1.Study of relationship between renal function and respiratory tract infections in the elderly

Objective To study the relation between kidney function impairment and respiratory tract infections(RTI) in the elderly.Methods Two hundred and twenty-four patients above 65 years old were divided into five groups according to stages of chronic renal disease and estimated glomerular filtration rate (e GFR):(A group,n =65):≥90 ml/(min · 1.73 m2),(B group,n =55):60-89 ml/(min · 1.73 m2),(C group,n =27):30-59 ml/(min · 1.73 m2),(D group,n =28):15-29 ml/(min · 1.73 m2) and(E group,n =49):< 15 ml/(min · 1.73 m2).Taking A group as the control baseline.Logistic regression was employed to analyze the relationship between kidney function and RTL Results Compare with A group(10.8%,7/65),the incidences of RTI in three groups(C group,D group and E group were 63.0% (17/27),85.7% (24/28) and 100% (49/49) respectively.The incidence in these groups was higher than that in A group(x2 =26.95,49.46 and 89.00 respectively,P < 0.05).We identified the risk factors with RTI were smoking (Waldx2 =17.66,P < 0.01),respiratory underlying diseases(Waldx2 =14.38,P <0.01),invasive operation history(Waldx2 =4.50,P < 0.05),diabetes (Waldx2 =3.86,P < 0.05),stay in hospital for above 2 weeks (Waldx2 =18.18,P < 0.01),Gender(Waldx2 =4.12,P < 0.05),eGFR (Waldx2 =13.57,P < 0.01).After adjusting for other risk factors of RTI,Logistic regression analysis showed that the risk of RTI still increased with the decrease of eGFR (eGFR≥90 ml/(min · 1.73 m2):(Waldx2 =0.053,P >0.05);60-89 ml/(min · 1.73 m2):(Waldx2 =0.046,P>0.05) ;30-59 ml/(minl · 1.73 m2):(Waldx2 =7.61,P<0.05),15-29 ml/(min · 1.73 m2):(Waldx2 =4.38,P<0.05); <15 ml/(min · 1.73 m2):(Waldx2 =13.57,P<0.01)).Conclusion Kidney function impairment in patients above 65 years old is an independent risk factor for RTI.
2.Screening of 12 elemental impurities in pharmaceutical excipient grades of titanium dioxide from various sources and their correlations with whiteness

In the process of promoting the implementation and transformation of the Q3D guideline of the International Council for Harmonisation of Technical Requirements for Pharmaceuticals for Human Use (ICH) in China, the risk assessment of elemental impurities in naturally sourced excipients often faces challenges such as multiple types of elemental impurities and insufficient detection methods. In this paper, an inductively coupled plasma mass spectrometry (ICP-MS) method for screening 12 elemental impurities in titanium dioxide was established using the optimized acid extraction pre-treatment method. The accuracy and repeatability of the method were good. The recoveries of cadmium, lead, mercury, cobalt, nickel, lithium, barium, molybdenum, copper, chromium and iron were 83.6%−101.6%, the recovery of antimony was 54.1%, and the RSD was 1.1%−10.2% (n=6). The evaluation results of elemental impurities showed that the risk of cadmium, mercury, cobalt, nickel, lithium, barium, molybdenum, copper, chromium and antimony was low, while the risk of lead was high, and it is recommended that a lead salt test be added to the pharmacopoeia standard for this product. Based on the statistical data, the differences in the control levels of elemental impurities and whiteness of 29 batches of titanium dioxide samples from 10 domestic and foreign manufacturers were compared and analyzed. The Pearson correlation coefficient method was used to analyze the correlation between residual elemental impurities and whiteness, and a heat map was used to visualize the results. The results showed that whiteness was significantly negatively correlated with the amount of residual barium and the sum of residual elemental impurities. It is suggested that the manufactures should include whiteness as an internally controlled indicator to further improve the quality of titanium dioxide used as a pharmaceutical excipient.
3.Effect of fish oil emulsion on early stage of experimental severe acute pancreatitis

