2.The predictive factors of fever after percutaneous lithotripsy

Objective To analyze the predictive factors of fever after percutaneous renal stone surgery,and to provide reference for clinical treatment.Methods A total of 147 patients underwent percutaneous nephrolithotomy in after operation was chosen in the Department of Urology in our hospital from January 2014 to January 2016.According to the existence of fever,patients were divided into fever (n =25,heating rate 17.0%) and control (n =122) groups.Preoperative information were collected,including age,gender,preoperative serum creatinine,stone size and shape,the involvement of calyceal number,water,urine culture results,operative time,blood loss,intraoperative perfusion volume,pyonephrosis,puncture channel length,hospitalization time and other information including intraoperative,postoperative information including fever,and postoperative renal fistula complications if there is information.SPSS 18.0 was used for statistical analysis.Results The fever group stone surface area,CT value affected calyx number,stone shape,stone properties,the involvement of calyceal number,degree of hydronephrosis,operative time,intraoperative blood loss,intraoperative perfusion,hospitalization time,and renal fistula complication rates were higher than the control group (P < 0.05).There were no significant differences between two groups (P >0.05).The results of Logistic regression analysis found that the stone surface area (OR =5.19),stone,stone shape (OR =7.86) properties (OR =3.87),operation time (OR =5.68),intraoperative perfusion (OR =5.24),and renal fistula complications (OR =2.65) for the influence factors of fever.Conclusions The stone surface area is large,stone nature infection stones,stone shape for staghorn calculi,longer operation time,and intraoperative perfusion of large renal fistula complications were more prone to postoperative fever in postoperative.
5.The function of bushen qiangdu recipe containing serum in OPG/RANKL pathway of ankylosing spondylitis patients.

OBJECTIVETo study the mechanism of Bushen Qiangdu Recipe (BQR) for regulating osteoprotegerin receptor activator for nuclear factor kappa B ligand (OPG/RANKL) pathway in ankylosing spondylitis (AS).
METHODSThirty active AS inpatients or outpatients were recruited from Department of CM Rheumatology, China-Japan Friendship Hospital from January to May 2009. All patients were treated with BQR for 3 successive months, one dose daily, once in the morning and once in the evening. Besides, 30 healthy volunteers were recruited. The serum of patients and volunteers were collected. The osteoblast cell lines hFOB1. 19 were divided into 3 groups: the pre-treatment group, the post-treatment group, and the healthy volunteer group (as the control group). All cell lines were cultured by corresponding culture medium containing each serum. The supernatant from osteoblast cell lines was collected. The protein content of OPG/RANKL was detected using ELISA, and the protein expression of OPG/RANKL was detected using RT-PCR.
RESULTSCompared with the control group, the OPG content, the mRNA and protein expressions of OPG, and the mRNA and protein expressions of OPG/RANKL all decreased, while the mRNA expression of RANKL increased in the pre-treatment group, showing statistical difference (P<0.05, P<0.01). Compared with the pre-treatment group, the OPG content, the mRNA and protein expressions of OPG significantly increased, and the mRNA and protein expressions of OPG/RANKL increased, while the mRNA expression of RANKL decreased in the post-treatment group, showing statistical difference (P<0.05, P<0.01).
CONCLUSIONSAS patients' serum could directly inhibit the expression of OPG in osteoblasts, promote the expression of RANKL, and down-regulate the OPG/RANKL ratio. BQR containing serum might promote the osteogenesis and inhibit the bone resorption possibly through directly up-regulating the OPG/RANKL ratio in osteoblast, thus inhibiting the differentiation and function of osteoclast.

6.Effect of selenium on apoptosis of spleen ceils, T-cell subset of fluorosis chicken

