1.Native Assessment to the Montreal Battery of Musical Evaluation

Objective: This research carried out the native assessment to the Montreal Battery of Musical Evaluation, including MBEA and Musical emotion test. Methods: 60 normals were tested by MBEA (including 30 university students). Also 40 students were tested by musical emotion recognition battery. The correct rates were recorded and compared with western normals’data. Results: There were significant differences in pitch judgments and musical emotion recognition be- tween Chinese and Westerners, while rhythm perception and memory of music did not have significant difference between the two groups. Conclusion: Culture difference mainly influences the musical pitch perception in brain but rhythm. Chinese have its own understanding of western musical excerpts. Although MBEA is wildly accepted abroad, native analogy testing tool is acquired.

Wtth the development of imaging technique,particularly the use of magnetic resonance diffusion weighted imaging,it can easily detect silent stroke lesiom.Both the prevalence and incidence of asyrrotomatic stroke were higher than those of symptomatic stroke.Age,hypertemion and atrial fibrillation are its accepted risk factors.Although silent stroke lacks clinically overt stroke-like symptoms by definition,it often has mild physical and cognitive impairment with careful examination.Furthermore,asymptomatic stroke is also correlated with the occurrence of subsequent symptomatic stroke.decreased cognitive function and dementia.Therefore,clinicians should pay more attention to it,positively control its risk factors and improve its prognosis.
3.Investigation and thought on the teaching modes of the course of guide to learn in nursing under graduate education

Objective To investigate the teaching modes of the course of guide to learn in undergraduate nursing students in order to improve their quality of leaning.Methods Questionnaire survey and interview were conducted to collect the students'general appraisal on learning Science and teaching content and teaching modes as well as ingathering and suggestions of the course.Results Totally 79.5%among 117 students thought it necessary to open the course of Learning Science.96.6%students considered ten subjects of the course reasonable.83.8%students were satisfied with the alternate five-teachers'teaching modes.The most digestible teaching approach Was illustration,and the most valuable experience wag how to regulate the psyehology of learning.Conclusions The course of guide to learn in nursing undergraduate education has positive significance.Nursing educators should always explore and improve the teaching mode of this course so as to help students develop SOundly.
4.Maintenance effects of acupoint catgut embedding at early time on gastrointestinal function in patients with craniocerebral injury.

OBJECTIVETo explore the maintenance effects of acupoint catgut embedding at early time on gastrointestinal function in patients with craniocerebral injury.
METHODSSixty craniocerebral injury patients with 5 to 12 points of Glasgow coma scale (GCS), according to treatment order, were alternately divided into an observation group and a control group, 30 cases in each one. Patients in the control group were treated with regular treatment and nursing care. Based on this, patients in the observation group, according to different pathogenesis and symptoms presented within 24 h into hospitalization, were additionally treated with acupoint catgut embedding. The recovery time of borborygmus, time of first anal aerofluxus, time of first defecation, abdominal pressure at different time points, the occurrence rate of complications (upper gastrointestinal hemorrhage, diarrhea, vomiting), time of enteral nutrition tolerance rate reaching 30 kcal/kg x d were observed and recorded.
RESULTSThe recovery time of borborygmus, time of first anal aerofluxus, time of first defecation and time of enteral nutrition tolerance rate reaching 30 kcal/kg x d in the observation group were all earlier to those in the control group (all P<0.01). At 48 h, 4 d and 7 d into hospitalization, the abdominal pressures in the observation group were all lower than those in the control group [(11.10 +/- 1.47) mmHg vs. (13.50 +/- 1.43) mmHg, (8.40 +/- 1.25) mmHg vs. (11.90 +/- 1.56) mmHg, (6.73 +/- 0.74) mmHg vs. (10.80 +/- 1.30) mmHg, all P<0.01]. There were 8 cases with complications of gastrointestinal hemorrhage, diarrhea and vomiting in the observation group with the occurrence rate o 27% (8/30), which was lower than those in the control group (70.0% (21/30), P<0.01.
CONCLUSIONThe acupoint catgut embedding at early time in craniocerebral injury patients could improve the recovery of gastrointestinal function, reduce intolerance of enteral nutrition and occurrence rate of various complications.

