer and metastatic lymph node tissues,with manual no-staining frozen sections microdissection,human gastric adenocarcinoma cells from primary or metastatic lymph node tissues were isolated,and then the total proteins were extracted and purified.Highly sensitive 2-D DIGE was used to separate the total protein differentially expressed in the cells.The proteins were visualized by using a fluorescence scanner at appropriate wavelengths for Cy2,Cy3 and Cy5 dyes (Typhoon 9400).Image analysis was carried out with the DeCyder-Differential analysis software (Biological Variation Analysis version 5.0).Results Not only can the study procure defined adenocarcinoma cell populations from gastric primary or metastatic lymph node tissues,but also can resolve the problem of the change in 2-D DIGE patterns because of the varying in protein changes owing to dyeing.All these showed that the technique was simple,easy to perform,versatile and of particular usefulness when laser capture microdissection (LCM) was practically unavailable.The 2-D DIGE patterns with high resolution and reproducibility from adenocarcinoma cells in gastric primary or metastatic lymph node tissues were obtained.The number of spots in Gel1,Gel2 were 1 416 (similar 1 062,decrease 277,increase 77),1 299 (similar 1 050,decrease 157,increase 92),respectively.A total of 11 differential proteins were acquired by image analysis with DeCyder-Differential analysis software (Biological Variation Analysis version 5.0).Conclusions In this report,a simple,easy to proform method of protein epuration,manual no-staining frozen sections microdissection is described,and have used the highly sensitive 2-D DIGE for the identification of proteins differentially expressing in human gastric adenocarcinoma cells from primary or metastatic lymph node tissues.These results provide a fundamental basis for further study of metastatic mechanism of gastric cancer and screen its specific markers.

Objective To investigate the relationships between serum concentration of RBP-4 and inflammatory factors,insulin resistance in patients with gestational diabetes mellitus.Methods20 patients newly diagnosed with impaired glucose tolerance (IGT),27 patients newly diagnosed with gestational diabetes mellitus(GDM) and 50 subjects with normal glucose tolerance(NGT) were enrolled in this cross-sectional study.Additionally,another subset analysis was performed,subjects were divided on the basis of BMI into normal weight (NW) and overweight or obesity (OW/OB).Serum RBP-4 was measured with ELISA;serum concentrations of hs-CRP,free fatty acids (FFA),triglyeride(TG),total cholesterol (TC) and insulin were measured in fasting status.Insulin resistance was assessed by HOMA-IR.ResultsLn(HOMA-IR) and the concentrations of FFA,ln(hs-CRP) in IGT or in GDM were significantly higher than that of NGT(P＜0.05).In the group of OW/OB,FFA[(0.90±0.31)mmol/L vs (0.71±0.28)mmol/L,t＝2.73,P＜0.05)],ln(hs-CRP)(0.62±1.00 vs -0.17±1.07,t＝2.90,P＜0.05),ln(HOMA-IR),lnRBP(0.56±0.27 vs 0.30±0.14,t＝2.86,P＜0.05)were significantly higher than that of NW.In the group of GDM,lnRBP-4 showed a positive relationship with FFA (r＝0.231,P＜0.05)and ln(hs-CRP) (r＝0.237,P＜0.05).There was a positive relationship between lnRBP-4 and FFA(r＝0.371,P＜0.05) or ln(hs-CRP)(r＝0.247,P＜0.05) in the group of OW/OB,but no association in the group of NGT,IGT or NW.There was no relationship between lnRBP-4 and ln(HOMA-IR) in IGT,GDM and NW,while in the group of NGT and OW/OB,lnRBP-4 showed a positive relationship with ln(HOMA-IR) (r＝0.276,0.290,P＜0.05).In a multiple linear regression analysis,ln(hs-CRP),FFA or ln(HOMA-IR) was not the independent factor of lnRBP-4 in any group.ConclusionsIn patients with gestational diabetes mellitus,lnRBP-4 may be a new marker of inflammation.

Objective To evaluate the diagnostic value of MRI in hepatic echinococcosis.Methods MRI features of 37 cases with hepatic echinococcosis proved by operation and pathology were analysed in comparison with that of CT.Results The hepatic echinococcosis could be divided into five types according to the manifestations of CT and MRI:simple cyst in 8 cases,multiple daughter cyst in 13 cases,ruptured cyst in 7 cases,consolidated and calcified cyst in 4 cases and mixed cyst in 5 cases.Conclusion MRI can clearly show and determine the location,size,form,number and types of hepatic echinococcosis,that are helpful for the plan of operation.
