1.Simultaneous determination of four flavonoids in Rubus chingii by MIPs-HPLC

AIM To establish a molecular imprinted polymers (MIPs)-HPLC method for the simultaneous content dertermination of four flavonoids in Rubus chingii Hu.METHODS Fe3O4 magnetic MIPs was added into ethyl acetate fraction solution of R.chingii to prepare mixed MIPs.The analysis of mixed MIPs methanol eluent was performed on a 30 ℃ thermostatic Diamonsil C18 column (250 mm ×4.6 mm,5 μm),with the mobile phase comprising of acetonitrile-0.1% formic acid flowing at 1.0 mL/min in a gradient elution manner,and the detection wavelength was set at 266 nm.RESULTS Tannins,lilsonide,quercetin and kaempferol showed good linear relationships within the ranges of 2.4-1 232.0 μg (R2 =1)、7.1-3 648.0 μg (R2 =0.999 9)、4.7-4 840.0 μg (R2 =0.999 9)、4.8-2 440.0 μg (R2 =0.999 9),whose average recoveries were 98.94%,99.33%,99.26% and 98.67% with the RSDs of 2.04%,1.40%,1.76% and 1.75%,respectively.CONCLUSION This accurate and reliable method eliminates impurity interference,which can be used for the content determination of flavonoids in R.chingii.
2.Effectiveness of tobacco-control intervention program among high risk population of chronic non-communicable diseases in institutions from Chongqing

Objective To explore the effect of the classified management intervention model on the smoking status of high risk population of chronic non-communicable diseases in institutions from Chongqing, so as to provide scientific basis for the management of high risk population of chronic non-communicable diseases. Methods Workers from governments and institutions of four districts in Chongqing were sampled by cluster sampling. High risk population of non-communicable disease were screened and enrolled into the intervention group and control group. The intervention group were intervened by classified management. And the control group weren’t intervened. To evaluate the change of smoking and smoking hazard cognition in intervention group and control group after one year intervention. Results The result of analysis of the net effect of intervention measures using difference in difference regression model showed that the intervention measures could reduce the smoking rate, the current smoking rate and the daily smoking rate in subjects (OR=0.32, 95% CI: 0.21-0.49; OR=0.31, 95% CI: 0.20-0.48; OR=0.34, 95% CI: 0.21-0.54). The intervention measures could increase the awareness rates of smoking causing serious diseases, stroke and lung cancer in subjects (OR=2.19, 95% CI: 1.08-4.42; OR=1.73, 95% CI: 1.17-2.57; OR=2.17, 95% CI: 1.25-3.77), the difference was statistically significant (all P<0.05). Conclusion The intervention model of classified management for high risk population of chronic non-communicable diseases in institutions can effectively improve the awareness rate of smoking hazards among the subjects, and then reduce the smoking rate. Therefore, this intervention model is worth popularizing and applying.
3.Application of Precise Intracoronary Retrograde Thrombolysis During Primary PCI in Patients With Acute ST-segment Elevation Myocardial Infarction

Objective: In comparison with thrombus aspiration, to study the safety and effcacy of precise intracoronary retrograde thrombolysis during primary percutaneous coronary intervention (PCI) in patients with acute ST-segment elevation myocardial infarction (STEMI).
Methods: A total of 123 consecutive patients with acute STEMI received primary PCI in our hospital from 2014-01 to 2015-12 were enrolled.The patients were randomly divided into 2 groups: RT group, the patients received precise intracoronary retrograde thrombolysis (RT),n=60 and TA group, the patients received thrombus aspiration (TA),n=63, among them, 3 patients with failed TA were excluded. Primary end points included occurrence rates of no-lfow after PCI and ST-segment resolution (STR)≥50% at (60-90) min after PCI; primary safety end points included occurrence rates of in-hospital stroke and TIMI-hemorrhage events.
Results:①Compared with TA group, RT group showed decreased no-lfow rate after PCI (1.7% vs 15.0%),P=0.008 and increased rate of STR≥50% after PCI (65.0% vs 45.0%),P=0.028, improved LVEF by echocardiography (50.7±8.6) % vs (46.7±8.3)%,P=0.011. The in-hospital MACE occurrence rate was similar between 2 groups,P>0.05.②No in-hospital stroke or TIMI-hemorrhage events occurred in neither group.
Conclusion: Intracoronary retrograde precise thrombolysis had the similar safety to thrombus aspiration during primary PCI in patients with acute STEMI, it may reduce no-relfow rate and improve left ventricular function after PCI.
4.Clinicopathologic features and immunohistochemistry of the basal-like subtype of invasive breast carcinoma.

