2.Expression of HIF-1α,COX-2 and the correlation with lymph node metastasis and angiogenesis in gastric carcinoma

Objective To study the expression of hypoxia inducible factor-lalpha (HIF-1α) and eyclooxygenase-2 (COX-2) in gastric carcinoma and its correlation with lymph node metastasis and microvascular density (MVD). -Methods HIF-1α and COX-2 expression were detected by immunohistochemistry in 64 cases of gastric carcinoma and 15 cases of normal gastric mucosa tissues, MVD of the section marked with CD34 antibody was calculated. Results The positive rates of HIF-1α and COX-2 in 64 cases of gastric carcinoma were 67.19% and 78.13% respectively. The expressions of HIF-1α and COX-2 were significantly higher in gastric carcinoma than in normal gastric mucosa tissue(P < 0.05). The expressions of HIF-1α and COX-2 in gastric carcinoma were significantly associated with TNM stage, invasive depth and lymph node metastasis (P< 0.05) except for patient's age, sex and tumor grade.There were positive correlation between HIF-1α expression with COX-2 (rs=0.584, P<0.01). The MVD of cases with positive HIF-1α expression(36.78±10.46) was significantly higher than those without HIF-1α expression(17.92±3.71), the MVD of cases with positive COX-2 expression(37.59±10.75) was also higher than those without COX-2 expression (18.43±4.15). The expression of HIF-1α and COX-2 was associated with MVD(rs=0.624, 0.697, P<0.01). Conclusion HIF-1α and COX-2 play important roles in the pathway of carcinogenesis and progression of gastric carcinoma, and they may contribute to tumor angiogenesis through the same way, thus combined detection of HIF-1α and COX-2 may be recommended for diagnosis of gastric cancer and estimation of prognosis.
3.Effect of ischemic postconditioning on endoplasmic reticulum stress during cerebral ischemia/reperfusion in rats

Objective To evaluate the ischemic postconditioning on endoplasmic reticulum stress during cerebral ischemia/reperfusion (I/R) in rats.Methods Ninety adult male Sprague-Dawley rats,aged 350-450 g,were randomly divided into 3 groups (n =30 each) using a random number table:sham operation group (S group),I/R group,and ischemic postconditioning group (group P).Focal cerebral I/R was induced by electrocoagulation of left middle cerebral artery and 30 min occlusion of bilateral common carotid arteries followed by reperfusion.In group P,bilateral common carotid arteries were subjected to 3 cycles of 30 s reperfusion and 10 s ischemia at the beginning of reperfusion.At 24 h of reperfusion,neurological deficit was scored,and cerebral infarct size was detected by TTC staining.At 6,12 and 24 h of reperfusion,the expression of glucose-regulated protein 78 (GRP78),C/EBP homologous protein (CHOP) and caspase-12 in the ischemic area were measured (using immunohistochemistry).The neuronal apoptosis in the ischemic area was detected by TUNEL.Results Compared with S group,the neurological deficit score was significantly increased,cerebral infarct size was enlarged,the neuronal apoptosis was increased,and the expression of GRP78,CHOP and caspase-12 was up-regulated in I/R and P groups.The neurological deficit score was significantly lower,cerebral infarct size was smaller,the expression of GRP78 was higher at 12 and 24 h of reperfusion,the neuronal apoptosis was lower at 24 h of reperfusion,and the expression of CHOP and caspase-12 was lower in group P than in group I/R.Concluion Ischemic postconditioning can inhibit neuronal apoptosis mediated by endoplasmic reticulum stress during cerebral I/R,which dose not play a leading role in cerebral protection in rats.
4.Effect and safety of Statins therapy in very old patients

Objective To observe efficacy and safety of long-term Statins treatment in elderly patients aged over 80 years,and to provide clinical evidence for the rational use of Statins.Methods 224 patients aged 80 years and over were divided into two groups:Statin group given Statins (n=107),and control group not given Statin (n=117).The serum levels of total cholesterol (TC),lowdensity lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol (HDL-C) and triglyceride (TG) were measured,and liver and renal function,alanine aminotransferase (ALT),creatine phosphokinase (CPK) and epidermal growth factor receptor (EGFR) were tested.Results Pretreatment levels of TC and LDL-C levels were (4.6 ± 1.0) and (2.6 ± 0.8) mmol/L and were decreased to (4.3±0.9) and (2.4±0.6) mmol/L after 1 year treatment (t=3.535,3.448,both P<0.05).The target rate in statin group for LDL C was 35.5 % in patients at very high risk.There were no significant differences in levels of ALT,CK and eGFR between pre-and post-treatment in both two groups [(20.2±9.2) U/L vs.(21.4±12.1) U/L,(86.5±53.9) U/L vs.(86.6±67.5) U/Land (53.1±17.3) ml/min vs.(51.9±18.0) ml/min,all P>0.05].Compared with pre-treatment,1 year treatment showed that glucose level was increased [(5.9± 1.0) mmol/L vs.(6.0± 1.0) mmol/L,t=2.287,P<0.05].Conclusions Long-term Statin treatment in elderly patients is safe and effective.We should correctly administer statin drugs,and closely monitor the safety indicators.
5.Effects of acupuncture and moxibustion on chromatin remodeling-related enzymes in the colon tissue of rats with Crohn disease

