OBJECTIVETo establish an animal model of acute blunt scrotal trauma (BST) and evaluate the types of lesion by conventional ultrasonography (CUS) and contrast-enhanced ultrasonography (CEUS).
METHODSWe made acute BST models in 21 healthy male New Zealand rabbits by striking 3 - 12 times the unilateral testes randomly selected with a 0. 5 kg iron ball falling freely from a 30 cm height. Then we evaluated the lesion types in the models by CUS and CEUS and verified our evaluation against pathological results.
RESULTSAcute BST models were successfully established in all the 21 animals, including contusion in 10, hematoma in 6, and rupture in 5, all confirmed by pathology. CUS clearly manifested the morphology, internal echoes, and blood flow of the testes, but had a low rate of accurate diagnosis in testicular contusion for over 6 hours as well as in complex lesions. CEUS revealed an earlier perfusion of the contrast agent and shorter arriving time (AT) and time to peak intensity ( TP) in testicular contusion than in the control testes (P <0.05) , but showed no statistically significant difference between the two groups in the half time of descending peak intensity (P>0.05). For testicular hematoma, contrast agent clearly presented its outline and a delayed low enhancement in the surrounding tissue, with significant differences from the control in AT and TTP. In severe testis rupture, occasional outflow but no perfusion of contrast agent was observed.
CONCLUSIONBST models can be established in rabbits by repeated strikes of the unilateral testes lesion of contrast agent was observed. with a freely falling iron ball. Simple contusion injury can be induced by less than 6 strikes, while complex injuries can be inflicted by more than 10. Combined application of CUS and CEUS can improve the accuracy of diagnosis of different types of lesion.

4. Relationship between Polymorphism of MTHFR and Curative Effect of Adjuvant Chemotherapy in Postoperative Colorectal Cancer Patients
Chinese Pharmaceutical Journal 2017;52(18):1644-1648
OBJECTIVE: To investigate the association between methylene tetrahydrofolate reductase (MTHFR)C677T polymorphism and the efficacy of the adjuvant chemotherapy with XELOX regimen for patients who had underwent radical resection of colorectal cancer. METHODS: Sixty-two patients who had received chemotherapy with XELOX regimen following radical resection of colorectal cancer were tested for MTHFR C677T polymorphism using Kompetitive Allele-Specific PCR to analyze the association of MTHFR C677T polymorphism with the prognosis and adverse reactions to chemotherapy. RESULTS: Among the 62 patients with colorectal cancer, there were 3 allelotypes (C/C, C/T and T/T)at the MTHFR C677T locus, and their frequencies were 46.8%, 40.3%, and 12.9%, respectively. The recurrence free survival time was prolonged in C/T and T/T group than C/C group(Log-rank=4.778, P<0.05). Specifically, the relapse rate was 58.6% in patients with C/C allelotype, which was significantly higher than 33.3% in patients with T/T and C/T allelotype(Log-rank=3.985, P<0.05). TNM stage(HR=5.326, P<0.05) and MTHFR C677T polymorphism(HR=0.284, P<0.05) were shown to be the prognostic factors for postoperative adjuvant chemotherapy. The toxicities of chemotherapy were primarily gastrointestinal reactions and bone marrow suppression, without statistically significant differences across different allelotypes (P>0.05). CONCLUSION: MTHFR C677T polymorphism is associated with the prognosis with adjuvant chemotherapy with XELOX regimen, and is not associated with the toxicities of chemotherapy. TNM stage IV is predicative of worse prognosis with postoperative adjuvant chemotherapy.

Objective To explore the safety and feasibility of the "many people, many point, many cooperation (3M)" precise locating handling method in transferring general anesthesia patients according to the business process reengineering (BPR) theory. Methods A comparative analysis on 138 patients who had undergone general anaesthesia operation between October 2016 and January 2017 was performed. The research objects were divided into two groups randomly. The patients in observation group (n=69) was transferred by 3M method, while the patients in control group (n=69) was transferred by traditional method. The indicators of the two groups were compared, including transferring time, the number of people involved, the rate of tube slippage during transferring, the changes of heart rates and systolic blood pressure in one minutes before and after handling. Results The time used to handle the patient is much shorter in observation group (134.38±41.69) s than that in control group (201.71±49.34)s, with statistical difference (P<0.05). It needed four people to transfer in all cases of the observation group, while more than four people are needed in 9 cases of the control groups. There was no tube slippage case in the observation group, while 6 tube slippage cases in control group. The changes of heart rates and systolic blood pressure of patients within one minute before and after carrying are much higher in control group than in observation group, the differences were statistically significant (P<0.05). Conclusions In contrast to the traditional transferring method, 3M handling method is safer and more efficient for patients after general anaesthesia operation. It was also showed having less effect on patients heart rate and blood pressure. In conclusion,3M handling method is well worth clinical application.