OBJECTIVETo investigate the clinicopathologic features and immunohistochemical of the basal-like subtype of invasive breast carcinoma (BLBC), and to discuss the diagnosis standard.
METHODSImmunohistochemistry was performed in 448 cases of breast carcinoma and these cases were categorized into luminal A, luminal B, null subtypes, HER2-overexpressing and basal-like and their clinicopathologic features were observed under light microscope with stains of HE and immunohistochemical InVitrogen staining.
RESULTSAmong the breast cancer patients, the incidence of BLBC was 15.4% (69/448). Morphologic features significantly associated with BLBC constituently included nest structure and showing diffuse growth pattern, large scarring areas without cells in tumor, geographic necrosis, pushing margin of invasion, lymphocytic infiltrate in various degree in tumor stroma, syncytial tumor cell without clear boundaries, tumor cell showing vesicular unclear chromatin and nucleolus, markedly elevated mitotic count, metaplasia (all P < 0.01). Meanwhile, most BLBC showed strong immunoreactivity for CK5/6, CK14, CK17 (all P < 0.01).
CONCLUSIONBLBC showed distinct morphologic and immunophenotypic features.

5.Direct Versus Pre-dilated Rotational Atherectomy for Treating Calcified Coronary Lesions

Objective: To compare the efifcacy of direct and pre-dilated atherectomy (RA) for treating the patients with calciifed coronary lesions. Methods: A total of 137 coronary artery disease (CAD) patients received RA treatment in our hospital from 2010-04 to 2014-09 were retrospectively studied. The ischemic related lesions were all deifned as calciifed coronary lesions. The patients were divided into 2 groups: Direct RA group,n=81 and Pre-dilated RA group, the patients received balloon dilatation followed by RA,n=56. The procedural features, complications, in-hospital and 1 year occurrence rates of major cardiaccerebral vascular events (MACCE) were compared between 2 groups. Results: Compared with Pre-dilated RA group, Direct RA group had the less pre-stent balloon application,P=0.000 and the higher maximum post-dilatationpressure,P=0.004; lower rate of in-operative complication (14.8% vs 32.1%),P=0.016; higher rate of acute lumen gain (128.52±75.77) % vs (77.12±27.01) %,P=0.004; lower MACCE occurrencerate(7.3% vs 23.6%) at 1 year period,P=0.006.Cox regression analysis presented that the following indexes were related to MACCE occurrence within 1 year of RA treatment: balloon dilatation before RA (HR=8.166, 95% CI 1.872-35.614,P=0.005), left main disease (HR=13.649, 95% CI 2.983-62.440,P=0.001), minimum post-operative lumen area (HR=0.583, 95% CI 0.378-0.879,P=0.010), post-dilatation (HR=0.066, 95% CI 0.013-0.332,P=0.001) and EF>40% (HR=0.019, 95% CI 0.002-0.158, P=0.000). Conclusion: Direct RA had the lower MACCE occurrence rate at 1 year period, this might be related to less operative complication and the optimal lumen gain.
6.Auditory brainstem response may predict behavior threshold of children with hearing loss

Objective To investigate the impact of degree of hearing loss on auditory brainstem response predictions of behavioral thresholds. Methods A total of 179 patients with different hearing loss degree ( a total of 311 ears) patients from January 2012 to January 2016 in the affiliated hospital of Yan’an university were treated with auditory brainstem response (ABR), performed threshold behavior test, analyed the auditory brainstem electric response and behavioral threshold correlation, the factors that affect the ABR thresholds and behavioral threshold value and threshold of ABR and correction factor as well as linear regression equation for different frequency stimulation. Results ABR threshold was positively related to threshold behavior for the different frequencies stimulation, ABR threshold may predicte threshold behavior( P<0.05) ; ABR threshold value can predicte behavioral threshold differ-ence error and the degree of hearing loss( P<0.05) , through the relevant correction factors can reduce the predic-tion error. Conclusions Auditory brainstem responses may effectively predicte the hearing loss in children with be-havioural thresholds, differences in degree of hearing loss influence auditory brainstem response and behavioral threshold value, constant correction coefficient is conducive to auditory brainstem electric response and facilitates accuracy of the behavior threshold.
7.Effect of dopamine combined with norepinephrine on the renal function in patients with septic shock.

