1.THE STUDY OF INFLUENCE OF LONG-TERM SMOKING ON THE EXPRESSION AND MUTATION OF p53 AND K-ras GENE IN RAT LUNGS

Objective To observe the influence of long-term smoking on the expression of p53 and K-ras in rat lung tissues, and to study the relationship of smoking to the mutation of p53 and K-ras gene. Methods The model of Wistar rat smoking was built up. Seventy-two healthy male Wistar rats were divided into the experimental group and control group at random. The rats of the experimental group were compelled to smoke, and the rats of the control group were given the same condition as the experimental group was, without smoking. The rats of the experimental group were smoked for 6 months. At the end of each month, 6 rats were chosen from the two groups respectively, their lung tissues were sampled and immunohistochemistry was applied to observe the expression of p53 and K-ras in lung tissues. Finally, the mutation which might happen in the exon 5, 6, 7-8 of p53 and the exon 1 of K-ras was examined by PCR-SSCP. Results The p53 protein was expressed in cell nucleus and K-ras in cytoplasm. The positive ratio of protein expression was increased with the extension of smoking time. The mutation of p53 was increased as the smoking time extended. But the effect of smoking time was not that significant on the mutation of K-ras.Conclusion Smoking can strengthen the expression of p53 and K-ras protein and can also result in gene mutation. As the time of smoking extended, those phenomenons were tending to rise. That provided the theoretical evidence which can be used to judge the lesion of lung tissues caused by smoking and help the early diagnosis of smoking-related lung carcinomas. It is of great theoretical and application values.
2.Reform of vocational nursing Maternal and Child Care course teaching methods during connection of secondary and higher vocational education

Objective By the reform of vocational nursing Maternal and Child Care course teaching methods,to solve the problems existing in higher vocational curriculum in five-year nursing specialty such as too much repetition in teaching content,low learning interest and enthusiasm in students,and not being able to reach target level of personnel training effectively.Methods Higher vocational nursing class 2 (51students) were extracted as the experimental group,class 3 (50 students) were extracted as as the control group from grade 2009.The experimental group adopted the project teaching method,the control group used the traditional teaching method.At the end of the course,the results of knowledge and operating skills between the two groups were compared.Results The total scores of the experimental group were better than the control group in memorizing theoretical knowledge examination,interpretation,application.There were no differences in individual skill examination skills assessment between the two groups,comprehensive skills assessment and total scores of the experimental group was better than the control group.Conclusions Application of project teaching method in higher vocational nursing specialty maternal and child care course teaching in higher vocational cohesion.It can effectively avoid the repetition of teaching content which may lead to the low utilization rate in class,stimulate students' interest in learning,and develop students' comprehensive occupation ability,and make vocational course objectives to achieve high stage.
3.Effect of Long-term Smoking on Structure and Protein Expression of P53 and VEGF in Pericancer and Surrounding Lung Tissue.

Objective To investigate the effect of expressions of P53 and VEGF in smoking lung cancer and surrounding lung tissue,and to offer us with theorial evidence of early diagnosing smoking-related lung carcinoma clinically.Methods126 lung cancer patients were recruited,including 96 long-term smoking and 30 nonsmoking patients.All clinical data was integrity and the patients had clear smoking history.None of patients underwent chemotherapy,radiotherapy and other tumor treatment.The bronchial epithelium,cancer tissues,pericancer lung tissues,surrounding lung tissues were observed by light microscopy,and the expressions of P53 and VEGF of lung cancer tissues,pericancer lung tissues and surrounding lung tissues were detected by Immunohistochemical methed in smoking and non-smoking group.The experimental data was analyzed by SAS statistical software and the degree of difference between the groups was compared accordingly.ResultsThere were different levels of expansion of alveolar wall in lung tissues of smokers,and alveolar wall became capsular to expand.Respiratory bronchioles were cystic expansion and small bronchial wall becomed thickening,with severe bronchial epithelial membrane ranking nuts-chaos and peeling.Goblet cells and the cell surface of the cavity mucous secretions and suppositories were more than those of non-smokers.The P53 protein was expressed in cell nucleus and VEGF expressed in the cytoplasm and endothelial cells of neovascularization.Both their positive behavior was granular brown.Either for smoking or for non-smoking lung cancer,the expressions of P53 and VEGF were higher in tumor tissues than that of in the pericancer and surrounding lung tissue(P
4.Clinicopathological study of 69 cases of secondary myelofibrosis of malignant blood diseases

