Objective To observe the efficacy of Shuilongjian in treating cerebellar infarction, and compare with the pure western medicine. Methods 67 inpatients with cerebellar infarction enrolled were divided into two groups according to the method of random digits table, 36 cases in the treatment group and 31 cases in the control group. The control group was given 500 mL dextran by venoclysis, once a day. The treatment group was given Shuilongjian as an additional drug. The therapeutic effect was evaluated according to the nerve function and the total capability in dealing with daily activities. Objective To evaluate the significance of immediate early 1 (IE) mRNA detection by nucleic acid sequence-based amplification (NASBA) in the diagnosis of human cytomegalovirus (HCMV) infection after renal transplantation.Methods The expression of IE-mRNA and pp67-(mRNA) was detected by NASBA in 55 patients after renal transplantation, and pp65 antigenemia (assays) were done for all cases.Results Twenty cases were positive for IE-mRNA. Thirteen cases were suffered from CMV disease with symptoms. The sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of IE-mRNA by NASBA were 92.3 %, 80.9 %, 60.0 % and 97.1 %, respectively. Objective To investigate the protective effects of Bauhinia championii Benth flavones(BCF) on oxidative stress of neonatal rat cadiocytes induced by H2O2.Methods Cadiocytes of neonate rat was cultivated for 72 hours and divided into six groups: a normal control group, a H2O2 group, BCF(60, 120 and 240μg/ml)+H2O2 groups and aShuxueninginjection (100μg/ml)+H2O2 group (n=8). 6 hours after the drugs were given, the morphology changes was observed and the survival rate was detected; the content of AST, CPK, LDH in culture medium were detected; the activity of SOD, CAT, GSH-Px and the content of MDA in cardiomyocytes were also determinted; the apoptosis rate were detected, and the expression of caspase-3 in cardiomyocytes was detected by Western blot.Results Compared with the H2O2 group, the activity of AST(28.8 ± 6.1 U/ml, 24.5 ± 5.3 U/mlvs. 36.2 ± 6.7 U/ml), CPK(1.8 ± 0.4 U/ml, 1.5 ± 0.3 U/mlvs. 2.5 ±0.4 U/ml), LDH(805.2 ± 160.9 U/L, 671.5 ± 128.7 U/Lvs. 916.5 ± 168.4 U/L) in culture medium were significantly decreased (P<0.05,P<0.01), the activity of SOD(84.8 ± 17.4 U/mg, 95.3 ± 18.2 U/mgvs. 55.7 ± 13.1 U/mg), CAT(23.4 ± 3.1 U/mg, 26.3 ± 3.5U/mgvs. 15.2 ± 3.0 U/mg) in cardiomyocytes were significantly increased and the content of MDA(8.1 ± 1.7 nmol/mg, 6.8 ± 1.5 nmol/mgvs. 11.1 ± 2.3 nmol/mg) were significantly decreased (P<0.05,P<0.01), the expression of c caspase-3(1.64 ± 0.16, 1.30 ± 0.12vs. 2.06 ± 0.25) and the apoptosis rate (24.2% ± 5.5%, 13.4% ± 3.9%vs. 51.2% ± 9.1%) were significantly decreased (P<0.05,P<0.01); the activity of GSH-Px (3.6 ± 0.9 U/mg vs. 2.4 ± 0.7 U/mg) in cardiomyocytes of BCF 240μg/ml treatment group was increased (P<0.05).Conclusions BCF could effectively improve the morphology of neonatal rat cadiocytes induced by H2O2, increase the survival rate, improve the activity of antioxidase, down-regulate the expression of caspase-3 and decrease the apoptosis rate, suggesting that BCF had dose-dependent protective effects on oxidative stress of neonatal rat cadiocytes induced by H2O2.

