1.Exogenous opioid peptide preconditioning in prevention of pulmonary ischemia-reperfusion injury in pigs after cardiopulmonary bypass

Objective:To evaluate the protective effect of ?-opioid peptide,[D-Ala~(2)-D-Leu~(5)]-enkephaline(DADLE), on pulmonary ischemia-reperfusion(I-R) injury after cardiopulmonary bypass(CPB),and to discuss the possible mechanism.Methods: Twenty-eight male pigs were evenly randomized divided into 4 groups,namely,the sham-operation group,the CPB group(only subjected to CPB),the DADLE pretreatment group(received DADLE 1 mg?kg~(-1)before CPB),and the Glibenclamide group(received DADLE 1 mg?kg~(-1)and glibenclamide 1 mg?kg~(-1) before CPB).Peak inspiratory pressure(PIP),PaO_(2),and the plasma malondialdehyde(MDA) concentration were measured in each group before CPB and 10 min,30 min,and 60 min after reperfusion.The changes of lung tissue wet weight/dry weight ratio(W/D) were calculated before CPB and 60 min after reperfusion.The animals were sacrificed 60 min after reperfusion to observe the ultrastructural changes of the lungs and to quantitatively assess lung tissue damage(LTD).Results: There were no significant differences among the 4 groups before CPB and the parameters in(sham-operation) group had no changes at all specified time points.One hour after reperfusion,the PIP,MDA,W/D,and LTD values in Glibenclamide and CPB groups were significantly higher than those in sham-operation and DADLE groups while the PaO_(2) was significantly lower(P

Acute traumatic coagulopathy (ATC) attacks suddenly with a rapid progress and has an extremely highly mortality.Of late years more and more relative studies have made a great progress on the pathogenesis,diagnosis and treatment of ATC.However,there are currently no medical guidelines for ATC.There are still some problems such as nonstandard treatment,outdated concept and so on in clinic.Based on the latest research progresses home and abroad,this paper expounds emphatically the first-aid and treatment of ATC in order to bring helps to the clinical doctors.
3.Clinic research on delivery mode selection of re-pregnancy after cesarean delivery

Objective To discuss the main reason of re -pregnant patients'delivery mode selection after cesarean delivery and to guide the future clinical work.Methods Retrospectively analyzed the 181 cases of re -pregnancy after cesarean delivery and compare the clinical data of 140 cases of reselecting cesarean delivery with 41 cases of selecting vaginal trial production.Results Calculated the risks of vaginal trial production and cesarean delivery of scarred uterus by Smith formula.There was no statistically significant difference in two groups (P >0.05). Reselection of cesarean delivery accounted for 77.0% among the cases of re -pregnancy after cesarean delivery.In the investigation on the reason for re -pregnant patients reselecting cesarean delivery,38 patients chose cesarean delivery for the first time owing to social factors,accounted for 27.1%,taking the first place.While in the investiga-tion on the reason for re -pregnant patients selecting vaginal trial production,10 patients were operated on cesarean section for the first time due to fetal distress,accounted for 24.4%,coming first.Conclusion In the investigation on the reason for re -pregnant patients reselecting cesarean delivery,the result shows that the high possibility of reselect-ing cesarean delivery lies on those who chose cesarean delivery for the first time owing to social factors.While those who were transferred to cesarean section due to abnormal fetal heart rate during the first vaginal trial production,may have high possibility to select vaginal trial production when re -pregnancy.As a result,the key is to decrease the rate of cesarean delivery for the first time.

OBJECTIVE: To discuss the impact of Zero Inventory of Chinese decoction pieces in our hospital. METHODS: Zero Inventory of Chinese decoction pieces was analyzed from aspects of hospital resource utilization, pharmacy management, and quality assurance of decoction pieces. RESULTS: Zero Inventory can help save precious hospital resources, streamline pharmacy management and ensure quality of decoction pieces supply. CONCLUSION: Zero Inventory of Chinese decoction pieces has positive impact on hospital pharmacy management; therefore it deserves to be popularized.
5.Calcium phosphate nanoparticles as new generation vaccine adjuvants

Adjuvants have been used as critical components of vaccine.They were used to stabilize the antigens and potentiate the immune responses.Dose sparing is also of interests in recent years to be applied in potential pandemic situations or to lower the costs of vaccines.With the emerging nanotechnology in many aspects including the use in biomedical applications,the application of nanomaterials as vaccine adjuvants also attracted a lot of attentions in recent years.With favorable biological properties,such as well-defined and well-formed nanoparticles,biocompatibility,biodegradability and ability to induce humoral immunity and cellular immunity simultaneously,calcium phosphate nanoparticles (CaP) have the potentials to be developed as an immune potentiator to be used as vaccine adjuvants.Here,we reviewed the basic properties of CaP and their applications as vaccine adjuvants with antigens of different modalities such as inactivated virus,protein and naked DNAs.The mechanism of the adjuvanting effects is briefly described.Further the development of CaP as vaccine adjuvants with some designed surface modification could lead to next generation vaccine adjuvants with improved immune potentiating properties and safety profiles.

7.Co-delivery of paclitaxel and cyclosporine by a novel liposome-silica hybrid nano-carrier for anti-tumor therapy via oral route.