Objective: To observe the effect of ω-3 fish oil emulsion on the experimental severe acute pancreatitis(SAP)through the morphologic alteration of pancreas,the level of serum amylase(AMS)and the functions of liver and kidney,and explore its possible mechanism. Methods: Rat model of SAP was produced by injecting 5% Sodium Cholate(1 ml/kg)into the biliopancreatic duct.Male Sprague-Dawley rats were randomly divided into 4 groups: normal group(n=6),fish oil emulsion treatment group(FOG,n=18),soybean oil emulsion treatment group(SOG,n=18) and normal sodium treatment group(NSG,n=18).Then fish oil emulsion(FO,10 ml/kg);soybean oil emulsion(SO,10 ml/kg)and normal sodium(NS,10 ml/kg)were intravenously injected respectively 120 minutes.The pancreatitis were confirmed by levels of serum AMS and histopathologic score.ALT,AST,BUN and Cr were tested 24 hours after the treatments.Expressions of IL-1β and IL-10 were tested by ELISA.The activated NF-kappa B was examined in the pancreases. Results: Lower level of serum AMS(P<0.05)and lower histopathology score(P<0.05) appeared in FOG.Compared with NS,FO decreased the levels of serum ALT and BUN significantly (P<0.05).FO significantly attenuated the expression of IL-1β (P<0.05).FO downregulated the activity of NF-kappa B efficiently.Conclusions: By down-regulating the levels of IL-1β together with up-regulating the level of IL-10,FO reduces inflammatory damage at the beginning of AP.
4.Clinical application of intraoperative sustained skin and soft-tissue expansion in augmentation mammaplasty using transaxillary approach with endoscopic assistance

Objective To evaluate the results of prosthesis augmentation,relieve suffering patient experience,improve shape and touch of breast.Methods According to design,in dual-plane mammaplasty transaxillary approach with endoscopic assistance,we used soft-tissue expansion before prosthesis,saline volume in expander was the same as prosthesis size at beginning; adjusting expander volume and position,then determined prosthesis size according to expander volume lastly.after intraoperative sustained expansion for 30 to 60 minutes twice prosthes volume,replace expander with prosthesis.Results This method was applied in 120 patients,showing that pain significantly reduced after expansive without use of analgesia pump; complications included that 3 patients appeared seroma and absorbed soon; 2 patients had poor prosthese location,which was adjusted in reoperation; one capsular contracture was repaired later.After 6-months to 2-years follow-up,the results showed that breast was nature and dynamic; satisfactory rate reached to 95 %.Conclusions In augmentation mammaplastic operation,pre-adoption of an expander can make it easy to adjust the location of prosthesis,to avoid being injured and to determine prosthesis volume by expander injection of saline volume,especially for patient with different size of both breasts.
5.Investigation and thought on the teaching modes of the course of guide to learn in nursing under graduate education

Objective To investigate the teaching modes of the course of guide to learn in undergraduate nursing students in order to improve their quality of leaning.Methods Questionnaire survey and interview were conducted to collect the students'general appraisal on learning Science and teaching content and teaching modes as well as ingathering and suggestions of the course.Results Totally 79.5%among 117 students thought it necessary to open the course of Learning Science.96.6%students considered ten subjects of the course reasonable.83.8%students were satisfied with the alternate five-teachers'teaching modes.The most digestible teaching approach Was illustration,and the most valuable experience wag how to regulate the psyehology of learning.Conclusions The course of guide to learn in nursing undergraduate education has positive significance.Nursing educators should always explore and improve the teaching mode of this course so as to help students develop SOundly.
6.Platypnea-orthodeoxia syndrome in a patient with a pre-existing patent foramen ovale successfully treated with an atrial septal occluder

Platypnea orthodeoxia syndrome is associated with dyspnea and arterial oxygen desaturation accentuated by an upright posture. It can be secondary to an intracardiac shunt. We report a case of platypnea-orthodeoxia syndrome (POS) in a 58-year old male patient who had a pre-existing patent foramen ovale (PFO) and substantial pulmonary pathologies. He was successfully treated by percutaneous transcatheter closure of the PFO. Our case highlights the importance of recognition of this rare syndrome in patients who present with unexplained hy-poxia for whom transcatheter closure of the interatrial shunt can be safely carried out.