Objective To study the effect of selenium on peripheral and splentic T-cell subset, apoptosis of spleen cells in fluorosis chicken and its mechanism. Methods One hundred and eighty 8-day Hailanhe chicks were randomly divided into 3 groups(each 60): ①control group: 195 mg/kg fluoride and 0.08 mg/kg of selenium; ②fluorine group : 1000 mg/kg fluoride and 0.08 mg/kg of selenium ;③selenium antagonism group : 1000 mg/kg On 30~(th), 60~(th), 90~(th) day, peripheral and splentic CD4~+, CD8~+ T-cell subset analyses underwent flow cytometry and apoptosis of spleen cells were detected by TUNEL for study subjects. Results Compared with control group, the CD4~+ T-cell subset of peripheral in fluorine group was decreased obviously in 30,60,90 days[ (35.36± 4.27)% vs (24.29 ± 2.96)%, (47.65 ± 5.42)% vs (41.62 ± 3.96)%, (49.58 ± 3.98) % vs (42.35 ± 6.03 )%, P < 0.05 or < 0.01 ], CD4~+/CD8~+ ratio also was decreased obviously [ ( 1.701 ± 0.145 )% vs (1.393 ± 0.163)%,(2.712 ± 0.345)% vs (1.781 ± 0.201)%,(2.438 ± 0.356)% vs (1.973 ± 0.229)%, P< 0.05 or < 0.01]. Compared with fluorine group, the CD4~+ T-cell subset of peripheral in selenium antagonism group [ (29.40 ± 3.38)%, (45.40 ± 6.01 )%, (46.85 ± 5.25)%, P < 0.05 or < 0.01 ] was increased obviously in 30,60,90 days,CD4~+/CD8~+ ratio in 60,90 days[(2.004 ±0.314)%,(2.211±0.229)%,all P<0.01]also was increased obviously.Compared with control group,the CD4~+ T-cell subset of spleen cells in fluorine group was decreased obviously in 30,60,90 days[(47.33±5.35)% vs(41.91±4.83)%,(49.28±5.24)% vs(41.26 ±4.56)%,(34.31±4.15)%vs(29.33±2.89)%,all P<0.01],CD4~+/CD8~+ ratio also was decreased obviously[(1.927 ±0.244)% vs(1.525 ±0.265)%,(1.847±0.224)% vs(1.640±0.198)%.(1.265±0.174)% vs(0.878±0.092)%,P<0.05 or<0.01].Compared with fluorine group,the CD4~+ T-cell subset of spleen cells in selenium antagonism group in 60,90 days[(44.87±5.43)%,(32.62±3.37)%,all P<0.05]was increased obviously,CD4~+/CD8~+ ratio in 30,60, 90 days[(1.703 ±0.201)%,(1.772±0.215)%,(0.991±0.124)%,P<0.05 or<0.01]also was increased obviously. The apoptosis ratio of spleen cells in fluorine group in 30,60,90 days[(2.31±0.36)%,(2.76±0.22)%,(3.04± 0.29)%]was higher than that in control group[(1.14±0.21)%,(1.23±0.23)%,(1.29±0.20)%,P<0.01].The apoptosis ratio of spleen cells in selenium antagonism group in 60,90 days[(2.42 ±0.32)%,(2.73±0.39)%]was lower than that in fluorine group(P<0.05 or<0.01).Conclusion A certain concentration of selenium can antagonize the immunity inhibition of fluorine by decreasing apoptosis and improving the unbalance of T-cell subset.
7.Causes analysis of early postoperative high intraocular pressure of angle - closure glaucoma underwent compound trabeculectomy

To analyze the causes and treatments of high intraocular pressure ( > 21mmHg ) of angle - closure glaucoma underwent compound trabeculectomy 1mo after surgery.
●METHODS: This was a retrospective study of our hospital, from March 2010 to March 2013. Thirty-four (38 eyes) of angle-closure glaucoma patients were collected, who underwent compound trabeculectomy with high intraocular pressure ( > 21mmHg) 1mo after operation. We analyzed the causes and summarized the treatments.
●RESULTS:The causes which lead to early postoperative high intraocular pressure included malignant glaucoma (9 eyes of 8 cases), blood clot and connective tissue block under the scleral flap (15 eyes of 13 cases), hyphema after surgery(5 eyes of 5 cases), sclera flap incision was incarcerated with iris tissue ( 3 eyes of 3 cases ), preoperative high intraocular pressure for a long time(5 eyes of 4 cases), 1 eye of 1 case for unknown reason. After proper treatments, intraocular pressures of all patients were bellowed 21mmHg.
● CONCLUSlON: Early postoperative high intraocular pressure of angle - closure glaucoma underwent compound trabeculectomy is caused by various factors, and the early prevention and timely treatment are key points of a successful operation.
8.3′-terminus shifted bases degeneracy primers increasing sensitivity of polymerase chain reaction

