The pulmonary surfactant protein D ( SP-D) plays an important role in host defense and innate immunity of the lung.It has increasing evidences that SP-D involves in the pathogenesis of respiratory syncytial virus (RSV)infection.Recent studies have demonstrated that SP-D can be used for treating infectious and inflammatory diseases of the lung .Here is to make a review on SP-D structure, function , relationship between RSV and pulmonary inflammatory disease treatment .aiming to provide some assistance for treatment of RSV infection .
2.Meta analysis of tension free vaginal tape and laparoscopic Burch for the treatment of female stress urinary incontinence

Objective To compare the objective cure rate and perioperative complications 0f ten-sion free vaginal tape(TVT)and laparoscopic Burch for the treatment of fernale stress urinarv inconti-nence. Methods Literature search was conducted for all randomized control trials(RCTs)between 1994 and 2006,using MEDLINE,EMBASE with the keyword"tension free vaginal tape","colpo-suspension","stress urinary incontinence",then a second search was performed with the following keywords:[(tension free vaginal tape),AND(colposuspension),OR(Burch)],AND stress urinary incontinence].Gray literature was search in The Cochrane central register of controlled trials、NRR (National Research Register)、CCT(clinical controlled trials),ISTP,ISI proceedings,OCLC First-search proceedings,(CACP),GreyNet(http://www.greynet.org/)、DARE(Database of Abstract:s of Reviews of Effects)、SIGLE(System for Information on Grey Literature in Europc),official website of ICS.Meta-analysis was performed with RevMan software 4.2. Results right studies were included comparing objective cure rate and perioperative complications of TVT and LB.Overall objective cure rates were 233/259 for TVT and 189/247 for Burch respectively.TVT shared higher objective cure (RR 1.19,95%CI 1.10-1.29,P<0.01)and lower perioperative complications(RR 0.48,95%CI 0.32-0.71,P<0.05). Conclusions The available evidence indicates that TVT is an effective treat-ment for stress urinary incontinence with higher objective cure rate and 10wer perioperative complica-tions compared with LB.
3.The progress and practice of pulmonary rehabilitation in patients with chronic obstructive pulmonary disease

Pulmonary rehabilitation has been shown to be effective in reducing dyspnea and improving exercise capacity and health-related quality of life in people with COPD.COPD patients with varying severity can benefit from pulmonary rehabilitation which includes exercise training,inspiratory muscle training and nutritional intervention,et al.Exercise training is the basis of pulmonary rehabilitation.In the last decade,new strategies have been developed to enhance the effects of pulmonary rehabilitation,which include promoting exercise and activity,education and self-management,psychosocial intervention.Although pulmonary rehabilitation has traditionally been provided in stable COPD patients,pulmonary rehabilitation could be started as soon as infection is controlled for AECOPD patients.As pulmonary rehabilitation is administrated,the patients should be encouraged to do exercise as much as possible with different kind of training.
4.Impact of diagnosis-related groups point payment on hospitalization costs of parturition among lying-in women

Objective:
To examine the effect of diagnosis-related groups (DRGs) point payment on hospitalization costs of parturition among lying-in women, so as to provide the evidence for alleviating the burdens and saving medical resources among lying-in women.
Methods:
Lying-in women's age, gestational age, parity, duration of hospital stay, DRGs grouping and hospitalization costs were collected from the Inpatient Medical Record System and DRG Operation Analysis System in a tertiary women and children's hospital in Ningbo City from 2020 to 2021. The changes of hospitalization costs of parturition were compared among lying-in women before and after DRGs point payments, and the association between DRGs point payments and gross hospitalization costs of parturition was examined among lying-in women using a multivariable logistic regression analysis.
Results:
A total of 11 505 lying-in women after DRGs point payments, including 6 216 women at age of 30 years and below (54.03%), and 10 871 lying-in women before DRGs point payments, including 6 208 women at age of 30 years and below (57.11%), were enrolled. The median (interquartile range) gross hospitalization expenses, material expenses and laboratory testing expenses of parturition were 8 519.19 (2 456.61), 881.38 (816.16) and 939.00 (310.00) Yuan among lying-in women after DRGs point payments, which were significantly lower than those [9 123.13 (2 660.33), 915.57 (825.26), 1 036.00 (385.00) Yuan] among lying-in women before DRGs point payments (Z=-21.971,-16.061 and -27.199, all P<0.001). Multivariable logistic regression analysis showed that DRGs point payment was statistically associated with lower gross hospitalization expenses of parturition among lying-in women after adjustment for age, duration of hospital stay, gestational age, parity, type of delivery and development of complications (OR=0.462, 95%CI: 0.432-0.494).
Conclusion
DRGs point payment is beneficial to reduce the hospitalization cost of parturition among lying-in women.
5.Correlation study on high performance liquid chromatography and antidepressant effect of different eluting fractions of acorus tatarinowii schott macroporous resin

