1.Analysis of the losing situation of nursing human resource in second-class and upwards hospital of Xinjiang

Objective To understand the internal structure,loss direction of loss nursing staff, compare the current condition of nurses loss between minorities and Han nationality nurses, analyze the relationship between nurse turnover intention and job satisfaction in second-class and upwards general hospital of Xinjiang. Methods Adopting stratified random method, nursing staff from 61 second-class and upwards general hospitals were selected to participate the questionnaire survey. Results Loss of nurses in Xinjiang increased year by year. Nurses with age under 35 years old, college education, employment and with primary titles were most vulnerable to loss. The turnover rate of nurses with Han nationality was higher than minority nurses. Income, occupational status, learning assistance services, opportunities for personal advancement and retirement benefits constituted negative correlation with turnover intention, among which income level was most related. Health care services, work scheduling arrangements and the components of job independence demonstrated no significant correlation. Conclusions To improve income level of nurses and increase working enthusiasm of nurses with higher education and employment nurses, especially nurses with Han nationality is an important means to reduce the loss of nurses.

Genomic imprinting plays a fundamental role in mammal fetal growth and behavior. Abnormal expression of imprinted genes is associated with some genetic diseases and cancers. Imprinted insulin-like growth factor 2 (IGF-2) controls fetal growth by regulating nutrient transportation in placenta. Paternal uniparental disomy, duplication of paternal allele and loss of imprinting are 3 molecular mechanisms of IGF-2 overexpression that can cause Beckwith-Weidemann’s syndrome (BWS). Some assisted reproductive techniques may cause some epigenetic changes that affect embryonic and postnatal development.
3.Evaluation of the quality of life of Hazak patients with hypertension in Urumqi Nanshan pasturing area

Objective To evaluate the quality of life(QOL)of Hazak patients with hypertension in Urumqi Nanshan pasturing area.Methods Totally 900 Hazak hypertensive patients were investigated by using questionnaire of SF-36 quality of life,Stepwise regression analysis was used to study the correlated factors.Results The scores of the quality of life was (413.98±146.31),Their physical function,bodily pain,general health and mental health dimensions had lower scores.The factors which had influenced the QOL of patients was age,education level,the per capita monthly income,the payment of medical expenses,medical treatment,the nearest clinique,energy,sleep quality,availability of negative life events,compliance,whether or not having chronic diseases,social support,pain and others.Conclusions The quality of life of Hazak hypertensive patients in Urumqi Nanshan pasturing area was poor,medical workers should pay high attention to health eduction for these patients,and improve their life quality through increasing cognition of hypertension-related knowledge and simplify treatment protocol.
4.A survey of patients compliance and its influencing factors in Hazak hypertensive patients in Urumqi nanshan pasturing area

Objective To investigate the patients compliance and its influencing factors in Hazak hypertensive patients in Urumqi nanshan pasturing area and put forward corresponding nursing measures. Methods We investigated 625 Hazak hypertensive patients in their compliance behaviors by self-designed questionnaires and analyzed the results. Results 4.3% patients could maintain medication treatment,90.2% patients could keep physical exercises,2.5% patients could maintain reasonable diet,14.4% patients could quit smoking,30.3%patients could stop drinking and 3.8% patients could persist clinical follow-up. The patients compliance was related with their age, forgetting to take medicine, stop taking medicine because of hypertension alleviation and alteration of living pattern(P<0.01).Conclusion The patients compliance could be improved by increasing knowledge about hypertension, strengthened health education, predigested treatment scheme and support from society and families.
5.Regulation of ICC Autophagy -- New Target for Treatment of Intestinal Dysmotility in Ulcerative Colitis

Intestinal dysmotility symptoms,such as abdominal pain,diarrhea and tenesmus are frequently seen in patients with ulcerative colitis( UC). Interstitial cells of Cajal( ICC)has been accepted as a therapeutic target for gastrointestinal motility disorders and regulation of autophagy might be a potential strategy for various diseases. Study on relationship between intestinal dysmotility in UC and ICC autophagy and the intervention mechanism of related drugs will benefit the controlling of disease activity,relieving of dysmotility symptoms and improving of quality of life in UC patients. In this review article,the possible relationship between intestinal dysmotility in UC and ICC autophagy,as well as the possibility of regulating ICC autophagy as a treatment strategy for UC intestinal dysmotility were summarized.
6.The surgical reconstruction of congenital aural atresia via tympanic antrum approach

