1.The application of case management model on the stable patients with chronic obstructive pulmonary disease

Objective To evaluate the effect of case management model on the stable patients with chronic obstructive pulmonary disease (COPD),containing health knowledge level,the number of acute exacerbation and the quality of life.Methods Fifty patients were divided into the experimental group and the control group with 25 cases each by random digits table method.The experimental group received the case management model for 3 months.The control group received routine care for 3 months.The COPD health Knowledge Questionnaire(CKQ) scores and St George's Respiratory Questionnaire(SGRQ) scores were measured for 4 times (the beginning of the intervention and 1 months,2 months,3 months later),and the numbers of acute exacerbation were measured at 3 months later.Results The CKQ scores at 1,2,3 months after intervention were (28.04±1.43),(28.20±1.00),(28.40±1.04) points in the experimental group,and (18.08±2.30),(18.50±1.87),(18.33±1.66) points in the control group,and there were significant differences (t=18.109,22.556,5.320,all P < 0.01).The scores of SGRQ and symptom at 2 months and 3 months after intervention were (50.56±8.68),(49.40±8.05),(41.52±10.86),(37.16±8.90) points in the experimental group,and (57.42±10.22),(59.46±9.06),(54.50±8.67),(56.71 ±10.03) points in the control group,and there were significant differences (t=-7.222-2.536,P < 0.05 or 0.01).The influence scores at 1,2,3 months after intervention were (46.28±8.74),(44.40±8.11),(42.04±8.22) points in the experimental group,and (51.50±7.77),(51.79±8.06),(54.08±6.96) points in the control group,and there were significant differences(t=-2.206,-3.197,-5.523,P < 0.05 or 0.01).There was no statistically significant difference about activity scores between two groups before and after intervention (P > 0.05).The numbers of acute exacerbation during 3 months with 3,22 cases in 1 and 0 time in the experimental group and 9,15 cases in the control group,and there was statistically difference between two groups(x2=4.306,P< 0.05).Conclusions The case management model can promote the stable COPD patients to know more disease-related health knowledge,and improve the health knowledge level.The case management model can reduce the symptoms of the stable COPD patients,reduce the frequency of acute exacerbation,reduce the influence of disease on daily life,prevent further decline in activity endurance,and improve the quality of life.

4.Screening of High-yield and High-oil Curcuma kwangsiensis

OBJECTIVE:To establish a method for analyzing the volatile oil of high-yield and high-oil Curcuma kwangsiensis, and to provide reference for its breeding. METHODS:GC-MS was performed on the column of HP-5MS quartz elastic capillary column,carrier gas was high purity helium(99.999%),flow rate was 1.0 ml/min,inlet temperature was 250 ℃,the initial tem-perature of column was 50 ℃(temperature programmed),split injection with split ratio of 10:1. Mass spectrometry conditions:ionization mode was the electron impact ion source,ionization energy was 70 eV,the ion source temperature was 230 ℃,the quadrupole temperature was 150 ℃, transmission line temperature was 280 ℃,the electron multiplier voltage was 1588 V,and mass scanning range was m/z 45 to 500. High-yield and high-oil germplasm of were screened from 100 different germplasms,the volatile oil composition of single plant and relative percentage contents of each composition were compared,SPSS 22.0 software was used for cluster analysis. RESULTS:Totally 10 high-yield and high-oil germplasm were screened,54 kinds of compounds were identified,the common compositions of 10 different germplasms were camphor,1-caryophyllene,γ-elemene,curcumene, gemma ketone and new curdione,most germplasms contained borneol,isoborneol,δ-elemene,germacrene and calamine. The C78,C104,purpose 2,volatile oil content and relative percentage contents of active compositions in volatile oil were high. The 10 high-yield and high-oil germplasms can be divided into 3 groups. CONCLUSIONS:The study basically clears the main chemicalcomposition of volatile oil of high-yield and high-oil C. kwangsiensis,C78,C104 and purpose 2 are the more excellent strains in high-yield and high-oil C. Kwangsiensis germplasm.
6.Clinical study for mouse nerve growth factor on treating herpes zoster

Objective To observe the effect of mouse nerve growth factor (mNGF) on treating herpes zoster.Methods The 64 patients with herpes zoster were divided into treatment group and control group by random digits table with 32 cases each.The control group received only routine treatment,the treatment group based on routine treatment combined with mNGF 30 μ g,both groups were treated 14 days.Pain changes of two groups were observed in once every 5 days,a total of 1 month.Results The times of pain to lessen and the pain to vanish of treatment group were (4.36 ± 2.01) and (12.29 ± 1.98) d,those of control group were (7.23 ±3.57) and (20.36 ±2.24) d,there were significant differences (t =5.21 and 8.37,P <0.01).The effective rate of treatment group was significantly higher than that of control group [96.9%(31/32) vs.71.9%(23/32),P<0.05].Conclusion mNGF combined with routine treatment can significantly shorten the course of nerve injury and improve the prognosis with few adverse reactions.
7.Preparation and Properties of Tremella-like Fe-N-C Composite Catalyst for Oxygen Reduction Reaction

