2.Effects of Qinma Formula on Inflammatory Cell Related Factors in Rats with Chronic Obstructive Pulmonary Disease

Objective To investigate the effects of Qinma Formula on endogenous anti-inflammatory system in rats with chronic obstructive pulmonary disease (COPD); To discuss its mechanism of action. Methods Rat models of COPD were made by modified smoked lipopolysaccharide method. 70 healthy SPF male Wistar rats were randomly divided into normal group, model group, Western medicine control group (aminophylline group), TCM medicine control group (Liujunzi Decoction group) and Qinma Formula high-, medium-, and low-dose groups, with 10 rats in each group. Each medication group was given relevant medicine for gavage. The pathological changes of lung tissue were observed under light microscope; the alveolar lavage fluid (BALF) and venous blood were collected and the related indexes were tested; the levels of serum IL-1β, IL-6 and IL-10 were detected by ELISA. Results Compared with normal group, BALF white blood cells, neutrophils, macrophages, lymphocyte, hemoglobin and platelet count in rats of model group increased significantly (P<0.05). Compared with the model group, BALF white blood cells, neutrophils, and macrophages in rats of Qinma medium- and high-dose groups and Liujunzi Decoction group decreased significantly (P<0.05); white blood cells, hemoglobin, and platelet count in rats of Qinma medium-, high-dose groups and aminophylline group decreased significantly (P<0.05); levels of IL-1β and IL-6 in serum decreased significantly (P<0.05); level of IL-10 in serum increased significantly (P<0.05). Conclusion Qinma Formula can effectively inhibit the inflammatory response through the regulation of inflammatory response-related cytokines.
3.Value of MRI in assessing the invasiveness of intraductal papillary neoplasms of the bile duct

Objective To value the capability of MRI in assessing invasiveness of intraductal papillary neoplasms of the bile duct(IPNB). Methods Thirty-nine patients with pathologically confirmed IPNB, who had upper abdominal MR examination within 6 weeks before complete resection of the tumor, were included in the retrospective study. Patients were divided into noninvasive and invasive groups pathologically. Eighteen cases were noninvasive and 21 were invasive. All had undergone MRI plain scans, MR cholangiopancreatography as well as contrast enhanced scans including arterial, portal and delayed phases. Tumor size, location, biliary dilation, thread signs, lesion morphology, lobe atrophy, cholelithiasis, biliary hemorrhage, vascular invasion and intraperitoneal lymphadenopathy were observed on MRI. ADC values and enhancement level of lesions were also measured. Between invasive and noninvasive groups, laboratory results, enhancement levels and ADC values were compared by t test or Mann-Whitney U test, and categorical variables like location and lesion morphology were compared by χ2 test. The diagnostic accuracy was calculated using receiver operating characteristic(ROC)curve analysis. Results No difference was found between invasive and noninvasive groups on gender, age, lesion morphology, bile duct diameter, location, existence of thread signs or cholelithiasis(P>0.05). While the differences on serum CA19-9 level, lesion size, ADC value, and lymph nodes/vascular invasion between groups reached statistical significance (P<0.05). Other than on plain scan(P>0.05), CNR and enhancement levels were also statistically different on arterial, portal and delayed phases between both groups(P<0.05). CA199, enhancement level and CNR of portal phase, as well as ADC value exhibited areas under the ROC curve(AUC)of 0.790, 0.891, 0.817 and 0.882 respectively in invasiveness judgment. Conclusion MR demonstrated relatively high value in assessing invasiveness of IPNB.
4.Role of microRNA-335 in chronic liver diseases

In recent years, the regulatory role of microRNAs in liver pathological process has attracted more and more attention. In viral hepatitis and steatohepatitis, microRNA-335 (miRNA-335) regulates the progression of hepatitis via the transcription factor sex-determining region Y-box 4; in the development and progression of progressive liver fibrosis and liver cancer, miRNA-335 affects collagen production, deposition, and degradation in the liver via the target genes including hypoxia-inducible factor 1α, phosphatase and tensin homologue, Rho-associated coiled-coil containing protein kinase 1, plasminogen activator inhibitor-1, twist family bHLH transcription factor 1, and mesenchymal-epithelial transition factor and thus regulates the migration and invasion of hepatic stellate cells and hepatoma cells. This article summarizes the research advances in the role of miRNA-335 in hepatitis, liver fibrosis, and liver cancer in recent years, and based on existing data, it is pointed out that the miRNA-335/QSOX1 regulatory axis may mediate artesunate against schistosomal liver fibrosis by inhibiting hepatic stellate cell activation, so as to provide new ideas for the treatment of liver fibrosis and other liver diseases.
5.Clinical Application Value of Lactobacillus Plantarum PS128 in Patients with Anxiety Disorders