Objective To research the correlation on high performance liquid chromatography (HPLC) and antidepressant effect of different elution fractions of acorus tatarinowii schott macroporous resin .Methods Prepared different fractions by using AB‐8 macroporous resin .Establish HPLC characteristic chromatograms of crude drugs and different elution fractions from AB‐8 acorus tatarinowii schott macroporous resin by using gradient elusion .Confirm characteristic peaks in chromatograms .despair mod‐els of male mice ,including tails suspension test (TST ) ,forced swimming test (FST ) were used to evaluate antidepressant effects by taking orally .Results HPLC chromatograms of crude drugs and different fraction of acorus tatarinowii schott showed that they had same characteristic peaks .Elution of 90% and 70% alcohol solution from AB‐8 macroporous resin can obviously reduce TST and FST time .The anti‐depressant effect were associated with characteristic peaks on chromatogram .Conclusion Acorus tatarinowii schott has anti‐depressant effect ,and mainly from 70% and 90% alcohol solution eluted from AB‐8 macroporous resin .HPLC char‐acteristic chromatography and data relationship between relative peaks can be used to judge the antidepressant effects of acorus ta‐tarinowii schott ,and give reference to research and product development .

Object To separate and characterize the chemical constituents of a plateau plant Halenia elliptica D Don Methods Elemental analysis (EA), 1HNMR and 13 CNMR, MS, FTIR and UV spectrometry, as well as DSC were employed Results Two needle shaped crystal chemical constituents obtained from H elliptica were confirmed to be 1 hydroxy 3, 7, 8 trimethoxyxanthone and 1, 7 dihydroxy 3, 8 dimethoxyxanthone, respectively Conclusion This is the first time for these two chemical constituents to be separated from this Tibetan medicinal plant
7.Application and comparative studies on problem-based learning and lecture-based learning of clinical teaching in burn surgery

To improve the quality of clinical teaching in burn surgery,79 students of Grade 2005 were randomly divided to problem-based learning(PBL)group and lecture-based learning(LBL) group.The teaching quality of two teaching methods were compared.The results showed that the average score,the score of teaching effectiveness appraisal targets in PBL group were sig-nificant higher than that in LBL group.This indicated that the teaching effectiveness of PBL was better than that of LBL,PBL should be applied in clinical teaching in burn surgery.
8.Construction and identification of expression of superantigen SED mutants

Objective To construct staphylococcal enterotoxin D (SED) mutants expressed in Escherichia coli ( E. coli ). Methods The expected mutants were introduced into the SED DNA by megaprimer PCR method. The PCR products ligated to plasmid pTrcHis B were transformed into E. coli DH5? for IPTG induced expression. The target protein was purified by Ni NTA metal affinity chromatography and analyzed by SDS PAGE and immunoblotting. Results The sequencing results showed that mutant nucleic acids were successfully introduced at the expected sites of SED gene. SDS PAGE and immunoblotting confirmed that the proteins of SED mutants were obtained by Ni NTA metal affinity chromatography. The mutants were named as SEDN23A, SEDN23A/H26R, SEDF45A, SEDL59A, SEDN61A, SEDI92A, and SEDF203A, respectively. Conclusion Several SED mutants are successfully constructed, which lays a foundation for subsequent studies of immune recognition of SED.
9.Application value of MR diffusion weighted imaging of apparent diffusion coefficient in diagnosis of breast nodular lesions

