Objective To investigate the effect of somatostatin on the electrophysiological changes of early diabetic rats, and to clarify its therapeutic effects on retinopathy in early diabetic rats. Methods 20 Wistar male rats, which were induced into diabetic rat models by intraperitoneal injection of streptozotocin (STZ),were randomly divided into somatostatin therapy group and model control group, 10 in each group. The rats in somatostatin therapy group were injected with somatostatin 10μg·kg-1 ·d-1,for 8 weeks;the rats in model control group received the same volume of saline.Another 10 male Wistar rats were used as normal control group. The changes of blood glucose and electroretinogram were measured, and the indicators were used for statistical analysis. Results Compared with model control group, the blood glucose was declined in somatostatin therapy group at the 8th week (P<0.05).Compared with normal control group, the latent periods of a-wave,b-wave of rod-response and max-response were significantly extended in somatostatin therapy group and model control group at the 1 st, 2nd,4th,6th,and 8th week (P<0.05).The latent periods of a-wave,b-wave of rod-response and max-response in somatostatin therapy group were shorter than those in model control group at the 8th week (P<0.05 ). Compared with model control group, the amplitude had no changes at each observation time point in somatostatin therapy group (P>0.05).Conclusion Somatostatin has a therapeutic effect on early diabetic retinopathy in rats.

ObjectiveTo investigate the changes and relevance of H reflex and spasticity after cervical or thoracic spinal cord injury (SCI).MethodsH reflex of 13 healthy adults as controls and 30 SCI patients (subacute, n=18; chronic, n=12) was measured twice from soleus muscle once a month. In the same time, the evaluation of spasm was performed by the modified Ashworth scale.ResultsThere was no significant difference in the mean Hmax amplitude and Hmax/Mmax ratio between different SCI stages(P>0.05). However, the mean Hmax amplitude of the patients was smaller than that of controls(P<0.01). During subacute stage there was quadratic correlation between the mean Hmax amplitude and spasticity, Hmax/Mmax ratio and spasticity. But there was linear correlation between Hmax/Mmax ratio and spasticity during chronic stage.ConclusionThere was closer relation between spasticity and Hmax/Mmax ratio after spinal cord injury. However, the quantitative evaluation need to be proved by larger samples.

Objective:To explore the influencing factors of turnover intention of pre-hospital emergency nurses in Beijing, and provide references for reducing the turnover rate of pre-hospital emergency nurses and stabilizing the pre-hospital emergency nursing team.Methods:Pre-hospital emergency nurses from 2 emergency centers in Beijing were selected as the survey objects, and a self-designed questionnaire was used to conduct an online survey in September 2019. The questionnaire covered such aspects as the main demographic characteristics, workload, doctor-patient relationship, professional identity, job burnout and turnover intention. Descriptive analysis was conducted for the data, while rank-sum test and Kappa consistency test were used for univariate analysis, and ordered logistic regression analysis was used for multivariate analysis. Results:A total of 340 valid questionnaires were received, among which 41.5% (141) of the nurses said that they occasionally considered quitting, 11.7% (40) said they often considered quitting, while the scoring of professional identity was (33.29±6.00), and that of job burnout was (63.70±14.90). Univariate analysis showed that age, work units, self-rated health status, professional identity, job burnout, seniority, workload, doctor-patient relationship, and pressure of title promotion were significant ( P<0.05). The results of multivariate analysis showed that job burnout, average number of car trips per shift, and self-rated health status were positively correlated with turnover intention, while professional identity was negatively correlated with turnover intention ( P<0.05). Conclusions:The turnover intention of pre-hospital emergency nurses in Beijing was found at a high level. The authorities are recommended to rationalize the scheduling system and increase the staffing of pre-hospital emergency nurses; establish the diversion policy, title promotion system and post risk special allowance for such nurses; and train medical aid workers to undertake the transfer and lifting of patients, so as to reduce the turnover intention of these nurses.

Objective:To investigate the feasibility and clinical value of endovascular treatment of iliac vein disease via the great saphenous vein approach.Methods:Eighty-six patients with nonthrombotic left iliac vein compression lesions identified by anterograde lower limb vein angiography were divided into 2 groups: group A ( n=46) was treated via great saphenous vein and group B ( n=40) was treated via femoral vein. The success rate of puncture, time consuming of puncture catheterization, postoperative bed immobilization and complication rate of puncture were observed and compared between the two groups. Results:The puncture success rate was 97.8% (45/46) in group A and 100% (40/40) in group B, there was no significant difference between the two groups( P>0.05). The average puncture time was (9.4±2.7) min in group A and (5.5±1.3) min in group B ( P<0.05). The complication rate of group A was lower than that in group B ( P<0.05). The patency rate of iliac vein stent was 100%, as indicated by venous color ultrasound or angiography. Conclusion:Endovascular treatment of iliac vein disease via great saphenous vein approach is a safe and feasible method with less trauma and easier postoperative care.

Objective:To evaluate the technical aspects and examine patency rates of stent placement across the inguinal ligament for managing long-segment iliac venous obstruction. Methods:From Jan 2012 to Jan 2017, a total of 73 patients (42 cases of post-acute deep venous thrombosis(DVT) thrombolysis and 31 of post-thrombotic syndrome (PTS)) with long-segment iliac venous obstruction were treated with interventional surgery. All patients underwent balloon dilatation and placement of at least two stents extending below inguinal ligament. Results:The technical success rate was 100%. No serious perioperative complications occurred. The mean duration of the procedure in the DVT group was shorter than PTS group(35±12) min vs. (62±12) min, P<0.05). The mean number of stents did not differ between the two groups.Mean follow up periods was 28.34 months. Overall cumulative primary, assisted primary, and secondary stent patency rates were 100%, 100%, and 100% at 6 months and 87.7%, 95.9%, and 100% at 12 months and 75.8%, 85.1%, and 96.3% at 24 months, respectively(all P>0.05). Edema and pain alleviated significantly in the two groups. There was no stents compressed or fractured. Conclusions:Stenting across the inguinal ligament for treatment of long-segment iliac venous obstruction is a safe, effective, and feasible method.
