1.Significance of Ranawat''s triangle method in determining the rotation center of normal hip joint

BACKGROUND:It is crucial to determine the location of the rotation center of the hip joint using X-ray.Ranawat's triangle is commonly used to define the hip rotational center in bilateral developmental dysplasia of the hip and revision hip surgery especialy in cases with bone stock deficiency,but there are rare articles about its value in normal hip.OBJECTIVE:To compare the difference between Ranawat's triangle method and Mose circle method in determining the healthy hip joint rotational center.METHODS: We selected 30 patients with normal traumatic femoral neck fracture,who underwent unilateral total hip arthroplasty.Ranawat's triangle method and Mose circle method were applied to measure the anteroposterior radiographs of the pelvis after arthroplasty and to compare the distance between the hip rotational center and the reference lines (X-axis: teardrop line; Y-axis: a line perpendicular to the X-axis,drawn from the intersection of theKohler line and the teardrop line.The distance from the hip joint rotational center to X-axis is dx1,dx2 and the distance to Y-axis is dy1,dy2 in Ranawat's triangle and Mose circle respectively.).We also compared the ratios (dx1/H,dy1/W; dx2/H,dy2/W) in the pelvic height (H: the distance between the highest point on the iliac wing and the lowest point on the ischial tuberosity) and width (W: the distance between the most lateral points on the iliac crests).RESULTS AND CONCLUSION:When the values were compared with both methods,highly significant differences were observed for both X and Y,Ranawat's triangle method dx1=(19.52±3.03) mm,dy1=(24.43±2.26) mm; and Mose circle method dx2=(11.90±3.55) mm,dy2=(34.29±3.79) mm (P<0.001) and these ratios (dx1/H=0.0992±0.0133,dy1/W=0.0855±0.0069; dx2/H=0.0611±0.0194,dy2/W=0.1201±0.0178) in pelvic height and width (P< 0.001).Scatterplot results demonstrated that the hip rotational center identified by Ranawat's triangle method is near to the proximal end and medial end,which may affect mechanical environment surrounding the joint.For cases of bilateral hip dysplasia and joint revision,because of the changes in structure surrounding the acetabulum,Ranawat's triangle method can be a feasible method to identify the hip rotational center.For cases of unilateral lesion,Mose circle method can exactly identify the hip rotational center taking healthy side as a control.
2.Vacuum-compression therapy for ischemic disease of the extremities: experience with 40 patients

Objective To study the clinical effects of vacuum-compression therapy for ischemic disease of the extremities. Methods A total of 40 cases of peripheral arterial disorders, including 23 thromboangiitis obliterans (31 limbs) and 17 arteriosclerosis obliterans (23 limit), were treated by a self-made vacuum-compression therapeutic apparatus. Results The effective rate in thromboangiltis obliterans and arteriosclerosis obliterans groups was 96.77% and 92.23%, respectively. The cuffs on the apparatus were improved to eliminate discomfort in the patients during treatment. "Rebound symptom" was observed during treatment, which had not been reported previously. Conclusion Vacuum-compression therapy has a good prospect for treating ischemia of the extremities.
3.Molecular regulation of skeletal satellite cell's self-renewal.

Skeletal muscle possesses a remarkable ability for its regeneration and injured tissue repair. This ability depends on the activity and contributions of muscle satellite cells. Proliferating satellite cells, termed myogenic precursor cells or myoblasts, are activated and driven out of their quiescent state upon muscle injury. In this summary, we present a review to summarize the molecular regulation in skeletal satellite cells to light on the satellite cells' self-renewal mechanism.

4.Study on relationship between expression of survivin, galectin-3 and TPO and BRAFV600E mutation in patients with papillary thyroid microcarcinoma and its clinical significance

Objective:To detect the expressions of survivin, galectin-3 and thyroid peroxidase in papillary thyroid microcarcinoma ( PTMC) and cancer adjacent normal tissue to explore the clinical significant.The correlations between the expressions of survivin,Gal-3 and TPO with BRAFV600E gene mutation in PTMC were analyzed.Methods: The expressions of survivin,Gal-3 and TPO were measured by immunohistochemical ( IHC ) method in 56 cases of PTMC tissue and adjacent normal tissue; BRAFV600E mutation was detected by PCR amplification and subsequently sequencing.Chi square test was used to analyse the relation in the expression rates of survivin,Gal-3 and TPO and BRAFV600E gene mutation.Results:The survivin and Gal-3 were strongly expressed but TPO was negatively expressed.The survivin and Gal-3 were negative in adjacent normal tissues but TPO was expressed.There were sig-nificant differences in the expression rates of survivin, Gal-3 and TPO between PTMC tissue and adjacent normal tissue (all P<0.001).The BRAFV600E mutation rate (32.1%) and the expression rates of survivin,Gal-3 and TPO in PTMC tissue were found to be positively related to lymph node metastases (P=0.009,P=0.025,P=0.007,P=0.008),and negatively related to gender and age (all P>0.05).There were no correlation was found between the expressions of survivin,Gal-3,TPO and the BRAFV600E gene mutation in PTMC(all P>0.05).Conclusion: The strong expressions of survivin and Gal-3,the mild expression of TPO and BRAF mutation may be important in the development of PTMC,and the detection of BRAFV600E gene mutation and the expressions of survivin, Gal-3 and TPO could be used to the judgment of pathogenetic condition and prognostic outcomes.
5.The value of CMV viral load test in diagnosis of cytomegalovirus infection in infants

