1.Clinical Observation of Budesonide Combined with Salbutamol in the Treatment of Infant Bronchiolitis

OBJECTIVE:To investigate therapeutic efficacy,safety and economics of budesonide for infant bronchiolitis based on salbutamot. METHODS:In prospective study,160 inpatient children with bronchiolitis during Oct. 2014-Apr. 2016 were divid-ed into observation group and control group according to admission order,with 80 cases in each group. Both groups received conventional treatments. Control group was given Salbutamol solution for inhalation 0.25 mL added into 0.9% Sodium chloride injection 3 mL,q8 h. Observation group was given Budesonide suspension for inhalation 2 mL added into 0.9% Sodium chlo-ride injection 1 mL+Salbutamol solution for inhalation 0.25 mL,q8 h. Both groups received oxygen driven inhalation,and treat-ed for 5-7 d. Clinical symptom disappearance time,hospitalization time and clinical efficacy were compared between 2 groups as well as therapy drug cost(aerosol inhalation,other therapy drugs). The occurrence of ADR was recorded. RESULTS:There was no statistical significance in cough disappearance time,wheezing disappearance time,lung rale disappearance time,tri-re-traction sign disappearance time and hospitalization time between 2 groups(P>0.05). There was no statistical significance in to-tal response rate between observation group (95.00%) and control group (92.50%)(P>0.05). The cost of inhalation drugs in observation group [(355.77±10.98)yuan] was significantly higher than control group [(26.83±2.86)yuan],with statistical signif-icance (P<0.05). There was no statistical significance in the cost of routine therapy drugs between 2 groups (P>0.05). There was no significant ADR between 2 groups during treatment. CONCLUSIONS:For infant bronchiolitis,aerosol inhalation of budesonide based on salbutamol sulfate can not significantly shorten disease,shorten hospitalization time and improve clinical ef-ficacy,but increase therapy cost.

Objective To explore the protective effect of vitamin E on the rabbit bladder after partial outlet obstruction artificially setup. Methods A total of 28 New Zealand white male rabbits were divided into 4 groups (group A in 6,group B in 6,group C in 8 and group D in 8).Group A,B and C were fed a regular diet,and group D were placed on a diet enriched with 600 mg vitamin E.After 4 weeks partial outlet obstruction was created in groups C and D,while group B underwent sham operation. After 4 weeks of obstruction each rabbit was sedated and cystometry was repeated.After cystometry the bladder was weighed.The gene expression of sarcoplasmic endoplasmic reticulum,calcium,magnesium,adenosine triphosphatase (SERCA2) in bladder was detected by using RT-PCR assay,while the protein level of SERCA2 was measured by Western blot analysis. Results All parameters measured were approximately identical in nomal rabbits(Group A) and shum operation rabbits(group B).Thus,these 2 groups were combined as the control group(Group A and B).Partial outlet obstruction resulted in bladder weight increased significantly in obstructed groups given vehicle group C(13.07?1.71)g and those vitamin E group D(11.80?2.01)g,4-fold higher than in the control group (2.81?0.30)g(P
4.Research on Tongue Image Characteristics among 157 Chronic Kidney Disease Cases at Different Renal Function Stages

This study was aimed to observe the tongue image characteristics at different renal function stages among 157 chronic kidney disease (CKD) patients. Tongue images of 157 CKD cases and 30 healthy people were recorded with the instrument designed by Shanghai University of Traditional Chinese Medicine. Parameters of the tongue image were compared among different renal function stages. The results showed that compared with the control group, there were obvious decreasing on the R and L indexes in groups of different renal function stages (P < 0.05). Compared with the control group, there were obvious decreasing on the G and B indexes in groups of CKD1, CKD2, CKD3 and CKD4 stages (P < 0.05). Compared to CKD4 stage, there were obvious decreasing on the R, G, B, L indexes from group of CKD1 and CKD2 stage (P< 0.05). Compared with the group of CKD5 stage, there were obvious decreasing in the group of CKD1, CKD2, and CKD3 (P< 0.05). Compared with the control group, the cracking index was obvi-ous increased in 5 stages (P < 0.05). There was obvious difference in the greasy index between the group of CKD5 stage and the control group (P< 0.05). Compared with the control group, there was obvious difference in the peeling index from the group of CKD3, CKD 4, CKD5 stage (P < 0.05). Along the decreasing of renal function, compared with the group of CKD5 stage, there was obvious decreasing on the greasy index and peeling index in the group of CKD1 and CKD2 (P< 0.05). It was concluded that Chinese medicine tongue and face diagnosis instrument can pro-vide certain objective evidence for the clinical diagnosis of CKD.

