1.Influence of blood glucose level at hospitalization on no-reflow and prognosis in non-diobetic patients with myocardial infarction after emergency coronary intervention

Objective:To explore the influence of different blood glucose level at hospitalization on no-reflow and prognosis in patients with acute myocardial infarction (AMI),but without diabetes after emergency percutaneous coronary intervention (PCI).Methods:A total of 678 non-diabetic AMI patients undergoing PCI were selected.Ac-cording to blood glucose level at hospitalization,patients were divided into normal group (n=312),blood glucose level higher group (higher group ,n=201)and blood glucose level extremely high group (extremely high group,n=165).Clinical characteristics,no-reflow after PCI and incidence of cardiovascular events within 30d after PCI were compared among three groups.Results:Compared with normal group and higher group,there were significant rise in age,percentages of >1 location of myocardial infarction and Killip's ≥ class II,white blood cell count instant af-ter hospitalization,peak value of creatine kinase (CK)and CK-isoenzyme (CK-MB)in extremely high group,P <0.01 all;Compared with normal group,the age,peak value of CK and CK-MB significantly rose in higher group (P<0.01 all).Along with blood glucose rose,incidence rate of no-reflow significantly increased (17.0% vs.38.8%vs.49.7%),left ventricular ejection fraction (LVEF)on 30d after PCI gradually reduced [(68.6± 5.7)% vs. (62.2±6.4)% vs.(55.6±6.9)%]in above-mentioned 3 groups in turn,there was significant difference between any two groups,P <0.05 or <0.01.Conclusion:Stress hyperglycemia possesses great influence on no-reflow and prognosis in non-diabetic AMI patients after PCI.
2.Propofol pretreatment combined with umbilical blood mesenchymal stem cell transplantation improves cerebral ischemia-reperfusion injuries

BACKGROUND:As propofol has a neuroprotective effect, and umbilical cord blood mesenchymal stem cel s have a high differentiation potential, their combination wil have a better therapeutic effect on cerebral ischemia-reperfusion injury. OBJECTIVE:To study the effect of propofol pretreatment combined with umbilical blood mesenchymal stem cel transplantation on cerebral ischemia-reperfusion injury in rats. METHODS:Sixty-three Sprague-Dawley rats were randomized into model, propofol, and combined group (n=21 per group). Rat models of cerebral ischemia-reperfusion injury were made using ligation of the middle cerebral artery occlusion in the three groups. Rats in the combined group were given 100 mg/kg propofol injection at 1 day before injury and injection of umbilical blood mesenchymal stem cel s via the tail vein (0.5 mL, 2×109/L). RESULTS AND CONCLUSION:Compared with the model group, the neurological function was improved significantly in the propofol and combined group, especial y in the latter one, presenting with a remarkable mitigation in brain injury and an increased level of survivn mRNA in the rat hippocampus. The content of serum malondialdehyde was lower but the activity of superoxide dismutase was higher in the combined group compared with the propofol group. These findings indicate that propofol pretreatment combined with umbilical blood mesenchymal stem cel transplantation has better therapeutic effects than propofol pretreamtnet alone for improving cerebral ischemia-reperfusion injury in rats.
3.Diffusion tensor imaging and the Montreal cognitive assessment for assessing severe traumatic brain injury

Objective To investigate any correlation between diffusion tensor imaging (DTI) results and Montreal cognitive assessment (MoCA) scores after severe traumatic brain injury (TBI).Methods Eight male pa-tients with chronic severe TBI were given the MoCA (including memory,attention,speech and executive function).DTI was used to quantify the fractional anisotropy (FA) of white matter fiber tracts in the radial and longitudinal fasciculus,under longitudinal fasciculus,internal capsule,corpus callosum genu and body,and the cingulate cortex.Pearson correlation coefficients were calculated to quantify the correlation between the FA values and MoCA scores.Results There was a positive correlation between FA in the corpus callosum body,corpus callosum genu and the superior longitudinal fasciculus and MoCA total scores.Conclusion The MoCA scores of patients with chronic severe TBI are related to white matter damage in the corpus callosum body,corpus callosum genu and the superior longitudinal fasciculus.
4.Influence of Qizhi Yifei Containing Serum on Regulating Expression of MMP-9 and TIMP-1 mRNA in Rat Ffibroblasts Stimulated by TGF-β1