Objective: To investigate the effect of surgical reconstruction of congenital aural atresia via tympanic antrum approach and the prevention for reatresia of the aural canal. Methods: From 1993 to 1998, 89 patients (94 ears) with congenital aural atresia were operated via tympanic antrum approach. All of them got aural canals reconstruction and tympanoplasty, including 80 ears of type Ⅱ, 11 ears of type Ⅲ and 3 ears for fenestration of inner ear. Kenacort\|A and dilator were used to prevent reastresia of the aural canals. Results: The tympanic antrums of 94 ears were encountered smoothly, and their hearings were reconstructed. The hearing improvement were found over 20dB in 80 ears (85.1%), over 25dB in 49 ears (52.1%). Kenacort\|A and dilator should be applied when the reastresia began to appear, and then the good effect could be made. Conclusion: It′s easy to master for surgical reconstruction of congenital aural atresia via tympanic antrum approach, and it also owns the characteristics of safety, save time and reliable effect. It should be managed as soon as the reastresia appears.
7.A survey of medical staff in Obstetrics and Gynecology department on prevention of mother-to-child transmission of hepatitis B virus

Objective To investigate the knowledge awareness of medical staff in department of Obstetrics and Gynecology,on prevention of mother-to-child transmission of hepatitis B virus(HBV).Methods Medicatl staff,who attended national or local medical conferences of Obstetrics and Gynecology from July 29th to October 25th in 2011,were invited to complete a questionnaire containing questions on clinical implications of HBV serologic markers,relevant prevention measures and including intervention measures for both pregnant women and infants.Data were collected and analyzed by Chi-square test or Fisher's exact test.Results A total of 559 answers were analyzed.Of the 559 staff,85.0%(n=475) were doctors and 15.0%(n=84) were nurses.The percentages ofstaff from primary,secondary and tertiary hospitals were 13.2%(n=74),36.9%(n=206) and 49.9% (n=279).Overall,90.0% (503/559) participants agreed that positive hepatitis B surface antigen (HBsAg) was contagious,but 27.5% (154/559) mistook that the presence of antibody against hepatitis B e antigen (anti-HBc) and/or antibody against hepatitis B core antigens (anti-HBc) with negative HBsAg was contagious.Totally,96.3% (519/539) respondents knew that pregnant women should be screened for HBV infection,and those from secondary hospitals had more consciousness of HBV screening for pregnant women than those from tertiary hospitals [99.0% (201/203) vs94.7%(250/264),x2=6.466,P=0.011].Although,95.3% (511/536) realized that infants of mothers with positive HBsAg should receive hepatitis B immunoglobulin (HBIG) and vaccination,but 61.6% (330/536) and 80.4% (427/531) of the enrolled medical staff knew the exact regimen.With the available immunoprophylaxis,13.8% (74/536) participants mistakenly believed that cesarean section may prevent HBV mother-to-child transmission,and only 13.0% (69/532) correctly answered that neonates of women with positive HBsAg can be breast-fed.Additionally,54.4% (290/533) participants mistakenly believed that HBsAg positive pregnant women needed HBIG injection during pregnancy to reduce HBV mother to-child transmission.Conclusions Medical staff in obstetric and gynecological department has essentially mastered the strategies of prevention of mother-to-child transmission of HBV,but much more details should be stressed.
8.A study on temporary pacing site selection in patients with inferior acute myocardial infarction and two or three degree atrioventricular block