ObjectiveTo estimate the applications of ADC value and rADC value in the diagnosis of nodular lesions of breasts.Methods Fifty-two cases with 66 nodular lesions of breasts confirmed by histopathology underwent diffusion-weighted magnetic resonance imaging.Three b values (0,800 and 1000 s/mm2) were applied.The mean ADC values of the breast nodules,the ADC values of ipsilateral breast( rADC1 )and ADC values of contralateral breast (rADC2 )were respectively measured.The independent-samples t-test and chi-square test were used for statistical analyses.ResultsOf the 52 patients,there were 18 patients with infiltrating ductal carcinoma and 34 patients with fibroadenoma.50 patients with 64 lesions were examined by DWI.( 1 ) at b = 800 s/mm2,the mean ADC values of malignant nodules [ ( 1.01 ±0.09) × 10-3 mm2/s],rADC800-1 (0.52 ±0.07)and rADC800-2 (0.51 ±0.06) were lower than that of the benign nodules [ ADC value = ( 1.54 ± 0.28 ) × 10 -3 mm2/s,t = 8.217,P < 0.01 ; rADC800-1 =0.77 ±0.15,t =9.339,P<0.01 ; rADC800-2 =0.76 ±0.14,t = 10.394,P <0.01 ].The one-side upper limits of 95% medical reference value of mean values of infiltrating ductal carcinoma were adopted as the threshold point to distinguish the malignant from the benign.The threshold value of breast malignant nodule ADC,the rADC800-1 and rADC800-2 were respectively 1.05 × 10-3 mm2/s,0.55 and 0.53.The sensitivities of the three methods were 75.0%,65.0% and 60.0% ; the specificities were 100.0%,95.7% and 97.8% ;the positive predictive values were respectively 100.0%,86.7% and 92.3% ; the negative predictive values were 90.2%,86.3% and 84.9%; the diagnosis accordance rates were respectively 92.4%,86.4% and 86.4%.( 2 ) at b = 1000 s/mm2,the mean ADC values of malignant nodules [ ( 0.93 ± 0.08 ) ×10-3 mm2/s],rADC1000-1 (0.53 ±0.09) and rADC1000-2 (0.52 ±0.07) were also lower than that of the benign nodules[ ADC value= (1.45 ±0.28) ×10-3 mm2/s,t=11.844,P<0.01; rADC1000-1 =0.75 ±0.16,t=5.820,P < 0.01 ; rADC1000-2 = 0.74 ± 0.15,t = 8.082,P < 0.01 ].The threshold value points breast malignant nodule ADC,the rADC1000-1 and rADC1000-2 were respectively 0.97 × 10-3 mm2/s,0.58,0.55.The sensitivities were all 70.0% ; the specificities were respectively 100.0%,95.7% and 93.5% ;the positive predictive values were 100.0%,87.5% and 82.4% ; the negative predictive values were 88.5%,88.0% and 87.8% ; the diagnosis accordance rates were 90.9%,87.9% and 86.5% respectively.There were no significant differences in specificities and the diagnosis accordance rates ( x2 = 1.232,2.263 ; P =0.942,0.812 ).Conclusions ADC value and rADC value are both important parameters of MRI in differentiating benign and malignant breast diseases.The study indicated that ADC value ( at b =800 s/mm2) was the most valuable parameter.
10.Selective application of type Ⅰ and type Ⅱ bundled pancreaticojejunostomy

Objective To analyze and summarize the efficacy and the experience in the application of type Ⅰ and type Ⅱ bundled pancreaticojejunostomy in pancreaticoduodenectomy. Methods Between Jan.2005 and Dec. 2009, a total of 38 patients who underwent bundled pancreaticojejunostomy was enrolled, and their clinical data were retrospectively analyzed. 20 patients received type Ⅰ bundled pancreaticojejunostomy and 18 patients received type Ⅱ bundled pancreaticojejunostomy. The operative time, postoperative hospital stay, mortality and complications were compared. Results The operative time of type Ⅰ bundled pancreaticojejunostomy was (91 ± 20) min, and it was (63 ± 21) min in type Ⅱ procedure, and the difference was statistically significant (P < 0. 05). The mortality and complications, postoperative hospital stay were 10.0%(2/20), 45.0% (9/20) and (20 ±2)d in type Ⅰ procedure, while they were 5.6% (1/18),38.9% (7/18) and(23 ±2)d in type Ⅱ procedure, and the difference was not statistically significant.Conclusions There was no significant difference in the effects between type Ⅰ and type Ⅱ bundled pancreaticojejunostomy. Carefully selective application of type Ⅰ and type Ⅱ bundled pancreaticojejunostomy helps complete these procedures.
