4.The Tolerability of Chinese Melanoma Patients to High-dose Interferon Adjuvant Therapy

Objective: To observe the tolerability of Chinese melanoma patients to four-week high-dose interferon alfa-2b(INTRON A(R),Schering-Plough)therapy. Methods:A total of 29 patients with high risk melanoma[American Joint Committee on Cancer Staging(AJCC)ⅡB-ⅢC]who received adjuvant interferon therapy in our hospital between September 2007 and May 2009 were retrospectively reviewed.Patients received 4 hours of intravenous infusion of interferon alfa-2b fdose range,22.00 million international unit(MIU)to 33.75 MIU]Ⅳ 5 days/week for 4 weeks.The adverse events were evaluated with National Cancer Institute Common Toxicity Criteria(NCI 2.0 version). Results: The average daily dose was 17.63 MIU/(m~2·d).The therapy was ended in two patients because of poor wound healing or intolerability to severe fatigue.The most common adverse events were myelosuppression.Grade 3/4 neutropenia was observed in 69% (20/29)patients and was rapidly reversed after conventional support interventions.Grade 1/2 abnormal hepatic function occurred in 18 cases(62%).Twenty-six patients were followed up for 3 to 22 months.Five patients developed early progression:one with local recurrence,two with regional lymph node metastasis one with in-transit metastasis in the affected limb,and one with distant metastasis. Conclusion: High-dose interferon alfa-2b regimen can be well tolerated by Chinese patients but cannot effectively inhibit subclinical lesions.
5.Effect of danpu capsule on inflammatory factors in rat model of autoimmune prostatitis.

OBJECTIVETo explore the acting mechanism of Danpu Capsule (DPC) in treating chronic prostatitis by observing its effect on inflammatory factors in autoimmune prostatitis rat model.
METHODSThe rat model was established by abdominal subcutaneous multiple points injection of rat's prostate tissue antigen. Thirty modeled rats were randamly divided into 3 groups, the DPC group, the Qianlietai Tablet (QLT) group and the model group. They were treated via gastrogavage respectively with DPC, Qianlietai Tablet and normal saline respectively. Besides, a control group was set up with 10 healthy rats. All animals were sacrificed 56 days after treatment. Pathologic change of prostatic tissue was observed by light microscopy, and the levels of interleukin 8 (IL-8), interleukin 10 (IL-10), tumor necrosis factor alpha (TNF-alpha) and prostaglandin E2 (PGE2) in blood serum and prostatic tissue were detected by enzyme linked immunosorbent assay (ELISA).
RESULTSCompared with the normal group, the serum and prostatic levels of IL-8, IL-10 and TNF-alpha, as well as the prostatic level of PGE2 in the model group were higher (P <0.05 or P <0.01). In the DPC group, the serum and prostatic levels of IL-8 was 3.07 +/- 0.61 ng/L and 7.32 +/- 2.44 ng/L respectively, which was lower than those in the model group (4.73 +/- 1.95 ng/L and 10.14 +/- 3.64 ng/L, respectively); that of TNF-alpha in the DPC group (85.34 +/- 19.20 ng/L and 87.01 +/- 15.4 ng/L) was also lower in the model group (111.48 +/- 31.57 ng/L and 119.88 +/- 14.13 ng/L); similar difference between the two groups was also shown in prostatic level of IL-10 (34.05 +/- 7.56 ng/L vs 47.20 +/- 15.97 ng/L), and so was PGE2 (603.97 +/- 114.62 ng/L vs 712.58 +/- 117.10 ng/L), all with statistical significance (P <0.05 or P <0. 01).
CONCLUSIONDPC could reduce the prostatic inflammatory response of model rats, and regulate the local immune condition.

7.The study on retention of gastroscopy skills after simulator training

ObjectiveTo investigate whether the eight- year program students retain the skills from the endoscopy simulator gastroscopy training.Methods4 trainees accepted virtual reality simulator gastroscopy training and performed a standardized VR gastroscopy scenario at the end of training,and after a median 12 months without practice ( retention ).The intensified training was done by trainees based on the differences between the training end and the retention for a median 12 months and the number of intensified training times was found.ResultsThe significant differences existed in the overinsufflation and opeirational force and time.The score at the training end was better than after retention.Through the average 5.5 times intensified trainings the original levels could be reached.ConclusionThrough Endoscopy Simulator the key skills could be retained well and through a litde training the original levels could also be reached.

