1.Determination of in vitro Percutaneous Rate and in vitro of Release of Compound Patch of Hyperosteogeny

Objective: To study the feasibility of percutaneous absorption of Compound Patch of Hyperosteogeny(CPH). Methods: The content of ferulic acid,an index composition in percutaneous receptor fluid and release receptor fluid were determined by HPLC.Results: The results of in vitro transdermal delivery experiment and in vitro release experiment showed that ferulic acid permeated at the constant speed of 0.2142?g?cm -2 ?h -1 in 24 hours and its release coincided with Higuchi Equation.Futhermore,the release rate was 14.53?g?cm -2 ?h -1/2 . Conclusion: CPH is a skeleton controlledtransdermal delivery system whose permeation speed is limited by skin.
3.Comparison of amino acid infusion with a forced-air warming system for rewarming hypothermic postoperative cancer patients without shivering

Background and purpose:Perioperative hypothermia will affect the prognosis of cancer patients. Amino acid infusion can increase the core temperature by endogenous thermogenesis. And the forced-air warming system has gained high acceptance as a measure for rewarming. This study aimed to find out whether amino acid infusion was effective to treat postoperative hypothermia and how well the treatment effect was when compared with the forced-air warming system.Methods:Fifty-seven ASAⅠ orⅡ patients aged 18-60 years undergoing elective esophageal or gastric cancer operation under epidural-general anesthesia and whose core temperature were below 36℃. When admitted to the recovery room wererandomly divided into 3 groups (n=19): GroupⅠ received intravenous infusion of mixed amino acid at a rate of 2 mL·kg-1·h-1 (A); GroupⅡ received a forced-air system (B); groupⅢreceived no therapy (C). Rectal temperature and thermal comfort were recorded per 5 min during the ifrst 1 h and oral temperature and thermal comfort were recorded at the 2, 6 and 24 h. ABG was recorded when patients were admitted to the recovery room and at the ifrst hour.Results:At the ifrst hour, the rectal temperature and thermal comfort of groups A and B were higher when compared with group C (P<0.05), and there was no difference between groups A and B (P>0.05). At the second and sixthhour, the temperature and thermal comfort of group A were higher when compared with group B and C (P<0.05), and there was no difference between groups B and C (P>0.05). At the 24th hour, there were no statistically signiifcant differences in the temperature and thermal comfort among the three groups (P>0.05).
Conclusion:The rewarming effect of infusion of mixed amino acid is better than that of the forced-air warming system. It is the more effective and convenient method to rewarm the postoperative hypothermia.
4.Genotyping of Yersinia pestis isolated from Gansu Province by clustered regularly interspaced short palindromic repeats

Objective To type Yersinia pestis isolated from Gansu Province,and to study the trend of diffreent strains in different administrative regions and different years.Methods Totally 193 strains were enrolled in this study,including 9 strains of Ganning Dauricus type,18 strains of Aerjin type,45 strains of Qilian type and 121 strains of Qingzang type.These strains were genotyped by clustered regularly interspaced short palindromic repeats (CRISPR),and genotypes were named according to international standard.Genotyping by CRISPR in different administrative regions and different years of Gansu Province was explored.Results Two clusters (Ca7 and Cb4),including four genotypes (genotypes 7,22,24 and 26) were classified by CRISPR.From the point of view of origin,genotype 24 was the main genotype in Akesai 36.36% (16/44),Subei 36.17% (17/47),Yumen 50.00% (5/10) and Su'nan 38.67% (29/75);the main genotype of Xiahe and Huining was genotype 26 (4/7);the main genotype of Shandan was genotype 22 (1/1).From the point of view of time,the main genotype of Yersiniapestis in Gansu Province during the years of 1960-1969,1970-1979 and 1980-1989 was genotype 26 [53.33% (8/15),60.00% (6/10) and 48.28% (14/29)];the main genotype was genotype 22 [40.91% (18/44)] during the years of 1990-1999;and the main genotype was genotype 24 [43.16% (41/95)] during the years of 2000-2009.Conclusion Four genotypes of Yersiniapestis in Gansu Province are quite different in different administrative regions and different years.
5.Research of antitumor immunity of gastric carcinoma by DC vaccine with AⅣ in vitro