OBJECTIVETo investigate the effects of dopamine and norepinephrine on the renal function in the patients with septic shock.
METHODSEighty-seven patients with septic shock were divided into three groups (group A, B, C) according to the biggest infusing rate of norepinephrine, with the infusing rate of 0.5 - 0.9, 1.0 - 1.5, 1.6 - 2.0 microg x kg(-1) x min(-1), respectively. Mean arterial blood pressure (MAP), heart rate (HR), urine output, blood urea nitrogen (BUN), creatinine (CRE), urine albumin (U-ALB) and urine beta(2)-microglobulin (Ubeta(2)-MG) as well as APACHE III score in all the patients were detected.
RESULTSBefore anti-shock therapy was given, hypotension, tachycardia and oliguria occurred in all the 87 patients, and CRE, BUN, U-ALB, Ubeta(2)-MG and APACHE III score were abnormal in most cases. With the anti-shock therapy, MAP, HR, urine output and BUN, CRE in all patients returned to normal levels gradually, and U-ALB, Ubeta(2)-MG levels and APACHE III score also restored but still remained abnormal.
CONCLUSIONSThe first aim of treating septic shock should be restoring the organ blood supply, and based on volume resuscitation, dopamine, noradrenaline and other vasoactive drugs could be combined to maintain circulatory stability.

8.Effect of matrine on Fas, VEGF, and activities of telomerase of MCF-7 cells.

OBJECTIVETo study the effect of matrine on Fas, VEGF, and activities of telomerase of MCF-7 cells.
METHODSIn vitro cultured human breast cancer MCF-7 cells were randomly divided into the experimental group and the control group. The matrine solution was added in cells of the experimental group. Equal volume of culture medium was added in cells of the control group or the negative control group. Zedoary Turmeric Oil, the telomerase inhibitor was added in cells of the positive control group. Morphological changes were observed under an inverted microscope. The telomerase activity was detected by TRAP-ELISA. Expressions of Fas and VEGF protein were detected by immunocytochemical assay.
RESULTSMatrine obviously inhibited the growth and induced apoptosis of breast cancer cells. MCF-7 cells were treated by matrine of different concentrations at 24, 48, and 72 h, the telomerase activity gradually decreased along with increased matrine concentration and prolonged action time, showing dose-effect and time-effect positive relations. Matrine could up-regulate Fas protein expression and downregulate VEGF protein expression of MCF-7 cells.
CONCLUSIONMatrine showed obvious effect in inhibiting the growth of MCF-7 cells and promoting the apoptosis, which might be achieved by up-regulating the expression of Fas protein, inhibiting telomerase activity induced apoptosis of breast cancer cells, down-regulating the expression of VEGF protein, and inhibiting the tumor vascular formation.

9.Dynamic prediction on the number of influenza-like cases in Gansu province based on data from the influenza sentinel surveillance program, from 2006 to 2011

Objective To understand the epidemiological trend on the number of influenzalike cases and to explore the feasibility of early warning systems of influenza in Gansu province.Methods Based on data from the influenza sentinel surveillance program,a sequence chart was used to analyze the epidemiological trend on the number of influenza-like illness (ILI) cases.Both control chart and mobile percentile method were used to select the threshold of premium alert for the ILI of sentinel surveillance program.Warning effects were assessed by statistical model.Results The prevalence of influenza were both low in 2007 and 2008.Alert thresholds for ILI of Sentinel surveillance was built.The thresholds were higher alert in winter,but lower in summer.Both Seasonal Exponential Smoothing Model and Multiplicative Seasonal ARMA Model (1,1,1) (0,1,0) were used to dynamically predict the weekly percentage of outpatient visits for influenza-like illness (ILI%)of 2011.The concordance rates (predicted=actual) were 100% for both of them.According to the RMSE values,the dynamically predicted effect of the seasonal exponential smoothing model was superior to ARIMA.Conclusion Dynamic prediction on the number of influenza-like cases could reflect the epidemiological trend of influenza in Gansu province,but with some limitations.

Isotemporal substitution model is a powerful tool to explore the real association between physical behavior and health outcomes, which has the potential of the application in large-scale cohort study. This paper systematically introduces the principle of isotemporal substitution model and its implementation method in specific analysis to provide analytical ideas for the epidemiological research related to physical behavior in China. The baseline data of Regional Ethic Cohort Study in Northwest China conducted in Shaanxi province were used to analyze the relationship between physical behavior and cardiovascular disease with single-factor model, partition model and isotemporal substitution model. The advantages and disadvantages of different models were compared, and the advantages of isotemporal substitution model in quantifying physical activity health risk were introduced. Isotemporal substitution model could qualify physical behavior and health outcomes, which has wide application value in epidemiological research.
