Objective To understand the mechanism of vitamin A deficiency(VAD) impairing the mucosal immunity and the influences of VAD on cytokines response of intestinal mucosa in rats. Method The rat model with VAD was established, and intestinal infection was induced by typhoid salmonella orally administered. The fresh ileum tissue samples were taken, and ELISA and RT-quantity PCR were used to detect the production of cytokines, including IFN-?,IL-2,IL-4,IL-6,IL-10 and IL-12,by intestinal mucosa in protein and mRNA levels. Results IL-12,the cytokine produced by dendritic cells, was markedly increased in VAD rats, and the level was further raised when the VAD rats were complicated by intestinal infection. The Th1 cytokines,IFN-? and IL-2,were reduced in VAD rats. IL-4,the main Th2 cytokine, was significantly decreased after intestinal infection, especially in VAD rats. IL-6 mRNA was lower in VAD rats and after intestinal infection compared with that of the VA normal control. IL-10 mRNA was also declined in VAD rats, and more markedly reduced after intestinal infection. Conclusion The regulation of VAD onproduction of cytokines in local intestinal mucosa is likely to be an important mechanism of VAD affecting intestinal mucosal immunity.

OBJECTIVETo explore the effects of early intervention of acupuncture on urinary function reconstruction of neurogenic bladder after spinal cord injury, so as to find the optimal treatment timing of acupuncture.
METHODSFifty-three patients of neurogenic bladder after spinal cord injury were randomly divided into a prior-to-catheter group (25 cases) and a posterior-to-catheter group (28 cases). For the prior-to-catheter group, one week before the removal of catheter, acupoints on the abdomen and back such as Guanyuan (CV 4), Zhongji (CV 3), Baliao (Shangliao (BL 31), Ciliao (BL 32), Zhongliao (BL 33), Xialiao (BL 34), Jiaji (EX-B 2) and acupoints at distal end such as Zusanli (ST 36) and Sanyinjiao (SP 6) were selected; after the catheter removal, simple method. of water column was used to measure bladder capacity pressure to classify the type of bladder, and then different acupuncture manipulations were given; intermittent catheterization was performed if condition was allowed. The posterior-to-catheter group applied identical treatment after removal of catheter as the prior-to-catheter group. The treatment was given 5 times per week; after one-month treatment, five aspects including the time of first effective urination, time of establishing reflex urination, average residual volume of urine, time of residual urine less than 100 mL and quality of life (QOL) were evaluated.
RESULTSAfter treatment, the time of first effective urination, time of establishing reflex urination, average residual volume of urine and time of residual urineless than 100 mL in the prior-to-catheter group were all superior to those in the posterior-to-catheter group (all P<0. 05) ; the score of QOL was improved in two groups after treatment (both P<0. 01), but the difference between two groups was not statistically significant (P>0. 05); each index of urination function was not significant among patients with different injured segments of spinal cord (all P>0. 05).
CONCLUSIONThe early intervention of acupuncture (prior to catheter) has obvious improving function on establishing balanced bladder in patients with neurogenic bladder after spinal cord injury, which is superior to acupuncture intervention after removal of catheter, however, the effects of different injured segments on establishing balanced bladder are not different.

Journal of Clinical Hepatology 2021;37(2):471-474
In recent years, the regulatory role of microRNAs in liver pathological process has attracted more and more attention. In viral hepatitis and steatohepatitis, microRNA-335 (miRNA-335) regulates the progression of hepatitis via the transcription factor sex-determining region Y-box 4; in the development and progression of progressive liver fibrosis and liver cancer, miRNA-335 affects collagen production, deposition, and degradation in the liver via the target genes including hypoxia-inducible factor 1α, phosphatase and tensin homologue, Rho-associated coiled-coil containing protein kinase 1, plasminogen activator inhibitor-1, twist family bHLH transcription factor 1, and mesenchymal-epithelial transition factor and thus regulates the migration and invasion of hepatic stellate cells and hepatoma cells. This article summarizes the research advances in the role of miRNA-335 in hepatitis, liver fibrosis, and liver cancer in recent years, and based on existing data, it is pointed out that the miRNA-335/QSOX1 regulatory axis may mediate artesunate against schistosomal liver fibrosis by inhibiting hepatic stellate cell activation, so as to provide new ideas for the treatment of liver fibrosis and other liver diseases.

