1.Metabolize characteristic and effects of cephalosporins and fluoroquinolones in ACST in human bile

Objective To investigate the regulation of metabolize characteristic of some antibiotics in ACST in human bile, and provide the theoretic basis for physician selecting antibiotic rationally when biliary tract has been infected. Methods Samples of ACST in human bile were obtained by ERCP + ENBD, contents of Cefradine, Cefoperazone, Ceftriaxone, Ciprofloxacin and Lomefloxacin in human bile were simultaneously measured by HPLC method. Results After antibiotics were given through iv 0. 25 ~ 1. 5 hour, an increase was observed in contents of all antibiotics in bile, and the changes of Ceftriaxone and Ciprofloxa-cin were the most significant ( P <0.01). Ceftriaxone and Ciprofloxacin had a higher Cmax than other antibiotics, and Cefoperazone had a longer tl/2. Conclusion As maintaining a little time to tmax , a higher Cmax and a longer tl/2 in the bile by iv, Cefradine and Ciprofloxacin are recommended in the patient with ACST.
2.Application of 3-dimensional CT reconstruction in the diagnosis of posterior urethral strictures or ankylurethria

Objective To study the diagnostic value of 3-dimensional CT reconstruction in posterior urethral strictures or ankylurethria.Methods Thirty patients with strictures or ankylurethria of posterior urethra caused by pelvic fracture underwent helical CT scan and 3-dimensional reconstruction of the urethral canal as well as radiographic urethrography before and post open urethral reconstruction to observe the urethral anatomy,the length and position of the urethral strictures,the depth of periurethral scar.Results The mean stricture or ankylurethria length measured by radiographic urethrography was 4.0 cm (range from 1.0 cm to 7.0 cm),and the correlation coefficient of stricture or ankylurethria length was 0.92,21 (70%) patients were diagnosed accurately by radiographic urethrography.The mean stricture or ankylurethria length measured by 3-dimensional CT reconstruction was 4.3 cm (range from 1.2 cm to 7.6 cm),and the correlation coefficient of stricture or ankylurethria length was 0.96,there were 28(93%) patients diagnosed accurately by 3-dimensional CT reconstruction.The mean stricture or ankylurethria length measured by open urethral reconstruction was 4.2 cm (range from 1.5 cm to 7.5 cm).Five patients with urethrorectal fistula were also diagnosed accurately by 3-dimensional CT reconstruction rather than by radiographic urethrography.Conclusions 3-dimensional CT reconstruction of the urethral canal can accurately evaluate the urethral anatomy,the length and position of the urethral strictures,as well as the depth of periurethral scar after crush injury and provide useful information for operation that may not be provided by radiographic urethrography.3-dimensional CT reconstruction may become the most valuable means for detecting posterior urethra strictures or ankylurethria with urethrorectal fistula.
3.125I seed fixity after implantation in parotid region

Objective To observe the fixity of 125I seed after implantation in parotid region. Methods Ten patients treated with 125I seed interstitial brachytherapy in parotid were randomly selected. Within one week after the treatment, two plane radiographs were taken by radiotherapy simulator and 125I seed was counted. All plane dosimetry analysis were performed by treatment planning system. The areas surrounded by 50%, 100% and 150% of prescription dose curve and dose equality index were calculated. Two months after the treatment, the same examination were repeated and were compared with t test. Results The number of seeds was equal in two examination. There were no signifieant differences in two examinations either for the areas surrounded by 50%, 100% and 150% of prescription dose curve or for the dose equality index. Conclusions The position of 125I seed after implantation in parotid region is quite fixable, which can ensure the curative effect.
4.The Mutations and Sequencing of HPV16E6E7Gene of Human Papillomavirus from Patients with HPV Infection in Beijing

Objective To investigate the gene sequence and mutations of human papillomavirus(HPV)type16E6E7in patients with HPV infection in Beijing.Methods Sample DNA was extracted from lesions in patients with HPV infection.HPV types were identified by polymerase chain reaction(PCR).E6E7gene,isolated from samples infected with HPV16only,was cloned into plasmid pGEM-3zf and sequenced.Results The recombinant plasmid pGEM/16E6E7was constructed successfully.The whole HPV E6E7gene was776bp in length which was equal to that of the standard strain.Three nucleotides exchanges,i.e.,p60PROE6,p96GLUE6,p565SERE7,were found in E6E7gene.Conclusion The data suggest that there are nucleotide differences of HPV E6E7gene between HPV obtained from Beijing and that of standard sequence.
5.Early endoscopic therapy for elderly patients with acute severe biliary pancreatitis

Objective To evaluate the clinical effects of early endoscopic therapy for elderly patients with acute severe biliary pancreatitis.Methods Ninety-two elderly patients with acute severe biliary pancreatitis were randomly divided into 2 groups:ERCP group ( n =43) and non-ERCP group ( n =49).Serum TNF-a,IL-6,IL-8,the recovery time of blood amylase,the duration of abdominal pain,hospitalization,mortality and complications were compared.Results ERCP group showed a greater decrease in serum TNF-α,IL-6 and IL-8 levels than the control group ( 45.16 ± 13.48 ) μg/L v.s.( 176.89 ± 47.35 ) μg/L,(31.76 ± 13.85)μg/L v.s.(68.48 ±24.87) μg/L,(113.39 ±63.78) μg/L v.s.(309.86 ± 117.13)μg/L) (P <0.05 ).The duration of abdominal pain,the recovery time of blood amylase and hospitalization in ERCP group were significantly shorter compared to the non-ERCP group [ ( 10.2 ± 1.7 ) d v.s.( 13.2 ±2.4)d,(3.3 ±1.0)dv.s.(5.5 ±1.2)d,(15 ±1.6)dv.s.(20±3.0)d] (P<0.05),and complication rate of the ERCP group was lower,too (5% v.s.22%,P < 0.05).Conclusion Early ERCP is safe and highly effective for the elderly patients with acute severe biliary pancreatitis.
6.Animal Model of Esophageal Cancer Based on Its Clinical Symptoms of Traditional Chinese and Western Medicine