Objective To observe the influence of Qizhi Yifei containing serum on the expression of MMP-9 and TIMP-1 mRNA in lung fibroblasts, and explore its mechanism. Methods Trypsin digestion method was used to extract fibroblasts from lung tissue in rats. All fibroblasts were cultured and trained to the fourth generation. Then they were randomly divided into control group, model group and drug serum group. The model group and drug serum group were firstly treated by DMEM with 0.002 5 μg/mL TGF-β1. The control group was treated by DMEM only. The control group and model group were then treated by DMEM with blank drug serum in concentration of 5%, and the drug serum group was treated by DEME with Qiahi Yifei containing serum in same concentration. After 48 and 72 hours, RT-PCR was performed to test the expression of MMP-9 mRNA and TIMP-1 mRNA of each group. Results After 48 hours, MMP-9 and TIMP-1 mRNA were significantly increased in model group and drug serum group compared with control group. There was no difference between model group and drug serum group. After 72 hours, MMP-9 mRNA was up-regulated in model group and was decreased in drug serum group compared with control group. There was no significant difference among the three groups on the expression of TIMP-1 mRNA. Conclusion Qizhi Yifei containing serum can decrease the up-regulated expression of MMP-9 mRNA in lung fibroblasts stimulated by TGF-β1.
5.Investigation and Analysis of the Medication Safety Status of Chinese Patent Medicines for Residents in 3 Cit-ies of Guangdong Province

OBJECTIVE:To investigate the medication safety status of Chinese patent medicines and related influence factors for residents in Guangdong province,and provide reference for better guarantee of medication safety for residents. METHODS:Questionnaires were randomly sent out among the residents in 3 cities of Guangdong province. The cognition,utilization habit and safety awareness,the ways to get the related information of Chinese patent medicines and purchase way were surveyed and ana-lyzed statistically. RESULTS:Totally 530 questionnaires were sent out and 514 valid questionnaires were collected with effective re-covery of 96.98%. There are 64.01% of respondents didn’t know the composition of Chinese patent medicines;50.39% didn’t know the contraindications of Chinese patent medicines;48.44% didn’t know the adverse drug reactions of Chinese patent medi-cines that they were using;23.47% had once broken the tablets when took them;65.18% didn’t know that the old people had less dosage than teenagers and 44.75% didn’t know that Chinese patent medicines couldn’t be taken with some western medicines. There were many ways to get the related information of Chinese patent medicines and purchase them,and the main sources were from doctors and pharmacists in drug stores;90.08% had purchased Chinese patent medicines in drug stores. CONCLUSIONS:During daily medication of Chinese patent medicines,the biggest problem for residents is that the weak awareness of medication safety,unreasonable medication behavior and habit caused by lack of related knowledge of medication safety,which may cause safety risks or happen medication safety problems. Therefore,the residents’education of self-medical knowledge should be intensi-fied,the management of prescription in drug stores should be enhanced,pharmaceutical service function of pharmacists should be played,supervision management of drug advertisement should be strengthened and the contents in drug instructions should be regu-lated.
6.Effect of electroacupuncture stimulation on apoptosis of nerve cells in a rat model of spinal cord contusion

BACKGROUND:Electroacupuncture has promoting effects on the functional recovery of the injured spinal cord, can decrease pain, and elevate postoperative effect after acute spinal cord contusion.OBJECTIVE:To observe the effect of electroacupuncture on apoptosis in the injured site after spinal cord contusion, and analyze its neuroprotective effects on neurological function in rats with spinal cord contusion. METHODS:A total of 66 adult female Sprague-Dawley rats were randomly divided into: sham surgery group (n=20), spinal cord contusion group (n=20), electroacupuncture stimulation group (n=20) because six rats were excluded due to modeling failure and death. Before model establishment, at 1, 3 days, 1, 2, 3 and 4 weeks after model establishment, motor functions were evaluated by BBB score and the inclined plate test. At 3 days after model establishment, apoptosis of nerve cels could be detected in the site of injury in each experimental group using TUNEL assay. mRNA and protein expression of bax, bcl-2 and caspase-3 was detected surrounding the injury site using RT-PCR and western blot assay. Morphological changes in the site of injury could be observed using hematoxylin-eosin staining. The regeneration of nerve fibers was observed using HRP tracing. RESULTS AND CONCLUSION:(1) Motor function score was significantly increased at various time points in the 2nd week of treatment in the electroacupuncture stimulation group than in the spinal cord contusion group (P< 0.05). (2) Apoptotic index was significantly lower in the electroacupuncture stimulation group than in the spinal cord contusion group at 3 days after model establishment (P < 0.05). (3) mRNA and protein expression of bax and caspase-3 was significantly lower in the electroacupuncture stimulation group than in the spinal cord contusion group at 72 hours (P < 0.05); bcl-2 gene and protein expression was significantly higher (P < 0.05). (4) The number of HRP-positive nerve fibers was highest in the sham surgery group, folowed by electroacupuncture stimulation group, and lowest in the spinal cord contusion group at 4 weeks (P < 0.05). Results indicated that electroacupuncture plays a protective role on the spinal cord contusion by reducing apoptosis of nerve cels at the site of injury.
7.Therapeutic effect of electroacupuncture plus neural stem cell transplantation on the hindlimb function of rats with spinal cord injury