ObjectiveTo study the surgical indications of using locking compression plate (LCP) to fix the posterior pelvic ring fracture,and to explore its surgical techniques.MethodsFrom September 2005 to February 2010,30 patients with posterior pelvic ring fracture were treated with LCP.Among them 21 were males,9 were females,with the average age of 38.7 years(ranged from 21 to 68 years).The injuries were caused by traffic accident in 14 cases,fall-down from high place in 10 cases,smash of heavy object in 6cases.According to Tile classification,4 patients were type B2,8 patients were type B3,12 patients were type C1,5 patients were type C2,and 1 patient was type C3.Concurrent injures including:3 cases with lung contusion; 3 cases with abdominal parenchymatous organs rupture,2 cases with intestinal rupture,5 cases with urethral injury,5 cases with traumatic brain injury,and 8 cases with fractures of limbs,spine and acetabulum.ResultsAll the incisions of 30 patients were healed by first intention.No skin necrosis,fat liquefaction and infection.Thirty cases were followed up from 13 to 40 months,with the average time of 25.1months.According to Majeed functional score system,20 cases were excellent,7 cases were good and 3 cases were acceptable,the excellent and good rate was 90％.Twelve cases got bone healing 3 months after operation,10 cases got bone healing 4 months after operation,and 8 cases got bone healing 6 months after operation.All the cases had no reduction loss,no limb length discrepancy,no deep venous thrombosis,and had not accelerated the previous nerve symptom.ConclusionUsing LCP to fix posterior pelvic ring fracture has many advantages,including wide indication,strong strength of fixation,well biomechanical properties,easy-to-use,and very good follow-up results.

A case with the diagnosis of the onset of inflammatory bowel disease (IBD) with chronic granulo-matous disease (CGD) in Children′s Hospital Affiliated to Zhengzhou University in June 2016 was chosen, and the patient′s clinical data and whole treatment process were analyzed.According to the relevant literature from Chinese and foreign databases, the clinical characteristics were analyzed and summarized, principles of diagnosis and treatment for children who had the onset of IBD with CGD.This patient was a child of 1 year and 9 months old, and the initial symptoms included repeated diarrhea and bloody stools.He was diagnosed as " ulcerative colitis" in the local hospital.After admission, the neutrophil respiratory burst test was positive.The genetic analysis result suggested that the CYBB gene was mutated, thus obtaining the diagnosis of CGD.Then, he was given prophylactic antibiotic therapy and symptomatic treatment.After the 3 months of follow-up after discharge, the patient still had intermittent diarrhea and bloody stools.CGD is a rare primary immunodeficiency disease, and current treatment methods of CGD include hematopoietic stem cell transplantation and anti-infection treatment.When IBD patients have complications other than gastrointestinal symptoms, the IBD treatment is not effective, or there are suspected parents who are married to close relatives, or with the family history of IBD, CGD should be considered.

Objective To observe the expression of zinc finger protein A20(A20), NF-κB and related inflammatory factors before and after lipopolysaccharide (LPS) stimulates degeneration of rabbit intervertebral disc nucleus pulposus cells.Methods The normal and degenerative nucleus pulposus cells were isolated and cultured, then divided into normal group,degenerative group,LPS stimulation group and NF-κB inhibition group.HE staining observe the morphological changes of nucleus pulposus and annulus fibrosus,immunohistochemistry was used to detect the expression of A20,NF-κB/p65 and COL-Ⅱ.Real-time PCR was employed to analyze the expression of A20,IL-1β,TNF-α,NF-κB and COL-Ⅱ,Western blot was used to observe the A20 protein,p65 and COL-Ⅱexpression in the four groups, and TNF-α, IL-1β in cell supernatant was determined by ELISA.Results The number of nucleuspulposus cells significantly decreased, aggregation occured in the degenerative group.COL-Ⅱ was obvious lower and A20, p65 significantly higher than that in normal group by immunohistochemical staining.Compared with the normal group,A20,TNF-α,IL-1β,p65 expression was significantly increased and COL-Ⅱ decreased in the mRNA and protein levels in degenerative group.Above indexes changed more significant in LPS stimulation group than in degenerative group.The expression of A20, TNF-α, IL-1β, p65 in the NF-κB inhibitor group was lower than that in the LPS group, and the expression of type Ⅱ collagen increased(P<0.05).Conclusions Intervertebral disc inflammatory response is closely related to the development of intervertebral disc degeneration, A20 may play an important role.