Objective:To understand the relationship between occupational stress and carotid atherosclerosis in police population of Tianjin City. Methods:A total of 866 male police officers who underwent medical examination and treatment were selected as the survey object. A uniformly designed health questionnaire and occupational stress scale were used for epidemiological investigation. The blood pressure, blood sugar and blood lipid testing, and carotid ultrasound examination were conducted. Multi-factor unconditional logistic regression model was used for statistical analysis. Results:According to the norms of occupational stress of security service personnel, the classification standards of occupational tasks and individual nervous, occupational stress levels were divided into low, moderate, medium and high level. The possibility of occurring CAS in the civilian police population with high and medium individual nervous is 4.37 (95% CI: 1.92~11.59) and 3.28 times (95% CI: 1.84~5.62) than that in the police without nervous. The possibility of occurring CAS in the civilian police population with medium and high level of occupational tasks is 0.19 times (95% CI: 0.06~0.32) and 0.26 times (95% CI: 0.07~0.53) than that in the police with low level of occupational tasks. Conclusions:Occupational stress may be an independent risk factor for CAS occurring in the police population. Therefore, regular psychological counseling is particularly important for the physical and mental health of the police population, which can alleviate occupational stress and psychological pressure.

Organ Transplantation 2020;11(4):492-
Communication with the family members of donors is an integral part of the organ donation and transplantation, and the core of it lies in building trust through interpersonal communication. Every word and deed from the communicator will directly affect the overall impression of family members of potential donors towards organ donation. Regardless of whether or not granted the donation ultimately, family members may share their personal experiences and feelings with friends and relatives around them, which develops a secondary dissemination. Therefore, "the choice of best candidate for communication with family members of organ donation" has been an issue that organ donation practitioners have been working on in clinical practice. Taking into consideration of the experiences from different countries or regions, various advices and practices on this issue have been proposed due to differences in social systems, cultural background, organizational structure, clinical practice,

OBJECTIVE: To investigate the association of myocardial blood flow (MBF) quantified by dynamic computed tomography (CT) myocardial perfusion imaging (MPI) with troponin level and left ventricle (LV) function in patients with ST-segment elevated myocardial infarction (STEMI). MATERIALS AND METHODS: Thirty-five STEMI patients who successfully had undergone reperfusion treatment within 1 week of their infarction were consecutively enrolled. All patients were referred for dynamic CT-MPI. Serial high-sensitivity troponin T (hs-TnT) levels and left ventricular ejection fraction (LVEF) measured by echocardiography were recorded. Twenty-six patients with 427 segments were included for analysis. Various quantitative parameters derived from dynamic CT-MPI were analyzed to determine if there was a correlation between hs-TnT levels and LVEF on admission and again at the 6-month mark. RESULTS: The mean radiation dose for dynamic CT-MPI was 3.2 ± 1.1 mSv. Infarcted territories had significantly lower MBF (30.5 ± 7.4 mL/min/100 mL versus 73.4 ± 8.1 mL/min/100 mL, p < 0.001) and myocardial blood volume (MBV) (2.8 ± 0.9 mL/100 mL versus 4.2 ± 1.1 mL/100 mL, p = 0.044) compared with those of reference territories. MBF showed the best correlation with the level of peak hs-TnT (r = −0.682, p < 0.001), and MBV showed a moderate correlation with the level of peak hs-TnT (r = −0.437, p = 0.026); however, the other parameters did not show any significant correlation with hs-TnT levels. As for the association with LV function, only MBF was significantly correlated with LVEF at the time of admission (r = 0.469, p = 0.016) and at 6 months (r = 0.585, p = 0.001). CONCLUSION: MBF quantified by dynamic CT-MPI is significantly inversely correlated with the level of peak hs-TnT. In addition, patients with lower MBF tended to have impaired LV function at the time of their admission and at 6 months.
