OBJECTIVE:To study the influence of processing method on the content of 3 kinds of alkaloids in Phellodendron chinense. METHODS:HPLC was used to determine the content of 3 kinds of alkaloids in P. chinense,such as stir-frying with wine,salt-roast process and carbonization. The separation was performed on Zorbax SB-C18(250 mm?4.6 mm,5 ?m) column. The mobile phase consisted of acetonitrile-0.1% phosphoric acid (48 ∶ 52,0.1 g SDS per 100 mL) with flow rate of 1.0 mL?min-1. Detection wavelength was set at 265 nm and column temperature was set at room temperature. Injection volume was 10 ?L. RESULTS:Except for special case,the content of berberine hydrochloride,pamatine hydrochoride and jateorizine hydrochoride were all decreased after processing,especially carbonization.CONCLUSION:The 3 kinds of alkaloids can be also used as index of quality control of processed P. chinense. The method is simple,rapid and accurate for the determination of 3 kinds of alkaloids in P. chinense.

Objective To explore changes of serum fibroblast growth factor 2 (FGF2) levels after treatment with imatinib mesylate in patients with unresectable or metastatic gastrointestinal sromal tumor.Methods Serum FGF2 levels (with ELISA) were determined in 27 patients with unresectable or metastatic gastrointestinal stromal tumor both before and after treatment with imatinib mesylate.Results Before treatment with imatinib mesylate,of 27 cases of unresectable or metastatic gastrointestinal stromal tumor,17 cases (63.0 ％) were found positive for serum FGF2,after treatment with imatinib mesylate,9 cases (33.3 ％) were found positive for serum FGF2 (P ＜ 0.05).Using imatinib mesylate for 3 months,complete remission rate was 0,partial remission rate was 51.9 ％ (14/27),stable disease rate was 48.1％ (13/27) and progressive rate was 0.Conclusion Patients with unresectable or metastatic gastrointestinal stromal tumor have elevated serum FGF2 levels.Imatinib mesylate treatment can reduce serum FGF2 levels in patients with unresectable or metastatic gastrointestinal stromal tumor.

Objective To investigate the effects of imatinib mesylate on recurrent or metastatic gastrointestinal stromal tumor.Methods Thirty-five cases of patients with recurrent or metastatic gastrointestinal stromal tumors treated with Imatinib mesylate from January 2007 to February 2010 in our hospital were analyzed retrospectively.Results After treated with Imatinib mesylate for 3 months,complete remission rate was 2.9％( 1/35 ),partial remission rate was 45.7％ ( 16/35 ),stable disease rate was 51.4％ ( 18/35 ) and progressive rate was 0;clinical benefit rate was 100％.The 2-year overall survival rate was 97.1％.The 2-year progression free survival was 82.9％.Conclusion Imatinib mesylate is effective for recurrent or metastatic gastrointestinal stromal tumors and the treatment is safe and reliable.

Objective To investigate the clinical efficacy and safety of phakic posterior chamber intraocular lens (ICL) implantation for high myopia. Methods Twenty-four eyes of 12 patients with high myopia were treated with ICL implantation. The best corrected visual acuity, intraocular pressure and lens changes shape between post-operative and pre-operative were compared. Results Successful implantations were done in all patients. All eyes had a significant increase in uncorrected visual acuity. Exactly improved visual function in patients with high myopia status. The intraocular pressure was ( 14.26 ± 2.13) mm Hg(1 mm Hg = 0.133 kPa) of pre-operative , and was ( 14.79 ± 2.77) mm Hg at 1 year after post-operative, the intraocular pressure was stability after the operation. UBM showed that there was a certain gap between ICL and the lens. During the follow-up period,no complications such as cataract,chronic uveal infusion were found. Conclusion Phakic posterior chamber ICL implantation is effective,safe and reliable method for the correction of high myopia.

ObjectivesTo explore the serum insulin-like growth factor-1 (IGF-1)levels in the patients with unresectable or metastatic gastrointestinal stromal tumors before and after imatinib treatment and to discuss its clinical significance.MethodsThe serum samples of27 patients with unresectable or metastatic gastrointestinal stromal tumors were collected before imatinib treatment(before treatment group) and three months after imatinib treatment(after treatment group).The other serum samples were collected from 20 healthy volunteers who were accepting health examination (control group). The serum IGF-1 levels of the samples were determined by enzyme-linked immunosorbent assay (ELISA).ResultsThe mean serum IGF-1 levels were significantly higher in before treatment group than in control controls[(463.61±120.98)ng/ml vs.(115.75±39.27) ng/ml, t=12.355,P=0.000)]. Three months after imatinib treatment,the mean serum IGF-1 levels were significantly lower in after treatment group than in before treatment group [(244.64±100.11)ng/ml vs.(463.61±120.98) ng/ml, t=7.582,P=0.000].ConclusionsSerum IGF-1 levels may help to judge therapeutic effect,progression,recurrence or metastasis of the gastrointestinal stromal tumor.