8.Status and equity of mental health resources allocation in Shanghai Municipality

Objective:
To analyze the current status and equity of mental health resources allocation in Shanghai Municipality, so as to provide data supports to formulate mental health action plans and relevant policies. .
Methods:
The data pertaining to mental health institutions, actual beds opened, certified or assistant psychiatrists and registered nurses was collected from the Survey of the Current Status on Mental Health Resources in Shanghai Municipality in 2020. The equity of mental health resources allocated by population and geographical area in Shanghai Municipality was evaluated with Lorenz curve and Gini coefficient
Results:
There were 96 mental health institutions, 15 060 actual beds opened, 257 certified or assistant psychiatrists and 2 887 registered nurses in Shanghai Municipality in 2020, with a physician-to-nurse ratio of 1∶2.30. The greatest numbers of actual beds opened in the department of psychiatrics, the number of certified or assistant psychiatrists and the number of registered nurses per 10 000 residents and per km2 were all found the central urban areas. The numbers of actual beds opened in the department of psychiatrics, the number of certified or assistant psychiatrists and the number of registered nurses per 10 000 residents were 6.06 beds, 0.51 physicians and 1.16 nurses, with Gini coefficients of 0.36, 0.42 and 0.44, respectively, and the numbers of actual beds opened in the department of psychiatrics, the number of certified or assistant psychiatrists and the number of registered nurses per km2 were 2.38 beds, 0.20 physicians and 0.46 nurses, with Gini coefficients of 0.72, 0.76 and 0.75, respectively.
Conclusions
There was a gross equity in mental health resources allocated by population and geographical area in Shanghai Municipality in 2020, which showed an improvement as compared to 2015. The equity in mental health resources allocated by geographical area was lower than that by population in Shanghai Municipality.
9.Latent classes of health risk behaviors in medical students and depressive symptoms

Objective:
To identify the latent classes of various health risk behaviors among medical students and further analyze the correlation between different classes and depression.
Methods:
Using the method of convenient sampling, 2 014 medical students were selected from Anhui Medical University in October 2018. Self-designed online questionnaire were used to collect demographic data, health risk behavior (including smoking, drinking, suicide, sleep disorders, mobile phone dependence and lack of exercise) and depressive symptoms. Latent Class Analysis was used to cluster individuals. Logistic regression was provided to further analyze the association between health risk behaviors and depressive symptoms for the groups.
Results:
The health hazard behaviors of medical students could be divided into three separate classes: substance dependence group (8.4%), mobile phone dependence group (22.4%) and low-risk group (69.2%). The distribution of health risk behaviors of medical students with different gender, major, grade, only child,father s educational level,monthly living expenses, academic achievement and number of friends were statistically significant (χ 2=99.37,19.07,12.05,6.64,14.28,19.35,20.61,26.39,P<0.05). The results of Logistic regression analysis showed that after adjusting for gender, major, grade, only child, father s educational level,monthly living expenses, academic achievement and number of friends, the mobile phone dependence group was positively correlated with depressive symptoms (β=1.75, P<0.01).
Conclusion
Different ratent classes of health risk behaviors have different correlation with depressive symptoms in groups.It is suggested that school health workers should make individualized intervention plan for different types of health hazard behavior of medical students in the future educational activities, carry out stratified intervention, alleviate the symptoms of depression and promote the mental health of medical students.
10.Effect of intravenous thrombolytic therapy mode on patients with acute ischemic stroke led by fast-track specialist nurses

Objective To investigate the effectiveness of intravenous thrombolytic therapy mode led by fast-track specialist nurses on patients with acute ischemic stroke(AIS).Methods This study involved 124 AIS patients who underwent intravenous thrombolytic therapy in the Department of Emergency of our hospital from March 2021 to February 2023.Among the patients,61 admitted between March 2021 and February 2022 received conventional AIS thrombolytic therapy were assigned to a control group.While the 63 patients who received AIS thrombolytic therapy under the specialist nurse-led intravenous thrombolytic therapy mode between March 2022 and February 2023 were assigned to an observation group.The two groups were compared in terms of the time from admission to completion of CT examination,time for signing the informed consent for thrombolytic therapy,door to needle time and percentage of DTN<60 minutes,as well as the post-thrombolysis scores according to the National Institute of Health Stroke Scale(NIHSS)and satisfaction to medical consultation.Results The observation group exhibited a significantly shorter time from admission to completion of CT examination,a shorter time for signing an informed consent for thrombolytic therapy,a shorter door to needle time and a higher percentage of DTN<60 minutes,all with significant difference in comparison with those in the control group.After thrombolysis,the NIHSS score of the observation group decreased more than that of the control group(P<0.05).The patients and their families in the observation group reported significantly higher satisfaction compared to those in the control group(both P<0.05).Conclusion The fast-track specialist nurse-led intravenous thrombolytic therapy mode demonstrates the superiority in reduction of the time from admission to completion of CT examination,time for signing an informed consent for thrombolytic therapy,door to needle time and the NIHSS scores,higher percentage of DTN<60 minutes as well as improvement of patient satisfaction.
