OBJECTIVE:To preliminarily study the stability of 3 pieces of Chinese medicinal formula(CMF)after decoction, and provide reference for guaranteeing storage quality of decocted liquid and improving safety of drug use. METHODS:3 represen-tative formulas of Gegen Huangqin Huanglian decoction(A formula),Wuling powder(B formula)and Didang decoction(C for-mula)from Shanghan Zabing Lun were selected,the decocted liquid were stored under ambient temperature(25 ℃)and refrigerat-ed temperature (4 ℃) after decocting by automatic boiling-machine and packing. The microorganism,precipitation,pH and con-tents of total flavonoids,alkaloid,polysaccharide,total protein after 1,7,14,21,28 d were detected. RESULTS:Compared with the first day,contents of total flavonoids,polysaccharide in formula A at ambient temperature group were significantly in-creased on the 28th(P<0.05),content of polysaccharide in refrigerated temperature group was significantly increased(P<0.05). Content of polysaccharide in formula B at ambient temperature group was significantly decreased(P<0.05). The pH and content of total flavonoids in formula C at ambient temperature group and refrigerated temperature group were significantly increased (P<0.05 or P<0.01). Other indexes showed no obvious changes during the trial period. CONCLUSIONS:Under ambient temperature and refrigerated temperature,liquid ingredients of above decocted CMF will change when storing for 4 weeks. It indicates that the storage time of decocted CMF should not be more than 3 weeks.

Objective:To analyze the clinical characteristics of adult abdominal migraine (AM), and improve the understanding and the accuracy of diagnosis of this disease.Methods:A case of adult AM diagnosed in the First Affiliated Hospital of Zhengzhou University in October 2019 was reported, and patients diagnosed with AM in the medical literature from January 2004 to May 2020 were searched and summarized.Results:A total of 634 articles were retrieved, among which 14 articles reported 17 adult AM patients,totally 18 adult AM patients (11 females),including the case diagnosed in the First Affiliated Hospital of Zhengzhou University. Only two cases were diagnosed within one year after the onset, and the other cases were diagnosed several years later, of which the longest diagnostic delay was 37 years. The location and nature of abdominal pain were diverse. The median frequency of episodes was 2.5 times per month. Sixteen patients had a duration of 2-72 hours per attack, and the longest one lasted for four days. Fourteen patients had nausea, 13 had vomiting, and seven had headache. Fifteen patients had a personal or family history of migraine. Sixteen patients′ episodes were separated by weeks to months. Because nonsteroidal anti-inflammatory drugs were not beneficial to adults with AM, abortive triptan therapy was recommended and anesthetics were considered when necessary. All patients responded to prophylactic migraine therapies.Conclusions:Adult AM is a rare disease. Patients with unexplained, recurrent and moderate to severe abdominal pain, combined with headache, a personal or family history of migraine, are highly suspected of having AM. Early diagnosis and prophylactic migraine therapies could contribute to good prognosis.

ABSTRACT:Objective To investigate the prevalence of thyroid nodules in Xi’an City by ultrasonography and study the correlation between thyroid nodules and the components of metabolic syndrome.Methods We recruited 4 527 subjects in this cross-sectional study and divided them into thyroid nodules (TN)group and non-thyroid nodules (NTN)group.The height,weight,blood pressure,blood sugar,lipid,and serum uric acid were measured and analyzed.The relationship between thyroid nodules and thyroid function was assessed in the subjects who received TT3,TT4,TSH,TGAB,and TPOAB tests.Results ① The prevalence of thyroid nodules was 31.8% and was significantly higher in the female patients (38.4% vs.27.1%,χ2=64.44,P<0.001).However,the prevalence of metabolic syndrome was 9.1% and was significantly higher in the male ones (11.4% vs.5.9%,χ2=40.39,P<0.001).② Compared with that in NTN group,TT3 in TN group was significantly increased (t=6.04,P<0.001), but the other indexes did not obviously change in the two groups (P>0.05).③ Age,systolic blood pressure,and TG,LDL-C and serum uric acid levels were remarkably higher in TN group than in NTN group (P<0.001 ). Conclusion The prevalence of thyroid nodules in Xi’an City is noticeably high.High systolic blood pressure level, dyslipidemia and elevated uric acid are associated with thyroid nodules.Therefore,screening and management of the individuals with aforementioned characteristics deserve more attention.

Objective To investigate the level and clinical significance of D-dimer(D-D) and fibrinogen(Fib) in patients with first-episode schizophrenia.Methods The 83 patients with first-episode schizophrenia from March 2014 to December 2016 were selected as the study group,meanwhile 58 healthy subjects without mental illness and major somatic diseases were selected as control group.Two groups of subjects were collected from fasting sodium citrate anticoagulated plasma to detect level of D-D and Fib.Resuits The level of D-D in the first-episode schizophrenia group was(240.42±245.78)μg/L,which was significantly higher than that in the control group(76.53±71.25)μg/L,and the difference was statistically significant(P＜0.05).While the level of Fib in the first-episode schizophrenia group (2.62 ± 0.49) g/L was significantly lower than that of the control group (2.87 ± 0.65) g/L,and the difference was statistically significant(P＜0.05).The increase in D-D levels and the reduction in Fib levels in patients with firstepisode schizophrenia were not related to gender.Conclusion he patients with first-episode schizophrenia in acute stage may have hypercoagulable status and secondary fibrinolysis.

