Objective:To determine the basic characteristics and clinical manifestations of hospitalized patients with psoriasis in Hunan, and to provide reasonable reference for the etiology and treatment of psoriasis.
Methods:Totally 190 patients with psoriasis from January 1, 2012 to December 31, 2012 treated in the Department of Dermatology, the Second Affiliated Hospital of Central South University were retrospectively analyzed. The data were analyzed by SPSS17.0.
Results:The ratio of male to female inpatient numbers was 3.13:1, aged was between 40 and 70 years. The most common subtype of psoriasis was psoriasis vulgaris (64.73%), followed by psoriatic erythroderma (18.42%). The distribution of the subtype of psoriasis did not differ by gender. Nineteen patients recalled clearly the cause of proriasis, 5 of which were caused by medicine, and 4 by drinking. Totally 96 cases accompanied with other diseases, 24.21%of which accompanied with cardiovascular system disease.
Conclusion:There is no season difference in the hospitalization of patients with psoriasis. there are more male than female inpatients. Treatment of psoriasis should consider clinical classification, drug adverse reactions, and individual factors for individual treatment.

AIM: To observe the changes in infiltrating macrophages (M?)and extracellular matrix (ECM) in the kidney in the progressive course of nephrotoxic nephritis (NTN). METHODS: NTN model was established with rabbit-anti-rat nephrotoxic serum. On day 3, 7, 15, 30 and 90, renal biopsies were performed. Renal histology was checked under light microscopy with HE and Masson's trichrome staining sections. M?, fibronectin (FN), type Ⅲ and Ⅳ collagen were examined with immunohistochemistry ABC method. RESULTS: Infiltration of M? appeared on day 3 of NTN and preceded changes of FN and collagen. On day 15 of NTN, all nephritic animals had significant proteinuria, increased serum creatinine, infiltration of M? and deposit of entracellular matrix. On day 90 of NTN, seven nephritic animals improved significantly, while other five developed renal scarring with diffuse infiltration of M? which positively paralleled to renal function and deposit of FN, type Ⅲ and Ⅳ collagen. CONCLUSION: M? infiltrating into renal tissue enhances deposit of ECM and therefore plays important roles in progression or improvement of NTN.

Objective To study the influence of oxy matrine combined with salvia mltiorrhiz on the hepatic fibrosis markers.Methods 60 patients with abnomal prameters of fibrosis level were randomly divided into two groups,30 patients of treatment group treated by salvia miltiorrhiz and oxy matrine,the control group was given liver protectiont treatment,then observed the changes of hepatic fibrosis markers:HA,LN,PCⅢ,and ⅣC.Results A significant decrease of HA,LN,PCⅢ,and ⅣC were observed in the patients treated with oxy matrine and salvia miltiorrhiz(P

Objective To compare the advantages and disadvantages of the four DNA extraction methods according to the endophytic diversity in the roots, stems, and leaves of mulberry analyzed by polymerase chain reaction-denaturing gradient gel electrophoresis (PCR-DGGE) , and by taking the yield, purity and PCR amplification as indexes. Methods Four common methods, i.e., cetyltrimethyl ammonium bromide ( CTAB) , sterile phosphate buffered saline (SPBS) vibration, liquid nitrogen grinding (LNG) , and KIT methods, were used to extract the total DNA from different tissues of mulberry, and then were compared based on the diversity analysis results for endophyte by PCR-DGGE. Results From the roots and stems of mulberry, we got the highest concentration of DNA by LNG extraction method, and got the lowest concentration by SPBS extraction method. But for the leaves of mulberry, the results of the four extraction methods were completely opposite to those for the roots and stems. For different tissues of mulberry, the purity of DNA extracted by KIT method was the best. According to the endophytic bacteria diversity analyzed by 16S rDNA PCR-DGGE, the appropriate method for extraction of DNA was LNG or CTAB, but was not KIT. And according to the results of endophytic fungi diversity analyzed by ITS PCR-DGGE, the best extraction method was KIT, and the unsuited methods varied from the tissues of mulberry. Conclusion The optimum DNA extraction method for mulberry varies from the tissues of mulberry and endophtic bacteria.