5.Investigation and analysis of attitudes of nursing students in higher vocational towards death

Objective To investigate and analyze the attitudes of higher vocational nursing students toward death and the related effect factors.Methods Totally 324 nursing students in a higher vocational school in Shaanxi province were investigated with death attitude profile-revised scale.Contents of investigation included physical and mental health status,family members' attitude towards death,family form and ranking,religion,presence of suicidal thoughts,exposure to mass media reports about death,reading books about death,death fear,death escape,natural acceptance,approach acceptance,avoidance acceptance,etc.SPSS 16.0 was used for data entry and statistic analysis,mean and standard deviation for statistical description,t test and variance analysis for statistical inference.Results ①Higher vocational nursing students got the highest average score (3.93 ± 0.55) in the dimension of natural acceptance.②In the dimension of avoidance acceptance,the scores of nursing students with excellent and good physical and mental health were significantly lower than those of students with general physical and mental health (F=4.80,P=0.009) ; the scores of nursing students in single parent family were significantly higher than those of students in other family forms (F=4.59,P=0.004) ; the scores of nursing students with suicidal thoughts were significantly higher than those of students without (t=-4.77,P=0.000).In the dimension of death avoidance,the scores of nursing students whose family discussed death openly were significantly lower than those of students whose family never done so (F=6.88,P=0.001).In the dimension of natural acceptance,the scores of nursing students with religious beliefs were significantly lower than those of students without (t=2.52,P=0.012) ; the scores of nursing students who gained information of death from mass media were significantly higher than those of nursing students who never done so.(t=-3.85,P=0.000),which was the same in the dimension of approach acceptance (t=-3.48,P=0.001).Conclusions The majority of higher vocational nursing students can naturally accept death reality.Attitude of higher vocational nursing students toward death are influenced by physical and mental health,discussing death in family,family form,religious belief,suicidal thought and news or report about death from the mass media.
6.Serum 25-hydroxyvitamin D level, chronic inflammation state and malnutrition in maintenance hemodialysis patients

Objective To identify the relationship among serum 25-hydroxyvitamin D[25(OH)D] level, chronic inflammation state and malnutrition in maintenance hemodialysis (MHD) patients. Methods The serum 25 (OH)D level of 119 patients on MHD was detected. All the patients were divided into three groups according to the serum 25(OH)D level. Vitamin D deficiency: 25(OH)D ≤ 15 ng/mL, vitamin D insufficiency: 15 ng/mL <25(OH)D ≤ 30 ng/mL, vitamin D normal: 25(OH)D > 30 ng/mL. Inflammatory factor and nutritive index were compared among the three groups. Result The average of serum 25 (OH)D level was 3.4~45.3 ng/mL (22.5~14.8 ng/mL), and the prevalence of 25 (OH)D deficiency and insufficiency was 88.2%. Significant differences existed in age, inflammatory factor (IL-6, TNF-α, CRP) and nutritive index (Alb, PA, SGA) between 25(OH)D deficiency, insufficiency groups and normal group (P<0.05). Conclusion The serum 25(OH)D level is possibly related to the malnutrition in MHD patients. It may inhibit chronic inflammation state , in which it can promote state of nutrition of MHD patients.
7.Clinical efficacy of Jiawei Dihuang decoction combined with valsartan in treatment of early diabetic nephropathy

One hundred patients with early diabetic nephropathy were randomly divided into control group and study group with 50 cases in each group.All patients received conventional antidiabetic therapy with valsartan tablets 80mg, q.d;patients in study group were given additional TCM Jiawen Dihuang decoction.TCM syndrome scores, relevant biochemical testing and renal function changes were evaluated before and after treatment in two groups.After 8 weeks of treatment, the results showed than the TCM scores in both groups were significantly reduced (study group: 18.9±1.6 vs.7.2±5.1, control group 18.8±1.5 vs.10.3±6.2.both P<0.05);the improvement rate of TCM syndrome in study group was significantly higher than that of control group (90% vs.68%, x2=5.3,P<0.05);and the improvement of relevant biochemical index and renal function in study group was more marked than that in control group.The study indicates that the integrated TCM and Western medicine can enhance the efficacy in treatment of early diabetic hephropathy.
8.Treatment of patients with bosentan in post-operation of congenital heart disease with pulmonary arterial hypertension: a double-blind, randomized controlled trial

