1.Effect of combination of insulin and selenium on insulin signal transduction in cardiac muscle of STZ-induced diabetic rats.

This study is to investigate the effect of low doses of insulin (1 u x kg(-1)) and selenium (180 microg x kg(-1)) in combination on general physiological parameters and insulin signal molecules in cardiac muscle of STZ-induced diabetic rats. The levels of blood glucose were estimated using One Touch SureStep Blood Glucose meter. HbA1c levels were estimated using microcolumn assay. TG and TC were estimated using enzymatic assay. The levels of PI3K and GLUT4 in cardiac muscle were examined by immunoblotting and immunohistochemistry. The result showed that insulin in combination with selenium could significantly lower blood glucose and blood lipid levels and markedly restored the PI3K and GLUT4 levels in cardiac muscle. It could be concluded that there was cooperation between insulin and selenium, and that treatment of diabetic rats with combined doses of insulin and selenium increased cardiac glucose uptake by upregulating the level of PI3K-mediated GLUT4 in cardiac muscle, eventually ameliorating myocardial dysfunction.
2.The clinical characters and surgical managements of congenital laryngeal cysts in infants.

OBJECTIVE:
To classify congenital laryngeal cysts in 62 newborn or infants, and to discuss the characteristics of different operation mode.
METHOD:
The clinical data of 62 infants with laryngeal cysts treated were reviewed retrospectively. After the examination of neck palpation, laryngoscope and imaging (CT or MRI), all patients were under general anesthesia of laryngeal cyst excision. we select operation mode according to the classification. Classification include: 61 cases of cysts were confined to the larynx, 1 case was beyond the larynx. Surgical managements include: an excision through external carotid approach in 1 case beyond the larynx, 61 cases had endoscopic excision under general anesthesia (12 cases with traditional bite exception, 12 cases with powered system and 37 cases with low-temperature radiofrequency ablation).
RESULT:
The operations was successful. No recurrence was found after 0.5 to 7 years follow-up.
CONCLUSION
To chose surgical management after classification and preoperative assessment, which could avoid unnecessary opening surgical approaches, reducing the recurrence, and preventing repeated endoscopic treatment or tracheotomy. The low-temperature radiofrequency ablation had the advantages of short operation time, less bleeding, little injury, low recurrence rate and light postoperative reaction. It is worthy of clinical promotion.

3.Effect of insulin in combination with selenium on blood glucose and GLUT4 expression in skeletal muscle of streptozotocin-induced diabetic rats

Objective To evaluate the effect of low-dose insulin [1 U/(kg · d)] in combination with selenium [180 g/(kg · d)] on general physiological parameters and glucose transporter (GLUT4) level in skeletal mnscle of streptozotocin (STZ)-induced diabetic rats. Methods Diabetic rats were treated with insulin, selenium, and insulin and selenium in combination for four weeks. The level of blood glucose was determined using One Tonch SnreStep Blood Glucose meter and the level of GLUT4 in skeletal muscle was examined by immunobiotting and immnnohistochemistry. Results Our data showed that insulin in combination with selenium could significantly lower blood glucose level and restore the disturbance in GLUT4 level in skeletal muscle. Treatment with insulin was only partially effective in restoring diabetic alterations. Conclusion It can be concluded that there is a synergistic action between insulin and selenium, and that treatment of diabetic rats with combined doses of insulin and selenium is effective in the normalization of blood glucose level and correction of altered GLUT4 distribution in skeletal mnscle of diabetic rats.
4.Therapeutic effect of Shexiang Baoxin Pill combined with treatment based on syndrome differentiation for acute ischemic stroke

AIM: To investigate the effect of treatment based on syndrome differentiation only and combined with Shexiang Baoxin Pill for acute ischemic stroke(AIS). METHODS: Fifty-eight cases of AIS were randomly (allocated) to two groups.Thirty cases of group A were treated with treatment based on syndrome differentiation combined with Shexiang Baoxin Pill and twenty-eight cases of group B with the treatment based on syndrone differentiation. 5.Analysis of deafness-related gene mutations in 88 non-syndromic hearing loss patients in Fuzhou city

OBJECTIVE To investigate the deafness-related gene mutation frequency and hotspots in patients of Fuzhou city with non-syndromic hearing loss (NSHL). METHODS Peripheral blood samples were obtained from 88 cases of patients with hearing loss after clinical history inquiry and clinical examination. Their genomic DNA was extracted from peripheral blood by extraction kits to undergo polymerase chain reaction, traditional capillary electrophoresis sequencing and High-throughput sequencing so as to detect the mutations of deafness-related gene. RESULTS Among the 88 patients with NSHL, the gene mutation frequency was 34.09%.In the patients, 14 cases had mitochondrial 12 S rRNA mutations, six cases had GJB2 gene mutations and three cases had SLC26A4 mutations, two cases had MYO15A mutations, the other five cases had MYO7A, OTOF, TECTA, TMC1 and ILDR1 gene mutation respectively. CONCLUSION Among the 88 patients with NSHL, the most frequent mutation causing hereditary deadness was mutation in mitochondrial 12 S rRNA, followed by GJB2 and SLC26A4, The other genes such as MYO7A, OTOF, TECTA, TMC1 and ILDR1 gene were infrequent. The study could provide theoretical reference in genetic diagnosis, prevention and cure of hearing loss.
6.Comparison on Contents of 23-Acetate Alisol B and Infrared Spectrometry Fingerprint Among Rhizoma Alismatis of Different Specifications