AIM: To determine whether the eukaryotic initiation factor-4E (eIF-4E) inhibition facilitates the degradation of heparanase mRNA and alters heparanase protein expression in human colon adenocarcinoma cell line, LS-174T. METHODS: A 20-mer antisense s-oligodeoxynucleotide (asODN) targeted against the translation start site of eIF-4E mRNA were introduced into LS-174T cells by lipid-mediated DNA-transfection. eIF-4E protein and mRNA levels were detected by Western blot and RT-PCR, respectively. The mRNA levels of heparanase were determined by Northern blot. The alterations of heparanase expression were confirmed by Western blot analysis. RESULTS: The 20-mer asODN against eIF-4E specifically and significantly inhibited eIF-4E protein expression. Following eIF-4E inhibition, a significant reduction of heparanase mRNA was observed on Northern blot, and at the same time, heparanase protein expression significantly decreased as well. CONCLUSIONS: The results indicate that the inhibition of eIF-4E strongly reduces the stability of heparanase mRNA in colon adenocarcinoma cell line, LS-174T and resultes in an apparent reduction in the expression of heparanase protein. AIM To observe the clinical role for Supplemented Xijiao Powder (Bubali cornu,Coptidis Rhizoma,Cimicifugae Rhizoma,etc.) in management of hepatitis B virus (HBV) associated acute-on-chronic liver failure.METHOD Seventy-five patients,including the ones receiving internal medicine treatment,were divided into control group and experimental group.The experimental group received conventional treatment and additionally took Supplemented Xijiao Powder for one month.The clinical symptoms and signs were observed.Levels of ALT,AST,TBIL,ALB,CHE,PTA were measured,and the incidence of liver failure was evaluated.RESULTS Supplemented Xijiao Powder could improve symptoms,signs and the level of PTA.The incidence of liver failure tended to reduce.The comparison of other indexes had no marked difference.CONCLUSION Supplemented Xijiao Powder has a certain therapeutic effect on HBV associated prioracute-on-chronic liver failure.Its mechanism may be related to blocking the occurrence of liver failure.

Objective To investigate the therapeutic efficacy of the combined therapy of daoyin prescription and cognitive training on vascular dementia(VD).Methods Totally 75 patients with VD were randomly divided into 3 treatment groups:daoyin prescription,cognitive training and two combined treatments(n =25 each) for 3 months.Mini mental state examination(MMSE),Loewenstein occupational therapy cognitive assessment(LOTCA)and activities of daily living scale(ADL)were used for assessing therapeutic efficacy among three treatment groups.Results There were no significant differences in baseline clinical data among the three groups (P ＞ 0.05),possessing data comparability.Scores of MMSE,LOTCA,ADL and LOTCA subitems of orientation,visual perception,spatial perception,visual movement,thinking operation and attention were significantly improved in the 3 groups after 3 months intervention versus pre-treatment with three treatments(all P＜0.01).There were significant differences in scores of MMSE,LOTCA,ADL and LOTCA subitems of orientation,visual perception,visuo-motor organization and attention between combination therapy group versus daoyin prescription group and cognitive training group (all P ＜ 0.05).Conclusions Combined treatments of daoyin prescription and cognitive training have better effect on vascular cognitive function in patients with and VD.It can markedly improve the activities of daily living and cognitive function.It is worthy of further research in clinical medicine.

ObjectiveTo analyze the risk factors for false negative diagnosis of obscure gastrointestinal bleeding (OGIB) by capsule endoscopy.MethodsA total of 133 OGIB inpatients,104 in true positive group and 29 in false negativc group,were reviewed.The features of demography,diseases and capsule endoscopy were collected and then analyzed,which included 10 variables like age,sex,time of bleeding,diseases accompanied,type and location of the disease,hemoglobin concentration,transit time of CE,quality of CE pictures and type of purgative agents.All data were analyzed with t test,and all the enumeration data were analyzed with chi square test.Logistic regression was used to analyze the correlation between the factors and results of diagnosis.ResultsAge ( t =2.095,P =0.038 ),concentration of hemoglobin ( t =2.143,P=0.034),type (X2 =20.222,P ＜0.001) and location (X2 =33.732,P ＜0.001) of the diseases,image quality of the CE (X2 =9.219,P =0.002 ) and the type of purgative agents (X2 =6.999,P =0.024) were found to have statistical differences between the two groups.Chi-square and partition Chi-square test revealed the occurrence of civerticulosis and lesion location,i.e.lower ileum and ileumcecum,were of statistical difference between the two groups (X2 =22.233,P ＜ 0.001 and x2 =24.412,P ＜ 0.001 ).Univariate logistic regression showed diverticulosis ( OR =0.102,P ＜0.001 ),lower bowel diseases ( OR =0.110,P ＜0.001 ),poor quality of CE pictures ( OR =0.258,P =0.004 ) and the use of sodium phosphate agent ( OR =0.367,P =0.027) were risk factors for false negative diagnosis,while older age facilitated diagnosis ( OR =1.024,P =0.041 ).However,multivariate logistic regression showed no statistic significance in type of purgative agent ( P =0.05 ) or the concentration of hemoglobin ( P =0.394).Furthermore,elder age facilitated positive diagnosis ( OR =1.031,P =0.032),while diverticulosis ( OR =0.118,P =0.001 ),lower bowel diseases ( OR =0.145,P =0.001 ) and poor quality of CE pictures ( OR =0.245,P =0.016) were correlated with higher probability of false negative diagnosis.ConclusionAge,disease type,disease location and image quality exert great influence on CE diagnosis.Diverticulosis,lower location of the diseases and poor CE image quality are risk factors for false negative diagnosis.