ObjectiveEsophageal cancer is a common malignant tumor of the upper gastrointestinal tract, and has high incidence and mortality in China. Its incidence is increasing year by year, and survival rate is low, thus seriously threatening human life and health. To further explore the pathogenesis of esophageal cancer and its systematic and efficient diagnosis and treatment methods, the animal models of esophageal cancer was evaluated according to the animal model evaluation method previously established by our team based on the characteristics of its clinical symptoms of traditional Chinese and western medicine, and suggestions for model improvement were proposed. MethodThe existing animal models of esophageal cancer were summarized through China National Knowledge Infrastructure (CNKI) and Wanfang Data. The relevant indexes of the models were assigned, and their coincidence with the clinical diagnostic guidelines of traditional Chinese and western medicine for esophageal cancer was evaluated. ResultExcept the spontaneous animal model of esophageal cancer with high clinical coincidence adopted in few studies, the animal model induced by methylbenzylnitrosamine was in good agreement with the clinical data, which simulated the etiology and pathogenesis of esophageal cancer to a certain extent. The model partially reflected some indicators of clinical diagnosis in western medicine, and also indicated the body weight loss, purple nail and increased number of drinking in traditional Chinese medicine (TCM). However, there was still a lack of differentiation of TCM syndromes. ConclusionOn the basis of the original model, the animal model induced by methylbenzylnitrosamine and the mouse model of xenogeneic gastric wall transplantation of human esophageal cancer cells were applied, which were intervened with the factors of phlegm and qi mutual obstruction syndrome, blood stasis and phlegm stagnation syndrome, Yin deficiency and internal heat syndrome and Qi deficiency and yang weakness syndrome, and were distinguished to reflect the performance of TCM syndrome. The animal model of esophageal cancer combined with TCM syndrome was thus obtained, which embodied the clinical symptoms of esophageal cancer in TCM, and the characteristics of the animal model combined with TCM syndrome, and simulated the clinical practice of traditional Chinese and western medicine, providing an accurate pathological model carrier for basic research.
7.Characteristics and Analysis of Animal-derived Drugs in 2020 Edition of Chinese Pharmacopoeia

ObjectiveTo statistically analyze the efficacy and application characteristics of animal-derived drugs in the 2020 edition of the Pharmacopoeia of the People's Republic of China (referred to as Chinese Pharmacopoeia), and provide data references for the research and application of animal-derived drugs. MethodAnimal-derived drugs included in the 2020 edition of the Chinese Pharmacopoeia were selected and screened. Information such as the classification, nature and flavor, meridian tropism, usage, dosage, toxicity or contraindications of single animal-derived drugs were statistically analyzed. The efficacy and clinical application analysis of externally applicable Chinese medicinal preparations containing animal-derived drugs, the dosage forms of preparations containing animal-derived drugs, and the contraindications of their use were also analyzed. The above information was recorded in an Excel spreadsheet for statistical analysis. ResultThe 2020 edition of the Chinese Pharmacopoeia clearly records a total of 51 types of animal-derived drugs. Among them, liver-calming and wind-relieving drugs and deficiency-tonifying drugs are the most common in the efficacy classification of animal-derived drugs. The properties are mainly categorized as neutral and salty, with meridians including liver, kidney, heart, and lung meridians with a frequency of >10 times. Most animal-derived drugs are used in decoctions and pills, with the decoction method being the primary method for decoction. The common dosage of animal-derived drugs is 3-9 g, 5-10 g, or 3-6 g, and animal-derived drugs are used externally in appropriate amounts. In addition to mentioning the toxicity of drugs, the most common precautions for animal-derived drugs are caution and prohibition for pregnant women. Externally applicable Chinese medicinal preparations containing animal-derived in the Chinese Pharmacopoeia are widely used in various departments such as pediatrics, gynecology, orthopedics and traumatology, stomatology, internal medicine, surgery, dermatology, ophthalmology, and otorhinolaryngology, with extensive clinical applications. The dosage forms of preparations containing animal-derived drugs are mainly pills, capsules, tablets, and capsules. Animal-derived drugs are mainly in the form of fine powder before use. The precautions for use in preparations mainly focus on the contraindications for pregnant women and children. ConclusionThe clinical application of animal-derived drugs included in the 2020 edition of Chinese Pharmacopoeia is extensive and has unique efficacy. However, there are still problems such as imperfect quality standards and unclear mechanisms of action for animal-derived drugs, which require further research to better utilize the advantages of internal and external use of animal-derived drugs. In addition, non-medicinal parts of animals also have development and utilization value, which should be fully explored and utilized to achieve the green development of animal-derive drug resources.