Objective To assess the value of CMV viral load test in the diagnosis and prognostic judgment of infantile cytomegalovirus infection with whole blood and urine specimens. Methods 50 infants with active CMV infection were selected, which pp65 antigen was positive in serological detection and either CMV-IgM positive or the titer of CMV-IgG ≥40. The viral load in whole blood and urine specimens was detected before and after a course of preemptive ganciclovir treatment and the pp65 antigen was determined after treatment. Results 98% patients were manifested as cytomegalovirus viral load quantitative measurement in whole blood positive before the treatment, while 14% after. The positive ratio of pp65 assay after therapy was 6%. And there was no significant difference between the results of the two kinds of detection methods measurement in urine before and after treatment was 98% and 90%, respectively. The results of urinary viral load quantitative detection did not coincide with clinical characteristics of CMV infection (P<0.05,Sp=0.11,PVP=0.55, (Youden's index)=0.09). Conclusions Good coincidence could be found between CMV-DNA quantitative measurement in whole blood and pp65 antigenemia assay. And the former could be used as a diagnostic index of CMV positive infection. While single urinary viral load quantitative detection had no significance for the distinction between active and latent CMV infection and dynamic monitoring of CMV treatment.
6.Association between arterial compliance and fundus oculi arteriosclerosis in elderly people

Objective To investigate the association between arterial compliance and fundus oculi arteriosclerosis in elderly people. Methods A total of 73 elderly subjects,including 61 elderly patients with fundus oculi arteriosclerosis and 12 healthy volunteers were recruited into the study.Capacitive arterial compliance(C1)and oscillatory arterial compliance(C2)were measured by DO2020 Cardiovascular Profiling Instrument.Moreover,fundus photographs were measured using Canon figure fundus camera. Results There were significant differences in systolic blood pressure(SBP),diastolic blood pressure(DBP),mean artery pressure,pulse pressure,capacitive arterial compliance (C1),and oscillatory arterial compliance(C2)between patients with fundus oculi arteriosclerosis and healthy volunteers(all P<0.05).In multiple regression analysis,the grade of fundus oculi arteriosclerosis was significantly correlated with SBP,DBP,mean artery pressure and pulse pressure (r=0.357,0.261,0.342,0.280,all P<0.05).Moreover,there were significant correlations between the grade of fundus oculi arteriosclerosis and Cl(r=-0.281,P<:0.05),and C2(r=-0.308,P<0.01). Conclusions These findings underscore the efficacy of C1 and C2 in identifying patients with fundus oculi arteriosclerosis.
7.Telmisartan decreases vascular endothelial growth factor and its receptor expressions in the kidney of insulin resistant rats with normal blood glucose

The results of RT-PCR and immunohistochemistry showed that the expressions of vascular endothelial growth factor (VEGF) and its receptor ( flk-1 ) in the renal cortex of insulin-resistant rats during the phase of normal blood glucose were significantly increased, which were decreased by telmisartan. The result suggests that telmisartan may ease kidney damage via decreasing VEGF and flk-1 expressions.

9.Preparation of polyvinyl alcohol/lota-carrageenan scaffolds and its biocompatibility

BACKGROUND:Polyvinyl alcohol (PVA) hydrogel with similar porous structure and mechanical properties to the natural cartilage is very suitable for the repair of articular cartilage. However, the pure PVA hydrogel after lyophilization wil be accompanied by the shrinkage of the polymer network and the col apse of the pores, leading to the inhomogeneous performance of the material even in the state of re-swel ing. Addition of the active polymer wil increase the cel adhesion ability of PVA hydrogel. OBJECTIVE:To construct PVA/lota-carrageenan (l-CA) composite materials with different mass fractions of l-CA and evaluate the biocompatibility with vascular endothelial cel s. METHODS:PVA/l-CA composite films with different contents of l-CA were fabricated and then co-cultured with vascular endothelial cel s. Attachment, proliferation and morphological changes of vascular endothelial cel s on the composite were observed by scanning electron microscope and MTT assay to evaluate its biocompatibility. PVA/l-CA three-dimensional scaffold with different contents of l-CA were constructed, and hemolysis experiment was conducted according to the biological evaluation standards of medical devices, and the porosity and pore size were observed using scanning electron microscope. RESULTS AND CONCLUSION:In vitro experimental results showed that the addition of l-CA could significantly increase the biological activity of PVA hydrogel, and promote the cel attachment and proliferation on the scaffold. The hemolysis rate of each experimental group was less than 5%(the accepted safety standard), suggesting that the composite materials were in accordance with the standard of medical devices for hemolysis experiment. These findings indicate that the composite scaffolds with 20%-30%l-CA possess the pore size suitable for cel growth and proliferation and the porosity beneficial for transportation of nutrients and metabolites, which can serve as an excel ent scaffold for tissue engineering.
10.Immediate Ureterovesical Reimplantation After Ureteral Rupture During Laparoscopic Hysterectomy

Objective To investigate the feasibility and clinical effect of immediate ureterovesical reimplantation after ureteral rupture during laparoscopic hysterectomy. Methods From August 2010 to December 2015, 5 cases of ureteral rupture during the operation of laparoscopic hysterectomy were treated with immediate ureterovesical reimplantation under laparoscopy. Results All operations were successfully performed without transversion to open surgery. No patients with urinary leakage occurred. The mean follow-up were 21 months (range 3-60 months) . No cases with ureteral stricture were observed. Slight urine reflux was found in two patients, of whom obvious hydronephrosis and renal damage were not found. Conclusion Immediate ureterovesical reimplantation under laparoscopy is a feasible, safe and minimal invasive method for treatment of ureteral ruputure during laparoscopic hysterectomy.
