Objective To explore the effect of extracurricular activities based on specialist nursing clinic. Methods Thirty-four undergraduate nursing students of grade 2013 in the Capital Medical University were enrolled in the study. After learning of nursing theories for senile dementia, the extracurricular activities based onspecialist nursing clinicwere carried out. Before and after the second class activities, the caring ability inventory scale and self scale-rating for self-directed learning (SRSSDL) were used to assess 34 students. Result After the activities, students' caring ability and self-directed learning ability were statistically significantly improved (P<0.01). Conclusion The extracurricular activities based onspecialist nursing cliniccan stimulate the learning interest of nursing students and improve their self-directed learning ability and humanistic care ability.
2. Meta-analysis of clinical efficacy of Helmet non-invasive ventilation and oxygen therapy on patients with hypoxemic respiratory failure

Objective:
To systematically evaluate the clinical efficacies of Helmet non-invasive ventilation and oxygen therapy on patients with hypoxemic respiratory failure.
Methods:
The randomized controlled trials (RCTs) for comparison of efficacy between Helmet non-invasive ventilation and oxygen therapy for treatment of patients with hypoxemic respiratory failure published by Wanfang database, China National Knowledge Infrastructure (CNKI), China Biology Medicine (CBM), PubMed, Embase, Cochrane Library and Web of Science were retrieved. The retrieval time was from the establishment of database to February 1st, 2019. The indexes of the study outcomes included oxygenation index, arterial partial pressure of carbon dioxide (PaCO2), endotracheal intubation rate, hospital mortality and intolerance rate. Literature search and data extraction was performed separately by two researchers. Quality assessment of literature was conducted according to the risk of bias criterion provided by Cochrane collaboration net. The extractive data were Meta-analyzed by RevMan 5.1.0. Funnel plot and Egger regression analysis was employed to detect publication bias.
Results:
Six RCTs including 5 English studies and 1 Chinese study were selected. Finally, 547 patients were enrolled, with 270 patients in Helmet non-invasive ventilation group and 277 in oxygen therapy group. The study quality assessment revealed that the overall risk of bias was low, and no publication bias was detected by the funnel plot and Egger regression analysis. Objective To explore the potential mechanism of recombinant ficolin 3(FCN3)in regulating non-small cell lung cancer cell line A549.Methods Bioinformatics analysis of FCN3 was performed using online software,and its overexpression vector and siRNA interference system were constructed.CCK8 assay and flow cytometry were used to detect the effects of overexpression and knockdown of FCN3 on the proliferation and apoptosis of A549 cells.Real-time fluorescent quantitative polymerase chain reaction was used to detect the influence of FCN3 on the expression of lung adenocar-cinoma-related genes[Chromobox Homolog 5(CBX3),Cyclin-Dependent Kinase Inhibitor 3(CDKN3)and Baculoviral IAP Repeat-Containing Protein 5(BIRC5)].Results After overexpres-sion of FCN3,the apoptosis level of A549 cells increased,and the proliferation level decreased(P<0.05).After knockdown of FCN3,the apoptosis level of A549 cells decreased,and the proliferation level increased(P<0.05).Overexpression of FCN3 inhibited the expression of CBX3,CDKN3 as well as BIRC5 mRNA,and promoted the expression of FABP4 mRNA;knockdown of FCN3 promoted the expression of CBX3,CDKN3 as well as BIRC5 mRNA,and inhibited the expression of FABP4 mRNA.Conclusion FCN3 can inhibit the proliferation of A549 cells and promote their apoptosis.FCN3 may serve as a potential therapeutic target for non-small cell lung cancer.

Meta-analysis showed that the oxygenation index in Helmet non-invasive ventilation group was significantly higher than that in oxygen therapy group [mean difference (MD) = 73.47, 95% confidence interval (95%CI) was 52.01 to 94.92, P = 0.000 01], and PaCO2 (MD = -2.46, 95%CI was -4.54 to -0.39, P = 0.02), endotracheal intubation rate [relative risk ratio (RR) = 0.38, 95%CI was 0.20 to 0.73, P = 0.004] and hospital mortality (RR = 0.35, 95%CI was 0.19 to 0.65, P = 0.000 8) in Helmet non-invasive ventilation group were significantly lower than those in oxygen therapy group. There was no significant difference in patient's intolerance between the two groups (RR = 2.38, 95%CI was 0.74 to 7.67, P = 0.15).
CONCLUSIONS
Compared with oxygen therapy, the Helmet non-invasive ventilation used for treatment of patients with hypoxemic respiratory failure can effectively improve the oxygenation index, decrease the PaCO2, reduce the endotracheal intubation rate and hospital mortality, and the patients are well tolerated to the Helmet method.

6.Analysis on value of using real time dynamic blood sugar monitoring during insulin pump treatment process in ICU patients

Objective To investigate and analyze the value of using real time dynamic blood sugar monitoring during insulin pump treatment process in ICU patients.Methods Sixty-two ICU cases needing insulin intervention due to own blood glucose abnormality in this hospital from February 2015 to February 2016 were selected and divided into the control group and observation group,31 cases in each group.The control group used the fingertip method for measuring blood glucose and simultaneously used the continuous intravenous insulin injection by micropump;the observation group used real time dynamic blood sugar monitoring system to measure blood glucose level.Results The two groups completed the related study without excluded case or fallen out case.The observation group was better than the control group in the aspects of the time reaching target blood sugar level,total treatment time,insulin dosage and hypoglycemia occurrence rate,the difference was statistically significant (P < 0.05).MBG,MAGE,MODD,SDBG and time percent of glucose drift (TBG≥7.8 mmol/L and TBG≤3.9 mmol/L) on 3 d in the observation group were significantly decreased compared with those on 1 d.The two monitoring methods showed significantly positive correlation (r =0.97,P=0.006).Conclusion ICU patients using the real time dynamic blood sugar monitoring system can obviously decrease blood sugar level,and is safe and effective.
7.Analysis of ND4 gene mutations in acute myelogenous leukemia.

OBJECTIVETo investigate the relationship of the mutational status of the ND4 gene and the clinical features of acute myelogenous leukemia (AML) patients with ND4 mutations.
METHODSUsing PCR combined with directly sequencing, we identified somatic mutations of ND4 in 121 primary AML patients to couple with their clinical features.
RESULTSThere were 58 male patients and 63 female patients (median age 49 years, 10-86 years). Eight of 121 patients (6.6%) with de novo AML were found harboring missense mutation of ND4 gene, including 3 patients with A131V (3/8, 37.5%), 2 patients with A404T (2/8, 25%), 1 patient with F149L (1/8, 12.5%), 1 patient with G242D (1/8, 12.5%) and 1 patient with Y409H (1/8, 12.5%), respectively. Patients with ND4 mutations were associated with good karyotype (P=0.049), regardless of gender, age, white blood cell, hemoglobin, platelet, blast cells of bone marrow or immunophenotype (P>0.05). There were no statistical significance in mutations of FLT3-ITD, NPM1, CEBPA, c-KIT and DNMT3A between patients with ND4 mutation and wild-type (wt) ND4 (P>0.05). The median overall survival of patients with ND4 mutations and wt ND4 were all not reached. The median relapse-free survival were not reached and 29(2-53) months, respectively (P>0.05). There was no significance in the ratio of CR and RR patients between wt ND4 and ND4 mutated groups (P>0.05).
CONCLUSIONIt was concluded that novel ND4 mutations could be found in de novo AML patients, especially in patients with good karyotype. Thus, ND4 mutations might play an important role in AML prognosis. However, whether the mitochondria dysfunction contribute to leukemogenesis needs to be further investigated.