Objective To investigate the effect of valsartan (angiotensinⅡtypeⅠreceptor antagonists) on neointimal proliferation and expression of CD34 after angioplasty in rabbits.Method Twenty-four male New Zealand White rabbits were randomly divided into three groups:the control group,fed up with common diet;the model group and the valsartan group,fed up with hypercholesterolemic diet for 4 weeks,f then and ballon angioplasty.At 4 weeks after operation,the model group was fed up with common diet,whereas the valsartan group was fed up with the admixture of valsartan 10 mg?kg~(-1)?d~(-1) and common diet.All the rabbits were killed at the end of the 12th weeks.The abdominal aorta was performed with pathologic and morphologic analysis,and expression of CD34 in endothelial cells was analyzed with immunohistochemical method.Results Compared with the model group,the neointimal thickness and area of the valsartan group decreased by 56.58%and 66.81%, respectively.The expression of CD34 of the valsartan group was significantly higher (P
9.Clinical Study on Qingfei Sanjie Pills Combined with Targeted Cryoablation Therapy for Treatment of Elderly Patients with Advanced Non-Small Cell Lung Cancer

Objective To study the clinical efficacy of Qingfei Sanjie Pills combined with targeted cryoablation therapy on elderly patients with advanced non-small cell lung cancer (NSCLC) and its influence on the levels of immune function in patients. Methods Totally 94 elderly patients with NSCLC were divided into observation group and control group, with 47 cases in each group. Both groups were given targeted cryoablation therapy and under CT review after surgery to observe lesions condition. Observation group was given Qingfei Sanjie Pills, once a bottle, twice a day, orally, for three months. Recent efficacy of the two groups were evaluated. The levels of IgG, IgA, CD3+, CD4+, CD8+, and complications in the two groups were detected. The survival rate of 1 year, 2 years and 3 years in the two groups were observed. Results Two cases in the observation group and three cases in the control group were invalid. The total effective rate was 84.4% (38/45) in observation group and 65.9% (29/44) in the control group, with statistical significance (P<0.05). After the treatment, the levels of IgG, IgA, CD3+, CD4+and CD8+in the observation group were significantly higher than the control group (P<0.05). After treatment, the incidence rates of increased cough, postoperative pain, bloody sputum, fever and nausea and vomiting in the observation group were lower than the control group. The 1 year, 2 years and 3 years survival rates were 91.1%, 75.6% and 64.4% in observation group, and 79.5%,63.6% and 52.3% in the control group,with statistical significance(P<0.05).Conclusion Qingfei Sanjie Pills combined with targeted cryoablation therapy on elderly patients with advanced NSCLC has good clinical efficacy, which can improve immune system of patients and increase survival rate.
10.Serum levels of endothelin-1, tumor necrosis factor-α and its clinical significance in patients with delayed encephalopathy after acute carbon monoxide poisoning

Objective To investigate the serum levels of endothelin-1 (ET-1), tumor necrosis factor- α (TNF-α) and their dynamic changes in patients with delayed encephalopathy after acute carbon monoxide poisoning (DEACMP) and their relation with the condition variation of the patients.Methods The serum ET-1 and TNF-α levels were determined with enzyme-linked immunno-sorbent assay (ELISA) in 31 patients with DEACMP, and the dynamic changes of patients' condition were monitored by use of the activity of daily living (ADL) scale, the information-memory-concentration test (IMCT) and the Hasegawa's dementia scale (HDS). The comparisons between patients with DEACMP and both 30 patients with acute carbon momoxide poisoning (ACMP) but without DEACMP and 30 normal controls were also conducted. Results At the acute stage of the DEACMP group, the serum levels of ET-1 and TNF-α were both significantly higher than those in the normal control group (P<0.05); that of TNF-α was significantly lower than that in the ACMP group (P<0.05), but that of ET-1 was not significantly different from that in the ACMP group (P>0.05). The serum levels of ET-1 and TNF- α in the ACMP group were both significantly higher than those in the normal control group(P<0.05). In the DEACMP group, the serum level of ET-1 at the convalescent stage was significantly lower than that at the acute stage (P<0.05), but the serum level of TNF-α was not significantly different from that at the acute stage (P>0.05). At the acute stage of the DEACMP group, ADL scores were significantly higher than those in norms, and IMCT scores and HDS scores were significantly lower than those in norms (P<0.05). In the DEACMP group, the ADL scores at the convalescent stage were significantly lower than those at the acute stage (P<0.05), IMCT scores and HDS scores were significantly higher than those at the acute stage (P<0.05). Significant correlations between scores of any 2 of 3 scales in patients with DEACMP at both acute and convalescent stage were noted (P<0.05). Conclusion The dynamic detection of serum ET-1 and TNF-α level variations could be used as an indicator for condition severity in patients with DEACMP.
