1.Impact of chronic constipation on quality of life and economic burden of patients

Chronic constipation (CC) is one of the most common digestive diseases with high prevalence and long course.CC patients often suffer from sleep disorders and psychological problems such as anxiety and depression.Somatic and mental symptoms may affect with each other, so a vicious circle will develop to impair patients′ quality of life (QOL) and increase their economic burden.The article reviews the impact of chronic constipation on QOL and economic burden of patients, indicating that clinician should pay sufficient attentions on chronic constipation.
2.Advances in Study on Antidepressants in Treatment of Irritable Bowel Syndrome

Irritable bowel syndrome(IBS)is one of the most common functional gastrointestinal disorders,which severely affects the life quality of patients. Currently,the pathogenesis of IBS has not yet been clarified. Visceral hypersensitivity and central algesia abnormality may be involved in the pathogenesis of IBS. Antidepressants can improve abdominal pain,abdominal discomfort,mental disorders and abnormal defecation and enhance the quality of life. This article reviewed the advances in study on antidepressants in treatment of IBS.
4.Trends in incidence and clinical feature of hypertriglyceridemic acute pancreatitis: clinical review of 20 years

Objective To retrospectively investigate the clinical features and incidence trends of hypertriglyceridemic acute pancreatitis (HLAP) in Guangzhou First People's Hospital for 20 years.Methods The medical records of 1 362 patients,who were admitted to Guangzhou First People's Hospital with acute pancreatitis during January 1991 to December 2010,were reviewed and 99 patients met the HLAP criterion.These patients were divided into four groups according to years (1991 ~ 1995,1996 ~2000,2001 ~2005,and 2006 ~2010).The incidence and clinical features were compared among the 4 groups.Results Among the 99 patients,there were 61 males and 38 females,the mean age was (44 ± 12) years old,and young male was predominant in HLAP.HLAP accounted for 7.3% of all AP patients,and the incidence was increased 1.5 times during the 20 years period (5.4%,5.7%,6.7%,and 8.3% respectively).The serum amylase level was (513.3 ±462.7) mmol/L,TG level was (12.7 ± 7.0) mmol/L,Ranson score was (1.2 ± 1.1),CTSI score was 2.2 ± 1.1,incidence of SIRS was 36.4%,and the incidence of MODS was 18.2%,and the incidence of pancreatic pseudocyst was 5.1%,but the incidence and mortality of SAP was not changed with time.The incidence and mortality of SAP of HLAP were significantly higher than those of acute biliary pancreatitis (20.2% vs 8.5%,6.1% vs 2.8%).The rate of blood purification increased from 0 to 10.7%,and the hospital stay was decreased from 26 d to 14 d,and the difference between the two groups was statistically significant (P < 0.05).Conclusions Incidence of HLAP is increased gradually,with a trend towards more severe disease,but the prognosis is improved.
5. Endoscopic findings and clinical implication of heterotopic gastric mucosa in upper esophagus

Heterotopic gastric mucosa in upper esophagus (HGMUE) is a rare clinical condition, which is often associated with congenital agenesis. The diagnosis of HGMUE mainly depends on endoscopic and pathological examinations. The detection rate is increasing with the development of endoscopic techniques, however, there is no consensus on the duration of follow-up and therapeutic strategies for HGMUE and its clinical implication need to be further studied.
6.Prenatal diagnosis and typing of fetal cystic adenomatoid malformation of the lung by ultrasound

Objective To explore the clinical value and typing characteristic of prenatal ultrasonography for fetal cystic adenomatoid malformation of the lung(CAML).Methods Ultrasonographic features and typing of fetal CAML in 41 cases detected by prenatal ultrasonography were analyzed retrospectively.All cases were followed up until to the induction of labor or birth.Results (1)Site of tumor: in the 41 cases,there were 22 cases on the left side,15 on the right side and 4 on bilateral.(2)Typing diagnosis of prenatal ultrasonography: three cases were CAML type Ⅰ,14 were type Ⅱ,and 24 cases were type Ⅲ.(3)Pathology diagnosis: thirty two cases were induced abortion.CAML was confirmed in 29 cases by autopsy and the classifications were consistent with the prenatal diagnosis of ultrasonography.Three pulmonary sequestration cases were prenatally misdiagnosed as CAML type Ⅲ by ultrasonography.(4)The other nine cases were followed up to birth.Three masses decreased gradually and then disappeared.Six newborns were confirmed as CAML by CT.Their typings were consistent with the postnatal diagnosis.(5)The diagnostic accuracy rate of prenatal ultrasonography for CAML was 92.7%(38/41).Its misdiagnostic rate was 7.3%(3/41).Conclusions Prenatal ultrasonography has a high accuracy rate for the diagnosis and classification of CAML and is the first choice to detect CAML early in pregnancy.It has an important clinical value.Pulmonary sequestration should be distinguished from CAML type Ⅲ because they tend to be confused.
7.Therapeutic strategies of chronic constipation in special patients