The Fe-N-C composite catalyst was prepared by the thermal decomposition of the chelate precursors based on Fe central ions and o-phenylenediamine ligands.It was observed from the scanning electron microscopy that the crumpled carbon micro-and nano-sheets were intertwined to form a free-standing tremella-like 3D structure.The N2 adsorption/desorption experiments revealed that the composite contained ample micro-and meso-pores and had a specific surface area of 290 m2/g.Graphitic C and multi-crystal Fe3C as main components were confirmed by the X-ray diffraction, and N-doping in the general form of graphite N and pyridine N was also verified by X-ray photoelectron spectroscopy.The electrochemical measurement showed that the tremella-like Fe-N-C composite catalyzed oxygen reduction through a four-electron path in an alkaline solution, and its activity was comparable to the commercial Pt/C catalyst.After 2000 cycles, the limited current density of the Fe-N-C catalytic electrode only decreased less than 5%, and the half-wave potential shift negatively 5 mV, which suggested that the Fe-N-C composite catalyst had better catalytic stability than the commercially used Pt/C catalyst.
8.Expression of bone sialoprotein in prostate cancer and its clinical significance

Objective To investigate the expression of bone sialoprotein (BSP) in prostate cancer and its clinical significance. Methods Prostate cancer tissues of different pathological grades (68 cases) and benign prostatic hyperplasia tissues (22 cases) were selected. SP method was used to detect the expression of BSP. Serum total prostate-specific antigen (tPSA) levels of prostate cancer were detected by electrochemiluminescence immunoassay before the operation. Results Compared with no or low expression in the adjacent normal glandular tissues, the detectable levels of BSP were examined in most of the prostate cancer tissues. The expression rate of BSP in prostate cancer tissues was higher than that in benign prostatic hyperplasia tissues [76.47%(52/68) vs 13.64%(3/22),χ2=27.614, P<0.001]. The expression rates of BSP in well differentiated, moderately differentiated and poorly differentiated tissues according to cell differentiating degree (Gleason system) were 75.0 % (12/16), 77.5 % (31/40) and 75.0 % (9/12) respectively. There was no significant difference in various pathological grading (χ2=0.057, P=0.972). The expression rates of BSP in pathological stage pT2, pT3 and pT4 tissues were 62.16%(23/37), 95.24%(20/21) and 90.0%(9/10) respectively. A statistically significant association was found between BSP expression and pathological stage (χ2=9.338, P=0.009). Serum tPSA level of prostate cancer group with BSP expression was higher than that with no BSP expression [(69.06±25.52)μg/L vs (38.00±21.64)μg/L, F=19.355, P<0.001]. Conclusion The high expression of BSP in prostate cancer has a relationship with pathological stage and serum tPSA level, it may play an important role in the biological behaviour of prostate cancer.
9.Clinics in China:development and distribution analysis

Objective To learn the recent development and regional distribution of clinics in China.Methods Based on statistics and a nationwide survey of clinics in the country,a simple linear regression was made to find factors determining clinics regional distribution.Results Clinics in China were found to have grown sizably from 134 000 in 2008 to 155 000 in 2014;medical technology workers to 2.31 per clinic in 2014;and the total revenue of these clinics accounted for only 0.724% of all medical institutions,while there are more clinics in the east than the west regions in China.Conclusions The role of clinics in attracting high quality medical resources to primary care should be further enhanced for development of the hierarchical medical system in China.
10.Effects of HAVCR2 gene silence on apoptosis and cell cycle in the radiation-caused genomic instable liver cells

Objective To explore the effects of HAVCR2 siRNA on apoptosis and cell cycle in the radiation-caused genomic instable liver cells.Methods RNAi was used to inhibit HAVCR2 gene transcription.Cellular apoptosis and cell cycle distribution were detected by flow cytometry (FCM).Expression of p53 gene was assayed by real time fluorescence quantitative PCR.Results HAVCR2 gene was effectively inhibited by RNAi (t =19.21,P < 0.05).After siRNA transferring,cell apoptosis (t =3.65,P < 0.05) and p53 gene expression (t =4.82,P < 0.05) were decreased,and G2-phase arrest was induced(t =-3.41,P < 0.05).Conclusion HAVCR2 siRNA can decrease the generation of apoptosis in the genomic instable liver cells and blocks the cells at G2 phase.