Objective To compare the chemical components among Rhizoma Alismatis of different specifications. Methods Rhizoma Alismatis of 8 different weights were chosen, and then contents of 23-acetate alisol B were determined by HPLC, and infrared spectrometry fingerprint was determined by Fourier transform infrared spectroscopy. Results The contents of 23-acetate alisol B in Rhizoma Alismatis of 8 different specifications were over 0.06%, and had no relation with specification of Rhizoma Alismatis (P>0.05). The similarities of infrared spectrometry fingerprint were above 0.9. Conclusion The chemical components among Rhizoma Alismatis of different specifications were basically the same. Contents of 23-acetate alisol B of Rhizoma Alismatis of 8 different specifications conformed to regulation of China Pharmacopoeia.
7.Effect of Aromatic Herbs for Resuscitation on NO Content and Expression of NOS in Brain Tissue of Rats with Cerebral Ischemia and Reperfusion Injury

[Objective] To investigate the effect of aromatic herbs for resuscitation such as Moschus and Borneol on nitric oxide (NO) content and expression of endothelial NO synthetase (eNOS) and inducible NO synthetase (iNOS) in brain tissue of rats with cerebral ischemia and reperfusion injury. [Methods] Forty rats were allocated to 4 groups: mimic operation group (Group A), model group (Group B), herbs treatment group (Group C) and nimodipine treatment group (Group D). Rat model of cerebral ischemia and reperfusion injury was established by Pulsinelli 4VO method. NO content in brain tissue was determined by nitrate reductase method. Expression of eNOS and iNOS in micrangium and iNOS in neurons was measured by semi-quantitative analysis. [Results] NO content was decreased and approached normal level in Group C and Group D (P 0.05 as compared with Group A). Objective To investigate the effects of different doses of dexmedetomidine on perioperative inflammatory responses in patients undergoing one-lung ventilation (OLV).Methods Thirty-six ASA T or Ⅱ patients (aged 43-72 years and weighing 50-78 kg) scheduled for esophagectomy were randomly divided into three groups (n =12 each):control group (group C),low dose dexmedetomidine group (group D1) and high dose dexmedetomidine group (group D2).Dexmedetomidine 1 μg/kg was infused intravenously 10 minutes before anesthesia induction,then infused at a rate of 0.2 μg· kg-1 · h-1 (group D1) or 0.5 μg· kg-1· h-1 (group D2) until 30 minutes before the end of operation.Group C received the equal volume of normal saline.Blood samples were collected before anesthesia induction (T0),immediately before OLV (T1),30 minutes after OLV (T2),90 minutes after OLV (T3),30 minutes after lung inflation (T4) and 2 hours after operation (T5) for monitoring serumlevels of tumor necrosis factor-alpha (TNF-α) and interleukin-8 (IL-8).Results Compared with T0,serum levels of TNF-α and IL-8 significantly increased at T3 and T5 in all the three groups (P < 0.05).Compared with group C,serum levels of TNF-α and IL-8 significantly decreased at T3 and T5 in group D2 (P < 0.05).There was no significant difference in the indexes mentioned above between group C and group D1 (P > 0.05).Conclusion Dexmedetomidine 1 μg/kg given before anesthesia induction and then infused at the rate of 0.5 μg· kg-1 ·h-1 during operation can reduce inflammatory responses in patients undergoing OLV.
10.Clinical analysis of nosocomial infection in patients with non-Hodgkin lymphoma

Objective To identify risk factors and preventive measures of nosoconial infections in patients with non-Hodgkin lymphoma (NHL). Methods Clinical data of 65 NHL patients admitted from January to December 2007 were retrospectively analyzed. Results According to WHO classification (2001), 58 patients were with B-cell lymphoma, 7 were with T-cell lymphorna. All patients received CHOP regimen as initial chemotherapy and 23 of them were with nosecomial infections. Logistic regression analysis demonstrated that age, length of stay, pathological type, bone marrow involvement, levels of serum lactate dehydrogenase (LDH), beta2-microglobulin and invasive treatment were identified as risk factors of nosocomial infections. Respiratory tract infections and infections with gram-negative microorganisms were the most popular. Conclusion High nosocomial infection rate is found in NHL patients, and control of risk factors may effectively prevent nosocomial infections in NHL patients.