Objective Through experiments in vitro,we explored the role of Chinese medicine monomer-Astragaloside Ⅳ with DC vaccine in the body immunologic function.Methods Peripheral blood mononuclear cells (PBMCs)from healthy volunteers were isolated,cultured and generated in vitro,pulsed with tumor antigen from SGC7901 gastric carcinoma cell lysates,produced DC vaccine.Observe T-cell proliferation responses stimulated by DC vaccine with AⅣ group,DC vaccine group and AⅣ group respectively,and the anti-tumor effects on SGC7901 cells in vitro.Results ①The T-cell proliferation rate of DC vaccine with AⅣ group and DC vaccine group were significantly higher than AⅣ group and T-cell group(negative control group)(P=0.000).The stimulating efficacy on T-cell proliferation of DC vaccine with AⅣ group was higher than that of DC vaccine group(S/R 1∶5,1∶10,1∶50,1∶100,P=0.013,0.014,0.017,0.019).Compared with T-cell group,the T-cell proliferation rate of AⅣ group had no statistically significance(P=0.185).②The killing rate of effector cells actived by DC vaccine with AⅣ group and DC vaccine group against SGC7901 gastric carcinoma cells were higher than that of AⅣ group and T-cell group(P=0.000).The Killing power of DC vaccine with AⅣ group was stronger than that of DC vaccine(E/T 5∶1,10∶1,20∶1,50∶1,P=0.023,0.012,0.016,0.011);while the group of AⅣ group and T-cell group cannot killing tumor cells.Both had no statistically significance(P=0.267).Conclusion AⅣ can stimulate T-cell proliferation and enhance the activity of killing tumor cells by DC,which induced specific antitumor response against stomach carcinoma cells effectively.
6.Preparation and application of polyclonal antibody of human transcription factor CTCF N terminus protein.

To investigate the function of CTCF and understand the pathogenesis of tumors better, we produced rabbit polyclonal antibody of human transcription factor CTCF protein and detected its expression in several kinds of human cancer cells and tissues. GST fusion protein of human CTCF-N domain was purified by GSTrap-FF affinity chromatography and was successfully expressed under induction of IPTG in E. coli BL21. Western blotting analysis demonstrated that the polyclonal antibody can recognize the endogenous CTCF from HepG2, MCF-7 and HeLa cells specifically. The produced antibodies can be used for gene expression regulation and tissue distribution study at protein level.

7.Status of professional identity for general practice residents standardized training and influencing factors

The professional identity and the influencing factors were surveyed among 110 resident physicians of general practice standardized training program in the First Affiliated Hospital of Xi'an Medical College with a self-designed questionnaire,and 100 valid questionnaires were received.The average score of professional identity was (66.10 ± 13.644),and the six dimensions ranked from high to low:professional behavior,professional efficacy,professional cognition,professional emotion,professional value and professional commitment.There were significant differences in the scores of professional identity among general practitioners with different monthly income(F=2.947,P=0.037).The survey indicates that the professional identity of the resident physicians of general practice standardized training program is generally low and the income is an important influencing factor.
8.Relationship of atrial fibrillation with hip fractures caused by falls in the elderly

