1.Quantification of sitagliptin in human plasma and urine by LC-MS/MS method and its application.

A rapid and sensitive liquid chromatography-mass spectrometry/mass spectrometry (LC-MS/MS) method for quantification of sitagliptin in human plasma and urine had been developed. This method was applied to the pharmacokinetics study of sitagliptin tablet after single- and multiple-dosing in Chinese population. Plasma samples were prepared by a liquid-liquid extracted method, and urine samples were diluted. Compounds were analyzed by multiple reaction monitoring (MRM) mode with a electrospray ionization (ESI) interface. Mobile phase consisted of methanol and water (85 : 15, v/v). The linear concentration range of calibration curve was 0.5-1 000 ng.mL-1. and 0.2-100 µg.mL , intra-run/between-run accuracy was 98.98%-103.69% and 97.63%-102.29%, intra-run/between-run precision was <5.51% and 4.26% for plasma and urine sample, respectively. The stability of sitagliptin stock solution was tested for 55 days at -30 °C. Sitagliptin was stable when stored under the following conditions: 24 hours in the autosampler after sample preparation; 24 hours at room temperature, after 3 freeze and thaw cycles (from -30 °C to room temperature), 40 days at -30 °C for plasma and urine samples. The absolute recovery in plasma was 71.1%, and no matrix effect was founded. This method was proved simple, specific, sensitive, rapid and suitable for pharmacokinetics study of sitagliptin in human being.

2.Diagnosis and treatment of solid-pseudopapillary tumor of pancreas

Objective To summarize the clinical experience of diagnosis and treatment of solid-pseudopapillary tumor of pancreas(SPTP),and to work out the diagnosis and treatment process which matches the conditions of China.Methods The clinical data of consecutive 50 patients with SPTP who had been admitted to our hospital from January 2001 to March 2007 were retrospectively analyzed.Of all patients,3 were male and 47 female.Their median age Was 24 years(13-60 years).Preoperative imaging examination revealed typical findings and tumor markers were negative.SPIP of 23 cases were located in the head of pancreas,3 in the neck of pancreas,2 in the uncinate process of pancreas.3 in the body of pancreas,18 in the tail of pancreas,and the location of SPTP of 1 case was uncertain.Forty-eight patients underwent the operation of SPTP resection,1 of exploratory laparotomy and 1 of CT-guided fine-needle aspiration biopsy of metastatic lesions.Results Forty-nine out of 50 patients were diagnosed"pancreatic mass"preoperatively.Eighteen of the 32 patients with intact tumor capsule and 11 of the 16 patients without intact tumor capsule underwent operation without pathological examination.Thirty-two patients were followed up for 3-55 months,and no metastasis or invasion of the SPTP was observed.Conclusions Yong females are in the high risk group of SPTP.CT is the most valuable preoperative imaging test.Complete surgical resection is the first choice treatment with good prognosis.Resection of the head of pancreas with the preservation of duodenum and resection of body and tail of pancreas+splenectomy are the commou modalities used for the treatment of SPTP.The intactness of the tumor capsule is an important factor in making surgical plan and deciding the necessity of intraoperative frozen section pathological examination.The most common postoperative complication of SPTP resection is pancreatic fistula.
3.Clinical study on the association of ocular dominance with accommodation in myopia adult