To reduce the risk of 3′-terminal mismatch between primers and template and increase the sensitivity of polymerase chain reaction (PCR) in the detection of variable region of DNA. Methods: A pair of special primer(WU,WD) was designed to amplify a fragment of HBV DNA P gene by PCR. Other 2 similar pairs of primer (MU1, MD1, MU2, MD2) were obtained by knocking off 1 or 2 bases at the 3′-terminal of WU and WD. (1) Special primers (WU, WD) and degeneracy primers(WU, WD, MU1, MU2, MD1, MD2) were used to amplify 27 samples respectively by PCR under the same condition. The sensitivity of each PCR was compared. (2) Using degeneracy primers, serum HBV DNA was amplified from 4 patients who were resistant to lamivudine. The PCR products were sequenced to evaluate the effect of the 3′-terminal mismatch of primers upon PCR. Results: (1) The sensitivity of special primers and degeneracy primers were 70.4%(19/27) and 85.2%(23/27) respectively (P<0.05). (2) The sequencing analysis of the PCR products suggested that the 3′-terminal mismatch of primers caused false negative in the PCR detection. Conclusion: When amplifying the variable region of DNA, the false negative result can be avoided by using 3′-terminus shifted degeneracy primers.
9.Study on pharmacological ingredients of wuzhuyu tang treating migraine by correlating absorption ingredients in everted intestinal sac and pharmacodynamics.

Wuzhuyu Tang is a classical formula for treating migraine, but its' pharmacological ingredients is unclear yet. Present study employed the everted intestinal sac model to collect the absorption samples of 10 kinds of Wuzhuyu decoction, and then analyzed the contents of 9 ingredients in Wuzhuyu Tang and absorption samples quantitatively or semi-quantitatively by HPLC-DAD method. Reserpine was used to establish the mice model of migraine, and then the contents and activities of 5-hydroxytryptamine, noradrenaline, dopamine, nitric oxide and nitricoxide synthase in brain tissues and serums were determined respectively after oral administration of Wuzhuyu Tang. Using the partial least squares regression method to correlate the total absorption quantity of 9 ingredients and pharmacodynamics. The result shows that limocitrin-3-O-beta-D-glucoside, ginsenoside Rg1 and Rb1, rutaevine, limonin, evodiamine and rutaecarpine are the main ingredients influenced the effects in absorption samples in everted intestinal sacs, especially ginsenoside Rg1, rutaevine, evodiamine and rutaecarpine among them have obvious improving effects to most pharmacodynamics index, might be the pharmacological ingredients influenced the therapeutical effects of Wuzhuyu Tang treating migraine.

10.Safety and immunogenicity of combined hepatitis A and hepatitis B vaccine according to 0 and 6 months schedule in healthy children.

OBJECTIVETo evaluate the safety and immunogenicity of the Bilive(TM) combined hepatitis A and hepatitis B vaccine in healthy children.
METHODSA total of 116 healthy children aged 1 - 10 years, who, without history of hepatitis A vaccine vaccination and anti-HAV negative, had completed the full immunization of hepatitis B vaccine were recruited in city of Changzhou in Jiangsu province. The Bilive(TM) combined hepatitis A and hepatitis B vaccine was administered according to a two-dose schedule (0, 6 months). The dosage was 250 U for hepatitis A antigen and 5 microg for hepatitis B surface antigen. The potential adverse effects were observed within 72 hours after vaccination. The serum samples were collected for the testing of anti-HAV and anti-HBs at month 1, 6 and 7 after initial dose.
RESULTSThe local and systemic adverse reactions after immunization were slight and temporary. The rates of local and systemic adverse reactions were 12.1% (14/116) and 6.0% (7/116). The sero-conversion rates of HAV were from 92.9% (92/99) to 100.0% (101/101) and the geometric mean titers (GMT) ranged from 47.0 mIU/ml to 2762.3 mIU/ml 1, 6, 7 months after initial dose. The sero-protection rate of HBV was 86.1% (87/101) before vaccination and came up to 100.0% (101/101) one month after initial dose, and the GMTs of HBV were from 894.3 mIU/ml to 3314.3 mIU/ml 1, 6, 7 months after initial dose.
CONCLUSIONThe Bilive(TM) combined hepatitis A and hepatitis B vaccine has good safety and immunogenicity in healthy children who had preexisting immunity to hepatitis B virus.