6.Effect of comprehensive prevention and control of soil-transmitted nemato-diasis in Runzhou District of Zhenjiang City

Objective To evaluate the effect of comprehensive prevention and control of soil?transmitted nematodiasis in Runzhou District Zhenjiang City Jiangsu Province from 1997 to 2012. Methods The comprehensive prevention and control measures included the helminthicide health education improvement of water supplier and harmless toilets and these mea?sures were implemented continuously. At the same time the infection rates of soil?transmitted nematodes the local economic in?dicators and the coverage rates of tap water and harmless toilets were surveyed. Results The mass chemotherapy was per?formed for 202 100 person?times and the diagnosed chemotherapy was performed for 2 006 person?times in Runzhou District from 1997 to 2012. The awareness rates of health knowledge were 57.18%in 1997 and 95.62%in 2012. The coverage rates of tap water and harmless toilets were 10.14%and 10.21%in 1997 and 100.0%and 90.38%in 2012 respectively. There were negative correlations between the awareness rate of per capita GDP per capita net income coverage rates of tap water cover?age rates of harmless toilets health knowledge and the infection rate of soil?transmitted nematodes respectively rper capita GDP=-0.526 P 0.05 rper capita net income=-0.671 P 0.01 rcoverage rates of tap water=-0.936 P 0.01 rcoverage rates of harmles toilets=-0.922 P 0.01 rawarenes=-0.774 P 0.01 . The statistical analysis showed that the infection rate of soil?transmitted nematodes had a downward trend as an expo?nential curve in Runzhou District from 1997 to 2012 y=42.031 7e?0.357 6x R2=0.803 6 F=57.28 P=0.000 . The infection rate of degradation by an annual rate was 29.18%. The infection rate in farmers was significantly higher than that in students χ2=17.998 P 0.01 . There was no significant difference between men and women in the infection rate of soil?transmitted nema?todes χ2=3.627 P=0.057 . Conclusion The comprehensive prevention and control measures and the development of so?cial economy contribute to the steady decline of soil?transmitted nematode infections.
7.Study on associated protein markers in urine of patients with chronic renal failure

Objective To investigate the associated protein markers in urine of patients with chronic renal failure (CRF) based on surface enhanced laser desorption and ionization time of flight mass spectrometry (SELDI-TOF MS)technique.Methods Urine samples were taken from 150 CRF patients and 50 healthy people,and investigated by proteomic techniques with H4 gene chip.Results Compared to healthy group,141 different protein peaks were identified within the range of 1000 to 20 000 M/Z in the protein map of CRF group,whose differences were all significant (all P<0.01).The decision tree model for CRF urine was constructed after bioinformation analysis to significantly differentiate between CRF and healthy group.The accuracy rate,sensitivity and specificity of the decision tree model were 96.0%,100.0% and 94.7% respectively.In CRF group and healthy group,the different protein peaks in urine were identified to probably be 17 proteins with reference to SwissProt database.Conclusions Candidate protein markers in urine are screened and prediction model of CRF urine is established.The markers are identified with the database which provides a more accurate prediction and solid evidence for early diagnosis of CRF.
8.Exploring the prediction model of chronic renal failure based on serum proteomics

Objective To Screen serum protein markers related to CRF and establish a diagnosis model,exploring and discussing its significance in serodiagnosis by comparing differences of serum protein spectrum expression between patients with chronic renal failure (CRF) and control group.Methods The trial included 62 CRF patients and 28 control ones.Serum samples were tested by surface enhanced laser desorption/ionization-time of flight-mass spectrometry (SELDI-TOF-MS).The data were analyzed to screen serum proteomic biomarkers.By bioinformatics analysis,decision classification tree models were to be established and tested.Results A total of 19 effective protein peaks were significantly different between CRF and normal control (P<0.001) at m/z range of 1 500 to 30 000,among which 18 showed low expression and 1 showed high expression in CRF.CRF and normal control were obviously different in the clustering;By bioinformatics analysis,a CRF-normal controls of the diagnostic decision tree model was developed,which was 87.8% in with prediction accuracy rate of 87.8% sensitivity of 87.1% and a specificity of 89.3%.Condusion Diagnostic decision tree model provides a more accurate prediction and solid experimental evidence for early clinical diagnosis.
9.Clinical study on treatment of mid-advanced crescentic nephritis by qingre huoxue recipe.

OBJECTIVETo evaluate the therapeutic effect of integrative Chinese and Western medicine in treating mid-advanced crescentic nephritis (MACN).
METHODSThirty-two patients, their diagnosis was confirmed as MACN by renal biopsy, were divided, adopting randomized, controlled method, into two groups, the treated group and the control group, they were all, excepting one, treated with impact therapy of methyl-prednisolone followed with oral intake of prednisone, to part of them cyclophosphamide or mycophenolate mofetil was given in addition, to those with hypo-hemoglobin (< 90 g/L), subcutaneous injection of erythropoietin was administered. Decoction of Qingre Huoxue recipe (QHR), consisted of oldenlandia herb 30 g, honey-suckle stem 30 g, violet herb 30 g, red peony root 15 g, rehmannia root 15 g, solomonseal rhizome 15 g, asiabell root 30 g, red sage root 30 g, prepared rhubarb 12 g and giant-hyssop herb 12 g, were additionally given one dose per day to patients in the treated group. The renal function, improvement of anemia and immunosuppressive agents needed in patients were observed after 3 months treatment.
RESULTSAfter treatment, renal function was improved in both groups, but the effect in the treated group was better than that in the control group (P < 0.05). Anemia was partially alleviated in the two groups with no significant difference. The dosage of glucocorticoids used in the treated group was obviously lesser than that used in the control group (P < 0.01).
CONCLUSIONIntegrative Chinese and western medicine could treat crescentic nephritis to obtain good effect, and reduce the quantity of glucocorticoid necessity for treatment.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