Th17 cell is a newly developed CD4 +T cell subsets,play a pro-inflammatory role and participate in the pathogenesis of many autoimmune diseases mainly through secretion of cytokines such as interleukin-17 (IL-17),many of the biological effects of Th17 cells are closely related to the IL-17,this article reviews the relation-ship between Th17/IL-17 axis and related cytokines and kidney the pathogenesis of disease,in order to more in-depth understanding of the pathogenesis of kidney disease,provide the basis for looking for new targets of diagnosis and treatment of kidney disease and new treatment strategies.

Objective To explore the predictive value of serum homo sapiens microRNA(hsa-miR)-30c-5p expression in patients with type 2 diabetes mellitus(T2DM)for microvascular complications.Methods A total of 205 T2DM patients admitted to Bazhong Central Hospital from May 2021 to September 2022 were selected as the diabetes group,and the diabetes group was further divided into diabetes with combined group(n=124)and non combined group(n=81)according to the microvascular complications of the patients.In addition,205 healthy people who underwent physical examination during the same period were selected as the control group.The expression of hsa-miR-30c-5p in serum was detected by reverse transcription-polymerase chain reaction(RT-PCR)and compared.The factors affecting microvascular complications were analyzed using multivariate logistic regression analysis,and receiver operating characteristic(ROC)curves were plotted to predict the value of serum hsa-miR-30c-5p expression in predicting microvascular complications in T2DM patients.Results The expression of serum hsa-miR-30c-5p in the combined group(0.58±0.06)and the non-combined group(0.72±0.08)were lower than that in the control group(0.89±0.21),and the differences were significant(t=16.038,7.079,all P=0.001).The combined group was lower than the non-combined group,and the difference was significant(t=14.289,P=0.001).The course of diabetes[(OR(95％CI):3.873(2.976～4.770)],uric acid[(OR(95％CI):2.125(1.211～3.040)]and glycosylated hemoglobin[(OR(95％CI):2.680(1.745～3.616)]were independent risk factors for microvascular complications in T2DM patients(all P＜0.05),while the time within the target range of glucose[(OR(95％CI):0.491(0.135～0.846)]and serum hsa-miR-30c-5p[(OR(95％CI):0.532(2.976～4.770)]were protective factors for microvascular complications in T2DM patients(all P＜0.05).The sensitivity,specificity and area under the curve(95％CI)of serum hsa-mir-30c-5p expression in predicting microvascular complications in T2DM patients were 81.45％,85.19％and 0.802(0.741～0.854),respectively.Conclusion The expression of serum hsa-miR-30c-5p in patients with T2DM is abnormally reduced,and serum hsa-miR-30c-5p is a protective factor for microvascular complications in patients with T2DM.It may have a certain predictive value for microvascular complications in patients with T2DM.

Objective To evaluate the relationship between heme oxygenase-1 (HO-1) expression and autophagy during acute lung injury in septic mice.Methods Forty-eight male C57BL/6 mice,aged 6-8 weeks,weighing 20-25 g,were randomly divided into 4 groups (n=12 each) using a random number table:sham operation group (group SH),sepsis group (group CLP),HO-1 agonist hemin plus sepsis group (group Hemin+CLP) and HO-1 inhibitor SnPP plus sepsis group (group SnPP+CLP).Sepsis was induced by cecal ligation and puncture in CLP,Hemin+CLP and SnPP+CLP groups.Hemin 50 μmol/kg and SnPP 50 μmol/kg were intraperitoneally injected at 2 h before operation in Hemin + CLP group and SnPP+CLP group,respectively.Arterial blood samples were taken at 24 h after operation for blood gas analysis,PaO2 was recorded,and oxygenation index was calculated.Lungs were removed for examination of pathological changes which were scored and for determination of autophagosome count (with an electron microscope),weight/dry weight ratio (W/D ratio) and expression of HO-1,Becling-1 and microtubule-associated protein 1 light chain 3 Ⅱ (LC3 Ⅱ) in lung tissues (by Western blot).Results Compared with group SH,oxygenation index was significantly decreased,and W/D ratio,autophagosome count and pathological scores were increased in CLP,Hemin+CLP and SnPP+CLP groups,and the expression of HO-1,Beclin-1 and LC3 Ⅱ was significantly up-regulated in CLP and Hemin+CLP groups (P＜0.05).Compared with group CLP,oxygenation index and autophagosome count were significantly increased,W/D ratio and pathological scores were decreased,and the expression of HO-1,Beclin-1 and LC3 Ⅱ was up-regulated in group Hemin+CLP,and oxygenation index and autophagosome count were significantly decreased,W/D ratio and pathological scores were increased,and the expression of HO-1,Beclin-1 and LC3 Ⅱ was downregulated in group SnPP+CLP (P＜0.05).Conclusion Up-regulated expression of HO-1 mediates autophagy,which is involved in the endogenous protective mechanism underlying acute lung injury in septic mice.