Objective To probe into the medical practicing of neurosurgery, thoracic surgery, and cardiac and great vessels surgery in Shanghai, discovering measures and suggestions for improvement Methods Such methods as onsite inspection and questionnaires were called into play to study the practice of neurosurgery, thoracic surgery, and cardiac and great vessels surgery in 68 secondary and tertiary hospitals. Results Such three disciplines in the 68 hospitals have in total 3288 beds, 1142 medical practitioners, 19 assistant medical practitioners, and 1983 nurses. In 10 of the 68 hospitals, the medical space for these disciplines is non-standard; In 3 hospitals, personnel use is non-standard in general; 8% of the medical records are incompliant in generals 15% of the prescriptions are incompliant in general. Conclusion Medical practicing of neurosurgery, thoracic surgery, and cardiac and great vessels surgery in Shanghai is found to be legally compliant in general. However, there exist defects in medical space, personnel use, as well as medical records and prescriptions. In this regard, enhanced management and inspection are recommended.

Objective To understand the drug resistance and changes of Pseudomonas aeruginosa in Tianjin Medical University Eye Hospita and to explore the relationship between the drug resistance and the dosage of antimicrobial agents, so as to provide the basis for clinical rational drug use.MethodsUsage of antibiotics in our hospital during 2012 to 2016 were analyzed retrospectively, and the drug resistance of pseudomonas aeruginosa were calculated respectively.SPSS 22.0 software was used to analyze the drug resistance and the frequency of use of antimicrobial agents (DDDs).ResultsResistance rate of pseudomonas aeruginosa to aztreonam, ceftazidime and meropenem were decreased gradually.Resistance rate of pseudomonas aeruginosa to gentamicin increased gradually.The resistance of pseudomonas aeruginosa to piperacillin, imipenem, cefepime, evil ciprofloxacin, piperacillin/tazobactam tazobactam and cefempidone/sulbactam is in a state of fluctuation.Piperacillin DDDs were significantly negatively correlated with gentamicin resistance.There was a significant positive correlation between imipenem DDDs and gentamicin resistance and there was a significant negative correlation between imipenem DDDs and drug resistance rates of piperacillin, imipenem, ceftazidime, meropenem, levofloxacin and cefoperazone/sulbactam.There was a significant positive correlation between aztreonam DDDs and ceftazidime, meropenem, ciprofloxacin and cefoperazone/sulbactam, There was a significant negative correlation between cephalosporin DDDs and gentamicin resistance rates.There was a significant positive correlation between the DDDs of piperacillin/tazobactam and the resistance rate of piperacillin/tazobactam;the resistance rates of cefoperazone/sulbactam DDDs to aztreonam and meropenem were Significant negative correlation.ConclusionDrug resistance of pseudomonas aeruginosa and the dosage of clinical antibacterial drug is closely related, suggesting that clinicians should use antibiotics for clinical rationally, in order to reduce the number of drug resistance of pseudomonas aeruginosa.

Objective To evaluate the changes in the activity of extracellular signal-regulated protein kinase 5 (ERK5) in the spinal cord in a rat model of bone cancer pain (BCP).Methods One hundred and eight female Sprague-Dawley rats,weighing 160-180 g,were randomly divided into 3 groups (n =36 each):control group (group C):sham operation (group S) and group BCP.In group BCP Walker 256 mammary gland cancer cell suspension (1 × 105 cells/ml) 5 μl was injected into the left tibia,while in group S normal saline 5μl was injected into the left tibia instead of cancer cell suspension.Group C underwent no treatment.Mechanical paw withdrawal threshold to von Frey filament stimulation (MWT) was measured at 1 day before inoculation and 3,5,7,14 and 21 days after inoculation.Six rats were sacrificed after the last measurement of MWT in groups C and S or after measurement of MWT at 3,5,7,14 and 21 days after inoculation in group BCP and the spinal cord was removed for determination of the expression of phosphorylated ERK5 (p-ERK5),ERK5 and Fos protein in the spinal dorsal horn by Western blot analysis.Results There was no significant difference in the MWT and expression of p-ERK5,ERK5 and Fos protein at each time point between groups C and S (P ＞ 0.05).Compared with groups C and S,MWT was significantly decreased at 7-21 days after inoculation,the expression of p-ERK5 and Fos protein was significantly up-regulated at 5-21 days after inoculation (P ＜ 0.05),and no significant change was found in the expression of ERK5 in group BCP (P ＞ 0.05).Conclusion Intra-tibial inoculation of Walker-256 breast cancer cells can increase the activity of ERK5 in the spinal cord in rats,leading to the increased release of Fos protein,thus inducing BCP.