Objective:To investigate the regulatory effects of acupuncture and moxibustion on chromatin remodeling complex core catalytic subunit of Brahma-related gene 1(Brg1),histone deacetylase(HDAC)3,HDAC9,and males absent on the first(MOF)in the colon tissue of rats with Crohn disease(CD). Methods:Using the random number table method,60 male Sprague-Dawley rats were divided into 5 groups,including a normal group,a model group,an acupuncture group,a medicinal cake-insulated moxibustion group,and an acupuncture-moxibustion group,with 12 rats in each group.CD rat models were prepared using 2,4,6-trinitrobenzene sulfonic acid(TNBS)in all groups except the normal group.The normal and model groups received no interventions.In the acupuncture group,rats were intervened with acupuncture at bilateral Zusanli(ST36)and Shangjuxu(ST37),20 min/session,once a day.The medicinal cake-insulated moxibustion group received medicinal cake-insulated moxibustion at Qihai(CV6)and bilateral Tianshu(ST25)with 2 cones per point per session,once a day.The acupuncture-moxibustion group received both acupuncture and moxibustion interventions simultaneously.Each intervention was performed for 10 consecutive days.Observations included general condition,disease activity,macroscopic damage,and pathological changes in the rat's colon tissue.Real-time fluorescence quantitative polymerase chain reaction was used to measure the mRNA expression of chromatin remodeling-related enzymes Brg1,HDAC3,and HDAC9,while Western blotting detected the protein expression of Brg1,HDAC3,HDAC9,and MOF in rat's colon tissue. Results:The model group showed significantly increased diarrhea score,occult blood score,macroscopic damage score of colon tissue,and colon macroscopic damage index(CMDI)score,as well as elevated mRNA expression levels of HDAC3 and HDAC9,protein expression levels of HDAC3,HDAC9,and MOF,and decreased mRNA and protein expression levels of Brg1 compared to the normal group(P<0.01).In contrast,compared to the model group,the diarrhea score,occult blood score,macroscopic damage score,CMDI score,mRNA expression levels of HDAC3 and HDAC9,and protein expression levels of HDAC3,HDAC9,and MOF were significantly reduced(P<0.05 or P<0.01),while the mRNA and protein expression levels of Brg1 were significantly increased(P<0.01)in the acupuncture group,the medicinal cake-insulated moxibustion group,and the acupuncture-moxibustion group. Conclusion:Both medicinal cake-insulated moxibustion and acupuncture,either used alone or in combination,can regulate the abnormal expression of chromatin remodeling-related enzymes Brg1,HDAC3,HDAC9,and MOF in the colon tissue,thus reducing colon inflammation in CD rats.
6.Three-dimensional printing technology preparation of bone tissue engineering scaffold materials in bone defect repair

BACKGROUND: Bone tissue engineering plays a very important role in the repair of bone defects, which can deliver bioactive substances, promote bone tissue growth and repair bone defects. Bone scaffolds act as one of the three elements of bone tissue engineering. Three-dimensional (3D) printing technology can achieve individualized bone tissue repair through customized artificial bone preparation.OBJECTIVE: To analyze the biological characteristics of several commonly used bone tissue engineering scaffolds and to explore the application of 3D printing technique in the construction of bone tissue engineering scaffolds.METHODS: The literatures of PubMed and Wanfang database related to bone tissue engineering scaffold materials and 3D printing technology were retrieved from 2005 to 2016. The Keywords were tissue engineering scaffold, bone defects,polymer materials, bioceramics, metal materials, composite materials, 3D printing in English and Chinese, respectively,which would appear simultaneously in title and abstract. Repetitive articles were excluded and finally 65 articles were included in result analysis.RESULTS AND CONCLUSION: The commonly used bone tissue engineering scaffold materials include polymer materials (natural and macromolecule polymeric materials), bioceramics, and metal materials. According to the characteristics of the materials, composite materials made of different materials can compensate for the shortcomings of a single material, and then developed into new tissue engineering scaffold materials. For the tissue engineering bone production, 3D printing technologies include melt deposition technology, selective laser sintering technology, low temperature deposition manufacturing technology, and etc. When the 3D printing technology is used to prepare a bone tissue engineering scaffold, the use of powder or adhesive must have limited conditions, such as flowability, stability and wettability. Powder materials used for 3D printing can be divided into synthetic polymers, natural macromolecules,bioceramics and their mixtures, with different advantages and disadvantages. Ultimately, the bone engineering scaffolds produced by 3D printing technology have unique advantages in mechanics, structure and individuality, and have wide application prospect in the manufacture of bone scaffolds.
7.Discussion on standardized training of resident physicians of acupuncture and tuina graduate students