Circular RNA (circRNA) is a ubiquitous class of noncoding RNAs (ncRNAs) in all kingdoms of life with diverse structures.CircRNA is an endogenous noncoding RNA molecule with covalently close cyclic structure which does not have 5'end cap and 3'poly (A) tail.It is found to be evolutionary conservative and stable with tissue specificity.CircRNAs have many biological functions.In addition to acting as miRNA sponges,many other functions are being revealed,including regulator of gene transcription,and splicing and protein translation.It is involved in ovarian epithelial neoplasms,pre-eclampsia and other development.CircRNAs could regulate the expression of numerous gynecological cancer-related microRNA (miRNAs).The circRNA-miRNA-messenger RNA(mRNA) axis is a known regulatory pattern of several cancer-associated pathways,with both agonist and antagonist effects on carcinogenesis.Numerous studies have shown that circRNAs may become potential targets and clinical diagnostic markers for reproductive and gynecological diseases.This review focuses on the biogenesis and properties of circRNAs,databases of circRNA,their functions and potential significance in gestational,tumor and gestational diseases.

Sepsis was induced by cecal ligation and puncture in S and LY+S groups. LY294002 30 mg∕kg was intraperitoneally injected at 2 h be?fore operation in group LY+S. Arterial blood samples were taken at 24 h after operation for blood gas analy?sis, PaO2was recorded, and oxygenation index was calculated. Lung specimens were obtained for examina?tion of pathological changes which were scored and for determination of autophagosome count(using trans?mission electron microscope), wet∕dry weight ratio(W∕D ratio)and expression of Akt, phosphorylated Akt(p?Akt), Beclin?1 and microtubule?associated protein 1 light chain 3 Ⅱ(LC3 Ⅱ). The p?Akt∕Akt ratio was calculated. Results Compared with group SH, oxygenation index was significantly decreased,and the W∕D ratio and pathological score were increased in S and LY+S groups, the autophagosome count was significantly increased, p?Akt∕Akt ratio was increased, and the expression of Beclin?1 and LC3Ⅱ was up?regulated in group S(P<0.05). Compared with group S, oxygenation index was significantly de?creased, and the W∕D ratio was increased, the autophagosome count was decreased, pathological scores were increased, p?Akt∕Akt ratio was decreased, and the expression of Beclin?1 and LC3Ⅱwas down?regu?lated in group LY+S(P<0.05). Conclusion PI3K∕Akt signaling pathway activation mediates autophagy and is involved in the endogenous protective mechanism of acute lung injury in septic mice.

Objective:To perform cross-cultural adaption of the KING′s Parkinson′s Disease Pain Scale (KPPS), explore its reliability and validity in Chinese Parkinson′s disease (PD) patients, and to create the new version of the pain scale which adapts to the Chinese PD patients.Methods:This study enrolled 225 patients, including 121 men and 104 women who were selected from the Outpatient Center of Movement Disorders Clinic of the Second Affiliated Hospital of Soochow University from July 2018 to July 2020. All patients completed the evaluation of the Chinese Version of KPPS (KPPS-CV). According to the preliminary evaluation results, the expert group modified KPPS-CV after discussion, and developed a Modified KPPS-CV (MKPPS-CV). These patients then completed the MKPPS-CV evaluation during the 3-month follow-up. Cross-cultural adaptation was performed according to published international guidelines that include translation, back-translation, expert review, and pretesting. The following psychometric properties were evaluated: basic item analysis; floor and ceiling effects; construct validity; content validity; criterion validity (Spearman′s rho between the KPPS-CV and Numeric Rating Scale); internal consistency reliability (Cronbach′s alpha); test-retest reliability (intra-class correlation coefficient, ICC).Results:In item analysis, 50% of the items had poor discrimination (critical ratio<3.0), and floor effect was found in all domains (proportion of 0 point>15%). The items were reclassified after exploratory factor analysis. The content validity of item 3, item 10 and item 11 was low (item-level content validity index<0.78). Criterion validity showed the highest correlations (Spearman′s rho>0.88) between the KPPS-CV and Numeric Rating Scale. While overall scale reliability was minimally acceptable at 0.46, which showed a poor reliability of this scale. Test-retest reliability was excellent for each item (Spearman's rho>0.85). The Cronbach′s alpha of MKPPS-CV (0.76) was higher than that of KPPS-CV (0.46). It showed a great improvement after the modifying.Conclusions:When using scales that are not developed for local populations, differences in culture and clinical practices should be taken into account. MKPPS-CV is an acceptable, valid measure to evaluate pain in Chinese PD patients, which is more suitable for Chinese people.