Objective Endothelin is a key role in the pathogenic of pulmonary arterial hypertension. High concentrations of endothelin-1 have been recorded in plasma and lungs of patients with pulmonary artery hypertension associated with congenital heart disease, and the concentrations of endothelin-1 was correlated with severity degree of pulmonary arterial disease.Endothelin exerting vasoconstrictor and mitogenic effects by binding to two distinct receptor isoforms in the pulmonary vascular smooth muscle cells: endothelin A and B receptors. Bosentan is an orally active dual ( A and B) endothelin receptor antagonist that has been shown to improve exercise capacity, haemodynamics, and clinical worsening in many clinical trials, but have no experience in post-operation of congenital heart disease with PAH. In our country, there are lots of elder patients of congenital heart disease accompany with pulmonary arterial hypertension. And they have poor prognosis after operation because pulmonary arterial hypertension leads to right ventricular failure and sudden death. So we hope bosentan can improve clinical outcome of these patients. This time we present the results of the bosentan trial in post-operative patients still with pulmonary arterial hypertension, a randomized controlled trial designed to assess the efficacy and safety of the dual endothelin receptor antagonist bosentan in post-operative pulmonary arterial hypertension, and present the first experience of bosentan for these patients.Methods This study was a prospective clinical trial. The objects were 30 patients ( age:4 months - 6.8years,weight:5 - 15 kg) who still had pulmonary artery hypertension at one week after cardiac defects repaired. They were randomized to controlled ( n = 15 ) or to bosentan ( n = 15 ). Dosage regimen: 10 - 20 kg patients, 31. 25 mg qd ( 4 weeks ) ,31.25 mg bid ( 8 weeks);5 - 10 kg patients, 15.6 mg qd ( 4 weeks), 15.6 mg bid ( 8 weeks). Evaluate the efficacy and safety of Bosentan through the amelioration of pulmonary arterial systolic pressure, WHO functional class, and clinical worsening. Results We monitored pulmonary arterial systolic pressure after operation by echocardiogram 2 times, baseline ( 1 week after operation) and at 12 weeks later. The pulmonary arterial systolic pressure decreased 19.5 mm Hg in Bosentan group( P =0. 000), and decreased 10.3 mm Hg in control group(P =0. 164), with the mean treatment effect of 9.2 mm Hg (P=0.049,95%CI:0. 1 -18.3). The effct of bosentan on haemodynamics is also reflected in the reduction plasma ET-1 concentration in bosentan group. Plasma ET-1 in control group increased (0. 15 ±0.1 )fmol/ml(P =0.77), however, decreased (2.01 ±0.3)fmol/ml (P=0. 03) in bosentan group; Bosentan prevented post-operation PAH. Bosentan treatment was associated with lower incidence of worsening NYHA functional class compared with controlled(0 in the bosentan group vs 13% in the placebo group) There was a delay in time to clinical worsening with bosentan compared wih controlled group. Abnormal liver function occurred in 2 cases in bosentan group but resolved after discontinuation of bosentan treatment, no other side effects. Bosentan produced hemodynamic improvement and was well tolerated in infant. Conclusion Bosentan administration in patients with postoperative PAH is safe and efficient. Bosentan is a new effective approach to therapy for postoperative pulmonary arterial hypertension in children.
9.Early and mid-term outcome of the arterial switch operation for transposition of great arteries: predictors and functional evaluation

Objective The aim of this report was to study the early and mid-term outcome in hospital and follow-up mortality, predictors for late pulmonary stenosis (PAS) and insufficiency of neo-aortic valve (neo-AVI) in patients with transposition of great arteries (TGA) and Taussig-Bing malformation undergoing arterial switch operation ( ASO ). Methods Between January 2004 and December 2007, 169 patients (129 male, 40 female; mean age of [(11.71 ± 26.3 ) months] with TGA or Taussig-Bing malformation underwent ASO. The patients were divided into Group Ⅰ (n = 56 ): TGA with intact ventricular septum and Group Ⅱ ( n = 113 ): TGA with ventricular septal defect (VSD). All patients were followed up in out-patient department by ultrasonic cardiogram. The mean follow-up periods was (27.66 ± 14.6 ) months. Multiple logistic regression analysis was performed to find out the risk factors. Results The overall hospital mortality was 11.24% (19/169)and there was no significant difference between the two Groups. With the improving of perioperative management, the hospital mortality decreased from 16.67% in 2004 to 3.92% in 2007. The overall actuarial survival at 1-, 3- and 5-year follow-up was 94.00%,91.33%, and 91.33%, respectively. The multivariate analysis revealed that age above 6 months was a strong predictor for poor postoperative survival. Predictors for neo-AVI were: combined with VSD, age > 6 months and postoperative neo-AVI Z-score > 1. Predictors for moderate to severe PAS were age < 1 months and pulmonary artery plasty with an unstretchable patch. Conclusion ASO remains the optimal choice for treating various forms of TGA with an acceptable early and mid-term outcome regarding overall survival rate. Patients with TGA should be treated as early as possible. Age >6 months is a predictor for poor postoperative survival and neo-AVI. Mismatch between the neo-aortic root and distal aorta may induce neo-AVI. Unstretchable patch in pulmonary artery plasty may induce PAS.