In this study, we developed a novel liposome-silica hybrid nano-carrier for tumor combination therapy via oral route, using paclitaxel and cyclosporine as a model drug pair. Optimization of the preparation of the drug-loading formulation and characterization of its physicochemical parameters and drug release profile were performed in vitro. Then in vivo pharmacodynamics and pharmacokinetics studies were performed. The results showed that the obtained formulation has a small particle size (mean diameter of 100.2 +/- 15.2 nm), a homogeneous distribution [the polydispersity index was (0.251 +/- 0.018)] and high encapsulation efficiency (90.15 +/- 2.47) % and (80.64 +/- 3.52) % for paclitaxel and cyclosporine respectively with a mild and easy preparation process. A sequential drug release trend of cyclosporine prior to palictaxel was observed. The liposome-silica hybrid nano-carrier showed good biocompatibility in vivo and co-delivery of cyclosporine and paclitaxel significantly enhanced the oral absorption of paclitaxel with improved anti-tumor efficacy, suggesting a promising approach for multi-drug therapy against tumor and other serious diseases via oral route.

9.Transfusion management analysis of perioperative cancer patients

Background and purpose:Reasonable proportion with blood less than half of the surgical system, cancer patients during surgery prone to acute massive blood loss, this study aimed to investigate blood transfusion in the patients who diagnosed with cancer during operation in the Afifliated Tumor Hospital of Xinjiang Medical University, and in order to improve the intraoperative blood transfusion and make it more scientiifc and more reasonable.Methods:The patients who were taken selective operations and heterogenous blood transfusions in the Afifliated Tumor Hospital of Xinjiang Medical University from Oct. 2012 to Dec. 2013 were enrolled, and the clinical data were retrospectively surveyed and investigated.The intraoperative use of blood components and reasonableness in each department were analyzed.Results:The medical records of 299 blood transfusion receipts proportions of reasonable transfusion of RBC from department of gastrointestinal surgery, gynecology, orthopedics, hepatobiliary surgery, neurosurgery, urological surgery and thoracic surgery were 88.9%, 91.8%, 94.3%, 96.3%, 91.6%, 100%, 81.3%,respectively. And the proportions of reasonable transfusion of plasma were 86.2%, 71.8%, 96.4%, 78.4%, 100%, 100%, 87.5%, respectively. The proportions of low-volume blood transfusion and the combined transfusion of RBC and plasma were 62.5% and 43.2% in unreasonable blood transfusion.Conclusion:Except for the unreasonableness in very few departments, the intraoperative blood component transfusion is carried on fairly well. The medical staff still should be further trained in the appropriate use of blood and strengthened the knowledge on blood transfusion.
10.Effect of anisodamine on myocardial connexin 43 expression in pig after resuscitation from cardiac arrest

ObjectiveTo investigate the effect of anisodamine on the expression of connexin 43 (Cx43) in swine ventricular myocardium after resuscitation from cardiac arrest.Methods The experiment was conducted on healthy pigs, and they were randomly divided into three groups, namely sham group, epinephrine group (control group) and anisodamine group (experimental group, animals were resuscitated combined with injection of 0.4 mg/kg of anisodamine), with 5 pigs in each group. Model of ventricular fibrillation was reproduced by alternating current challenge, and cardiopulmonary resuscitation (CPR) was performed 8 minutes after cardiac arrest. Left ventricular myocardium was harvested at 24 hours after restoration of spontaneous circulation. The expression and distribution of Cx43 were observed by immunofluorescence, Cx43 mRNA expression was assessed with reverse transcription-polymerase chain reaction (RT-PCR), and the protein expressions of Cx43 and phosphorylation of Cx43 (p-Cx43) were analyzed by Western Blot.Results The positive expression of Cx43 in ventricular muscle was distributed uniformly, mostly at the end-to-end linkage of myocardial cells, with a few side-to-side linkage in sham group. The positive expression of Cx43 in control group was significantly weaker than that in the sham group, and the signal intensity was significantly declined (4.35±2.10 vs. 10.02±3.66,P< 0.01). The positive expression of Cx43 at the end-to-end linkage and side-to-side linkage was irregular in experimental group, and the signal intensity was obviously higher than that in the control group (7.91±2.54 vs. 4.35±2.10,P< 0.05), but it was significantly weaker than that in the sham group (7.91±2.54 vs. 10.02±3.66,P< 0.05). For control group and experimental group, the Cx43 mRNA and protein expressions were significantly lower than those of the sham group [Cx43 mRNA (A value): 0.32±0.05, 0.32±0.03 vs. 0.48±0.07; Cx43 protein (A value): 0.43±0.03, 0.50±0.07 vs. 0.65±0.04, allP< 0.01], and there were no significant differences between experimental group and control group (allP> 0.05). The p-Cx43 protein expression of control group was significantly lower than that of the sham group (A value: 0.22±0.03 vs. 0.37±0.06,P< 0.01), and it was significantly higher in the experiment group than that in the control group (A value: 0.29±0.07 vs. 0.22±0.03, P< 0.01), but there was no significant difference with the sham group (P> 0.05). No significant difference in p-Cx43/Cx43 was found among sham, control, and experiment groups (0.57±0.09, 0.51±0.05, 0.58±0.06, all P> 0.05).ConclusionAnisodamine can improve the abnormal expression of Cx43 in ventricular muscle of pigs with cardiac arrest, which may be related to the protection effect of anisodamine on cardiac conduction.