Objective To study the feasibility,effectiveness and safety of temporary pacing at the right ventricular outflow tract area in patients with inferior acute myocardial infarction(AMI)and two or three degree atrioventricular block(AVB). Methods Ninety-five patients with inferior AMI and two or three degree AVB admitted into Cangzhou People's Hospital were randomly divided into right ventricular apex pacing group(47 cases) and right ventricular outflow tract region pacing group(48 cases). Seldinger method was used to perform the right femoral vein puncture in which a 6F sheath tube was placed,and a diode temporary pacing electrode was introduced into it in the two groups. Under the X-ray guidance,in the right ventricular apex pacing group,the electrode was pushed from the inferior vena cava to enter into the right atrium,then cross the tricuspid and finally reach the right ventricular apical portion pacing. In the right ventricular outflow tract region pacing group,under the X-ray guidance, after the electrode was withdrawn and rotated slightly clockwise,it was sent forward to the right ventricular outflow tract region pacing. The duration from the puncture at the right femoral vein to the implanted electrode reaching the pacing region,the pacing threshold,the un-favorable pacing,the incidence of ventricular arrhythmia and prognosis were observed in the two groups. Results The pacing time and pacing threshold in right ventricular apex pacing group were obviously higher than those in the right ventricular outflow tract region pacing group〔pacing time(s):336±150 vs. 354±152,pacing threshold(V):0.9±0.4 vs. 0.7±0.3,both P<0.05〕. The mortality in intensive care unit (ICU)〔0(0/48)vs. 2.13%(1/47)〕,the incidence of bad pacing〔10.42%(5/48)vs. 17.02%(8/47)〕and ventricular fibrillation(VF),sustained ventricular tachycardia,contraction of non-sustained ventricular tachycardia or frequent ventricular premature beat(VPB)and other arrhythmia incidence of complications〔10.42%(5/48)vs. 12.77%(6/47)〕in the right ventricular outflow tract region pacing group were all lower than those in the right ventricular apex pacing group. Conclusion The application of right ventricular outflow tract pacing in patients with inferior AMI and two or three degree AVB is safe and effective,and its stability is fine.
9.A Randomized and Controlled Study of rhGM-CSF on Leucopenia Induced by Chemotherapy on Cancer Patients

Objective: To evaluate the efficacy and tolerability of rhGM-CSF in prevention of neutropenia caused by chemothera- py of cancer patients. Methods: A muticenter, randomized,match and crossover clinical study was conducted. 64 enrolled patients were randomized into AB and BA groups and each patient received two cycles of combination chemotherapy.Results:59 patients were evaluted for clinical efficacy.rhGM-CSF significantly increased the number of WBC and ANC at the stage of nadir. The period when the patients, rhGM-CSF also resulted in a hastening recovery from leucopenia and neutropenia.Conclusion: The administraton of rhTM-CSF ensures the implement of sheduled combination chemotherapy. The main side effects such as fever, osteomyalgia,skin rash were generally tolerable.
10.Histological changes following surgically-assisted rapid tooth movement through resistance reduction and distraction osteogenesis in dogs

Objective To investigate the histological changes of rapid tooth movement in dogs treated by resistance reduction and distraction osteogenesis, aiming to establish an animal model and further to reveal the remodeling mechanism of rapid tooth movement. Methods A total of 8 local hybrid dogs were selected as subjects for this study. The second pre-molar was extracted on both sides. The experimental side underwent alvelor surgery for resistance reduction and a home-made tooth-borne intraoral distraction device was installed for rapid tooth movement, while for the other side (control side) only tooth-borne intraoral distraction device was used for rapid tooth movement. The longest active force-delivery span was 2 weeks, followed by 6-week retention. The distance between the moved tooth and anchor unit was recorded weekly, and radiography was performed for each side before and after distraction. The surrounding tissues including periodontal ligament and alveolar bone were sectioned for histological analysis. Results The average distance of tooth movement was 3.55mm on the experimental side and 1.11mm on the control side. The rate of tooth movement was notably higher (P<0.01) and no significant apical root resorption was detected by X-ray on the experimental side. The active alvelor bone remodeling was found on the tension and pressure sides. However, there was no significant difference between the experimental side and the control side after the retention period. Conclusion The rate of orthodontic tooth movement can be accelerated through resistance reduction and periodontal distraction without any unfavorable effects but at minimal anchorage loss.
