Objective To investigate the effect of adrenomedullin on epithelial barrier function of Caco-2 cells under hypoxia/reoxygen-ation injury and the molecular mechanism. Methods Build hypoxia/reoxygenation injury model,Caco-2 cells were randomly divided into three groups:normal control group( N) ,hypoxia/reoxygenation group( H/R) and hypoxia/reoxygenation plus 1 μmol/L adrenomedullin group(H/R+AM).Transepithelial electrical resistance(TEER) was determined, and the laser confocal microscope was used to detect the construction of tight junction. The protein expressions of tight junction proteins (Occludin and ZO-1) and nuclear factor kappa B(NF-κBp65) were examined by using western blotting. Results Pretreatment of AM significantly attenuated the disruption of morphological structure of tight junction caused by H/R.The TEER of N group,H/R group and H/R+AM group were(157.68 ±5.54),(96.06 ±3.42),(134.56 ± 4.72) Ohm/cm2 respectively. After AM pretreatment, the TEER were significantly raised compared with that in H/R group by 40.00%(P<0.05).The relative protein expressions of Occludin and ZO-1 in N group,H/R group and H/R+AM group were [(0.43 ±0.03), (0.27 ±0.04)],[(0.20 ±0.03),(0.15 ±0.07)],[(0.32 ±0.15),(0.21 ±0.03)]respectively.After AM pretreatment, the protein ex-pression of Occludin and ZO-1 were significantly raised compared with that in H/R group(by 60.00% and 40.00%).Simultaneously,the relative protein expressions of NF-κBp65 in N group,H/R group and H/R+AM group were (0.53 ±0.30),(2.89 ±0.16),(1.75 ±0.25) respectively. And H/R induced NF-κBp65 activation was significantly inhibited by AM treatment (decreased as compared with H/R group by 40.00%).Conclusion AM can attenuate intestinal epithelial barrier dysfunction by regulating the expression of intestinal tight junction pro-teins.The inhibition of NF-κB activation may be involved in this mechanism.

Objective To create a new animal model which can preferably simulate the human's scoliosis. Methods Forty-five 4-week-old female Wistar rats were randomly divided into 3 groups. Group 1 underwent subcutaneous left scapula-to-contralateral ilium tethering procedure with a non-absorbable suture, which made the spine convex towards right side, and then removed forelimbs and tails of rats to create the bipedal rats. Tethering sutures were cut at postoperative 8th week, and the spines of rats were then observed during 2 weeks. Group 2 were the same as group 1 but in which scapula-to-ipsilateral ilium tethering procedure was performed. Group 3 were the same as group 1 except that the bipedal rats were not created. All postoperative rats were fed separately in special high cages for group 1 and group 2 or in standard cages for group 3. Operative duration, postoperative first food-taking duration, operative mortality, body weight, relative length of the spinal cord, Cobb angles in the coronal and sagittal planes, modeling mortality, the incidences of re-operation, scoliosis and vertebral rotation among the 3 groups were compared. Results There were no significant differences in the degree of scoliosis or kyphosis among the 3 groups at the time of initial tethering. At 2 weeks after tether release, the incidence of vertebral rotation was significantly higher in group 1 than group 2 (86.7% vs. 33.3%), and the degree of scoliosis or kyphosis was significantly larger in group 1 than group 3 (coronal plane: 30.2°±10.4° vs. 20.5°±7.7°; sagittal plane: 55.6°±10.0° vs. 43.8°±6.3°). There was no significant difference in postoperative first food-taking duration among the 3 groups, and no death of rats occurred in all groups during intraoperative and postoperative 6 hours, although operative duration was significantly longer in group 1 or group 2 than group 3. There were no significant differences in body weight, relative length of the spinal cord, modeling mortality, the incidences of re-operation and scoliosis among the 3 groups. Conclusion The bevel and axial loads of the vertebrae can significantly enhance the incidence of vertebral rotation and the degree of kyphoscoliosis, respectively. The scoliosis model created by scapula-to-contralateral ilium tethering procedure in bipedal rats can preferably simulate the human's scoliosis.

Objective To explore the extent of axillary lymph node dissection in breast cancer patients with positive sentinel lymph nodes. Methods In this series,95 patients underwent complete axillary lymph node dissection (CALND) following positive sentinel lymph nodes identified and 97 patients underwent partial axillary lymph node dissection (PALND) at the absence of positive sentinel lymph nodes. The influence of different extent of axillary lymph node dissection was evaluated. Results The positive rate of level(Ⅰ + Ⅱ) non-sentinel lymph nodes (NSLN) and level Ⅲ lymph nodes was 68.4% and 29.5%,respectively,in CALND group. In PALND situation,57.9% CALND cases would have changed pathologic stage from pN1 to pN2 or pN3.But with level Ⅲ lymph node dissection followed PALND,only 2.1% cases had changed pathologic stage from pN2 to pN3.Multivariate logistic regression showed that the number of positive SLNs (OR =2.157) and positive rate of SLNs (OR =10.374) were risk factors for having positive level Ⅲ lymph nodes.CALND needed longer operation time and larger postoperative drainage volume. Conclusions In cases of breast cancer with ≥ 3 positive sentinel lymph nodes, complete axillary lymph node dissection should be considered.

Objective To observe colorectal tumor's growth in hyperglycemia mice and its vascular endothelial growth factor (VEGF)'s expression, insulin-like growth factor-1 (IGF-1)'s variation of blood through the experiment, then to ascertain whether type 2 diabetes mellitus (T2DM) danger factors to promote colorectal cancer happen and progress or not. Methods The mouse model of colorectal cancer combined T2DM was established. The volume of tumor was observed. After 5 weeks, all mice were executed and IGF-1 in the blood and the expression of VEGF in the tumor tissue was examined. Results The average tumor volume of colorectal tumor-diabetes group [(1628.5±882) mm3] were larger than that of colorectal tumor group [(1950.2±726) mm3] (P <0.05), and its expression IGF-1 of blood [(105.33±32.32) ng/ml] were higher than that of the control group [(69.83±25.57) ng/ml] and colorectal tumor group [(70.17±25.27) ng/ml] (P <0.05). The expression of VEGF [(70.0±11.5)%] in colorectal tumor-diabetes group were significantly higher than that of colorectal tumor group [(42.9±7.5)%] (P <0.05), too. Conclusion The model of T2DM and transplanted colorectal tumor can be duplicated successfully in ICR mice. Diabetes mellitus may be one reason of promoting colorectal cancer progress. Besides high blood glucose, its mechanism is the high level of IGF-1 which can inhibit apoptosis, promote cell differentiation and hyperplasia, and through inducing VEGF duplicating, heighten its expression, promote the tumor vessel growth, lead to tumor happen and metastasis.