Objective To explore the relationship between atrial fibrillation and hip fractures caused by falls in the elderly.Methods All patients aged 65 years or older who underwent surgical treatment for hip fractures caused by falls in our department from January 2015 to December 2017 were enrolled.The relationships of atrial fibrillation as well as chronic medical history with hip fractures were retrospectively analyzed.The control group was matched 1 to 1 by age,sex,and admission time.Chi-square test and Logistic regression analysis were used to calculate the odds ratio (ORvalue) of fractures in patients with atrial fibrillation.Results There was no significant difference in the history of smoking,drinking,diabetes,coronary heart disease,heart failure,chronic kidney disease,chronic obstructive pulmonary disease(COPD),and Parkinson's disease between the two groups.A total of 37 patients had a history of atrial fibrillation in the fracture group,compared to 18 patients with atrial fibrillation in the control group.The incidence of atrial fibrillation in the fracture group and the control group were 9.6% and 4.7%,respectively,with crude OR=2.18,95%CI(1.32-4.15),P=0.008 and adjusted OR=2.27,95%CI(1.37-5.20),P=0.02.Compared with the control group,the incidence of stroke (x2 =14.06,P < 0.05),osteoporosis (x2 =200.64,P < 0.01) and cognitive impairment(x2 =8.23,P <0.01)increased in the fracture group.When classified by age and sex in the fracture group,the incidence of atrial fibrillation had no significant difference between different gender and age subgroups.Conclusions Atrial fibrillation is an independent risk factor for hip fractures caused by falls in the elderly.There is no significant difference in the incidence of atrial fibrillation in hip fracture patients between the different ages(65-74 years,75-84 years,≥of 85 years)and genders.In addition,stroke,osteoporosis,and cognitive impairment increase the risk of hip fractures caused by falls in elderly patients.
9.Clinical characteristics and risk factors of occult pneumonia in advanced elderly patients

Objective To explore the clinical characteristics and risk factors of occult pneumonia in advanced elderly patients.Methods A total of 170 advanced elderly patients with community-acquired pulmonary infection in geriatric department of Xijing hospital from January 2014 to October 2017 were enrolled.Patients were divided into two groups:the observation group (with occult pneumonia,n=80)and the control group(without occult pneumonia,n =90).Clinical data of patients were collected,and clinical characteristics and related risk factors of occult pneumonia in elderly patients were analyzed.Results There was no statistically significant difference in the positive rate of sputum culture and the distribution of pathogenic bacteria between the two groups (P > 0.05).Gram-negative bacteria predominated in pathogenic bacteria.The hospitalization time was longer in the observation group than in the control group[(17.3 ± 3.0) d vs.(10.3 ± 3.9) d,P < 0.05].The mortality rate was higher in the observation group than in the control group(10.0 % vs.3.3 %,P < 0.05).The age was older in the observation group than in the control group[(87.0± 1.7) years vs.(82.0± 2.4) years,P < 0.05].The incidences of diabetes,Alzheimer's disease,hypothyroidism and long-term bed rest were higher in the observation group than in the control group (P < 0.05).Multivariate logistic regression analysis showed that age (OR =6.93),dementia (OR =4.32),hypothyroidism(OR =4.89) were independent risk factors for occult pneumonia (P < 0.05).Conclusions The pathogenic bacteria in advanced elderly patients with occult pneumonia are mainly gram-negative bacteria.The positive rate of CT scan confirmed-occult pneumonia is much higher than that by chest X-ray.The advanced elderly patients with occult pneumonia have longer hospitalization time and higher mortality rates.Old age,senile dementia and hypothyroidism are independent risk factors for occult pneumonia in advanced elderly patients.

Maintaining the stemness of the transplanted stem cell spheroids in an inflammatory microenvironment is challenging but important in regenerative medicine. Direct delivery of stem cells to repair periodontal defects may yield suboptimal effects due to the complexity of the periodontal inflammatory environment. Herein, stem cell spheroid is encapsulated by interfacial assembly of metal-phenolic network (MPN) nanofilm to form a stem cell microsphere capsule. Specifically, periodontal ligament stem cells (PDLSCs) spheroid was coated with FeIII/tannic acid coordination network to obtain spheroid@[FeIII-TA] microcapsules. The formed biodegradable MPN biointerface acted as a cytoprotective barrier and exhibited antioxidative, antibacterial and anti-inflammatory activities, effectively remodeling the inflammatory microenvironment and maintaining the stemness of PDLSCs. The stem cell microencapsulation proposed in this study can be applied to multiple stem cells with various functional metal ion/polyphenol coordination, providing a simple yet efficient delivery strategy for stem cell stemness maintenance in an inflammatory environment toward a better therapeutic outcome.
