1.Survey of the standardization of medical records writing by physicians

Objective To study the standardization of medical records writing by residents in hospitals. Methods With both quantitative and qualitative analysis, medical records made by residents were sampled in groups to study their standardization. Results Recognition rate of such medical records was 82. 55%, and the excellence rate was 24. 50% in the grade evaluation. For senior residents, their rate of writing excellence falls below those of junior ones; whether the evaluators have medical background bears no significant difference for grade evaluation of medical records writing, yet a significant difference was found with the recognition rate. Conclusion Writing of medical records by residents is found with incompliance now and then, and the recognition rate ought to be improved.
2.Analysis of 251 cases entered the procedure of medical malpractice identification

Objective To analyze the causes of medical disputes and to find an effective way to reduce medical malpractice and medical dispute.Methods The complete identification files of 251 cases of medical malpractice obtained from corresponding Offices of Technical Identification of Medical Malpractice in three cities at prefecture level in Shandong province were analyzed to compare the cause of medical malpractice identified by patients with that identified by experts.Results The main causes that identified by patients included violation(36.41%),default(35.60%),illegality(13.05%),tort(11.89%),and absence of management(3.05%).Among the litigation claims,58.53%were identified as existed factors causing disputes.Among the identified factors,72.66%should have been prevented and controlled.Conclusion Medical malpractice and medical dispute can be effectively prevented and controlled by putting management site forward,ensuring the implemention of the policies,and strengthening the substantive responsibility of the leadership.

5.Preliminary clinical study of regional cerebral blood flow and hemodynamics in patients with chronic cerebrovascular insufficiency

Objective To explore the changes in regional cerebral blood flow(rCBF)and cerebral hemodynamics in patients with CCVI.Methods Twenty patients diagnosed as CCVI were enrolled to undergo single photon emission computed tomography(SPECT)rCBF imaging or transcranial Doppler uhrasonography(TCD)examination.All the patients were free from cerebral structural abnormalities as demonstrated by X-CT or magnetic resonance imaging(MRI).Cranial arteries blood flow velocity,pulsating index(PI)were measured by TCD and compared with the mean of rCBF as measured by SPECT.Results On SPECT images, decreased rCBF lesions were found in 95 percent of patients(19/20).The areas of hypoperfusion were mainly located in frontal,temporal,parietal lobes and fondues nodus.Increased cranial arteries blood flow velocity was found in the anterior cerebral artery(ACA)and middle cerebral artery(MCA)in 80 percent of patients (16/20).There were no correlations among rCBF,the decreased percentage of rCBF,average velocity,and PI of cranial arteries.Conclusions By performing SPECT rCBF and TCD on CCVI patients,the degree of changes in rCBF and hemodynamics Can be evaluated easily,which is valuable for early clinical diagnosis and treatment of CCVI.

This paper lettered experts using delphi method,determined 3 the first indexes,14 the second indexes and 33 the third indexes.After indexes weights given,a comprehensive evaluation index system of science and technology program' s selection and outputs had been established.The comprehensive evaluation index system is simple and could be easily operated,used for population assessment of science and technology program's selection and outputs in population and health field.
7.The value of dual-energy CT in staging for gastric cancers

Objective To explore the image quality of gastric cancer improved by dual-energy CT,and to evaluate the clinical utility in staging gastric cancers.Methods 55 patients suspected gastric cancers were included in the study.Patients underwent dual-phasic scans using dual-energy CT with a dual-source scanner.The relationships image quality parameters in 100 kV,120 kV and 70 keV images were evaluated.Image quality parameters comprised mean density,signal-to noise ratio (SNR) and objective noise.The 120 kV and 70 keV images were compared in TNM staging.Results In arterial and portal phase,the attenuation was significantly higher at 70 keV and 100 kV compared to 120 kV images (P<0.001).In arterial phase,the SNR were higher at 120 kV and 70 keV in comparison to 100 kV(P<0.01).In portal phase,the SNR were higher at 70 keV in comparison to 120 kV and 100 kV(P<0.01).Overall accuracy in assessment of tumor invasion of the gastric wall (T stage) was 83.7% with 70 keV images and 72.1% with 120 kV images,this difference was not significant (P=0.25);Overall accuracy for lymph node (N) staging was significantly better with 70 keV images(74.4%) than with 120 kV images(62.8%) (P<0.05).Conclusion The image quality of gastric cancers using dual-energy CT can be improved by optimizing the energy level.The 70 keV images may be used to improve the N-staging accuracy.

Objective to study the characteristic typing of Helicobacter pylor(Hp)phenotypes and their sub-phenotypes in the patients with duodenal bulb ulcers(DU),and its clinical significance. Methods One hundred thirty-five cases with DU and 140 casses with chronic superficial gastritis were enrolled in this study. Determinations of serum cytotoxin-associated gene protein A (CagA),vacuolating cyto-toxin A(VacA),urease (Ure)A,UreB antibodies and their sub-phenotypes by immunoblotting were carried ou. Results Positive rate of middle-phenotypes of Hp infection in DU was significantly lower than that in chronic superficial gastritis (21.5%vs 27.9%,P<0.05).VacA and CagA antibodies might express alone. There had no significant difference among the expression rate of phenotype CagA, VacA antibodies and their sub-phenotype. But expression rate of Ure antibodies in Du was higher than that in chronic superficial gasstritis (P<0.05).In infection of Hp type I, the expression rate of sub-phenotypes 30ku UreA in DU was hronic superficial gastritis (P<0.05). Conclusions The VacA is not for expressing higher than that inpression of Hp exists many sub-phenotypes ( 128 ku CagA 116 ku CagA, 95 ku VacA, 91 ku CagA 0 ku UreA),and it probably causes formation of DU by comprehensive effect. Hp type I with sub phenotype expressing 30 ku Urea may be more pathogenic in DU formation.