Background Dominant eye is one of the functional asymmetric organ,and the dfference between dominant eye and undominant eye is a researching hotspot.But the study about accommodation in adult myopia is less.Objective This study was to determine the association between ocular dominances and accommodative factors in the subjects with adult myopia.Methods This study used prospective descriptive research method.Thirty-five subjects aged from 18 to 35 years with the myopia ranged from-2.00 D to-10.00 D and anisometropia less than 1.5 D,BCVA≥ 1.0 were recruited consecutively in this study.Ocular dominance was determined using the hole-inthe-card test and thumb test.Refractive error was measured with objective and subjective optometry,and amplitude of accommodation was measured by push-up test.Fusion cross cylinder(FCC) was used to measure the accommodative lag,and flipper test was applied to determine the accommodative facility.Oral informed consent was obtained from each subject before any relevant examination.Results No significant differences were found in the amplitude of accommodation (D),accommodative facility (cpm) and accommodative lag (D) between the dominant eye and undominant eye (accommodative amplitude:9.69 D±2.30 D vs.9.60 D±2.37 D,P =0.294 ;accommodative facility: 11.08 D±4.20 D vs.10.63 D± 4.60 D,P=0.260;accommodative lag:P=0.141).In the patients with the right eyes as dominance eyes,the accommodative amplitude of both eyes were (9.48±2.29) cpm and (9.33 ± 2.49) cpm,and accommodative facility were (10.50 ± 4.70) cpm and (9.99 ± 4.90) cpm.There were no significant differences between the right and left eyes in the accommodative amplitude,accommodative facility and accommodative lag (P =0.319,0.116,0.590).In the patients with the left eyes as dominant eyes,the accommodative amplitude of both eyes were (9.91±2.35)D and (9.88±2.26) D,and accommodative facility were (10.70±3.77)cpm and (11.25 ±4.27) cpm.No significant differences were seen between the right eyes and left eyes in the accommodative amplitude,accommodative facility and accommodative lag (P =0.749,0.295,0.238).Conclusions The amplitude of accommodation of the dominant eye is not significantly enhanced,and less accommodative lag and better accommodative facility also are found in the demonstrate eye in myopia adults with low anisometropia.
4.ANATOMIC STUDY OF MAXIMUM INTENSITY PROJECTION IN MRI FOR MEMBRANOUS LABYRINTH AND INTERNALAUDITORY MEATUS IN 15 CHINESE ADULTS

Objective The three-dimensional reconstructed images of maximum intensity projection (MIP) for membranous labyrinth and internal auditory meatus in Chinese adults were observed and measured in order to provide anatomic basis for otolosurgery and nerosurgery. Methods Thirty inner ears of 15 volunteers were scanned by using a circular temporal coil and three-dimension fast spin echo sequence with a 1^5T GE-signal MRI scanner. All original images were transferred to MRI workstation and all the structures of inner ear were reconstructed, rotated from various angles and measured by using maximum intensity projection. All the data were analyzed using SPSS 10.0 software. Results Anatomic structures of membranous labyrinth and internal auditory meatus were well demonstrated in three-dimensional reconstructed images of MIP in all volunteers. All three semicircular ducts, utricle, saccule, cochlear duct and internal auditory meatus produced high intensity signal. The results of measurement were statistically in significant.Conclusion Three-dimensional reconstruction of MlP might document, lively, stereoscopically and directly, the minute structures of membranous labyrinth and internal auditory meatus. We provide the basic data for the establishment of the MRI measurement criterion of inner ear.
5.Pharmacokinetics of amrubicin and amrubicinol in small cell lung cancer patients