Objective To compare the dosimetric effects of micro-multileaf collimator (MLC)(2 mm leaf width) and conventional MLC (10 mm leaf width) on inverse intensity-modulated radiotherapy(IMRT) in intracranial stereotactic radiosurgery(SRS). Methods In view of the fact that the micro-MLC has a small open field,30 patients with intracranial tumor with a<10 cm diameter were enrolled in this study. Their inverse dynamic IMRT plans were established using conventional MLC (conventional group) and micro-MLC (micro group) with the same other conditions. The radiation doses to the target volume and the organs at risk (OAR) were compared between the two groups with t test. Results Compared with the conventional group, the micro group had a significantly better dose distribution in the target volume (P=0.019). However, there were no significant differences in D98,D95,D50,and D3between the two groups (P=0.774,0.650,0.170,0.080). The micro group had a 58.7% lower mean homogeneity index and a 20.1% higher mean conformity index than the conventional group (P=0.000). The micro group had significantly lower radiation doses to OAR than the conventional group (P=0.044). The mean Dmeanand Dmaxof the brain stem in the micro group were 10.0% and 8.2%,respectively,lower than those in the conventional group (P=0.768,0.753). The mean Dmeanand Dmax of the right eye and left eye in the micro group were 16.5%,19.3%,21.4%,and 13.4%,respectively,lower than those in the conventional group (P=0.572,0.775 and 0.734,0.630). The mean Dmaxof the left lens, right lens, left optic nerve, right optic nerve, and optic chiasm in the micro group were 50.4%, 24.1%, 38.5%, 27.8%, and 5.7%, respectively, lower than those in the conventional group (P=0.172,0.467, 0.521,0.740,0.899). The PRV100,PRV50,and PRV25of the normal tissue in the micro group were no more than those in the conventional group(P=0.839,0.832,0.972). Conclusions In inverse IMRT in intracranial SRS,micro-MLC is better than conventional MLC because it can improve CI of the target volume and reduce the radiation doses to OAR.

OBJECTIVE: To observe the changes of pulmonary function and Notch signaling pathway of lung tissues in adjuvant-induced arthritis rats, and to investigate the mechanism of reduced lung function. METHODS: A total of 30 rats were randomly divided into a normal group and a model group. Rats in the model group were induced to establish the adjuvant arthritis AA model by intradermally injecting 0.1 mL Freund's complete adjuvant into the right paw. After 30 days, we observed the paw edema volume, arthritis index, pulmonary function, histomorphology, and Notch receptor/ligand of the lung tissue. RESULTS: Compared with the normal group, the paw edema volume, arthritis index, average expiratory flow within 0.3 s (FEV0.3/FVC), and the level of Notch3, Notch4 and Jagged2 of the lung tissue in the model group was significantly increased, while maximum expiratory flow at 50% of vital capacity (FEF50), maximum expiratory flow at 75% of vital capacity (FEF75), forced expiratory flow (PEF) and the expression of Notch1 of Jagged1 and Delta1 in the lung were significantly decreased (P<0.05, P<0.01). There were significant positive correlations between FEV0.3/FVC and Notch4. FEV0.3/FVC, FEF25, FEF50 and Notch3, Delta1 were negatively correlated, respectively (P<0.05, P<0.01). CONCLUSION While arthritis occurs in AA rats, pulmonary function declines and significantly correlates with the expression of Notch receptor/ligand. The deposition of immune complex in the lung after the injection of CFA activates the Notch signaling pathway, and results in further decline of pulmonary function by signaling cascades.