As one of the most serious types of psoriasis, pathogenesis of erythrodermic psoriasis (EP) is unclear so far. In this study, we aimed to detect the levels of Th1/Th2 cytokine-associated transcription factors and T-lymphocyte clone in peripheral blood mononuclear cells (PBMCs) derived from EP patients, and gene expression level of T-bet/GATA-3 in skin lesion. The potential role of Th1/Th2 reaction pattern played in the pathogenesis of EP was also discussed. Serum levels of IFN-γ, IL-2, IL-4 and IL-10 were quantified by ELISA among 16 EP patients, 20 psoriasis vulgaris (PV) patients and 15 healthy controls. The expression levels of T-bet/GATA-3 in the skin lesion and PBMCs were examined by real-time qPCR. The ratio of Th1/Th2 was measured by flow cytometry. The levels of IFN-γ, IL-2, IL-4 and IL-10 were higher in EP patients than in the healthy controls. The levels of IL-4 and IL-10 were 69.44±11.45 and 12.62±4.57 pg/mL, respectively, in EP patients, significantly higher than those in PV patients and healthy controls (P<0.05). Flow cytometry revealed the levels of both Th1 and Th2 in PBMCs from EP patients were higher than those in healthy controls, and the Th1/Th2 ratio was dramatically lower than in PV patients (P<0.01). The ratios of IFN-γ/IL-4 and T-bet/GATA-3 in EP patients were both less than 1.0, suggesting a reversal when compared with the other two groups. Our study indicated that the EP patients exerted a Th1/Th2 bidirectional response pattern, and the balance of Th cell subsets inclines to Th2, which might be one of the important mechanisms of EP pathogenesis.

China Tropical Medicine 2022;22(12):1215-
Abstract: Mycobacterium senegalense is one of the major pathogens causing bovine farcy, and reports of its infection in
human are rare. Here is a report on a woman who had been taking hormones and immunosuppressants for a long time for SLE
and underwent abdominal soft tissue infection with Mycobacterium senegalense after abdominal liposuction, to provide reference
for clinical diagnosis and treatment. The patient, female, 32 years old, has a history of SLE for more than 2 years, and currently
takes "methylprednisolone, hydroxychloroquine, and mycophenolate mofetil" regularly. Nine months before the patient was
admitted to the hospital, she once performed abdominal, waist and buttock liposuction in a medical beauty institution. One
month after the operation, several masses gradually appeared on the abdominal wall, accompanied by tenderness, one of the
masses had obvious fluctuation on palpation and purulent fluid could be drawn out. The location of the abdominal wall mass
was consistent with the insertion site of the liposuction needle. After the onset of the disease, the patient went to the medical
beauty institution for puncture of the abdominal wall mass, and 5 mL of purulent fluid was pierced and sent for bacterial
culture, and cultured "Mycobacterium Senegalense", after 3 days of treatment with "cephalosporin" antibiotics (specifically
unknown), the symptoms did not improve, so she went to the second affiliated hospital of hainan medical college. After
completing the relevant examinations during the hospitalization in our hospital, in order to clarify the etiology, another
abdominal puncture to extract pus was performed, the mycobacterial culture + identification results: Mycobacterium
senegalense. Consistent with the out-of-hospital results, the diagnosis of Mycobacterium senegalense infection was confirmed.
After 3 months of treatment with "cefoxitin, azithromycin, amikacin, and levofloxacin", the patient's abdominal wall soft tissue
infection was cured. Trauma or invasive procedures can lead to skin, muscle, or bone infection with nontuberculous
mycobacteria (NTM), which can manifest as chronic painless nodules that progress to purulent folliculitis and abscesses. NTM
infection should be suspected when the patient's wound has been exposed to water, there is a history of surgery, and empirical
anti-infection is ineffective. This is the first case of Mycobacterium senegalense infection caused by medical beauty, which tell
people that they should be cautious when choosing medical aesthetic projects and medical aesthetic institutions.

Objective To observe the effects of acupuncture on the behaviors and the expressions of Fis1 and OPA1, as well as mitochondrial ultrastructure in the hippocampus of mice with Alzheimer disease (AD); To explore the mechanism of action of acupuncture for AD.Methods Forty male SAMP8 mice were randomly divided into acupuncture group and model group, with 20 mice in each group. Another 20 male natural aging mice with the same age (SAMR1 mice) were set as the normal group. Acupuncture group chose Shenshu, Baihui, Xuehai and Geshu for intervention. 8 weeks later, Morris water maze was used to test the mice behaviors, and then hippocampus organization was taken. Western blot was used to detect the expressions of Fis1 and OPA1 and mitochondrial ultrastructure in hippocampal neurons of mice was observed by transmission electron microscopy. Results Compared with the model group, the escape latency of acupuncture group was significantly shortened (P<0.05), and the stay time in the former platform quadrant and former platform crossing times were significantly increased (P<0.05). The expression of Fis1 in the hippocampus of acupuncture group decreased significantly (P<0.05), while the expression of OPA1 increased significantly (P<0.05). The mitochondrial ultrastructure in hippocampus in the acupuncture group was effectively improved, and the mitochondrial surface density and body density were both increased in the acupuncture group compared with the model group (P<0.05).Conclusion Acupuncture may play a potential therapeutic role in AD by decreasing the expression of Fis1, increasing the expression of OPA1, recovering the injury of mitochondrial ultrastructure.