Objective To evaluate the pharmacokinetics characters of amrubicin and its active metabolite amrubicinol following single and mul-tiple administrations in Chinese small cell lung cancer patients.Methods This study was designed as a multi -center , open label , single and multiple doses study.Twelve patients with extensive stage small cell lung cancer were given iv as amrubicin hydrochloride 40 mg· m-2 for 3 days qd and cisplatin 60 mg · m-2 at day 1.Concentration of amrubicin and amrubicinol in plasma , blood and blood cell were analyzed by LC -MS/MS.WinNonlin was used to calculate the pharmacokinetics parameters.Results Following single -and multiple -administration of amrubicin hydrochloride , and multiple-administration of amrubicin hydrochloride ,for amrubicin, eathylene diamine tetra -acetic acid-K3(EDTAK3) anticoagulant plasma:cmax was (3460 ±1273) and (4080 ±1895 ) ng· mL-1 , AUC0-t was (2050 ±408 ) and (2190 ±412 ) h· ng· mL-1;heparin anticoagulant plas-ma:cmax was (3660 ±1382) and (4360 ±2065) ng· mL-1, AUC0-t was (2100 ±363) and (2220 ±404) h· ng· mL-1;whole blood:cmax was (3240 ±1133 ) and (3920 ±1582 ) ng· mL-1 , AUC0-t was (2150 ±465 ) and (2560 ± 853) h· ng· mL-1;blood cell:cmax was (2100 ±544) and (2500 ±796) ng· mL-1,AUC0-t was (2030 ±465) and (2340 ±653) h· ng· mL-1, respectively.For amrubicinol, EDTAK3 anticoagulant plasma: cmaxwas (18.2 ±3.3) and (30.1 ±3.8) ng· mL-1, AUC0-t was (258 ±43) and (992 ±182) h· ng· mL-1; heparin anticoagulant plas-ma:cmax was (17.9 ±3.8) and (30.9 ±5.7) ng· mL-1, AUC0-t was (259 ±51) and (978 ±190) h· ng· mL-1;whole blood:cmax was (50.3 ±12.8 ) and (81.9 ±19.3) ng· mL-1 ,AUC0-t was (723 ±198) and (2610 ±656) h· ng · mL-1;blood cell:cmax was (83.6 ±19.9) and (153.0 ±25.2) ng· mL-1 , AUC0-t was (1290 ±319) and (4400 ± 831) h· ng· mL-1 , respectively.Conclusion Amrubicinol shows an accumulation after multiple administration , which is similar to Japanese by reported;plasma AUC0-24 is also similar to Japanese after single and multiple administrations;cmax of amrubicinol in blood cells are comparable to Japanese patients , but cmax in plasma tends to be lower.
6.Computational analysis of genetic loci required for amphid structure and functions and their possibly corresponding microRNAs in C. elegans.

OBJECTIVETo examine the important roles of microRNAs (miRNAs) in regulating amphid structure and function, we performed a computational analysis for the genetic loci required for the sensory perception and their possibly corresponding miRNAs in C. elegans.
METHODSTotal 55 genetic loci required for the amphid structure and function were selected. Sequence alignment was combined with E value evaluation to investigate and identify the possible corresponding miRNAs.
RESULTSTotal 30 genes among the 55 genetic loci selected have their possible corresponding regulatory miRNA (s), and identified genes participate in the regulation of almost all aspects of amphid structure and function. In addition, our data suggest that both the amphid structure and the amphid functions might be regulated by a series of network signaling pathways. Moreover, the distribution of miRNAs along the 3' untranslated region (UTR) of these 30 genes exhibits different patterns.
CONCLUSIONWe present the possible miRNA-mediated signaling pathways involved in the regulation of chemosensation and thermosensation by controlling the corresponding sensory neuron and interneuron functions. Our work will be useful for better understanding of the miRNA-mediated control of the chemotaxis and thermotaxis in C. elegans.

7.Controlled clinical trials on the treatment of lumbar disk herniation by Dingweiban and Xieban manipulation.

OBJECTIVETo observe and compare the clinical effect of Dingweiban and Xieban manipulation, and to compare the change of the deviation of spinous processes between two methods.
METHODSOne hundred and twenty-two cases were divided into two groups. Sixty-two cases were treated with Dingweiban manipulation method and 60 cases by Xieban manipulation. The changes of Fairbank scores, the clinical effects and the difference of the deviation of the spinous processes (L3, L4, L5) from the lumbar posterior-anterior X-ray were compared.
RESULTSThe scores before and after treatment and 3 months after treatment were compared. There were significant differences between two groups (P < 0.05) by nonparametric test. The result of Dingweiban manipulation group: 53 cases cured, 5 cases better, 3 cases effective and 1 case no effect. The result of clinical Xieban manipulation group: 43 cases cured, 6 cases better, 7 cases effective and 4 cases no effect. The clinical effects had significant differences after treatment and 3 months after treatment (P < 0.05, P < 0.0l) by nonparametric test. After the first treating, there was clear difference of the deviations' distance of the L4 spinous process compared with the Xieban manipulation group (P < 0.05). After the last treating, there were clear differences of the deviation distance of the L4 and L5 spinous processes compared with the Xieban manipulation group (P < 0.05, P < 0.01).
CONCLUSIONDingweiban manipulation is better than Xieban manipulation in effects and has influence on the deviation of spinous processes, especially for the L5 spinous process.

