2.Drug delivery of sinomenine hydrochloride floating and pulsatile release tablet

AIM: To prepare a phase-specific drug delivery system with floating and pulsatile release of sinomenine hydrochloride and evaluate in vitro drug release behavior. METHODS: The floating and pulsatile-release of coat-core tablets were prepared by press-coated technics.The effects of factors influencing release characteristic of the drug were investigated by dissolution test,and to elucidate the mechanism of drug release of the tablets with erosion and water-uptake test. RESULTS: The tablets had typical floating and pulsatile release properties with a lag time rapid release.The lag-time was shortened with the increase of expansion ratio of tablet core and rotation speed of stirrer.The lag-time was prolonged with the increase of pH and ionic strength of dissolution media. CONCLUSION: The tablet could float and rapidly release drug at the predetermined time.
3.Mid-term treatment results of drug-eluting stent implantation for patients with multivessel chronic total coronary occlusion lesions

Objective To evaluate mid-term clinical result of drug-eluting stent (DES) implantation for patients with multivessel chronic total coronary occlusion (CTO) lesions. Methods We analyzed 89 patients who underwent interventional revascularization in their multivessel CTO lesions with DES from November 2002 to June 2005 (DES group). A control group was composed of 70 consecutive patients with multivessel CTO lesions treated with bare metal stents (BMS) from November 2000 to June 2003 (BMS group). Results There were no significant differences in basic clinical manifestations and success rate of intervention procedures between DES and BMS group. At 6-month follow-up, the cumulative rate of major adverse cardiac events (MACE) was 14.6% in the DES group and 28.6% in the BMS group (P=0.031). The incidence of restenosis was 16.9% in the DES group and 34.3% in the BMS group (P=0.011). The need for target vessel revascularization and target lesion revascularization in the DES group was significantly lower (9.0% vs 21.4%, P=0.003, and 7.9% vs 18.6%, P=0.043, respectively) compared with BMS group. Conclusion It is possible to obtain an ideal mid-term clinical outcome in patients with multivessel CTO lesions by DES implantation.
4.Genetic association of complement factor B gene polymorphisms with susceptibility and prognosis of IgA nephropathy

Objective To investigate the genetic association of complement factor B (CFB)gene polymorphisms with the susceptibility and prognosis of IgA nephropathy (IgAN).Methods Four hundreds and sixty-three Northern Han Chinese patients with IgAN and two hundreds and ninty-six geographically and ethnic matched healthy volunteers were recruited.Peripheral blood was collected from recruited individuals for DNA extracting.After amplified by plymerase chain reaction (PCR),genotyping of the four single nucleotide polymorphisms (SNPs) in CFB gene,which were rs4151667,rs12614,rs641153 and rs117314762,were performed by sequencing.Differences of allele and genotype frequencies were analyzed between IgAN patients and healthy controls.Moreover,the association between these SNPs and disease clinical manifestation,pathological features and long term renal outcome in IgAN patients were further analyzed.Results The G allele and GG genotype frequencies of rs117314762 in CFB gene were significantly higher in IgAN patients than that in healthy controls.No difference in allele and genotype frequencies of rs4151667,rs12614,rs641153 between IgAN patients and healthy controls was observed.Furthermore,no association was found between these SNPs in CFB gene and clinical manifestation,pathological features and long term renal outcome of IgAN.Conclusion Association between rs117314762 in CFB gene and IgAN susceptibility suggests that there may be functional variants in CFB gene or its linked genetic region,which needs further exploration.
5.Black-dot ringworm caused by Trichophyton tonsurans and analysis of its extracellular enzymatic activity

Objective To report a case of black-dot ringworm caused by Trichophyton tonsurans in a 3-year-old girl. Methods Lesional hair was obtained from the patient and subjected to direct microscopic examination as well as culture. Subsequently, the isolate underwent morphological, biochemical and molecular biology identification. The extracellular enzymatic activity of the isolate was analyzed. Results Microscopy revealed that the hair shaft was filled with fungal spores. Typical colony of the isolate was grayish-white with downy appearance. Slide culture showed centipede-like, lateral, rod-shaped microconidia. Urease test was positive. The amplification of ribosomal DNA (rDNA) ITS domains by PCR produced a 687 bp-sized fragment which had a 100% homology with the sequences of several Trichophyton tonsurans strains in the GenBank database. The extracellular enzymatic activity analysis showed an increase in the activity of alkaline phos-phatase, acid phosphatase, esterase (C4), β-glucosidase, leucine arylamidase, N-acetyl-β-glucosaminidase and a-mannosidase. Conclusions The pathogenic fungus is identified as Trichophyton tonsurans based on morphological and biochemical features as well as sequence of the ITS region of rDNA, and the child was diagnosed with black-dot ringworm.

7.Research on the Allocation of the Health Workforce in Primary Medical Institutions in Rural China:Based on Data of township hospitals from 29 provinces

The health workforce plays an important role in the development of township hospitals. It builds up the comprehensive evaluation index system as the first time, based on data of township hospitals from 29 provinces, to evaluate the allocation of the health workforce in primary medical institutions in rural China. The empirical results show that the core workforce allocation of Chinese township hospitals has largely focused on the local health status, which shows good fairness of allocation. Meanwhile, the equity level of the core workforce allocation is not far behind the overall fairness level, and both are slightly higher than the critical value. The core workforce allocation is in good consistency with the local level of economic development.

9.RETROSPECTIVE ANALYSIS OF PERCUTANEOUS CORONARY INTERVENTIONAL(PCI) TREATMENT IN 116 CASES WITH CHRONIC RENAL INSUFFICIENCY

To evaluate the clinical characteristics of percutaneous coronary interventional (PCI) treatment in patients with chronic renal insufficiency, 116 patients with serum creatinine ≥141?mol/L and 352 patients who had normal serum creatinine level were included for the stady. It was found that the ineidences of using alcohol, acute myocardial infarction, hypertension, diabetes mellitus, as well as hospital stay days after PCI and the entire hospital stay days, and mean number of diseased vessels were significantly higher in renal insufficiency group compared with the control group. On the other hand, the value for HDL, the time for PCI operation, and the dose of contrast medium were lower in renal insufficiency group than in control group. The results suggested that patients with renal insufficiency were able to tolerate PCI if proper peri-operative care was observed, though they had more predisposing factors of arteriosclerosis.
10.Effects of peridural and general anesthesia on respiration and circulation in gynecological laparoscopic operation

Objective To compare the influences of peridural and general anesthesia on respiration and circulation in gynecological laparoscopic operation. Methods Fifty-six patients, underwent gynecological laparoscopic operation during June to October, 2003, are divided into two groups. The items of respiration and circulation recorded before and 10, 20, 30, 40min after pneumoperitoneum as well as 5 min after deflation abdomen were as follows: blood pressure (BP), heart rate (HR), sphygmus oxygen saturation (SpO_2), end-expiratory pressure of CO_2 (P_ ETCO_2), tidal volume, frequency of respiration, minute ventilation volume and airway pressure. Results The re-consciousness time after the operation in the peridural anesthesia group is significantly less than that in the general anesthesia group (P0.05). Conclusion In gynecological laparoscopic operation, no significant difference was found concerning the influence of the two anesthesia manners on respiration and circulation.