Objective To study the clinical features bone marrow biopsy of secondary myelofibrosis(SMF).Methods 69 patients with secondary myelofibrosis were analyzed retrospectively,and the relationship between myelofibrosis degree and megakaryocyte count was analyzed. Results Analysis of 69 cases of patients with SMF, the original incidence included 20 cases (29.0 %)of chronic myeloid leukemia (CML),14 (20.3 %) cases of lymphoma,10 (14.5 %) cases of acute myeloid leukemia (AML),10 (14.5 %) cases of myelodysplastic syndrome (MDS),6 cases(8.7 %)of acute lymphoblastic leukemia (ALL),4 cases of multiple myeloma (MM),3 cases of myeloproliferative neoplasms (MPN),2 cases of chronic lymphocytic leukemia (CLL).There was no significant difference of the mygakaryocytes’ count and association of myelofibrosis with it was found (r=0.024,P=0.848). Conclusion Various clinical diseases of blood system may associated with secondary myelofibrosis. Bone marrow biopsy can be used to initial diagnostics of malignant disease of hematological system patients.When cellular examination of bone marrow is dry tap or dilution, which means higher incidence of SMF. Bone marrow biopsy plays an important role in diagnosis of SMF.
5.Application of ChemOffice Software in Teaching Medicinal Organic Chemistry

6.Application of PBL teaching method in the experimental course of medical organic chemistry

7.Cementless total hip arthroplasty versus hemiarthroplasty for the treatment of old femoral neck fracture

BACKGROUND:There is controversy on the treatment of old femoral neck fracture with hemiarthroplasty.
OBJECTIVE:To observe the clinical effect of cementless hemiarthroplasty in the treatment of old femoral neck fracture, and to compare with total hip arthroplasty.
METHODS:A retrospective analysis was performed on the clinical data of 23 old femoral neck fracture patients treated by artificial joint replacement from January 2009 to June 2010. Among the 23 patients, 11 cases were treated with cementless hemiarthroplasty, and 12 cases were treated with total hip arthroplasty. The time for off-bed activity, Harris score and the incidence of perioperative complications were compared between cementless total hip arthroplasty and hemiarthroplasty.
RESULTS AND CONCLUSION:Al the patients were fol owed-up for 12-18 months. The active straight leg raising angle, time for off-bed activity, incidence of early postoperative complications and Harris score at 1 week after treatment of the cementless hemiarthroplasty group were better than those of the total hip arthroplasty group;there were no significant differences in Harris score at 6 weeks, 3 and 6 months between two groups;the incidence of forward hip pain of the cementless hemiarthroplasty group was higher than that of the total hip arthroplasty group. So, we general y think that cementless hemiarthroplasty has better short-term effect in the treatment of old femoral neck fracture, but the long-term integrated efficacy needs to be further identified.

9.Effect of Different Types of Tube Feeding on Swallowing Function for Stroke Patients

Objective To evaluate the effect of different types of tube feeding on swallowing function of stroke patients. Methods 60 patients with dysphagia post stroke were divided into observation group and control group with 30 cases in each group. The control group received indwelling nasogastric tube feeding, and the observation group received intermittent nasogastric tube feeding. They were assessed with Kubota Water Swallow Test (WST) and VGF on the 1st day and 30th day after admission. Results The scores of drinking test and VGF were better in the observation group than in the control group (P<0.05) on the 30th day. Conclusion Long-term nasogatric tube feeding is effective on dysphagia in stroke patients.
10.Effects of different biofeedback training techniques on the static balance of hemiplegic patients

Objective To compare the static standing balance of stroke patients after different biofeedback protocols.Methods Thirty-two stroke patients were randomly divided into a knowledge of performance (KP) group,a knowledge of results (KR) group and a control group.All 3 groups received 4 weeks of conventional rehabilitation training plus another 30min of static standing balance training per day.The KP group received audio-visual feedback in real time during the training.The KR group received section result feedback.The control group received no feedback during the extra balance training.Before and after the training,the performance of the 3 groups was evaluated using Berg's Balance Scale (BBS) and a portable biofeedback device.Results Average BBS performance improved significantly more in the KP group (3.08± 1.08) than in KR group (1.30±0.67) and control group (1.20± 0.79) (P<0.05).No significant difference was detected between the KR and control groups (P>0.05).The average improvements of the KP group in terms of Standing with Eyes Closed and Tandem Standing (0.92±0.79 and 0.83± 0.39) were significantly highcr than those in the KR (0.30± 0.48 and 0.20± 0.42) and control groups (0.01 ± 0.01 and 0.40±0.52) (P<0.05).Average trunk angular displacements in all four directions [Anterior (2.83±0.93;6.15± 1.85),Posterior (2.56±0.88;5.97±1.74);Left (2.86±1.16;6.49±2.42),Right (2.68±1.43;5.98±2.05)] in the KP group was significantly higher than in the others (P<0.05).No significant differences were detected between the KR and control groups in BBS results or in posture.Conclusions Static standing training should incorporate real time biofeedback.It is then more effective than conventional standing training or training with section results feedback.It is worth spreading in clinical applications.