Chronic constipation is one of the common gastroenterological conditions,which impairs physical and mental health of patients,compromises quality of life and increases medical cost.Constipation is usually managed by changing life styles,using enemas,laxatives,probiotics or antidepressants,performing bio-feedback or behavioral therapy.However,some patients are not satisfied with treatment.It is important to give individualized therapy to special patients with chronic constipation in order to enhance the therapeutic efficacy and to improve the quality of life.
8.Point Mutation Analysis on Tumor Suppressor Genes in Stool of Patients with Colorectal Carcinoma by Silver Stain Polymerase Chain Reaction Single Strand Conformation Polymorphism

Objective:The aim of this study was to seek a new approach for early detection and screening colorectal carcinoma. Methods: Point mutation analysis through 8% non denaturant polyacrylamide gel electrophoresis(acrylamide∶ N,N, methylene bisacrylamide=29∶ 1) was applied to examine stool of patients with colorectal carcinoma. Results: Forty five patients with different histological categories at all clinical stages and normal cases were examined. There were 14 individuals with lymph node metastasis while thirty one without lymph node involvement. Of 45 samples investigated, twenty eight patients with adenomatous polyposis coli(APC) or/and mutated in colorectal cancer(MCC) mutations in the stools were identified, which representing 62.2% of all patients and 93.3% of the patients in whom the tumor tissue with gene mutations. APC or MCC determined in the stool was 91.3% or 84.6% respectively corresponding to the carcinomas in which APC or MCC aberrants had been detectable. There were no significant relationships between genetic alterations in the stools and histological types, Dukes stages,lymph node involvements, sizes or locations of the tumor (P >0.05), and between genetic abnormalities in the stools and histomorphologies or blood occult tests of the patients (P >0.05). Conclusions: The method presented here may provides a useful noninvasive technique for screening,detection and prognosis of colorectal neoplasms.
9.Preparation and quality control of soybean isoflavone dropping pills.

OBJECTIVETo establish the optimized preparation procedure and study the method to determine the content for soybean isoflavone(SIF) Dropping Pills.
METHODThe preparation conditions, such as the proportion between SIF and PEGs, the temperature of mixture of SIF and PEGs, dropping distance, etc., were studied with Uniform Design and One-way ANOVA. SIF was identified by TLC and the content of SIF was determined by UV spectrometry at 262 nm detection wavelength.
RESULTThree batches of the prepared products meet the standards of the Chinese pharmacopoeia on dropping pills. SIF can be identified by TLC. Using UV spectrometry, the linear range of SIF was 0. 407 2 to 4. 072 g x mL(-1) and the correlation coefficient was 0. 999 8. In high, middle and low concentration, average recovery were 96. 54%, 97.27% and 97.21%, respectively (RSD were 1.3%, 0.78% and 0.71%).
CONCLUSIONThe preparation procedure is feasible, simple and suitable, the method established in this paper can be adopted for the quality control of SIF dropping pills, and the determination method is simple, relatively fast and accurate.

10.The treatment and diagnosis of primary seminal vesicle carcinoma:report of 4 cases

Objective To approach the clinical presentation,treatment and diagnosis of primary seminal vesicle carcinoma.Methods The records of 4 patients who diagnosed seminal vesicle carcinoma were retrospectively reviewed,including the symptoms signs and examination results as well as operation program,postoperative therapy.Considered to the literature reports.Bilateral seminal vesicles,bladder, prostate and rectum were totally removed in one case.Seminal vosiculectomy and partial cystoprostotectomy were performed in 2 cases,and the another one,bilateral lower ureterectomy and ileum substitute bladder was be done.Results Followed up for 3 months to 5 years,and no recurrence had been observed so far, one died of colon cancer after 2 years.Conclusions Early symptoms of primary seminal vesicle carcinoma are unobvious,so that early diagnosis of seminal vesicle carcinoma is difficult and the misdiagnosis is so usually.Thus,accurate recognition is important for early diagnosis.Radical surgery appears to offer the best chance and different approaches can be selected according to tumor stage and invasive condition of the circumambient organ.Comprehensive treatment like hormonal therapy,along with the 5-fluorouracil,paclitaxel,and oxaliplatin chemotherapy regimen appears to be effective against adenocarcinoma.