Objective:
PS128 is a novel psycho biotic strain, it has been reported to play an important role in neuropsychiatric disorders. This study investigated the clinical effect of PS128 supplementation on patients with anxiety.
Methods:
A total of 200 patients with anxiety were recruited, and divided into two groups (n = 100/group). The control group received oral treatment with citalopram, and the PS128 group received PS128 capsules based on citalopram treatment. Hamilton Anxiety Scale (HAMA) and Self-Rating Anxiety Scale (SAS) were used to evaluate the anxiety levels.After 2 months of continuous administration, clinical efficacy was evaluated according to HAMA score.
Results:
There was no significant difference in HAMA and SAS scores between the two groups before treatment. With the treatment prolonged, the HAMA and SAS score decreased gradually in both control and PS128 groups, and the decrease rate of PS128 group was significantly greater than that of the control group. The clinical effective rates of PS128 group were higher than those in the control group, high levels of clinical cure rate were also detected in the PS128 group. Compared with the control group (22%), the incidence of adverse reactions was significantly reduced for patients in the PS128 group (4%).
Conclusion
The treatment effect of citalopram combined with PS128 against anxiety is satisfactory clinically. It can greatly improve the anxiety symptoms of patients, increase the cure rate, reduce adverse reactions.
6.Rate of force development and its relationship with functional performance in patients after anterior cruciate ligament reconstruction

BACKGROUND:The maximum muscle strength is typically used for evaluating the recovery of muscle function after anterior cruciate ligament reconstruction.Recent studies have suggested that neuromuscular function should also be considered,such as rate of force development,which measures the slope of the force time curve at different time intervals under conditions of isometric muscle contraction. OBJECTIVE:To elaborate on the current research status and shortcomings of muscle isometric rate of force development in patients undergoing anterior cruciate ligament reconstruction surgery,and analyze the degree of defects in quadriceps femoris and hamstring isometric rate of force development at different times after surgery;to analyze the effect of isometric rate of force development on postoperative functional performance,thereby providing important information for optimizing postoperative rehabilitation following anterior cruciate ligament reconstruction,reducing secondary injury to patients,and reducing the incidence of knee osteoarthritis. METHODS:Literature retrieval of CNKI,VIP,WanFang and PubMed was performed using"anterior cruciate ligament,rate of force development"as Chinese search terms and"anterior cruciate ligament,rate of force development,rate of torque development"as English search terms.Finally,69 articles were included according to inclusion and exclusion criteria. RESULTS AND CONCLUSION:Most studies have found defects in bilateral muscle isometric rate of force development in patients undergoing anterior cruciate ligament reconstruction within 6 months.Early isometric rate of force development of the bilateral hamstring muscles(i.e.the slope of the force time curve at any time interval during muscle contraction of 100 ms)showed significant improvement after 6 months.However,long-term defects in early isometric rate of force development of the bilateral quadriceps indicate long-term damage to the neuromuscular function of the quadriceps after surgery.There is limited research on late isometric rate of force development(the slope of the force time curve at any time interval after 100 ms of muscle contraction),and conclusions cannot be drawn.Regarding landing exercises(jumping to the ground,lateral cutting,etc.)and daily activities(walking,running),early isometric rate of force development of the quadriceps is more correlated with isometric peak torque.Abnormal biomechanical changes during exercise are considered an important risk factor for secondary injury and traumatic knee osteoarthritis in patients.Actively improving early isometric rate of force development of the quadriceps may reduce the incidence of secondary injury and traumatic knee osteoarthritis.Currently,there is limited evidence to suggest that whole-body vibration training can improve early isometric rate of force development of the quadriceps femoris in patients undergoing anterior cruciate ligament reconstruction.It is recommended to use neuromuscular electrical stimulation to intervene in the quadriceps and hamstring in the early postoperative stage and implement explosive force and high resistance training in the late postoperative stage,which may improve the isometric rate of force development in patients.Generating sufficient muscle strength in a short period of time is necessary to effectively protect the anterior cruciate ligament,while the relationship between isometric rate of force development in the hamstring muscle and functional performance is still unclear,which may provide information on preventing secondary injury in patients.It is recommended to use the isometric rate of force development as one of the evaluation indicators for guiding rehabilitation and restoring movement.In addition to focusing on improving symmetry and differences from normal individuals,the hamstring to quadriceps strength ratio should also be considered.An appropriate range of ratios can ensure the balance of muscles during rapid muscle exertion,which may reduce the occurrence of secondary injuries.However,the normal range of ratios is not yet clear.Future research should consider the effects of graft type and knee flexion angle on isometric rate of force development,in order to identify neuromuscular dysfunction in patients as much as possible and help them recover better.