The standardized training of resident physicians of Chinese medicine specialized graduate students (standardized training) is a great reform of clinical postgraduate education and a major initiative to improve professional degree graduates education. It contributes to higher professional qualities of clinicians in China. At this stage, the standardized training in our school just started and some problems existed such as department arrangement, training and checking system, curriculum and tutors instruction. Here, taking the standardized training in our school as an example, this paper discussed some issues on the training and put forward suggestion. This will help standardize our training, improve the training quality of our graduate students and develope medical professional talents.
8.Effect of CDR1/CDR2 or MDR1 genes overexpression on oxidative stress in Candida albicans

To investigate the effect of CDR1/CDR2 or MDR1 genes overexpression on oxidative stress in Candida albicans,we evaluated the effect of H2O2 on cell viability in C.albicans overexpressing genes CDR1 /CDR2 or MDR1 and their parent strains.After establishing an oxidative stress model with H2O2,we detected reactive oxygen species (ROS),mitochondrial membrane potential (Δψm) and the expression of oxidative stress response-related genes (CAP1 and GRP2) and ROS clearance related-genes (SOD2 and SOD5).The results showed that C.albicans growth were inhibited by 100% after the treatment of 5 mmol/L H2O2.HeO2 caused more ROS accumulation and Δψm reduction in parent strains than in CDR1/CDR2 or MDR1 genes overexpressed strains (P<0.05).Compared to parent strains,the up-regulated expression of CAP1 and GRP2 were relatively less in CDR1/CDR2 or MDR1 genes overexpressed strains,moreover,the down-regulated expression of SOD2 and SOD5 were also relatively less in CDR1/CDR2 or MDR1 genes overexpressed strains (P<0.05).In conclusion,the overexpression of CDR1/CDR2 and MDR1 genes could reduce the oxidative stress response and enhance the adaptability of C.albicans to oxidative stress.
9.Application of Cystatin C-based formulas for prediction of glomerular filtration rate

Objective To investigate the applicability of Cys C-based formulas for prediction of GFR in Chinese patients with CKD.Methods A total of 176 adult patients with CKD including 90 males and 86 females collected from 4 hospitals located in different geographic regions of China (Beijing,Shanghai,Dalian and Changsha) were enrolled in this study from September 2007 to July 2009.The rGFR was measured using 99mTc-DTPA clearance rate two-sample method.Cystantic C was measured by PETIA and PENIA respectively.The results of eGFR in the Larsson formula,Grubb formula,Hoek formula,Filler formula,Stevens formula and Hojs formula were compared with the rGFR to evaluate the calculation coherence,bias,precision,accuracy and the performance of correct phasing of the formulas.Results The mean 99mTc-DTPA clearance was [40.70 ( 19.09 - 86.49)] ml · min-1 · ( 1.73m2 ) -1.Significant difference was witnessed in the evaluation of GFR estimation formulas calculated by PETIA and PENIA (P <0.01).ICC and Spearman correlation analysis revealed a significant correlation between eGFR and rGFR.The ICCs of eGFR and rGFR ranged from 0.874 to 0.938.Compared with rGFR,the 30% accuracy of all the eight evaluation formulas using PETIA and PENIA method were lower than 60%.The percentages of correct phasing in all the 5 stages of CKD were not ideal.With these formulas,percentages of correct phasing from CKD stage 2 to CKD stage 4 were lower than 65%.The eGFRs were underestimated by formulas evaluated by PENIA in CKD stage 1.All the eGFRs were overestimated remarkably by all equations in CKD stage 5.Conclusions None of the eight Cys C based formulas are ideal for estimation of GFR in Chinese CKD patients,and they can not be applied to Chinese patients directly.For this patient population,further studies will be needed to develop a more accurate Cys C-based GFR estimation formula that includes ethnicity,age and other factors.
10.Ultrasound study of brachial plexus

Objective To explore the anatomic character of brachial plexus nerve by ultrasonography,and provide a basis for ultrasound-guided brachial plexus block.Methods Twenty-four healthy volunteers were chosen for high-frequency sonography examination,the features of brachial plexus distributions were studied in the axilla location and the interscalene location.Results Brachial plexus of all the 24 cases were detected.At the axilla level,main braches of brachial plexus nerves were very close to the axillary artery.At the interscalene level,the brachial plexus was consistently found between the scalenus anterior and medius muscles in the interscalene groove.Conclusions Using high-frequency sonography,the main braches of brachial plexus can be observed clearly.