8.Semen quality analysis of 2 640 infertile men in Suzhou area.

OBJECTIVETo study the semen quality of male infertility patients in Suzhou area, China.
METHODSWe detected the semen quality of 2 640 infertility patients in Suzhou area using computer-assisted semen analysis technology.
RESULTSOf the 2 640 semen samples, 27.35% were found normal in all seminal indexes, 47.35% (1 250/2 640) abnormal in the percentage of grade a + b sperm, and 42.39% (1 119/2 640) abnormal in sperm motility. The percentage of grade a + b sperm and sperm motility were decreased with the increase of age, more obviously in those over 40 years. The incidences of asthenospermia and oligospermia were 37.31% (985/2 640) and 8.94% (236/2 640), rising with the increase of age, especially in those older than 30 years. The 9 sperm motion parameters, including VSL, VCL, LIN, MAD, VAP, STR, WOB, ALH and BCF, were significantly reduced with the decrease of sperm motility and sperm concentration.
CONCLUSIONThe infertile men in Suzhou area are mainly characterized by a decrease in sperm motility, especially in the percentage of grade a + b sperm. The correlation of age with sperm motility and incidence of asthenospermia and oligospermia suggests that men also have an appropriate childbearing age.

9.Effect of thyroid hormone on the contents of NOS and CO in the penile corpus cavernosum of rats.

OBJECTIVETo determine the contents of NOS and endogenous CO in the penile corpus cavernosum of Wistar rat models of hyperthyroid and hypothyroid, to detect the effects of thyroid hormone and endogenous CO on rat penile erection, and to further investigate the influence of thyroid hormone on human penile erection.
METHODSFifty 3-month-old male Wistar rats were randomly and equally divided into 5 groups: a hyperthyroid, a hyperthyroid treatment, a hypothyroid, a hypothyroid treatment and a control group. The contents of NOS and CO were detected by ultraviolet spectrophotometry.
RESULTSCompared with the controls, the content of NOS was significantly decreased in both the hyperthyroid and the hypothyroid groups (P < 0.01), even lower in the latter than in the former (P < 0.01), and so was the content of CO (P < 0.01), lower in the former than in the latter (P < 0.01). After treatment, the levels of NOS and CO were both elevated in either the hyperthyroid or the hypothyroid group, with no significant difference from that of the controls (P > 0.05).
CONCLUSIONThe contents of NOS and CO in the penile corpus cavernosum decrease with the imbalance of thyroid hormones, but return to normal once the imbalance is corrected. Under the same conditions, hypothyroid inflicts more damage on penile erection than hyperthyroid.

10.Clinical contrast of cervical pedicle screw fixation assisted by C-arm fluoroscopy or 3D navigation system.

OBJECTIVETo evaluate the feasibility and accuracy of cervical (C(2)-C(7)) pedicle screw fixation assisted by C-arm fluoroscopy or 3D navigation system (CT-based navigation system or Iso-C 3D navigation system).
METHODSOne hundred and forty-five cervical pedicle screws inserted with C-arm fluoroscopy and 187 pedicle screws inserted with 3D navigation system were observed by postoperative CT or Iso-C 3D scan. The process of navigation was investigated.
RESULTSIn the 145 screws inserted with C-arm fluoroscopy, 133 screws are accurate (91.7%), and in the 187 screws inserted by 3D navigation system, 183 screws are accurate (97.9%) (chi2 = 6.705, P = 0.010), and the 4 misplaced screws occurred in the early stage of navigation system application. The process of navigation was investigated in twenty-five patients. With CT-based navigation, the mean time for registration and surface matching was 3.5 minutes; with Iso-C 3D navigation, the mean time for images collection and transfer was 6.2 minutes. The mean time for screw-marker insertion was 2 minutes.
CONCLUSIONS3D navigation system can increase accuracy of cervical pedicle screw fixation obviously.