BACKGROUND:Neural stem cel transplantation alone has achieved unsatisfactory outcomes in the repair of damaged spinal cord tissues. To promote the survival, proliferation and neuronal differentiation of transplanted cels in vivo, it is necessary to further improve the micro-environment of spinal cord injury.
OBJECTIVE:To investigate the effect of neural stem cel transplantation plus electroacupuncture on the hindlimb function and electrophysiological changes of rats with spinal cord injury.
METHODS: Animal models of spinal cord injury were made in 72 Sprague-Dawley rats and randomized into four groups: control group with injection of culture mediumvia the tail vein; neural stem cel group with injection of neural stem cel suspensionvia the tail vein; electroacupuncture group given 1-week electroacupuncture atDu meridian and body points starting from 6 hours after modeling; combined group given injection of neural stem cel suspension via the tail vein+1-week electroacupuncture atDu meridian and body points starting from 6 hours after modeling. Motor functional recovery in rats was assessed by Basso-Beattie-Bresnahan score and inclined plane test before and at 1, 3 days and 1-4 weeks after modeling. At 4 weeks after modeling, hematoxylin-eosin staining was performed for pathological observation; fluorescence microscope was used to observe the survival and distribution of CM-Dil-labeled neural stem cels; horseradish peroxidase tracer was used to observe nerve fiber regeneration; rat neurophysiological recovery was assessed by determining motor evoked potentials and somatosensory evoked potentials.
RESULTS AND CONCLUSION:At 2-4 weeks after modeling, the hindlimb function was better in the combined group than the neural stem cel group and electroacupuncture group; while it was better in the neural stem cel group and electroacupuncture group than the control group. At 4 weeks after modeling, there were few nerve axon-like structures and smal voids in the spinal cord of the neural stem cel group and electroacupuncture group; however, in the combined group, there were more nerve axon-like structures and no void in the spinal cord. At 4 weeks after modeling, the number of nerve fibers positive for CD-Dil and horseradish peroxidase was ranked as folows: combined group > neural stem cel group and electroacupuncture group > control group, and there were significant differences between groups (P < 0.05). The latencies of motor and somatosensory evoked potentials were significantly lower in the combined group than the neural stem cel group and electroaucpuncture group folowed by the control group (P < 0.05), whereas the amplitudes of motor and somatosensory evoked potentials were significantly higher in the combined group than the neural stem cel group and electroacupuncture group folowed by the control group (P < 0.05). In conclusion, these findings indicate that neural stem cel transplantation combined with electroacupuncture can promote synaptic regeneration and improve the motor and electrophysiological functions of rats.
8.A prospective cohort study on hormone-dependant metastatic breast cancer treated with Shuganyishen formula

Objective Observing the effect of Shuganyishen formula on hormone-dependant metastatic breast cancer and the mechanisms of anti-resistance and endocrine therapy. Methods 226 ITT cases were divided into two cohorts by treatment. 126 cases were treated with the combination of Shuganyishen formula and standard western medicine including endocrine therapy, and 100 cases were treated with western medicine alone. The end points were time to progress (TTP) and overall survival and quality of life (QOF). The effect of Shuganyishen formula on growth of TAM-resistance cell line in vitro was observed. Results There were statistical significances of median TTP between combination and western medicine in Luminal A (ER+ /PR+, HER2-) as well as Luminal B (ER+/PR+, HER2+), but no significances of OS; The extent and incidence of hot flush, sweat and syndromes related to bone were decreased significantly in combination treated group. Shuganyishen formula could reverse resistance to TAM. Conclusion Patients with LuminalA and LuminalB metastatic breast cancer benefit from Shuganyishen formula, which maybe related with its reverse resistance to endocrine therapy.