Objective To investigate the prevalence of A2058G or A2059G mutation within 23S rRNA in Treponema pallidum (Tp) from primary syphilis patients chancre samples. Methods Simple PCR was used to screen the positive samples containing Tp DNA. Nested PCR was adopted to amplify the region of the Tp 23S rRNA and the purified amplicons were digested by restriction endonuclease MboⅡand Bsa I respectively and sequenced. Results 39 qualified samples were obtained from 43 chancre samples and all of them were found harboring the A2058G mutation, whereas the A2059G was not detected. Conclusion High frequency of the A2058G mutation within 23S rRNA implicated in macrolide resistance emerges in the circulating Tp in Hengyang. Therefore, macro-lide antibiotics such as azithromycin should be cautiously used as an optional therapy for syphilis.

OBJECTIVE: To investigate the clinical characteristics of ocular symptoms in patients with allergic rhinitis (AR) and the impact of the ocular symptoms on patients' quality of life. METHOD: AR patients' history and clinical data were collected and analyzed. One hundred cases were extracted from adult patients with and without ocular symptoms in each group and their quality of life were evaluated using rhino-conjunctivitis quality of life questionnaire (RQLQ). RESULT: Totally 1119 cases were collected and 859 cases had ocular symptoms. Of the patients with ocular symptoms, 582 cases were mild, 234 cases were moderate, 43 cases were severe. Eye itching was the most common symptom, followed by tears, hyperemia and swelling. Patients with ocular symptoms had longer disease history than non ocular symptom group (P < 0.05). There was a positive correlation between ocular and nasal symptoms (P < 0.01); patients with ocular symptoms had more severe nasal symptoms than patients without ocular problems; while patients with moderate to severe AR had more severe ocular symptoms than mild AR patients (P < 0.01). Female patients had higher incidence (P < 0.05) and ocular symptoms score (P < 0.05) than male. Children less than 10 years old had a relative lower incidence and score of ocular symptoms. While the incidence and score had an increasing trend for patients older than fifty. There was no difference regarding the type and number of allergen in the patients with and without ocular symptom. Moreover, patients with ocular symptoms had higher scores in the domains of non-nose/eye symptoms, practical problems, ocular symptoms, emotional function and total score in RQLQ than patients without ocular symptom. CONCLUSION It was common for the AR patients to have ocular symptoms, and ocular symptoms had obvious influence on the patients' quality of life. So controlling of the ocular symptoms should not be ignored during the diagnosis and treatment of AR.

Objective We aimed to assess effectiveness of Risk Score Scale (RSS) for aortic Dissection (AD) in early triage stage.Methods A total of 251 patients with acute chest or back pain complaints were allocated by triage nurses according to the pre-established RSS from 2011 to 2013.87 cases were assessed to have 5 points or more,and 80 cases were assessed to have 6 points or more from 95 patients diagnosed as aortic dissection (the diagnosed group) by CT angiography and magnetic resonance imaging while excluded 156 cases of control after discharge (control group).The general conditions,distribution of aortic dissection risk factors,aortic dissection risk score,sensitivity and specificity of RSS between two groups were compared.The misdiagnosis rate,missed diagnosis rate,RSS triage sensitivity and specificity and OR value were compared between RSS ≥5 points and RSS ≥6 points.Results There was significant differences in abrupt onset of severe pain intensity,ripping or tearing pain,pulse deficit or systolic blood pressure differential between extremities [upper extremity≥20 mmHg (1 mmHg=0.133 kPa) or lower extremity≥ 10 mmHg],x2 value was 62.6,94.2 and 20.2.RSS≥5 points had high sensitivity and specificity,lower missed diagnosis rate and high OR value for triage application.Conclusion RSS≥5 points could serve as a reliable evidence for early triage of aortic dissection.

Objective To assess the usefulness of left ventricular contrast echocardiography in diagnosis of left ventricular myocardium noncompaction.Methods Contrast echocardiography was done in ten patients who were diagnosed or suspected with left ventricular noncompaction by common transthoracic echocardiography,for further study of the trabecular muscles extent,the continuity of the endocardium,the compact myocardium thickness,and the contrast agent in the trabecula recessus.Results By contrast echocardiography,noncompaction myocardium thickness can be perspicuously observed,the turgor of the contrast agent was vividly detected in the trabecular recessus.Especially for the measurement of compaction myocardium,the contrast echocardiography was more accurate than in the condition of the common echocardiography.Conclusions Left ventricular contrast echocardiography can be used in the diagnosis of left ventricular noncompaction,it was a good added method of conventional echocardiography.