Liver is no longer the only organ in the concept of anatomy,but it is a concept that has some specific functions in the visceral manifestations of liver.Liver is wood in the five element theory.It is yang of yin.Liver qi connects to the spring qi.Therefore,the liver function is to dredge and smooth qi of the whole body,which influences the blood and body fluid operation,the operation of the spleen and stomach,emotional changes and reproductive function and many other aspects.Suwen said "liver stores blood",which means that liver has the function of storing,regulating and controlling blood."Liver corresponds to the spring",which means changes in four seasons can affect the function of organs,but also contain the disease occurrence with a seasonal concept,which is an important part of visceral manifestation of liver.Modern medical research achieved some results from the nerve-endocrine-immune network,traditional four-axis,liver-blood vessels as the starting point for the liver as the theoretical mechanism of in-depth study.It was found that depression due to liver failed in soothing was related to the central cortex,the marginal system and the hypothalamicpituitary-adrenal (HPA) axis.The study covered neuropeptide and neurotrophic factor,monoamine neurotransmitter,central amino acid level,and etc.Liver stores blood,which is related to human coagulation system,EPO pathway and hemorheology.Liver corresponds to the spring,which is related to the neuroendocrine-immune network.And the pineal gland may be the highest regulatory site.At present stage,the study was conducted on the microcosmic mechanism of liver visceral manifestations from the gene and protein level.It was used to verify the regulation of liver function abnormality caused by the regulation of liver function in order to greatly explore the biological basis of traditional Chinese medicine (TCM) theory.
10.Effect of entecavir antiviral therapy on the prognosis of patients with hepatitis B virus-related hepatocellular carcinoma after transcatheter arterial chemoembolization

ObjectiveTo investigate the effect of entecavir (ETV) antiviral therapy on the prognosis of patients with hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC) receiving transcatheter arterial chemoembolization (TACE). MethodsA total of 170 HCC patients who received TACE for the first time in Liver Cancer Center of Nanfang Hospital from January 2011 to March 2018 were enrolled, among whom 114 patients were treated with ETV (ETV treatment group) and 56 patients did not receive antiviral therapy (control group). Baseline demographic data, alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBil), albumin (Alb), platelet count (PLT), Child-Pugh class, HBeAg and HBV DNA levels, alpha-fetoprotein, and BCLC stage were recorded before treatment, and the changes in HBV DNA level, ALT, AST, TBil, Alb, and Child-Pugh class were observed at weeks 4-8 after treatment; long-term survival was also observed after treatment. Short- and long-term clinical benefits (overall survival) were observed for all patients. The t-test or the Wilcoxon signed-rank test was used for comparison of continuous data between two groups, and the chi-square test was used for comparison of categorical data between two groups. Multivariate logistic regression analyses were performed for related clinical indices before treatment to identify the risk factors for HBV reactivation. The Kaplan-Meier method was used to analyze the survival curves of overall survival, and the log-rank test was used for comparison of survival curves. ResultsThere was no significant difference in the incidence rate of HBV reactivation between the ETV treatment group and the control group (15.79% vs 16.07%, χ2=0.002,P=0.962). The univariate analysis showed that PLT was a risk factor for HBV reactivation (Z=-2.183,P=0.029), and the multivariate analysis showed that HBV DNA level was an independent risk factor for HBV reactivation (hazard ratio =1.000, 95% confidence interval: 1.000-1.000, P=0.015). The 1-, 3-, and 5-year survival rates were 56.20%, 30.30%, and 13.20%, respectively, in the ETV treatment group and 60.60%, 27.20%, and 16.30%, respectively, in the control group. There was no significant difference in overall survival rate between the two groups (χ2=0.049,P=0.755). ConclusionAntiviral therapy can reduce the incidence rate of HBV reactivation after TACE in patients with HBV-related HCC.
